Solicitation HSTS04-14-R-CT2043 Past Performance Data

Past Performance Data 1:

(1) A detailed explanation of the work performed on the contract. Include customer name, period of performance, dollar value (differentiate between total dollars obligated and total ceiling, if applicable), and whether the offeror acted as the prime or subcontractor. If this work was performed as a subcontractor, include the prime contractor information and describe only the work under the subcontract, not the total scope of work as performed by the prime contractor.

(2) A detailed explanation of how this contract is similar to the requirements of this solicitation, specifically in terms of size, scope, and complexity.

(3) Point(s) of contact for the customer (Include name, title, email, and phone number). If this work was performed as a subcontractor, include additional point(s) of contact for the prime contractor. Attachment J.10 (Past Performance Questionnaire) may be completed by either the prime contractor or the customer.

(4) If this work was performed as the prime contractor, indicate the major subcontractors, the estimated percentage of work performed by each, and their associated socio-economic status. If this work was performed as a subcontractor, briefly describe the overall scope of the prime contract and estimate the percentage of work that the subcontract represents out of the entire effort (indicate period of performance if different and dollar value if known).
