TECHNICAL EXHIBIT - License and Certification Requirements The Contractor shall maintain copies of all licenses and certifications readily available for Government review.

SOW Section Functional Alignment Certification and/or License

3.1 Program and Project Management

3.1 Program Manager • Project Management Professional (PMP)

• Incident Command System (ICS) 300 Level

3.2 Quality Management

3.2 Quality Manager • Lead ISO 9001 auditor certified

3.2.4 IACUC Certified Professional IACUC Administrator (CPIA) Qualifications

3.4 Animal Care and Welfare

3.4 Veterinarian

Services (Clinical Veterinarian)

• Doctor of Veterinary Medicine (DVM) or equivalent. Licensed to practice in one state within the US. At least 1 year of clinical experience with large animals.

3.4 Veterinarian

Services (Attending Veterinarian/Contr olled Substances Officer)

• Doctor of Veterinary Medicine (DVM) or equivalent. Licensed to practice in one state within the US. At least 5 years of clinical experience with large animals and/or board eligible for lab animal medicine (ACLAM).

The Contractor shall maintain copies of all licenses and certifications readily available for Government review.

3.4

Animal Care Staff • On-site Supervision: Certified as Certified Manager of Animal Resources (CMAR) with AALAS or CMAR eligible and finish certification within 12 months

• Animal Health Technician: Associate’s Degree in veterinary technology or Bachelor’s Degree in animal science or equivalent. Graduate of an AVMA accredited school in veterinary technology. Licensed in one U.S. state as Veterinary Technician. Certified or eligible for certification as LAT with AALAS

• Animal Care Technician: Certified as Assistant Laboratory Animal Technician (ALAT) with AALAS or eligible for certification (and obtain certification within 12 months). Bachelor’s Degree in animal science or eqivalent, Associate’s Degree in veterinary technology, or High School Degree with specialized animal experience.

3.5 Laboratory and Biocontainment

Services

3.5.1 Class II biological safety

cabinets (BSCs) Class II Biological Safety Cabinets (BSCs) to be monitored, maintained, repaired and certified by a qualified National Sanitation Foundation (NSF) accredited Class II Biosafety Cabinet field certifier

3.5.4 Regulatory Pesticide

Applicator

New York State Department of Environmental Conservation certified commercial category 9 pesticide applicator

3.5.4 Ornamentals Shade Trees

and Turf

New York State Department of Environmental Conservation certified commercial category 3A pesticide applicator

The Contractor shall maintain copies of all licenses and certifications readily available for Government review.

3.5 Courier Current, valid driver’s license, a proven record of safe driving

3.6 Utilities and Facility Systems

3.6.7, 3.6.9 Maintenance of chiller plant, and refrigeration equipment

Un limited CFC license

3.6.4 Operation and

maintenance of Wastewater Treatment Plant

Current NY State Grade 3A Wastewater Treatment Plant License

3.6.10 Electrical Distribution

3.8 Safety, Health, Environmental and Emergency Services

3.8.1 Fire truck and

ambulance drivers Training Program for Operation of Emergency Vehicles: A National Standard Curriculum (EVOC)

3.8.1 Shift supervisors, Fire, Rescue, and Emergency Medical Services, ICS Command Staff

Incident Command System (ICS) 300 Level

3.8.1 Fi re, Rescue, and

Emergency Medical Services

• Firefighter Level I certification

• New York State or National Certified Critical Care Emergency Medical

Technician (EMT-CC)

• NYS Licensed Paramedic

• Confined space rescue

• Current certification as paramedic and an Advanced Life Support (ALS) responder

3.8.1 Fire Chief • New York State (NYS) Firefighter II (or equivalent), National Fire Protection Association (NFPA) Fire Instructor I (or equivalent) and HazMat Technician certifications

Required to be obtained and maintained within 12 months of assuming the position of Fire Chief: Confined Space Rescue Tech; NYS Emergency Medical Technician (EMT)-B; Federal Emergency Management Agency (FEMA) Courses/Certifications FEMA ICS-300, FEMA ICS-400, FEMA IS-800A and FEMA IS-139 Exercise Design

3.8.2.2 Health Clinic Clinic Physician - MD and Board Certified in Occ Health. MD Degree; Licensed in New York to practice. Complete and submit DHS Form 5207.

The Contractor shall maintain copies of all licenses and certifications readily available for Government review.

3.8.2.2 Health Clinic BSN or Physician Assistant Degree; Licensed in New York to practice. Certified Nurse Practitioner or Physician Assistant. Complete and submit DHS Form 5207.

3.8.2.3 Health and Safety Certified Industrial Hygienist (CIH) Certification by the Board of Certified Safety Professionals

3.9 Transportation

3.9.1 Mari ne Vessel

Operations and maintenance (Master captain)

• USCG 1 00 Ton Inland Waterway License Radar certification (minimum level

TBD)

• Advanced Shipboard Firefighting certification

• FCC Marine Radio Operator Permit

3.9.1 Marine Vessel

Operations and maintenance (Master captain) Fi re, Rescue, and Emergency Medical Services Environmental Protection

Hazardous Materials Training and certification

3.9.1 Marine Vessel

Operations and maintenance (Master captain, deckhands)

Transportation Worker Identification Credential

3.9.1 Marine Vessel

Operations and maintenance (deckhands)

USCG Documented deck hand

The Contractor shall maintain copies of all licenses and certifications readily available for Government review.

3.9.2 Transportation:

• bus drivers ( 16 passengers or more)

• animal truck/trailer

• fuel oil trucks

• all vehicles with a gross vehicle weight over 26,000 pounds Exceptions include emergency vehicle (fire truck, ambulance) drivers

• Commercial Driver’s License (CDL) - Class B

• Heavy Equipment Officer should also have a Class A

3.10 Buildings, Roads and Grounds

3.10.1.1 Asbestos Work (Heating, Cooling, Ventilation ;

Liquid Effluent Decontamination Plant)

New York State Asbestos Hand ling Certificates (with one additional person as asbestos supervisory trained)

3.11 Hospitality and Staff Services

3.11.1 Cafeteria staff At Hire ServSafe Food Handler certificate or equivalent
