AMENDMENT 0002

Title of Contract

Type of Contract

Name, address and current telephone number of contract administrator or contracting officer

Description of project including information on how project overcame constrains noted in paragraphs 2.) and 3.) above

List of Key Personnel or Subcontractors involved with the prior project who are now involved with the proposed effort.

(b) SUBFACTOR 2 – Past Performance Questionnaire: The purpose is to evaluate the offeror’s overall quality of work and customer satisfaction with the contractor’s performance on similar work.

RFP REQUIREMENTS:

1.) Provide five (5) past performance questionnaires to customers whom you have completed a construction project for, within the past three years to demonstrate customer satisfaction with overall job performance and quality. The nature of the work and customer characteristics should be representative of the current proposed effort.

2.) Instruct customers to complete the questionnaire and fax, mail directly to the Coast Guard Contracting Officer for evaluation.

3.) It is the offeror’s responsibility to ensure the questionnaires are completed and forwarded directly to the Coast Guard Contracting Officer.

4.) Customer’s comments will be considered Source Selection Sensitive information and therefore customer’s names and comments will be confidential. To maintain the integrity of the source selection process, customers will be requested to not divulge the offeror’s name being considered nor to discuss customer comments with any other individuals.

(b) Volume II – Price

(1) The Offeror must submit the following detailed information for each definite work item to support the technical proposal for the seed project. The seed project will be used for evaluation purposes and will be awarded as a task order at contract award. Offerors may use their own format for the required information:

(i) Pricing breakdown of labor, materials, transportation, equipment and other direct costs.

(ii) Indirect costs (overhead), contingency and profit.

(iii) If proposed, price information for each subcontractor shall be furnished in the same format and level and detail as prescribed in sub-paragraphs (i) and (ii) above. Additionally, the offeror shall submit the following:

(A) A description of the items/work to be furnished by the subcontractor.
