DEERS – EHR ICD (draft) v

Scope This Interface Control Document (ICD) specifies the interface between the United States Coast Guard (USCG) Electronic Health Record (EHR) system and the Defense Eligibility and Enrollment Reporting System (DEERS).

System Identification DEERS is a computerized information service. DEERS data is used to verify the eligibility of individuals to receive Uniformed Services benefits. It is development by Electronic Data Systems (EDS) in support of the:

DEERS Program Office 1600 N. Wilson Blvd.

Suite 400 Arlington, VA 22091

EHR is a Commercial Off-the-Shelve (COTS) software package that supports the USCG Health Care system through its USCG healthcare clinics and deployed forces. It integrates the management of all health care services under a single system.

Document Overview The purpose of this document is to describe the interface between EHR and DEERS. It is not to be used as a requirement documents. It serves as a resource for understanding this interface as it is envisioned to exist in the future EHR.

This document describes the concept of operations for the interface, defines the message structure and protocols which govern the interchange of data, and identifies the communication paths that the data is expected to flow. The document contains message specifications and communication requirements.

1-1

Concept of Operations The operational objective of the interface between EHR and DEERS is to provide USCG healthcare clinics with accurate and timely patient’s eligibility, enrollment and Non-Availability Status (NAS) information. The EHR on deployed forces does not directly interface with DEERS but rather with the EHR central system located in Martinsburg, WV during periods that sufficient connectivity exists between the ship and the central system.

System Overview The functionality and architecture of the participating systems is described in the following paragraphs. They briefly summarize each system; place special emphases on functionality related to the interface and identify key hardware and software components of the DEERS. The EHR system components will be identified once the system has been acquired.

EHR

EHR is a Commercial Off-the-Shelve (COTS) software package that supports the USCG healthcare system throughout its USCG healthcare clinics and deployed forces. It integrates the management of all USCG health care services under a single system.

The USCG healthcare clinics include medical and dental clinics and provide integrated information services in support of patient care. The system supports application modules, patient care and documentation, administrative and management functions, and other infrastructure to support the USCG health care program.

Application Modules include:

· Ambulatory Care

· Urgent Care

· Dental

· Physical Therapy

· Optometry

· Behavioral Health

· Immunization

· Audiology

· Radiology

· Ambulatory Pharmacy

· Laboratory

· Patient Portal

Patient Care and Documentation include:

· Clinical Content and Decision Support

· Patient Problems, History and Laboratory Tests

· Charting and Drawing

· Medications

· Sick Call / Triage Center

· Ophthalmology

Administrative and Management include:

· Billing

· Health Care Management

· Scheduling

· Worklists

· End User Dashboard

· Reporting and Forms

· Health Information Management (HIMS)/ Medical Records

Other Infrastructure includes:

· Remote Access to Patient Clinical Information for USCG Physicians and Staff

· Remote Access to Patient Clinical Information for Affiliates/ Contract/ Network Providers

· Remote Access to Patient Clinical Information for Ships

· Kiosks

· User Interface

· User Roles/ Security

EHR will be implemented as a COTS package in a CITRIX environment with server hosting in Martinsburg, WV and clients at USCG healthcare clinics. Remote stations will be used with deployed forces and will synchronize with the server when connectivity is sufficient or when the ship returns to port.

DEERS

DEERS is a reporting system designed to aid the Department of Defense (DoD) in improving distribution of personnel and health benefits to entitled beneficiaries by providing interfacing systems with patient’s medical benefits eligibility information. The system also supports the Health Care Management, Research, and Liaison activities. DEERS data provides information for eligibility verification and identification (ID) card issuance for individuals entitled to Uniformed Services Benefits.

DEERS operates as two systems: Enrollment and Eligibility. The Enrollment System represents the past, present, and potentially eligible beneficiaries of the system. The Eligibility System provides an Online Inquiry function to determine the benefit eligibility of sponsors and their dependents that are entered in DEERS.

Functional Allocation The interface between EHR and DEERS provides EHR with access to data held by DEERS:

· Patient’s eligibility to receive uniformed services medical benefits

· Patient’s demographic and organ donor information

· Reciprocal disenrollment information

· Non-Availability Statement information

· Non-Availability Statement information The interface between EHR and DEERS provides DEERS with data held by EHR:

· Patient’s demographic, organ donor status and blood type

· Patients MCP enrollment information

· Non-Availability Statement information Eligibility for Care Patient data related to eligibility for medical care as a uniformed service's benefit is collected and maintained in DEERS. EHR requests eligibility data from DEERS as needed. EHR does not collect eligibility data locally and does not modify eligibility data in DEERS. When DEERS responds with a patient's eligibility, it also sends selected information they hold relative to the patient's demographic data, sponsor identification and enrollment information. EHR notifies DEERS when a decision is made to provide patient care to an individual whom DEERS classifies as ineligible.

Patient Demographics When patients are registered at an USCG healthcare clinic, EHR collects the following patient information, which may be updated during subsequent encounters:

1. Patient Name and FMP/SSN

2. Sponsor Name, Rank, and SSN

3. Patient Gender and DOB

4. Patient Address and Phone number

5. Patient Blood Type

6. Organ Donor information.

EHR notifies DEERS when any of the above information is modified (added, changed or deleted) during registration or subsequent editing.

Managed Care Enrollment Patient enrollment in and disenrollment from an MCP is performed on EHR. EHR notifies DEERS of patient enrollments, disenrollments and other changes to enrollment information. On request, DEERS forwards to the requesting EHR site the identity of its patients who were reciprocally disenrolled by a remote EHR site. The requesting EHR site will use this information to update its database.

Non-Availability Statements EHR generates non-Availability Statements (NAS) to document the authorization for non-military treatment facilities to provide services for patients who are eligible to receive treatment at an USCG healthcare clinic. EHR notifies DEERS when issuing or updating a NAS on EHR. DEERS maintains this information and provides EHR with NAS Reports, upon request.

Interface Transactions Table 21 shows the transactions/events on EHR that trigger the messages sent to DEERS. The paragraph, in this document, where each message’s format and content are defined is also listed. All messages received from DEERS are triggered upon DEERS receipt of a message from EHR; these are also listed in Table 21. For additional information on the specific transaction see the paragraph of this document referenced in the last column of this table.

Data Transfer EHR and DEERS exchange patient data via custom messages, which are transmitted as triggering events occur. EHR and DEERS exchange patient data via custom messages, which are transmitted as triggering events occur. Computer connectivity between EHR and DEERS occurs in two ways. Most EHR sites connect via communications processes using the Defense Information Systems Network (DISN) Wide Area Network (WAN) and based on the Internet Protocol (IP) network address of the DEERS McData gateway. A few EHR sites connect using the Defense Medical Systems Support Center (DMSSC) network with leased telephone lines and modems. Specific communications requirements are itemized in Section 4 of this document.

Security and Integrity Existing security protocols will continue to be used. Currently the network performs validations of IP address of remote systems are performed.

C2 Requirements All participating systems shall adhere to the security requirements as defined for division C (discretionary protection), class C2 systems (controlled access protection) in the Department of Defense Trusted Computer System Evaluation Criteria (TCSEC), DoD 5200.28-STD (also known as the Orange Book).

Privacy Requirements Participating systems of the CDI interface send and receive Sensitive But Unclassified (SBU) administrative, personal, and medical data. Examples of such information include patient name, social security number, and benefit information. This data is subject to the provisions of the Privacy Act of 1974 and DoD Privacy Program, DoD 5400.11-R. Additionally, its personal and medical data fall within the content of Exemption Number 6 of the DoD FOIA Program, DoD 5400.7-R. Both the DoD Privacy and FOIA programs mandate adequate data protection.

EHR/DEERS Message Structure The following sections describe the basic rules that are followed for all messages sent and received by EHR.

Encoding Rules EHR and DEERS exchange patient data via a custom set of messages. Each message is composed of a defined set of data fields of fixed length. Padding must be used to maintain the fixed field and message length. The fields are organized in a specified sequence with no delimiters. The data fields are of two data types: string and numeric. Fields are found within the message by employing an offset scheme. The required padding scheme for each data field is specified in its associated message description in Section 3.2 and 3.3 of this document. Table 32 defines the nomenclature used to specify the padding scheme for each data field.

Table 32 Legend -- Data Field Padding Nomenclature

A message requires all specified data fields. If there are no available data or applicable business rules directing that the information associated with the data field not be transmitted, the field must be padded as indicated in the associated message description. Any unexpected fields will be treated as an error. See Section 4.3 for error management rules.

All data must be represented as displayable ASCII characters (hexadecimal values between 20 and 7E, inclusive). All special separator and other special characters must also be displayable ASCII characters, except the message terminator, which must be the ASCII Carriage Return character.

Conventions This section describes the layout of the tables used to depict the messages and responses exchanged by EHR and DEERS.

As applicable, the following items of information are listed as part of each message description:

Transaction ID: The EHR message identification. The value contained in this field tells DEERS what type of message this is. (See Section 3.2 – Many messages have the same Transaction ID; the value in the Transaction Type differentiates them.)

Transaction Type: The value contained in this field tells DEERS what type of transaction this is and, accordingly, the purpose of the message.

Source: The originator of the message.

Target: The receiver of the message.

Repeat at Element Name: Name of the data element in the message that identifies the beginning of a repeat segment. (This applies only to some DEERS messages).

Repeat Value: The maximum number of times the data stream can be repeated in a message. The value can contain an actual numerical value or a data element name (value by reference). This only applies to some DEERS messages.

Repeat Condition: A numerical value that identifies the type of report and thereby the data content of the report. (This only applies to the NAS Monthly Report response message from DEERS.)

Each column header used in the message layouts shown in Sections 3.2 and 3.3 represents an attribute of the data fields listed for each EHR and DEERS message. The table format in Table 33 depicts EHR messages. The table format in Table 34 depicts DEERS messages.

RP – Repetition - Whether a data field may repeat. An asterisk under this heading indicates that the identified data field and all data fields that follow it repeat as a segment.

FILE - The EHR database ID Number for the EHR File that is the source of value in the data field. For example, File = 2 - Patient File.

Note: Where File = API - The EHR Application Software is the source of the value in the data field.

FIELD - The EHR database ID Number for the EHR Field that is the source of the value in the data field.

VALID RANGE - The range of valid values for the data field.

DESCRIPTION – A high-level description of the contents of the data field.

EHR Messages EHR sends unsolicited update messages to DEERS and queries (requests) DEERS for information when triggered by an event initiated by an authorized EHR user.

Paragraphs 3.2.1 through 3.2.20 describe the EHR messages transmitted to DEERS.

Table 35 provides the triggering events for these messages.

Address/Log/Organ Donor Update Message This unsolicited message will notify DEERS when EHR updates a patient's demographic information, organ donor status or blood type information. Additionally, this message will notify (log) DEERS when EHR accesses patient information via a request for patient eligibility.

The specification for this message is provided in Table 35.

Enrollment/Disenrollment (8) Message This message notifies DEERS that a patient's MCP status on EHR, relative to their most current enrollment record (MCP plan), is changed from 'disenrolled' to 'enrolled'.

Upon successful receipt of this message, DEERS will change the patient's DEERS current enrollment record to 'enrolled' status, if the following are true:

· Patient's enrollment status on DEERS = 'disenrolled'

· MCP Enrollment Date in the AQC48 message matches the one on DEERS

· MCP Disenrollment Date in the AQC48 message = current or future date If any one of these is not true, DEERS will return one of the following Discrepancy Codes: A3, A4, or A5. Upon receipt of any Discrepancy Code, EHR will restore the patient's enrollment status to 'disenrolled' and the Enrollment End Date to the date it contained prior to the Cancel Disenrollment process.

The specification for this message is listed in Table 315.

Reciprocal Disenrollment Message This message queries DEERS for all the requesting EHR site's patients who are reciprocally disenrolled. A EHR site can reciprocally disenroll a patient from the EHR site where they are currently enrolled by sending DEERS an AQC43 or AQC44 message. DEERS stores the reciprocal disenrollment for the patient relative to the patient's current DMIS (Defense Medical Information System). A patient's current EHR site cannot reciprocally disenroll them.

When DEERS receives an AQC5J message from a EHR site, DEERS provides that site with all its patients who have been reciprocally disenrolled.

The specification for this message is listed in Table 317.
