PERFORMANCE WORK STATEMENT

FOR CUSTODIAL SERVICES AT

DEFENSE SECURITY COOPERATION AGENCY

(DSCA)

DEFENSE INSTITUTE OF SECURITY COOPERATION

Part I

General Information

PERFORMANCE WORK STATEMENT FOR CUSTODIAL SERVICES

1.0 General

The Defense Security Cooperation Agency (DSCA) is responsible for coordination and oversight the

Department of Defense (DoD) Security Cooperation (SC) program. The DoD utilizes the SC program to create trust and influence, while promoting access and interoperability vital to US national security.

This is a non-personal services contract to provide custodial services to the Defense Security Cooperation

Agency (DSCA) / Defense Institute Security Cooperation Studies (DISCS) and Case Writing Division

(CWD). The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the

Contractor who, in turn, shall be responsible to the Government.

1.1 Description of Services

The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, material, supervision, and other items and non-personal services necessary to perform the services outline in this performance work statement. The contractor shall perform to the standards in this contract.

1.2 Objectives

1.2.1 DISCS/CWD requires the services of a contractor to perform custodial services to present a clean, neat and professional appearance in accordance with commercial standards to include all local, state and federal laws and the requirements of this Performance Work Statement (PWS). Contractor shall establish internal quality control procedures for ensuring contract service requirements are met and contract deficiencies are corrected in a timely manner.

1.2.2 The Contractor shall provide all management, tools, equipment, and labor necessary to ensure that custodial services are performed at DISCS/CWD, Building 52, Wright Patterson AFB, Ohio in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance.

The contractor shall accomplish all restroom cleaning tasks and DISCS/CWD will provide all hand soap, toilet paper and paper towels in order to meet the requirements of this PWS and the Performance

Requirements Summary (PRS). Cleaning frequencies are established in Service Standards. All work performed by the contractor shall be performed in accordance with all standards, instructions, and commercial practices.

1.3 Hours of Operation

1.3.1 The Contractor shall perform regularly scheduled services beginning at 1700 hours Monday through

Friday. The contractor shall leave only after all scheduled tasks are complete, but no later than 2100 hours. Therefore, the window for performance is 1700 – 2100 hours Monday through Friday. The contractor may report up to 30 minutes before 1700 hours in order to allow for the commencement of services at 1700 hours. The contractor will not be required to work on Federal Holidays or periods when the government facility is closed due to local or national emergencies, administrative closures, or similar government directed facility closing. The Contractor shall at all times maintain an adequate workforce for the uninterrupted service when the government facility is not closed for the above reasons.

1.3.2 On Call: The contractor shall be available for on call requirements upon emergencies such as toilet flooding, etc., and shall have a person(s) on site within two hours from DISCS placing the call.

1.4.1 When a holiday falls on a Saturday, the preceding Friday shall be considered a holiday. When a holiday falls on a Sunday, the following Monday shall be observed. If a periodic task, as scheduled by the contractor, falls on one of the above listed holidays, the contractor shall perform said task on the next workday following the holiday.

1.4.2 Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

1.4.3 In the event the base commander determines it necessary to close the base due to adverse weather, a base or local disaster, security problems or other event, the following radio and television stations will be notified:

1.4.4 Additionally, base status information can be obtained by calling (937) 656-7669 (i.e., 656-SNOW) or by going to the public, off-base website at http://www.wpafb.af.mil/.

1.4.5 The contractor shall not receive any other form of notification of a base closure from the government. The contractor is responsible for the notification of his/her employees.

1.4.6 Continuation of Contractor Services during Base Exercises, Local Emergencies or Contingencies, and Implementation of Local Contingency Plans. It has been determined that none of the services http://www.wpafb.af.mil/ specified in this PWS are required to be performed in support of base exercises, local emergencies or contingencies or implementation of local contingency plans.

1.5 Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes.

1.5.1 The contractor shall comply with all requirements and procedures IAW AFFAR 5352.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference.

1.5.2 The contractor is responsible for ensuring employees comply with all security requirements imposed by the Base Commander at all times while its employees are on the installation. The contractor shall also follow the instructions of each organizational commander pertaining to security.

1.5.3 The contractor shall not be granted access to classified information, but shall be required to perform duties in controlled access area(s) where classified information is processed and stored. If the contractor inadvertently gains access to classified information, the contractor shall notify 88 SFS/SFAI at (937) 255-

4443 within 30 minutes of the incident.

1.6 Place of Performance

The Contractor shall perform tasks Building 52, DISCS/CWD, Wright-Patterson AFB, OH 45433.

1.7 Period of Performance

The period of performance for this effort shall be for one (1) base year of 12 months and four, 12-month option years. The period of performance reads as follows:

Base Year 1 April 2018 - 30 March 2019

Option Year 1 1 April 2019 - 30 March2020

Option Year 2 1 April 2020 - 30 March 2021

Option Year 3 1 April 2021 - 30 March 2022

Option Year 4 1 April 2022 - 30 March 2023

1.8 Pre-award Conference/Post Award Conference/Periodic Progress Meetings

The contractor shall attend any conferences convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The Contracting

Officer, Contracting Officers Representative (COR), and other government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the Government views the contractor's performance and the contractor shall apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.9 Contractor Personnel

1.9.1 The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the contracting officer. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager or alternate shall be available during normal business hours as outlined above, Monday thru Friday except Federal holidays or when the government facility is closed for administrative reasons. The contractor shall be responsible for managing and overseeing the activities of all contractor personnel, as well as any subcontractor efforts used in performance of this effort. The contractor's management responsibilities will include all activities necessary to ensure the accomplishment of timely and effective support, performed in accordance with the requirements contained in the PWS. Resumes shall be submitted for the Contract Manager assigned to perform under this PWS will contain documented experience directly applicable to the functions to be performed.

Further, these prior work experiences will be specific and of sufficient variety and duration that the employee is able to effectively and efficiently perform the functions assigned.

1.9.2 Key personnel are those personnel considered essential to successful contractor performance, including the Contract Manager and other personnel providing supervision or critical technical oversight designated above. The contractor shall provide resumes of all key personnel in response to this PWS and all subsequent contract modifications. The resumes must identify all key personnel and their proposed role in the execution of the PWS.

1.10 Contracting Officer’s Representative (COR)

The COR will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communications with the contractor concerning technical aspects of the contract; issue written interpretations of technical requirements, including government drawings, designs, specifications; monitor contractor's performance and notifies both the Contracting Officer and contractor of any deficiencies; coordinate availability of government furnished property; and provide site entry of contract personnel. A letter of designation issued to the

COR, a copy of which is sent to the contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting contract.

1.11 Contract Employees

1.11.1 Contract personnel shall present a neat appearance and be easily recognized as contractor employees by wearing a Security Identification Badge at all times while on government premises. When contract personnel attend meetings, answer phones, and work in other situations where their status is not obvious to third parties, they must identify themselves as such to avoid creating the impression they are government employees.

1.11.2 Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured as required by the local government facility.

1.11.3 The contractor shall be responsible for ensuring employees comply with all security requirements imposed by the Base Commander at all times while his/her employees are on the installation. The contractor shall also follow the instructions of the local organizational commander pertaining to security.

1.12 Contract Personnel Conduct

Contract personnel’s conduct shall not reflect discredit upon the government. The contractor shall ensure that personnel present a professional appearance. The contractor’s employees shall observe and comply with all local policies and procedures concerning fire, safety, environmental protection, sanitation, security, and possession of firearms or other lethal or illegal weapons or substance. The contractor is responsible for ensuring that any contractor employees providing services under this contract conduct themselves and perform services in a professional, safe, and responsible manner. The contractor shall remove from the job site any employee for reasons of misconduct or security violations in accordance with DOD Directive 5500.7-R, “Joint Ethics Regulation”. Contract employees must avoid improperly influenced in the execution of their duties under the contract. Particular attention must be applied to acceptance of gifts/ gratuities, and on non-disclosure of sensitive or classified information. The contractor shall ensure employee conduct complies with 41 U.S. C 423 relative to release of acquisition related information or actions or discussions which may prejudice future competitions. The contractor shall ensure no contractor employees conduct political related activities or events on Wright-Patterson Air

Force Base Facilities.

1.13 Traffic Laws

The Contractor and their employees shall comply with base traffic regulations.

1.14 Weapons, Firearms, and Ammunition

Contract employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately owned vehicle while on Wright-Patterson AFB.

1.15 Reporting Requirements

Contract personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information.

1.16 Key Control

1.16.1 The contractor shall establish and implement methods of making sure all keys issued to their employees by the Government are not lost or misplaced and are not used by unauthorized persons.

NOTE: All references to keys include key cards. Keys issued to the contractor by the Government shall not be duplicated. The contractor shall develop procedures covering key control that shall be included in the Quality Control Plan. Such procedures shall include turn-in of all issued keys by personnel who no longer require access to locked areas. The contractor shall immediately report any the loss of any keys and key cards to the Contracting Officer.

1.16.2 In the event keys, other than master keys, are lost or duplicated, the contractor shall, upon direction of the Contracting Officer, re-key or replace the affected lock or locks; however, the government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the contractor.

1.16.3 The Contractor shall prohibit the use of government issued keys by any persons other than the

Contractor’s employees. The Contractor shall prohibit the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer.

1.17 Payment Information

Invoicing Instructions: Requests for payments must be submitted electronically via the Internet through the Wide Area Workflow – Receipt and Acceptance (WAWF-RA) system at https://wawf.eb.mil. All contractors must create two documents (Invoice and Receiving Report; (i.e. Combo Documents) with a single-data entry session.

1.18 Quality Control

The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract. The QCP is to be delivered within 30 days after contract award, and three copies of a comprehensive written QCP shall be submitted to the Contracting Officer and COR within 5 working days when changes are made thereafter. After acceptance of the quality control plan the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC system.

1.19 Quality Assurance

The Government shall evaluate the contractor’s performance under this contract in accordance with the

Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.20 Manpower Reporting

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the DSCA via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:

http://www.ecmra.mil/ Reporting inputs will be for the labor executed during the period of performance during each government fiscal year (FY), which runs October 1 through September 30. While inputs maybe reported any time during the FY, all data shall be reported no later than October 31 of each http://www.ecmra.mil/ calendar year, beginning with 2014. 2. DEFINITIONS, INITIALISMS AND ACRONYMS

2.1 DEFINITIONS:

2.1.2 CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.3 CONTRACTING OFFICER. An individual with warranted authority to enter into, administer, terminate, and make contractual determinations and findings on behalf of the U.S. Government. Note:

The only individual who can legally bind the government.

2.1.4 CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.4 DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.

2.1.5 DELIVERABLE. Anything that can be physically delivered, but may include non- manufactured things such as meeting minutes or reports.

2.1.6 PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

2.1.7 QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

2.1.8 QUALITY ASSURANCE Surveillance Plan (QASP). An organized, written document specifying the surveillance methodology used for surveillance of contractor performance.

2.1. 9 QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.

2.1.10 SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

2.1.11 WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.

2.1.12 WORK WEEK. Monday through Friday, unless specified otherwise.

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

3. GOVERNMENT FURNISHED ITEMS AND SERVICES

3.1 Services

The Government will provide all utilities required to support the assigned area. The estimated square footage for the building is 50,874 square feet.

3.2 Facilities

The Government will provide 3 storage closets within building 52 wherein the contractor may store cleaning supplies and equipment. Janitorial closets will be available for use by the contractor, without cost, for the purpose of storing materials and equipment, excluding flammable materials. The contractor will be required to maintain odorless and clean closets. Closets will be free of clutter and debris. Cleaning supplies and tools will be stored and organized in such a manner as to allow easy access and movement in closet. Closets provided for janitorial service use will be locked if locks are available. The Government will not be responsible for the contractor’s stored supplies or equipment kept in the building or janitor’s closets or for the contractor’s employees’ personal belongings.

3.3 Refuse removal, Entomology Services, Police and Fire Protection

The Government will provide the contractor these services as necessary in performance of the tasks outlined in this PWS.

PART 4

CONTRACTOR FURNISHED ITEMS AND SERVICES

4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES

4.1 The Contractor shall furnish all cleaning supplies, equipment, facilities supervision, and services required to perform work outlined in the PWS and associated work schedules, technical exhibits and performance frequencies.

4.2 The contractor shall maintain storage spaces to the same standards as similar area as occupied by the

Government and use space only in connection with performance under this contract. The contractor shall not make any alterations to the space except with the prior written permission of the building manager.

4.3 The contractor shall notify the Government of any circumstances of needed repair of assigned space or of any area that may affect performance of contract work, present unsafe or unhealthy condition, or cause any delays or interference of work by employees of the contractor

4.4 Contractor-Furnished Equipment/Vehicles. The contractor shall provide and maintain all equipment and vehicles necessary to perform the requirements of this contract. Contractor vehicles shall have the company name prominently displayed on both sides of the vehicle and be maintained to present a neat, professional appearance. Contractor vehicles shall be used solely for the intended purpose of performing services in this performance work statement during normal hours of operation.

4.5 All equipment used in the performance of this contract shall be in good operable condition and carry a

U/L (Underwriters Laboratory) listing. The contractor shall furnish a listing of all contractor-furnished equipment utilized during the contract period to the contracting officer not later than the pre-performance conference for approval by the Contracting Officer.

4.6 Cleaning Products Approval. The Contractor shall provide a list of all cleaning products to be used

Material Safety Data Sheets (MSDS). The contractor shall provide to the Government 3 copies of the

MSDS for all cleaning products used during the performance of this contract. The contractor shall use no cleaning product until approved by WPAFB Bioenvironmental personnel on the base facility.

PART 5

SPECIFIC TASKS

5.1 Facility Cleaning Services

5.1.1 Non-carpeted floors (Hard Floors: Vinyl Tile, Ceramic Tile, Wood, and Concrete. Hard floors shall be maintained free of grit, soil, dust, scuff and heel marks, stains, spills, debris, litter and other foreign matter by effective routine cleaning. Cleaning shall be accomplished by the most appropriate method

(vacuum, sweep, dust mop, damp mop, or spot clean and with cleaning solutions if applicable), for the specific floor type. After cleaning, floor surfaces to include grout shall have a uniform, clean appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film, or standing water.

Baseboards, corners, and wall/floor edges shall also be clean. Chairs, trash receptacles, and other easily moveable items shall be moved to maintain floors underneath these items and returned to their original and proper position after cleaning. The contractor shall not place unsanitary items such as chairs, mats/rugs, and trash receptacles on food preparation/eating surfaces. The contractor shall not place paper products or dispensable items on the floor prior to installation or replacement in proper dispensers. Cleaning of the

Non-carpeted floors shall be conducted daily.

5.1.2 Carpeted floors. Carpets shall be maintained free of soil, dirt, debris, litter and other foreign matter by effective routine vacuuming. Contractor shall utilize High Efficiency Particulate Air (HEPA) vacuum cleaners that offer high airflow, high efficiency filtration, and an adjustable rotating brush agitation for more effective soil removal. Any spots and spills shall be removed by the carpet manufacturer’s approved methods or other commercially accepted practices as soon as noticed. All tears, burns, and raveling shall be brought to the attention of the facility manager. Area and throw rugs shall also receive this service.

Chairs, trash receptacles, and other easily moveable items shall be moved to maintain floors underneath these items and returned to their original and proper position after cleaning. Cleaning of Carpeted floors shall be conducted daily.

5.1.3 Stairways/Elevators/Handicap Lifts/Entryways. All floor surfaces shall be cleaned in accordance with paragraph 5.5, as appropriate. Stair guards, handrails, wall caps an baseboards shall be free from grease and grime. The contractor shall remove all marks, dirt, smudges, scuffs, and other foreign matter from adjoining stairwell walls, up to 72 inches height from stair tread level, to provide or maintain a clean, uniform appearance. Contractor shall clean dry all erase boards as required. All cleaning required at this paragraph shall be conducted daily.

5.1.4 Clean Floor (Walk-Off) Mats, Area and Throw Rugs. Vacuum and clean interior and exterior floor mats, area and throw rugs. Mats, area and throw rugs shall be free of all visible lint, litter, debris, soil and other foreign matter. Soil and moisture underneath mats shall be removed and mats returned to their normal location. All cleaning required at this paragraph shall be conducted daily.

5.1.5 Trash Collection/Removal. The contractor shall collect and dispose of all trash from these locations.

The contractor is not responsible for purchasing or replacement of plastic trash bags. Trash collection/removal shall be conducted daily.

5.1.6 The contractor shall remove recyclable trash (i.e., paper) to include shredded from centralized locations at the same frequency as regular trash. The contractor shall keep all recyclable trash separate from regular trash during collection/removal and place the recyclables in the appropriate outside dumpster/compactor. These tasks shall be conducted daily.

5.2 Restrooms and Kitchen Areas Cleaning Services

5.2.1 Clean and Disinfect. All surfaces of sinks, toilets, urinals, lavatories, dispensers, plumbing fixtures, saunas, doors, partitions, stalls, stall doors, entry doors (including handle, kick plates, ventilation grates, and metal guards), walls and other such surfaces shall be cleaned and disinfected using a germicidal detergent. Restrooms shall have a clean scent or no odor at all. Toilet and urinals will be free of spots, water spots, scale buildup, soap scum, odors, and any other deposits. Mirrors shall be clean and have no streaks or other removable matter. Partitions shall be smudge, stain free. Vents (within 72 inches of the floor) shall be clean. Restroom sinks, countertops, and fixtures shall be free from water and scale deposits, soil, streaks, and other removable matter. Walls and grout shall be free of all film, spots, and detergent buildup. All cleaning required at this paragraph shall be conducted daily.

5.2.2 Floor Care. All floor surfaces shall be cleaned daily and moveable items shall be tilted or moved to sweep and damp mop underneath. Grout on the wall and floor tiles shall be free of dirt, scum, mildew, and residue. All cleaning required at this paragraph shall be conducted daily.

5.2.3 Sweep and Mop Floor. Floors shall be swept and mopped with a detergent, water, and bleach solution to sanitize and disinfect. “WET FLOOR” signs shall be posted in all areas having received service whenever building occupants are present in order to avoid mishaps. Acceptable service includes removal of floor maintenance solutions and all mop strands, splash marks, or mop streaks from furniture, fixtures, walls, and baseboards. All cleaning required at this paragraph shall be conducted daily.

5.3 Stock Restroom Supplies

Each day the Contractor shall ensure restrooms are stocked so any provided supplies including toilet tissue, paper towels, and hand soap do not run out. Additional supplies shall be stored in designated areas.

5.4 Drinking Fountains

Contractor shall clean all facility drinking fountains, typically located near or in the vicinity of restrooms.

All porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountains shall be cleaned and disinfected. Drinking fountains shall be free of streaks, stains, spots, smudges, scale, and other obvious soil. Tasks shall be conducted.

5.5 Clean Walls/Partitions/Door Knobs

Clean door knobs with sanitizing solution once daily.

5.6 Restroom Trash Removal

All trash containers in restrooms and locker rooms shall be emptied, trash disposed, and containers returned to their original locations. The contractor shall replace any obviously soiled, worn or torn plastic trash can liners. Trash receptacles shall be left clean, free of foreign matter and free of odors. Tasks shall be conducted daily.

5.7 Periodic Cleaning Services (Services performed as specified in Service Summary).

5.7.1 Periodic Hard Floor Maintenance. Hard floors shall be cleaned, scrubbed, sealed, polished, waxed, and stripped as required for the appropriate surface to maintain, protect and ease normal routine floor cleanings. After periodic maintenance, floor surfaces to include grout shall have a uniform, clean appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film, or standing water. At a minimum this service shall be performed three (3) times a year.

5.7.2 Floor Maintenance. All items (i.e., chairs, trash receptacles, mats/rugs, and tables shall be moved then returned to their original position once service has been completed.

After floors have been thoroughly serviced, the contractor shall apply an even coating of non- skid floor finish. Surface shall have a uniform, glossy appearance free from hair, dirt, debris, dust, scuff/heel marks, floor finish build-up, stains, discoloration, and all other foreign matter once service has been completed.

All floor maintenance solutions shall be removed from baseboards, furniture, and trash receptacles. The contractor shall not place unsanitary items such as chairs, mats/rugs, and trash cans on food preparation/eating surfaces. Mop water shall be disposed of only in areas designated by the Activity

Director. Floor maintenance equipment, cleaners, solutions, or wax (opened/unopened) cannot be stored or left behind in work areas. At a minimum this service shall be performed three (3) times a year.

5.7.3 Periodic Carpet Restorative Cleaning. Carpets shall be deep cleaned in order to remove embedded soil from carpet fibers. Deep cleaning shall employ an effective technique to address soil suspension, soil extraction and drying. After deep cleaning, the carpeted area shall show an improvement in visible appearance. All cleaning solutions shall be removed from baseboards, furniture, trash receptacles, chairs, and other similar items. Chairs, trash receptacles, and other items shall be moved to clean carpets underneath and returned to their original location after the carpet has dried. At a minimum this service shall be performed three (3) times a year.

5.7.4 Dusting/High Dusting/Spot Cleaning. Low dusting/spot cleaning shall be accomplished to a height of

6 feet and must be free of dust, lint, cobwebs, and litter. All surfaces to include washable surfaces of white boards, walls, partitions, vents, grills, stairwell walls, stair guards, baseboards, handrails, doors/door guards/handles/push bars/kick plates, temperature controls, fixtures, and light switches are free of dust, lint, dirt, litter, smudges, fingerprints, marks, streaks, film/residue, spots, scuff/heel marks, soil, dirty buildup (grime), or other foreign matter. Once service has been completed, surfaces shall have a clean, uniform appearance. Metal surfaces of doors (kick plates, handles, push bars) shall be cleaned, polished, and have a uniform appearance, free of streaks and film/residue. High dusting includes surfaces from 6 to

10 feet from the floor. Surfaces include tables, shelves, bookcases, window sills, hand rails, pictures, clocks, window blinds, and ceiling fans. Dusting the window sills in the library, student lounge, staff and faculty lounge on a weekly basis.

5.7.5 Exterior Windows. Exterior windows shall be cleaned at least three (3) times per year.

5.7.6 Microwaves: Clean microwaves (inside and out) at least 3 times per week.

5.7.7 Miscellaneous: All doors, partitions, walls, display cases, and directory boards shall show no traces of film, dirt, smudges, water, or other foreign matter.

5.8 Special Cleaning Requirements

5.8.1 EMERGENCY OR SPECIAL EVENT CLEANING SERVICES (Minor, Medium, and Major). Upon notification, the contractor shall perform emergency or special event cleaning required in any area or room covered under this contract. The contracting officer shall request cleaning services (to include carpet shampooing) for the appropriate and required work task(s). Contractor shall begin emergency work, as determined by the contracting officer, within one hour of notification, which may be verbal. The

Contracting Officer or designated representative will notify the contractor as soon as a special event requirement is known, but not less than 24 hours prior to the event. Completion schedule shall be determined for each emergency or special event cleaning service performed.

5.8.2 Minor. Cleaning service performed with a minimum of 3 square feet and a maximum of 500 square feet per service.

5.8.3 Medium. Cleaning service performed with a minimum of 500 square feet and a maximum of 2000 square feet per service.

5.8.4 Major. Cleaning service performed with a minimum of 2000 square feet and a maximum of 6000 square feet per service.

5.9 On Call: The contractor shall be available for on call requirements upon emergencies such as toilet flooding, etc., and shall have a person(s) on site within two hours from DISCS placing the call.

5.10 Management/Performance Approach Plan

The contractor shall develop and maintain Management/Performance Approach Plan with consideration of parameters outlined in paragraph 1.3 that indicates the days of the week, task, and frequency of service to be performed. The contractor shall coordinate with the COR to agree upon a cleaning time that is convenient both to the contractor and the building occupants. Contractor shall provide written notification to the KO/COR informing him/her of the day cleaning will be accomplished within the building. Proposed schedule changes will be coordinated with the COR. The contractor shall comply with the submitted schedule as approved by the building manager. Revised service schedules shall be submitted two weeks prior to the end of the month and the new schedule shall begin no sooner than the beginning of the month following submittal.

5.11 Workload Estimate

The Government's estimate of square footage represents the Government's best estimate of the actual square footage of designated areas. The contractor shall notify the COR in writing when actual conditions or actual measurements differ from those provided by the government. If the contractor does not inform the COR of differing conditions, the Government shall not be responsible for any additional cost that may be incurred by the contractor. The contractor shall also notify the COR if a facility undergoes renovations, which prevents the contractor from performing cleaning duties.

5.12 Solid Waste Collection and Disposal

The Contractor shall use existing bulk containers to dispose of trash or refuse generated from accomplishment of services detailed in this PWS. The contractor shall adhere to all base level recycling programs.

5.13 Cleaning and Restroom Supplies

a. The contractor shall furnish a listing of all cleaning and restroom supplies the contractor will utilized during the contract period, to the government representative not later than the pre-performance conference for approval by the COR. After approval, any new cleaning supplies added to the contract will need approval before being used on the installation.

b. The cleaning inventory supply list shall contain the following information:

c. The contractor shall be responsible for the proper disposal of all their cleaning supplies. A material safety data sheet (MSDS) shall also be provided for each cleaning and restroom supply item and employees will be trained, by the contractor, on the proper use and disposal of each item. All cleaning supplies shall be non-flammable.

d. NOTE: Products containing chlorofluorocarbons (CFC’s) or ozone depleting substances shall not be allowed or approved for use.

5.14 Hazardous Material Identification. Material Safety Data Sheets (MSDS) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. MSDSs must be submitted by the Contractor upon contract award.

Failure to provide MSDSs or certificate when requested could result in the Contractor being considered non-compliant and result in termination of the contract.

5.14.1 Cleaning Products. Custodial cleaning products required in the performance of this PWS shall meet as a minimum, Green Seal Product Standards (http://www.greenseal.org). If it is determined that a product does not meet government performance requirements, the contractor shall submit a proposed alternative that would meet the performance requirements with the lowest environmental impact for evaluation and acceptance. Products that fall under the Environmental Protection Agency (EPA) Comprehensive

Procurement Guidelines (CPG) (https://www.epa.gov/) shall meet the minimum recovered (recycled) content. Bio-based products shall be used upon issuance of the bio-based product listing from the United

States Department of Agriculture (USDA) (http://www.usda.gov). The contractor shall purchase and use

Energy Star or other energy-efficient items listed on the Department of Energy’s Federal Energy

Management Program (FEMP) Product Energy Efficiency Recommendations product list. Supplements or http://www.epa.gov/cpg) amendments to listed publications from any organizational level may be issued during the life of the contract.

5.14.2 HAZMART. The government HAZMART personnel will inventory all chemicals that the

Contractor brings on to Wright-Patterson AFB, or any property under the control of Wright- Patterson

AFB. Any products that meet the criteria of “Hazardous Waste” must be bar coded and tracked until permanently removed from government property. The inventory will be performed prior to commencement of work. Criteria for identifying hazardous waste, is contained in Subpart C of 40 CFR, Part 261.

5.14.3 Hazardous Material/ Waste Management. The Contractor will be briefed on Wright- Patterson AFB

Hazardous Material/Waste Management Plan at the pre-performance conference.

5.14.4 Hazardous Material Handling. The Contractor shall have approval from the base Bio-

Environmental (BEE) and HAZMART sections prior to purchasing, handling, using, and storing any chemicals, solvents, lubricants, and other products that require MSDSs. The Contractor shall identify these materials and products on Air Force Form 3952 Chemical/Hazardous Material Authorization Request, Process Identification Form, and Shop Disposal Form. The Contractor shall provide one copy of the

MSDSs for each item to BEE and HAZMART sections for review prior to any chemicals being brought onto Wright-Patterson AFB. The Contractor shall maintain one copy of the MSDSs for each hazardous material line item used within the work center.

The Contractor shall not use products that are or contain Toxic 17 chemicals, Extremely Hazardous

Substances (EHS), Ozone Depleting Substances (ODS), and persistent Bioaccumulative and Toxic (PBT) chemicals. Any HM containing one of these banned substances will not be allowed on base.

5.14.5 HAZMART Training. The Contractor shall ensure all employees complete the local Air Force

Hazardous Communication, HAZMART, and hazardous waste training. The Contractor shall appoint a primary and alternate HAZMART and Hazardous Waste monitor. Monitors are responsible for training all

Contractor personnel regarding hazardous material containers maintained within Wright-Patterson AFB and complete MSDSs immediately upon receipt of new chemicals, products, or materials. The Contractor shall submit MSDSs to HAZMART as required.

5.14.6 Spill Response. The Contractor will be briefed on Wright-Patterson AFB, spill response procedures at the pre-performance conference. The Contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations, in the event that it is necessary to utilize government material, equipment or personnel to clean up a Contractor caused spill, the Contractor shall be required to reimburse the Government for all associated costs.

5.14.7 The contractor shall notify the base civil engineers at (937) 257-3131, the CO and the COR within

5.15 Miscellaneous Requirements

Notify COR when unauthorized or suspicious persons are seen on the premises

Employees entering any facility or area after it has been secured shall enter through one door and lock the door behind them. These facilities or areas shall not be left unsecured at any time, including the period when work is being performed. Upon completing work in a facility or area, Contractor employees shall ensure all doors and windows that were secured upon their arrival are closed and secured upon their departure.

The Contractor shall display the appropriate caution signs when cleaning floors or any other surface that might pose a risk to injury for any personnel that are present.

6.1 Quality Control Plan (QCP)

The contractor shall develop and maintain a quality control program to ensure custodial services are performed in accordance with commonly accepted commercial practices and services identified in this

PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur. As a minimum, the contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Service Summary.

6.1.1 Quality Control Plan. The Quality Control Plan shall demonstrate how the contractor intends to ensure quality performance during the contract period of performance. The contractor shall maintain the

Quality Control Plan throughout the life of the contract. The contractor shall provide any updates to the plan to the CO for acceptance with a copy to the COR when changes occur. The Quality Control Plan shall include the following:

6.1.2 Inspection System. Written procedures for the inspection system includes the title/name of the individual responsible for the Quality Control Plan and who shall perform the inspections. The location of inspection records and key control logs shall be made available to the Government at all times during the term of the contract.

6.1.3 Deficiency Prevention. An inspection form describing the method for identifying and preventing defects in the quality of service performed.

6.1.4 Inspection Record. The inspection record provides the following information:

a) Date, time, and location (building number) of the inspection.

b) Title and signature block for the person who performed the inspection.

c) Ratings of acceptable or unacceptable for all work performance inspected.

d) Areas (room number, section) found to be deficient and corrective action taken.

e) Total number of observations and defects.

f) Type of inspection (100%, random, periodic

g) Frequency (weekly, monthly, daily) of inspection.
