About this file
The document is a Past and Present Performance Questionnaire for a federal contract opportunity (Solicitation Number H9224025RE002) issued by the Naval Special Warfare Command. The four-page form is designed to collect detailed performance feedback from previous customers about a contractor's performance, with a closing date of 09 July 2025 at 1200 Eastern Time. The questionnaire requires a comprehensive evaluation across multiple performance dimensions including quality, schedule, cost control, management, and regulatory compliance, with rating options ranging from Unsatisfactory to Exceptional.
The form solicits specific information about a previous contract, including contract identification details (type, number, cost), customer/agency information, geographic service locations, and a narrative summary of the contractor's performance. Section 4 provides a detailed matrix for rating the contractor's performance across seven key areas of management and one area of regulatory compliance. The final section allows the evaluator to provide additional comments, indicate any reservations about future work with the contractor, and disclose any contract terminations. The completed questionnaire must be submitted directly to specified government email addresses and is considered source selection sensitive information.
