About this file
This is an addendum document related to a mine safety and egress re-bid project (GovTribe ID: 2015-109M14) issued by the State of Colorado, involving the Lebanon Mine and Everett Tunnel. The project focuses on opening the Everett Tunnel to its full length of approximately 768 feet from its portal to establish pedestrian access to the Lebanon Mine. The work includes supporting ground above and around a cave-in located approximately 205 feet from the tunnel portal, excavating rock from the cave-in area, surveying and mapping tunnel geology, and installing lighting to the tunnel's end. An Add/Alternate bid option is available for construction of the proposed pedestrian cross passageway. A mandatory walk-through was held on December 10, 2015, and only participating bidders are eligible to submit proposals. Construction is tentatively scheduled to begin in March 2016 and conclude by the end of April 2016. Bid opening is set for February 16, 2016 at 2 P.M. at the History Colorado Center. Proposals must include a Bid Bond, with Performance Bond and Labor & Material Bond required upon contract award.
The contractor's mobilization costs for both the base bid (Everett Tunnel) and the Add/Alternate bid (Lebanon/Cross Passageway Tunnel) shall not exceed 7.5% of the total bid price for each component, with separate mobilization costs detailed on the new bid sheet. The project was re-bid due to initial bids exceeding available funds. If the Everett Tunnel phase is successfully completed, Add/Alternate work will proceed if remaining funds are adequate; any additional rock slides encountered during construction will be addressed using funds earmarked for add alternates. Bidders must reference the revised drawings on sheet TN-2 and consult the attached project schedule sheet (SCH 1) and general layout sheet (CDOT 1) for additional details.

Project Description 2015-109M14 Lebanon Mine Safety and Egress
Re-Bid Addendum
2.5.2016
The bids received exceeded the available funds. The project is being put out to bid a second time with a focus on opening the
Everett Tunnel to the site where the pedestrian cross passageway to the Lebanon Mine is specified on the stamped drawings, along with proposals for an Add/Alternate bid for the construction of the proposed pedestrian cross passageway. The goal is to open the Everett to its full length of approximately 768 feet from its portal. The Everett Tunnel is currently blocked by a cave-in located approximately 205 feet from the tunnel portal. The first phase of construction is to support the ground above and around the cave-in, excavate the rock from the cave-in area, survey and map the geology of the Everett Tunnel, and complete lighting run to the end of the tunnel. If other rock slides are encountered in the process of this portion of work, we will address with funds earmarked for add alternates. If the Everett portion of the project is successfully completed, we will address the add alternates if remaining funds are adequate.
We have already done the surveying and geologic mapping in the Lebanon Tunnel. Tentatively, construction is planned to begin in March 2016 and be completed at the end of April 2016. See attached project schedule sheet SCH 1 for more details.
The general layout of the two mines, pedestrian cross passageway and the location of I-70 as it relates to these structures are shown on sheet CDOT 1.
Only bidders who participated in the mandatory walk-through on Thursday, December 10th 2015 are allowed to bid on the project. Please pay close attention to the revisions on TN-2 in the attached stamped drawings. Proposals must include Bid
Bond. When awarded contract, Performance Bond and Labor & Material Bond are required. Bid Opening is2.16.2016 at 2
P.M. at the History Colorado Center.
The contractor’s dollar amount for base bid/Everett Tunnel (item #1 Mobilization), and Add/Alternate/Cross Passageway
Tunnel (item #16 Mobilization), shall not exceed 7.5% of the total bid price for each. Use new bid sheet to detail separate mobilization costs for Base Bid (Everett Tunnel) and Add/Alternate (Lebanon/Cross Passage Way Tunnel).
