About this file
This is an insurance requirements document issued by the City of Meriden, Connecticut, for a contractor providing flooring replacement services at Edison Middle School. The City of Meriden is seeking a contractor to remove and replace approximately 4,088 square feet of VCT flooring in the SOAR hallway, gymnasium, teacher offices, and connected side halls at Edison Middle School. The project scope includes ripping out and properly disposing of existing flooring, floor preparation, installation of a moisture mitigation epoxy system (Mapie Planiseal, Sika MB, Ardex MC Rapid, or equivalent), and installation of new VCT flooring to match existing patterns. The contractor is also responsible for providing and maintaining dumpsters for material disposal throughout the project duration. Work may be completed during summer hours from 6:00 am to 2:30 pm daily, with furniture moving and VCT waxing to be performed by others.
Contractors must maintain specific minimum insurance coverages throughout the contract term, with the City of Meriden named as an Additional Insured on a primary and non-contributory basis on all policies except Workers' Compensation. Required coverages include General Liability of $1,000,000 to $2,000,000, Auto Liability of $1,000,000, and Employers' Liability coverage with minimum limits of $1,000,000 per accident and per disease. All insurance policies must be written with carriers approved in Connecticut and carrying a minimum AM Best's rating of "A-"VIII. Original Certificates of Insurance must be provided prior to contract issuance, with replacement certificates due at least 30 days prior to policy expiration. Claims-made policies require continuous renewal for a minimum of two years following project completion, and any policy cancellation or modification requires 30 days' written notice to the City of Meriden.
