About this file

This is a Proposed Program Plan document for the City of Santa Monica's FY 2027-28 Human Services Grants Program, a competitive funding opportunity open to eligible nonprofit organizations. Applicants must submit comprehensive proposals not exceeding ten pages that address community needs and demonstrate how their programs contribute to the wellbeing and safety of Santa Monica residents. The program plan template requires detailed responses across nine major sections: community need assessment, program participant eligibility and intake processes, program description with evidence-based practices, accessibility and outreach strategies, partnerships and referrals, program evaluation metrics, operational capacity and staffing, past performance history, and policies regarding racial equity and employee retention. Grant agreements will be executed for a four-year term (FY 2027-31) with annual funding contingent upon available funds, satisfactory outcomes, effective program performance, and City Council approval.

Applicants must demonstrate robust experience administering human services programs and develop realistic implementation timelines for program launch, including site acquisition, staff hiring, and training. All proposals require definition of a minimum of two and maximum of five performance indicators (outputs and outcomes) to be documented in the Program Budget, Demographics, and Performance Workbook. The RFP is open to qualifying agencies with or without prior City funding history, provided they present viable plans for delivering services locally. Program design must prioritize trauma-informed, person-centered, culturally appropriate approaches and establish low-barrier access for economically, ethnically, and socially diverse populations. Applicants must identify Santa Monica Program Participants (SMPP) eligibility criteria, define low-income household classifications, and describe strategies for ensuring fee-based services remain accessible regardless of participant ability to pay.
