About this file

This file is a Program Budget, Demographics, and Performance Workbook for the City of Santa Monica's Human Services Grants Program (HSGP) for fiscal year 2027-28. The workbook serves as a comprehensive application tool for nonprofit organizations seeking funding through the four-year HSGP (2027-2031), which was approved by City Council on August 11, 2026. Applicants must select either the Cash Match or Living Wage budget option and complete the corresponding workbook tabs, including sections for agency funding sources, participant demographics, leadership demographics, and performance indicators. Organizations applying for multiple programs must submit separate proposals for each program with corresponding workbooks. The program term runs from July 1, 2027, through June 30, 2028, with mid-year reporting required from July 1, 2027, through December 31, 2027, and year-end reporting covering the full fiscal period through June 30, 2028.

The workbook establishes specific budget and compliance requirements dependent on the selected funding model. Under the Cash Match option, organizations must provide a minimum 30% cash match and may pay staff at rates set by the agency, with budgets categorized by staff salaries, fringe benefits, consultant services, operating expenses, direct client support, and indirect administrative costs. The Living Wage option eliminates cash match requirements but mandates that all staff at 90% or more FTE to the program meet the City's living wage rate of $25.00 per hour (adjusted annually, with a one-year implementation delay). Indirect administrative costs are accepted at 15% or less of the total Santa Monica grant without documentation, or above 15% only with federally-negotiated indirect cost rate documentation. Performance indicators require a minimum of two and maximum of five indicators, each containing one output and one outcome with documented measurement methods. All reports submitted to the City are considered public record subject to the Public Records Act, and applicants must certify the accuracy and compliance of all information and disbursements.

BUDGET - LIVING WAGE

City of Santa Monica HSGP
Program Budget - Living Wage

COMPLETE ONE BUDGET TAB FOR YOUR PROGRAM. IF YOU COMPLETE THIS TAB, LEAVE THE CASH MATCH BUDGET BLANK.

INSTRUCTIONS
Complete this budget template if your program is choosing to meet the City's living wage for all staff that are 90% or more FTE to the program. Your program is not required to provide cash match.

Provide line-item detail as specified in each section of the template and verify all calculations. Budgets will not be accepted if any of the required information is incomplete or if the calculations are incorrect.

Cells containing formulas are set to calculate automatically. Report any issues to humanservices@santamonica.gov

Only input data into the highlighted cells.
SECTION I: BUDGET SUMMARY
Enter the name of your Agency and Program.
SECTION II: LINE ITEM DETAIL
Complete columns A-H with program budget details. If you propose a program that is fully funded by the HSGP, the Total Program Budget (column G) and SM Grant Budget (column H) will be the same.

1A. Staff Salaries: All staff that are 90% or more FTE to the program (column E) must meet the City's current living wage rate of $25.00 per hour and complete the "Living Wage Certification" attachment.

Note: The City adjusts living wage annually. HSGP grantees get a one-year delay in wage compliance (i.e., budgets for Fiscal Year 2027-28 must comply with the City's living wage rate for Fiscal Year 2026-27).

SECTION III: NON-CITY PROGRAM FUNDING (OPTIONAL)
If your program receives any non-City funding, provide a brief description of the funding sources included in your Total Program Budget. For government grants, indicate the type of funding source (Federal, State, or County/Local). Enter the Projected/Actual amounts in Column H as appropriate. Leave this section blank if you do not anticipate any non-City funding.

Performance Indicators
INSTRUCTIONS
Programs are required to have a minimum of two (2) and a maximum of (5) performance indicators. A performance indicator is a set of one (1) output and one (1) outcome.

Complete the table below with the following information for each performance indicator:

1. List the outputs and annual targets the program aims to achieve. Outputs describe the result of an activity and can include number of people served or number of services performed. Outputs should be a number.

2. List the outcomes and annual targets that measure program impact. Outcomes assess measurable impacts to participants’ lives or improvements that result from the program. Strong outcomes are based on measurable data sets, not anecdotal or casual self-reports of behavior change. Participant satisfaction surveys may be an acceptable form of measurement if they are conducted in an objective, uniform manner. Outcomes should be a percentage.

3. List how you will document the performance indicators. Define the tools used to record and measure the performance indicators. Examples of strong documentation methods include pre- and post-tests, clinical assessments, and databases. Programs should utilize objective, standardized measurement tools.

Funded programs will work with City staff to refine and finalize their indicators.
