About this file
This is a structural inspection report prepared by Fortress Engineering Group, LLC for the Clay County Board of County Commissioners regarding foundation rehabilitation services at 2651 Sapp Lane, Middleburg, Florida. The project, funded through the Community Development Block Grant (CDBG) Entitlement Program, involves complete removal and replacement of deteriorated structural floor framing components and restoration of the building's substructure to safe and code-compliant conditions. The Request for Proposal (RFP) process requires qualified Florida-licensed contractors with a minimum of five years of foundation rehabilitation experience to submit proposals by April 6, 2026, at 4:00 PM through the County's e-Procurement Portal. A mandatory pre-bid meeting is scheduled for March 19, 2026, at 1:00 PM at the project site, with the final inquiry deadline of March 25, 2026, at 4:00 PM. The Board is expected to make a recommendation by April 28, 2026. Substantial completion is required within 120 days of Notice to Proceed, with final completion no less than 30 days after punch list delivery, and liquidated damages assessed at $100.00 per day for delays beyond the final completion date. Contractors must provide a two-year warranty on all workmanship and materials.
Proposal evaluation will be based on five criteria: Executive Summary (10 points), Qualifications of Project Personnel (25 points), Project Approach (30 points), Pricing Proposal (25 points), and Similar Projects and References (10 points). The lump-sum pricing must cover all labor, materials, equipment, supervision, permits, inspections, overhead, and profit, with no additional compensation available for unforeseen site conditions. Performance and Payment Bonds at 100 percent are required, indicating the project value exceeds $200,000. Required insurance includes Commercial General Liability coverage of $1,000,000 per occurrence and $2,000,000 aggregate, Workers' Compensation at statutory limits, and Builder's Risk coverage at 100 percent of project value. Section 3 preference provisions apply to this federally funded project, allowing qualified Section 3 businesses to match the lowest non-Section 3 bid within 10 percent. Contractors must comply with Davis-Bacon wage requirements, OSHA regulations, and the Housing and Urban Development Act of 1968, and submit required forms including Electronic Funds Transfer Authorization, Form W-9, and a Contractor Warranty Letter prior to contract award.
