About this file

This document is a Massachusetts Substitute Form W-9 Request for Taxpayer Identification Number and Certification. It is used by the Commonwealth of Massachusetts for vendors and contractors to provide their taxpayer identification number and certify compliance with tax and ethical requirements. The form collects the business name, federal tax classification, legal and remittance addresses, and social security number or employer identification number of the contractor. It also requires the contractor to certify they are not subject to backup withholding and are a U.S. citizen or other U.S. person. Additionally, the form asks the contractor to indicate if they are an active Commonwealth of Massachusetts state employee and to certify compliance with state ethics requirements.

The form does not provide any details about a specific state or local contract opportunity. It is a standard tax and compliance form required by the Commonwealth of Massachusetts for business relationships and transactions.
