Summary of Form 3 - Foreign Countries of Concern Attestation
Form 3 (PUR 1355) is a mandatory compliance attestation required by the Florida Division of Emergency Management for all entities submitting bids, proposals, or replies for the ITB-DEM-25-26-020 WebEOC contract opportunity. The form must be completed and signed under penalties of perjury by an authorized officer or representative of any bidder seeking access to Personal Identifying Information through the contract. The attestation certifies that the bidding entity is not owned by a foreign government of concern, is not organized under the laws of a foreign country of concern, does not have its principal place of business in a foreign country of concern, and that no foreign government of concern maintains a controlling interest in the entity.
This attestation form references Rule 60A-1.020, F.A.C., which provides the regulatory framework defining "Foreign Countries of Concern" and establishes the requirement for entities pursuing contracts involving access to sensitive personal information. Bidders must provide printed name, title, signature, and date fields for both the primary signatory and a secondary authorized representative. Failure to complete and submit this form as part of the bid package will result in the submission being considered non-responsive to the solicitation requirements established by the Florida Division of Emergency Management.
