II. DEFINITIONS
A. The Information Technology Infrastructure Library (ITIL) and ISO/IEC 20000-1:2018 3)
Terms and Definitions were referenced in providing the below definitions related to change management and change control.
B. CAB: Change Advisory Board; ISA Chiefs and delegates that approve or make recommendations to the Change Manager in the Assessment, prioritization, and scheduling of changes. This board includes non-voting representatives from IT, the Business and Third-Party Vendors/Suppliers.
C. CMB: Configuration Management Board – The purpose of this board is to provide accurate information used to enter and audit Configuration Items (CIs); and for the addition of attributes / components associated with the record as well as relationship data/details (upstream and downstream)
D. ECAB: Emergency Change Advisory Board; Voting CAB members who make decisions about expedited high impact Emergency Changes through defined approval processes.
E. Change: The addition, modification or removal of approved and supported hardware and software to support network, application, environment services or associated documentation.
F. Change Management: The processes for managing changes to the day-to-day operations of the business, responsible for controlling the lifecycle of all changes. Ensures each change is assessed for risk, impact, and communications.
G. Change Type and Approval Levels:
1. Emergency – Changes that need to bypass the normal change flow based on criteria defined in Emergency Changes listed below.
2. Normal – Planned changes scheduled for implementation following the next scheduled CAB meeting, typically to occur within the next five to seven days or later.
a) These changes must go through the normal change approval process before implementation is approved as scheduled.
b) Scheduling of changes must allow for adequate risk and impact mitigation, as well as adequate notice to impacted customers.
3. Standard – These are prior approved changes that pose little to no risk and little to no impact on customers using/receiving those identified services. Standard Changes are relatively common with the following attributes:
a) They follow a defined/documented procedure or workflow and a defined process for communicating to those involved with the change including:
(1) Implementation Plan
(2) Back Out Plan
(3) Communication Plan
(4) Production Verification Plan
b) Approval handled by the Change Manager after validation as an approved standard change, further approval is not required by CAB.
c) Standard Changes must be approved by the CAB before placing on the Approved Standard Change list as found in SharePoint.
4. Emergency changes: Changes that need to be evaluated, assessed and either rejected or approved in an expeditious manner by ECAB (Emergency Change Advisory Board.
a) To resolve a major incident or service outage impacting business functionality.
b) Critical Security patching outside of planned patch updates, also known as Out of
Band Patching
c) Urgent changes needed to facilitate Budgetary or Legislative constraints.
5. Submission of an Emergency Change does not guarantee the change will be implemented. The emergency change workflow will assess the change for adequate risk, impact, and communication plans. Accountability for risk mitigation resides with the CAB and not Change Management.
H. IT Environment: All environments used for meeting the performance needs of our customers as outlined in the Service Level Agreements. These mirror the environments as applied in application lifecycle management. Environments are listed and defined below:
1. Development: The development environment (dev) is the environment in which changes to software are developed
2. Test: The purpose of the test environment is to allow human testers to exercise new and changed code via either automated checks or non-automated techniques
3. Stage: A stage, staging or pre-production environment is an environment for testing that exactly resembles a production environment. It seeks to mirror an actual production environment as closely as possible and may connect to other production services and data, such as databases.
4. Production: The production environment is also known as live, particularly for servers, as it is the environment that users directly interact with.
I. Maintenance Windows: ISA has a defined standard maintenance window to ensure defined levels of service are being met. All planned outages will be communicated to impacted customers. The two established maintenance windows are listed below:
1. FLHSMV: Weekly on Sunday mornings from 06:00 AM ET until 12:00 PM, excluding patching management.
2. FHP: Weekly on Monday and/or Tuesday mornings from midnight, 12:00 AM to
05:00 AM ET
J. Patching Management/Patching Schedules: The following patching schedules are based on the industry established Patch Tuesday or Update Tuesday, typically the 2nd Tuesday of each month as Wk-0. These are followed by the following weeks Wk-1, used for systems that host development environments, Wk-2, used to apply patching to systems that typically host Test or Stage environment systems and Wk-3 used to schedule patching for systems that host production environment systems.
K. Project Manager: The person responsible for managing a software development or infrastructure project from start to finish (including the submission of changes during the project warranty period).
L. Assignee: The personnel responsible for the work performed toward implementation of the change.
M. Change Requestor: Definition, (responsibility, may fulfill the closure tab – contains the information regarding the implementation end state, success, or failure)
N. Back Out Plan (Roll Back): The plan to remove a change and revert the changed system to pre-implementation condition and functionality.
O. Implementation Test Plan: How will the changed functionality be released, tested, verified and by whom, to ensure the agreed upon change was delivered as agreed.
P. Communication Plan: A plan outlining the method and information needed to be sent to customers or stakeholders of a change.
Q. TRB: Technical Advisory Board – This board service in the capacity of a risk and impact assessment workgroup providing expertise from all work sections within ISA. The group also provides the initial approval for all Normal changes in FLHSMV’s Change
Management.
R. Warranty Period: The warranty period is the time allocated for stabilization of the implemented solution, prior to change control closure. Following implementation. no further changes may be made out of the scope of change management.
III. POLICY

H. All other requests for change to CIs defined in the Change Management policy will be managed using the change management process.
I. A change calendar containing details of the approved changes and their proposed deployment dates will be established and communicated to interested parties. The Change Calendar will be used as the basis for planning the deployment of releases.

P. The change requester and or implementer will report the results of the implementation within three days of the scheduled implantation end date. Any change which passes the implementation end date by three days, 72 hours, without the reported implementation results will be indicated Results Unknown and promoted through the workflow to CMDB review.
