Proposals are due by February 22, 2024 for the FAA's requirement to acquire air traffic control services at approximately 264 airports through ten geographic areas. Two of the areas are set aside for small businesses, with one also designated for small disadvantaged businesses. Questions about the solicitation should be directed to the contracting officer by January 22, 2024.

PAST PERFORMANCE REFERENCES AND SURVEYS
Part A: Past Performance Summary
Offerors must complete a Past Performance Summary for up to three (and no more than three) Past Performance References in accordance with Section L.8.5.1 using the form provided on Page 2 of this questionnaire.
Submit Past Performance Summaries within Volume V of your proposal by the date and time identified in Section L.5.
The Offeror must complete Part A, which must not exceed three pages in length.

Part B: Past Performance Survey
1. Instructions to Offerors:
The Offeror must provide the Past Performance Survey (Part B) to appropriate customer Points of Contact (POCs) for three (3) Past Performance References identified in Part A, Past Performance Summary, above. For Government references, the Past Performance Survey must be provided to two (2) references for each of its past performance examples. The references should include individuals with firsthand knowledge of the Offeror’s performance such as a Contracting Officer (CO), Contracting Officer’s Representative (COR), or Program Manager (PM).
For past performance under contracts not performed for Federal Government entities, such as commercial contracts, the Past Performance Survey must be provided to individuals with duties similar to those of the Government CO and COR.
To aid the person completing the survey, the Offeror (i.e., Contractor) must complete Block 1 of the Past Performance Survey to identify its name and the specific contract being referenced.
The Offeror is responsible for following up to ensure that references submit the Past Performance Survey.
The due date for the completed Past Performance Surveys is the same as the due date for all proposal volumes identified in Section L.5. The Past Performance Survey must be submitted directly by the person completing the survey via email to the Federal Aviation Administration (FAA) CO, identified below in Section 2 of this document.
2. Instructions to the Person Completing the Past Performance Survey:
In order to assess Offerors responding to the FAA’s solicitation for the FAA Contract program, the FAA is seeking past performance information with respect to each Offeror. One of the Offerors has selected you as a Past Performance Reference.
The name of the Offeror, contract title, and contract number are identified on the next page under Block
1. Please correct any information in Block 1 that is incorrect. In the Past Performance Survey, the Offeror is referred to as “the Contractor” relative to its support of your organization.
Please complete Block 2 and provide a rating for areas 1-5 in the Past Performance Survey. Narrative explanations are only requested when a rating of “Marginal” or “Unsatisfactory” is indicated. If a question is not applicable to your contract, please indicate “N” for not applicable in the box provided.
Please sign the completed Past Performance Survey in the final block at the end of the survey and email the survey to the FAA Contracting Officer, Christian Lindsay, at Christian.C.Lindsay@faa.gov no later than 2:00 PM Eastern Time on February 22, 2024 subject line in the submission email should clearly indicate: FCT Past Performance Questionnaire Submission for <Contractor’s Name>. The email message must originate from your Government or Corporate email address, as applicable. If needed, you may receive a follow-up phone call or email to confirm or clarify information.

The rating scale is defined as follows:

Excellent Contractor consistently exceeded requirements and/or customer expectations. The contractual performance was accomplished with few minor problems, for which corrective actions taken by the Contractor were highly effective.
Good Contractor met and sometimes exceeded requirements and/or customer expectations. The contractual performance was accomplished with some minor problems, for which corrective actions taken by the Contractor were effective.
Satisfactory Contractor’s performance met contractual requirements and/or customer expectations. The contractual performance contained some minor problems, for which corrective actions taken by the Contractor appear or were satisfactory.
Marginal
Contractor’s performance did not consistently meet contractual requirements and/or was sometimes below customer expectations. The contractual performance reflect a serious problem, for which the Contractor has not yet identified corrective actions, or the Contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory Contractor frequently did not meet requirements and/or was often below customer expectations. The contractual performance contains serious problem(s), for which the Contractor’s corrective actions appear or were ineffective.

In the sections that follow, please provide your assessment of the Contractor’s performance relative to each item by placing an “X” under the rating you feel best describes their performance based on the definitions in Table 1 above.
