a. This amendment is issued to make the follow ing corrections:

1. The proposal extension due date is extended from 1200 on 13 May 2015 to 1200 on 22 May 2015.

2. Attachment 2, Statement of Work dated 31 March 2015 is being replaced w ith Statement of Work dated 07 May 2015 and reflects a revised Appendix A, Workload Estimate.

3. Attachment 7, L-1, ELIN Schedule is replaced w ith a “Revised ELIN Schedule” and is dated 07 May 2015. Please ensure you discard the original ELIN Schedule as this Revised ELIN Schedule shall be used w hen submitting your proposal. Failure to do so w ill result in your proposal being unacceptable.

4. Section L, L001 Proposed Preparation Instructions, is revised. The last sentence of Part III(b) stating “Submit three paper copies and one electronic copy of Part B for each PPQ sent out”, is deleted.

b. Please note that the changes made to Attachment 2, Statement of Work, Appendix A, Workload Estimate are the same changes that have been made to Attachment 7, L-1, ELIN Schedule.

c. Block 10a of the original solicitation and the descriptions of CLINs 0001 and 0002 is modified to reflect the new Statement of Work. In addition, Section J - List of Documents, Exhibits, and Other Attachments is modified to reflect the new Statement of Work, Attachment 1 as well as the Revised ELIN Schedule, Attachment 7.

d. Offerors shall acknowledge this amendment by completing blocks 8 and 15 of Amendment 0001 and returning 1 copy by the proposal due date of 1200 on 22 May 2015. Failure to do so may result in your proposal being unacceptable.

SECTION A - SOLICITATION/CONTRACT FORM

The required performance has changed from a. This is a Request for Proposal (RFP) for a contractor to provide Demolition and Abatement services at Edwards Air Force Base, California in accordance with (IAW) the Statement of Work (SOW), dated 31 March 2015, located in Section J and the Specifications, dated 31 March 2015, located in Section C. b. The proposed acquisition shall be a Firm-Fixed-Price (FFP) Indefinite Delivery Indefinite Quantity (IDIQ) contract. c. This acquisition is a competitive 8(a) program set-aside, restricted to 8(a) firms with a bona fide office in the state of California IAW 13 CFR 124.507(c)(2).d. All 8(a) participants are deemed eligible to submit offers with the NAICS code 238910 Site Preparation Contractors, among their approved NAICS codes. e. The Basic Contract's Period of Performance is five years, plus a six month option (if needed) IAW FAR Clause 52.217-8. f. The magnitude of construction for the Basic Contract is between $25,000,000 and $100,000,000. g. The Site Visit and Pre-Proposal Conference have been scheduled for 30 April 2015. See FAR 52.236-27 and AFFARS 5352.215-9001 for details regarding registration and more. to a. CLIN 0001

The CLIN extended description has changed from The contractor shall provide Demolition and Abatement services as specified in each task order issued against this contract. All work shall be performed in accordance with terms and conditions of this contract including the Statement of Work (SOW) and the Specifications, both dated 31 March 2015, located in Section J and each task order's complimenting SOW.

CLIN 0002

The CLIN extended description has changed from In accordance with FAR Clause 52.217-8 Option to Extend Services, CLIN 0002 allows the Demolition and Abatement Services to be extended for a maximum of six months after the period of performance end date of CLIN 0001.

The contractor shall provide Demolition and Abatement services as specified in each task order issued against this contract. All work shall be performed in accordance with terms and conditions of this contract including the Statement of Work (SOW) and the Specifications, both dated 31 March 2015, located in Section J and each task order's complimenting SOW.

Attachment 7 will be used to price all task orders. to In accordance with FAR Clause 52.217-8 Option to Extend Services, CLIN 0002 allows the Demolition and Abatement Services to be extended for a maximum of six months after the period of performance end date of CLIN 0001.

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS

INSTRUCTIONS TO OFFERORS

L-001 PROPOSED PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.

The proposal response shall consist of three separately bound parts, Part I – Contract and Price, Part II – Technical, and Part III – Past Performance Information.

B. The contracting officer has determined there is a high probability of adequate price competition for this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness of the price.

1. Requirements of the RFP (Exhibit Line Items Numbers (ELINs), Statement of Work (SOW), and Specifications), and government standards and regulations pertaining to the

SOW.

2. Proposal Preparation Instructions are in Section L and the Evaluation Factors for Award are in Section M of this RFP.

3. Page limitations if set forth, will be specified below in para D, Specific Instructions.

Pages exceeding the page limitations will not be read or evaluated, and will be removed from the proposal.

4. Format for each proposal shall be as follows:

(a) A page is defined as one face of an 8 ½” x 11” sheet of paper containing information.

(b) Typing shall be Times New Roman Font, size 12.

(c) Submit each volume of proposal in three ring binders. Number of copies will be specified for each proposal part below.

(d) Electronic files requested must be in Adobe (.pdf) or a combination of Microsoft Word Version 2010 or earlier. Excel files must be in Excel 2010 or earlier format. CDs are the only permissible electronic media, flash drives, DVDs and email proposals will not be accepted.

5. Proposal must be mailed or hand delivered to the address below. Submissions by fax and/or via email will NOT be permitted.

6. It is the sole responsibility of the offeror to ensure their proposal is delivered by the correct time and date and to the correct location.

7. There will be no financial considerations made to offerors for the development of their proposals in response to this solicitation

D. Specific Instructions:

1. PART I - CONTRACT AND PRICE PROPOSAL

(a) Submit three paper copies and one electronic copy of each document requested below as Part I of your proposal.

(b) Complete, the “Offeror” section on page two of the Standard Form (SF) 1442, which includes blocks 14 through 20 as well as all necessary fill-ins and certifications within Sections A through K. In doing so, the offeror accedes to the contract terms and conditions as written in the request for proposal, Sections A-K. Offerors shall submit Sections A through J in its entirety, only the necessary fill-ins and certifications in Sections C through K, and Section K shall only be returned for Representations and Certifications that are not completed in the System for Award Management (SAM), or that specify the offeror will complete their representations and certifications for each solicitation on an individual basis.

(c) Ensure that your company’s data is current in the System for Award Management (www.sam.gov).

(d) Provide a letter from the Small Business Administration (SBA) showing confirmation of 8(a) status including the firm’s actual graduation date.

(e) If any sort of teaming arrangement exists for this solicitation, provide complete information as to the arrangement signed by both parties, including how responsibilities are going to be divided among the companies to verify the Prime Offeror’s compliance with FAR 52.219-14, Limitations of Subcontracting and to confirm whether the existence of an ostensible subcontractor is present as described in 13 CFR (Code of Federal Regulations) 121.103(h)(4).

(f) In order for an offeror to be eligible to submit an offer for this solicitation, it must receive from SBA a determination that it has a bona fide place of business in the state of California prior to submitting its offer.

(g) Complete the proposed unit prices ONLY for each Exhibit Line Item

Number (ELIN) in Attachment L-1. The totals will auto calculate. Include the offeror’s name on the top right corner of each page in Attachment L-1.

(h) Place the contract total amount from Attachment L-1 into Contract

Line Item Number (CLIN) 0001 located in Section B of the solicitation.

Then place the six month option total also from Attachment L-1 into Contract Line Item Number (CLIN) 0002 located in Section B of the solicitation.

2. PART II - TECHNICAL PROPOSAL

(a) Submit three paper copies and one electronic copy of each document http://www.sam.gov/ requested below as Part II of your proposal.

(b) SUBFACTOR 1 – PROOF OF BONDING CAPABILITY: The offeror

(Prime Contractor) must submit bonding capability documents that evidence the ability to obtain a minimum single project bonding capacity of $750,000.00 and a minimum aggregate bonding capacity of $3,000,000.00 from an approved corporate surety whose name appears on the list contained in the Treasury Department Circular 570. The document must also include the surety company’s name, address, phone number and points of contact.

(c) SUBFACTOR 2 – SEED PROJECT PROPOSAL: A separate proposal must be submitted by the offeror in reference to the Seed Project Package (Attachment L-2). The proposal must include Exhibit Line Item Number (ELIN) types, quantities, and a period of performance ONLY. The seed project will be awarded to the same offeror who is awarded the Demolition and Abatement contract and will be issued as Task Order 0001 concurrently with the basic contract award to fulfill the minimum contract value.

3. PART III – PAST PERFORMANCE INFORMATION

(a) PAST PERFORMANCE INFORMATION FORM (PPIF): Offers shall use the Past Performance Information Form (Attachment L-3) to submit references for recent and relevant past efforts only. Refer to Section M for definitions of recent and relevant. Only references for relevant past efforts/contracts are desired. No more than 4 recent and relevant contracts are permitted and each PPIF must not exceed 5 pages. Submit three paper copies and one electronic copy for each PPIF submitted.

(b) PAST PERFORMANCE QUESTIONNAIRE (PPQ): Offerors must fill out Part B, Respondent Information of the Past Performance Questionnaire and Cover Letter (Attachment L-4) send to all points of contact listed in each PPIF. The responsibility to timely send out and track the completion of the PPQs rests solely with the offeror. The offeror shall exert its best efforts to ensure the points of contact complete and send the PPQ directly to the Contract Specialist as specified on the first page of the PPQ by the closing date of the solicitation.

(c) SUBCONTRACTOR CONSENT FORM: Past performance Information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. If a subcontractor’s information is used as a part of the proposal, a letter MUST be provided from each subcontractor, consenting to the release of their past performance information to the prime contractor. See Attachment L-5. Submit three paper copies and one electronic copy for each Subcontractor Consent Form submitted.
