SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
INSTRUCTIONS TO OFFERORS
L-001 PROPOSED PREPARATION INSTRUCTIONS
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
The proposal response shall consist of three separately bound parts, Part I – Contract and Price, Part II – Technical, and Part III – Past Performance Information.
B. The contracting officer has determined there is a high probability of adequate price competition for this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness of the price.
C. Documents submitted in response to this request for proposal (RFP) must be fully responsive to and consistent with the following:
1. Requirements of the RFP (Exhibit Line Items Numbers (ELINs), Statement of Work (SOW), and Specifications), and government standards and regulations pertaining to the
SOW.
2. Proposal Preparation Instructions are in Section L and the Evaluation Factors for Award are in Section M of this RFP.
3. Page limitations if set forth, will be specified below in para D, Specific Instructions.
Pages exceeding the page limitations will not be read or evaluated, and will be removed from the proposal.
4. Format for each proposal shall be as follows:
(a) A page is defined as one face of an 8 ½” x 11” sheet of paper containing information.
(b) Typing shall be Times New Roman Font, size 12.

(c) Submit each volume of proposal in three ring binders. Number of copies will be specified for each proposal part below.
(d) Electronic files requested must be in Adobe (.pdf) or a combination of Microsoft Word Version 2010 or earlier. Excel files must be in Excel 2010 or earlier format. CDs are the only permissible electronic media, flash drives, DVDs and email proposals will not be accepted.
5. Proposal must be mailed or hand delivered to the address below. Submissions by fax and/or via email will NOT be permitted.

6. It is the sole responsibility of the offeror to ensure their proposal is delivered by the correct time and date and to the correct location.
7. Offeror’s whom do not have access to the base and require a sponsor to hand deliver their proposal shall do so between the hours of 0700 and 1200 on Friday 22 May 2015.
The Visitor’s Control Center (VCC) is located near the entrance of the West Gate.
Offeror’s must check-in at the VCC and provide a valid form of identification, in addition to their vehicle registration and insurance. Two sponsors that can be contacted are the following:
a. Ashley Priestley, Contract Specialist, (661) 277-7131
b. Ty Okuhara, Contracting Officer, (661) 277-5435
It is still the offeror’s sole responsibility to ensure their proposal is delivered by the correct time and date and to the correct location. Therefore, we recommend arriving at the VCC right at 0700 to ensure you arrive to the proposal delivery location of B2800, 5 South Wolfe Avenue, Edwards AFB, CA 93524-1185 by the proposal due date time of 1200.
8. There will be no financial considerations made to offerors for the development of their proposals in response to this solicitation
D. Specific Instructions:
1. PART I - CONTRACT AND PRICE PROPOSAL
(a) Submit three paper copies and one electronic copy of each document requested below as Part I of your proposal.
(b) Complete, the “Offeror” section on page two of the Standard Form (SF) 1442, which includes blocks 14 through 20 as well as all necessary fill-ins and certifications within Sections A through K. In doing so, the offeror accedes to the contract terms and conditions as written in the request for proposal, Sections A-K. Offerors shall submit Sections A through J in its entirety, only the necessary fill-ins and certifications in Sections C through K, and Section K shall only be returned for Representations and Certifications that are not completed in the System for Award Management (SAM), or that specify the offeror will complete their representations and certifications for each solicitation on an individual basis.
(c) Ensure that your company’s data is current in the System for Award Management (www.sam.gov).
(d) Provide a letter from the Small Business Administration (SBA) showing confirmation of 8(a) status including the firm’s actual graduation date.
(e) If any sort of teaming arrangement exists for this solicitation, provide complete information as to the arrangement signed by both parties, including how responsibilities are going to be divided among the companies to verify the Prime Offeror’s compliance with FAR 52.219-14, Limitations of Subcontracting and to confirm whether the existence of an ostensible subcontractor is present as described in 13 CFR (Code of Federal Regulations) 121.103(h)(4).
(f) In order for an offeror to be eligible to submit an offer for this solicitation, it must receive from SBA a determination that it has a bona fide place of business in the state of California prior to submitting its offer.
(g) Complete the proposed unit prices ONLY for each Exhibit Line Item
Number (ELIN) in Attachment L-1. The totals will auto calculate. Include the offeror’s name on the top right corner of each page in Attachment L-1.
(h) Place the contract total amount from Attachment L-1 into Contract
Line Item Number (CLIN) 0001 located in Section B of the solicitation.
Then place the six month option total also from Attachment L-1 into Contract Line Item Number (CLIN) 0002 located in Section B of the solicitation.
2. PART II - TECHNICAL PROPOSAL

(a) Submit three paper copies and one electronic copy of each document requested below as Part II of your proposal.
(b) SUBFACTOR 1 – PROOF OF BONDING CAPABILITY: The offeror
(Prime Contractor) must submit bonding capability documents that evidence the ability to obtain a minimum single project bonding capacity of $750,000.00 and a minimum aggregate bonding capacity of $3,000,000.00 from an approved corporate surety whose name appears on the list contained in the Treasury Department Circular 570. The document must also include the surety company’s name, address, phone number and points of contact.
(c) SUBFACTOR 2 – SEED PROJECT PROPOSAL: A separate proposal must be submitted by the offeror in reference to the Seed Project Package (Attachment L-2). The proposal must include Exhibit Line Item Number (ELIN) types, quantities, and a period of performance ONLY. The seed project will be awarded to the same offeror who is awarded the Demolition and Abatement contract and will be issued as Task Order 0001 concurrently with the basic contract award to fulfill the minimum contract value.
3. PART III – PAST PERFORMANCE INFORMATION
(a) PAST PERFORMANCE INFORMATION FORM (PPIF): Offers shall use the Past Performance Information Form (Attachment L-3) to submit references for recent and relevant past efforts only. Refer to Section M for definitions of recent and relevant. Only references for relevant past efforts/contracts are desired. No more than 4 recent and relevant contracts are permitted and each PPIF must not exceed 5 pages. Submit three paper copies and one electronic copy for each PPIF submitted.
(b) PAST PERFORMANCE QUESTIONNAIRE (PPQ): Offerors must fill out Part B, Respondent Information of the Past Performance Questionnaire and Cover Letter (Attachment L-4) send to all points of contact listed in each PPIF. The responsibility to timely send out and track the completion of the PPQs rests solely with the offeror. The offeror shall exert its best efforts to ensure the points of contact complete and send the PPQ directly to the Contract Specialist as specified on the first page of the PPQ by the closing date of the solicitation.
(c) SUBCONTRACTOR CONSENT FORM: Past performance Information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. If a subcontractor’s information is used as a part of the proposal, a letter MUST be provided from each subcontractor, consenting to the release of their past performance information to the prime contractor. See Attachment L-5. Submit three paper copies and one electronic copy for each Subcontractor Consent Form submitted.
