STANDARDIZED COMMENT MATRIX PRIMER

The matrix below is a Word document table to be used as a template for submitting comments on this draft Request for Proposal. Except as noted below, an entry is required in each of the columns. To facilitate consolidating matrixes from various sources, do not adjust the column widths.

Column 1 – ITEM Numeric order of comments (1…n).

Column 2 – SOURCE Insert company name.

Column 3 – TYPE C – Critical (Contentious issue that will cause non-concurrence with publication) M – Major (Incorrect material that may cause non-concurrence with publication) S – Substantive (Factually incorrect material) A – Administrative (grammar, punctuation, style, etc.)

Column 4 – PAGE Enter the page number(s).

Column 5 – PARA Paragraph number that pertains to the comment expressed.

Column 6 – LINE Line number of the designated paragraph.

Column 7 – COMMENT Provide comments using line-in-line-out format. To facilitate adjudication of comments, copy and insert complete sentences into the matrix. This makes it unnecessary to refer back to the publication to understand the rationale for the change. Do not use Tools, Track Changes mode to edit the comments in the matrix. Include deleted material in the comment in the strike through mode. Add material in the comment with underlining. Do not combine separate comments into one long comment in the matrix, (i.e. 5 comments rolled up into one).

Column 8 - RATIONALE Provide concise, objective explanation of the rationale for the comment.

Column 9 - DECISION A - Accept R – Reject (Rationale required for rejection.)

M - Accept with modification (Rationale required for modification.)

NOTE: This column is for OPR use only. No rationale required for accepted items. Rationale for rejection is placed in the rationale comment box and highlighted for clarity. For modifications, the complete modified language will be placed (and annotated) as the bottom entry for that item in the “Comments” column and the rationale for the modification placed in the rationale comment box and highlighted for clarity.

Additional CRM Instructions:

--Comment text will be in line-out/line-in format, with edits following the flow of the text. Include material to be deleted via strike-through in red (line out) and material to be added via underlining in blue (line in)
--Do NOT use “track changes” to simulate line-out/line in
--Comments not submitted using line out/line in format will not be accepted

Comment Matrix and Line-out/Line-in Format”

--When suggesting additions or changes to text, specific text must be included in order for it to be incorporated
--Rationale should include specific references whenever possible
--Copy complete sentences into the matrix, providing context, so we can better understand rationale
--There is no need to make minor admin comments regarding spacing, fonts, acronym use, grammar, et cetera
--Completed CRMs must be in Microsoft Word format; PDFs will not be accepted
