1. Small Business Participation Commitment Document (SBPCD) The contractor is required to submit a Small Business Participation Commitment Document. The contractor shall submit an SBPCD in its own format which will be put on contract as Attachment 3b. The two tables below shall be included in the SBPCD. The contractor shall provide the level of participation of small businesses (as a small business prime and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goals set forth in Attachment 6, Evaluation Criteria, for the evaluation of small business participation.

For the purposes of this commitment document, the Offeror shall base the percentage upon total Launch Service Price (CLINs YYX1 and YYX2) (reference Attachment 5, Instructions to Offerors, paragraph 8.2.2). The Offeror is required to meet the Minimum Quantitative Requirement (MQR) level of small business participation, which is 4%, for each assigned launch service.

Describe the extent of commitment to use small businesses (e.g. that types of commitments, if any, are in place for this specific acquisition either – written contract, verbal, enforceable, non-enforceable, joint venturing, mentor-protégé, etc. Small business prime Offerors should include their own participation as well).
The contactor shall provide a copy of the Individual Subcontract Report (ISR) or Summary Subcontract Report (SSR) submitted in the Electronic Subcontracting Reporting System (eSRS) to the PCO within 30 days of the required submittal time per FAR 52.219-9, Small Business Subcontracting Plan (DEVIATION 2018-O0013).
