1.

Page Limitations Page limits apply to both hard and electronic copies. Each page shall be counted except the following: blank pages, tabs, glossaries, schedules, Contractor Furnished Parts (CFP) list, Requirements Correlation Matrix, and those noted in the Proposal Organization Chart as unlimited. Page limitations shall be treated as maximums. Any excess pages will not be read or considered in the evaluation of the proposal and (for paper copies) will be returned to the Offeror as soon as practical. Page limitations may also be placed on responses to Evaluation Notices (ENs).
Pricing Information All pricing information containing dollar values shall be addressed ONLY in the Price and Contract Documentation volumes, except dollar values specifically requested in relation to small business subcontracting and past performance.

Cross Referencing Each volume shall be written on a stand-alone basis so that its contents may be evaluated without cross-referencing other volumes of the proposal. Information required for proposal evaluation, which is not found in its designated volume, will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is permitted when its use would conserve space without impairing clarity.
Requirements Correlation Matrix The Offeror shall complete the cross-reference matrix provided in Table 2.3.8 to include: the proposal reference information as it relates to the instructions, PWS contents, RFP sections, CLIN numbers and the corresponding proposal paragraph in the section which addresses the reference item as part of Volume I - Technical.

3.
Factor 2 – Past Performance - (Volume II)
General
Volume Organization Each Offeror shall submit a Past Performance Volume with the proposal, containing past performance information IAW the format contained in ITO Attachment 1.1. This information is required of the Offeror and each significant subcontractor, teaming partner, and/or joint venture proposed to perform the work outlined in the solicitation. Offerors are cautioned that the Government will use data provided by each Offeror in this volume and data obtained from other sources in the evaluation of past performance.
Note: A “significant” subcontractor, teaming partner, and/or joint venture is defined as a business entity that contributes more than 5% of the overall effort, and/or is responsible for a key component or service of the contracted effort.
Additional Information In addition to the information required in this paragraph, the Offeror shall submit a consent letter (ITO Attachment 1.4) executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the Offeror so the Offeror can respond to such information. For each identified effort for a customer, the Offeror shall also submit a client authorization letter (ITO Attachment 1.5) authorizing release to the Government of requested information on the Offeror’s performance.
Early Proposal Information Each Offeror may submit Volume II Past Performance for each relevant contract at any time prior to complete proposal due date and the Government will accept them. The final past performance volume submission is not required until complete proposal due date. Offerors must use the PPI Tool (use latest version from FBO.gov, ITO Attachment 1.1) in order to electronically submit the PPI portion of the Past Performance Volume IAW the RFP (See ITO paragraph 2.2.8 for submission details). Failure to submit early proposal information will not result in Offeror disqualification.
As soon as practicable, Offerors shall:
a. Complete Section 1 of the attached Past Performance Questionnaire (PPQ) (ITO Attachment 1.2)
b. E-mail the PPQ and the Performance Questionnaire Letter (ITO Attachment 1.3) to all points of contact (POCs) the Offeror has listed in the Past Performance Information Sheets (PPIS) (ITO Attachment 1.1)
c. The POCs will complete the questionnaires and forward them, directly to the Past Performance Evaluation Team (PPET) (Attn: John Seacrist) or e-mail John.Seacrist@us.af.mil
RESPONDENTS TO THE QUESTIONNAIRES SHALL NOT SEND THE COMPLETED
INFORMATION SHEETS BACK TO THE OFFEROR. Offerors shall not follow-up with respondents to ensure they have completed the questionnaires. The PPET will conduct such follow-up with any POC as necessary.
Relevant Contracts PPI Each PPIS shall not exceed six (6) pages. Each Offeror may submit up to three (3) PPIS for the prime contractor and up to three (3) PPIS for each significant (see 4.1.2) subcontractor, teaming partner, and/or joint venture.
Organizational Structure Change History Many companies have acquired, been acquired by, or otherwise merged with other companies and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between the conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, Offeror’s shall include a "roadmap" describing all such changes in the organization of the Offeror’s company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the Offeror(s), the "roadmap" should be both specifically applicable to the efforts identified, yet general enough to apply to efforts on which the Government receives information from other sources.
4.
Factor 3 – Price – (Volume III)
General Instructions These instructions are to assist you in submitting information required to determine a Total Evaluated Price (TEP) for your proposal as well as evaluate the reasonableness, balance and completeness of your proposed pricing.
4.1.1.1. Price Data
All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price volume. Compliance with these instructions is mandatory and failure to comply may result in your proposal being ineligible for award. Data beyond that required by this instruction shall not be submitted, unless the Offeror believes additional information is necessary to justify or clarify proposed pricing. All pricing information shall be addressed ONLY in the Price and Contract Documentation volumes.
4.1.1.2. Cost or Pricing Data Requirements
FAR Part 15 applies to this acquisition regarding price analysis as a basis for determining reasonable and balanced pricing. IAW FAR 15.403-3-1(b), prices based on adequate price competition do not require submission of certified cost or pricing data; however FAR 15.403-3(a)(1) provides for obtaining data other than certified cost or pricing data if needed to determine price reasonableness. If requested by the CO, data other than certified cost or pricing data shall be provided to make a determination of price reasonableness and balance. This information will only be requested if all other sources have been insufficient to support a determination of reasonable and/or balanced pricing.
If, after receipt of initial or subsequent proposals, the CO determines that there is insufficient information available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the Offeror may be required to submit certified cost or pricing data IAW FAR 15.403-5.
4.1.1.3. Price Reasonableness, Unbalanced Pricing and Completeness Offerors’ proposals will be evaluated for reasonableness, balance, completeness and TEP. Unreasonable proposed prices, initially or subsequently, may be grounds for eliminating an Offeror from competition. Unbalanced pricing may pose an unacceptable risk to the Government and may be grounds to find an Offeror’s proposal unawardable. For completeness, Offerors must comply with all requirements of this section. Offerors’ proposals should be sufficiently detailed to demonstrate their reasonableness, balance and completeness.
4.1.1.4. Estimated Quantities
The estimated quantities, to be completed during specific periods of performance, are provided in the Pricing Matrix, Section J, Attachment 7 of the solicitation. All such quantities will be used for evaluation purposes only and do not obligate the Government to exercise or guarantee such quantities during the contract performance period.
4.1.1.5. Rounding
All proposed dollar amounts shall be rounded to the nearest dollar.
4.1.1.6. Government Field Support Agencies
Identify the cognizant Defense Contract Audit Agency (DCAA) and the Defense Contract Management Agency (DCMA) office responsible for administration of the Offeror’s Government contracts. Include names, physical addresses, telephone numbers, and e-mail addresses.
4.1.1.7. Approved Accounting System
Provide verification that the Offeror has an approved accounting system.
4.1.1.8. Cost and Software Data Reporting (CSDR) System
The offeror shall use Section J Attachment 6 to provide a plan that meets the requirements of DFARS 252.234-7003(b) and proposes a standard CSDR system. This plan shall be able to be easily separated from the proposal in its entirety and will be forwarded to the Defense Cost and Resource Center to determine if the offeror proposed a standard CSDR system prior to contract award.
Pricing Support Information
4.1.1.9. Pricing Support Narrative and Assumptions
Offerors shall provide a narrative of the basic pricing approach and methodology utilized to generate proposed prices. Offerors shall provide any ground rules and assumptions necessary for the Government to understand what the prices represent. This narrative, including assumptions, shall correlate to and/or supplement information provided elsewhere in the Offerors’ Price volume to add clarity. All significant assumptions, scope limitations and/or qualifications of the pricing proposal shall be addressed and explained.
4.1.1.10. Annual Price Changes
Offerors shall provide information to address price reasonableness and balance for proposed unit price increases or decreases of significant variability per year. Unexplained unit price variances between sequential performance period years could suggest unbalanced pricing.
4.1.1.11. Pricing Matrix
Offeror shall complete (fill-in all yellow shaded cells only) and submit the completed Pricing Matrix, Section J, Attachment 7 to the solicitation. Submission of an incomplete Pricing Matrix, will cause the Offeror’s proposal to be non-compliant and may render the proposal ineligible for award. The Pricing matrix submitted must comply with the requirements listed in 2.3.6.
Volume Organization.
The Price Volume shall consist of the following sections:
