PERFORMANCE-BASED WORK STATEMENT (PWS)

FOR

EQUIPMENT & FACILITY

SUPPORT SERVICES

309th MAINTENANCE WING (MXW)

1. DESCRIPTION OF SERVICES: This Performance Work Statement (PWS) defines the non-personal service requirements for equipment and facility support services. The contractor shall provide all management and labor necessary to clean, operate, install, relocate, modify or remove property (that is not real property), facilities and equipment (that is not industrial plant equipment (IPE)) in support of the 309th Maintenance Wing (MXW). The contractor may be exposed to and be required to work with various chemicals such as mineral spirits, cleaning solvents, hydraulic oils, engine oils, soaps, high pressure cleaners and steam cleaners to clean buildings, equipment and machinery. The types of equipment used in support of these services are listed in Appendix D while the estimated workload is described in Appendix B.

This requirement is considered non-mission essential.

1.1 Basis Services:

1.1.1 Equipment Support: In support of the 309th MXW and 309th Groups, the contractor shall:

1.1.1.1 Clean industrial plant equipment and machinery such as working lathes, milling machines, grinders, hones, drill presses, electrical maintenance, hydraulic and electric presses, work benches, work stands and work stations.

1.1.1.2 Tape or paint areas of the floor for staging equipment, safety purposes, move non- IPE equipment into designated areas, straighten such areas, sort and dispose of non-IPE obsolete equipment and office furniture and clean designated areas.

1.1.1.3 Repair, disassemble, relocate and assemble, audio/visual aids, office equipment, furniture, desks, chairs, work benches, shelving units, filing cabinets and tables.

1.1.1.4 Attach Radio Frequency Identification (RFID) tags to ground support equipment, recording and inputting which tags are associated with each piece of equipment by inputting the data into two databases (Equipment Management Database and Air Force Global Enterprise Tracking (AFGET)). The contractor shall remove, separate, organize, and track which tags have a “low battery status”, “bad tags”, and which tags with “no data” being transmitted from the equipment.

1.1.1.5 Tag and stage Precision Measurement Equipment Laboratories (PMEL) equipment and maintain respective listings.

1.1.2 Facilities Support: In support of the 309th MXW and 309th Groups, the contractor shall:

1.1.2.1 Clean the inside and outside of industrial areas. Contractor shall be exposed to and work with solvents, and soaps used to clean surfaces.

1.1.2.2 During the winter season: clear snow and ice from walkways, fire hydrants, entrances and landings within 75 ft of buildings to include delivery areas, docks and fire hydrants. In addition, the contractor shall lay ice melt crystals to all entrances of buildings and remove icicles within reach. During all other seasons, the contractor shall clear debris from walkways, entrances, landings, delivery areas and docks to include FOD removal from designated flight line areas.

1.1.2.3 Empty trash receptacles in common use areas, i.e. hallways, break rooms, entryways.

1.1.2.4 Operate equipment to clean work bays, aisle ways and break room floors in industrial production areas. Spot clean carpet and floor tile as needed.

1.1.2.5 Minor maintenance of facilities such as tightening of door handles and knobs, minor plumbing (clogs of toilets, urinals and sinks). Replace expended light bulbs or fluorescent lamps up to a height of 10 feet. Expended light bulbs shall be disposed as follows; keep the original packaging to repackage the expended lights for transport, if needed, boxes for expended bulbs can be obtained from building 514 or 893. Each box must have the term “Universal Waste Lamps” written on the box. Write the date of the first bulb placed in the box and keep the box closed at all times when adding spent tubes. Boxed of expended bulbs need to be turned into building 514 or 893 within 9 months of the start date. Coordinate with Facility Mangers.

1.1.2.6 Escort and/or observe when needed, other contracted personnel who must work in restricted industrial and flight-line areas and who do not have the appropriate credentials. Contract personnel who possess a controlled area badge can escort other contracted personnel. This service and building locations will be identified, when needed, at the task order level.

3. Government Furnished Property, Equipment and Services:

3.1. If required by a specific task order, the government shall provide the contractor personnel with use of workspace, furniture, telephones, and computer equipment and access within the OO-ALC. If the specific task order requires the contractor to gain access to either the Hill AFB local area network (LAN) or the use of the internet from Hill AFB, or Hill AFB email service, the 309th security manager will forward the contractor’s request to the 75ABW/CS/BCC for a computer account.

3.2. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

3.3. Record and report promptly (within one hour) to the contracting officer or designated government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

3.4. Furnished Equipment/Supplies: Wrenches, drills, screw drivers, hammers, snow blower, ladders, nail gun, paint rollers, weed eater, leaf blower, forklift, aprons, drill bits, cleaning gloves, cleaning towels, cleaning solvents, hand tools, ear plugs, safety goggles, electrical and air powered tools and transportation between buildings as available. Additional equipment listed in Appendix D.

4.0 Deliverables:

4.1 Monthly Report – The contractor shall provide a monthly report summarizing work performed on all task orders. Report is due by the 3rd business day of each month. The report shall show the progress, status, man-hours and expenditures on task projects. The monthly report shall be electronically submitted to the Wing COR, respective Group COR and Project Manager (if applicable). This report shall be in contractor format and error free. (CDRL A001).

5.0 General Information:

5.1 Special Requirements:

5.1.1 Contracted personnel who provide services to the 309 AMXG and 309 CMXG shall be required and responsible to make arrangements for expenditures on the following; respirators, respirator fit tests and physical exams. Copies of the certifications will be provided to the CO. Specific building locations requiring these certifications and government provided surveys are identified in Appendix D.

5.2. TRAINING

5.2.1 All contractor personnel shall attend mandatory government training. These training requirements will be accomplished by government Points of Contact (POC). The contractor shall provide the COR a roster of personnel trained upon request. Mandatory Contractor (production and administrative staff) Initial/Recurring Training courses:

5.2.3. The contractor shall furnish employee training for the following courses, including any additional training required at the task order level.

a. Hazardous Materials (HazMat)

b. Lockout/Tagout

c. Confined Spaces

d. CPR

e. Workforce Hazardous Communication Training

5.2.4. The contractor shall be responsible for ensuring individuals under this contract are trained with one year experience, certified or licensed with a Utah State Drivers License knowledge and ability to operate industrial cleaning equipment, government vehicle (up to five ton). The contractor shall ensure employees are certified or licensed with a Flight Line Drivers License as required. The contractor shall provide all training records, certification, licenses that must be maintained to operate JLG man lifts scissor extension lifts, forklifts, floor cleaning equipment, snow blowers, snow removal equipment, weed eaters and leaf blowers to the CO after award.

5.3 Security Requirements:

5.3.1. The contract officer shall complete a "Request for Identification Credential (AFMC Form 496)" for each employee of the contractor requiring access to Hill AFB. The requests shall be submitted to Pass and Registration (South or West Gate Visitors Center).

5.3.2. The unit security manager shall complete an “Unescorted Entry Authorization Certificate” (AF Form 2586) for each employee requiring entry into controlled areas. The request shall be submitted to Pass and Registration (Gates Visitor Center).

5.3.3. For contractors requiring access to computer networks on Hill AFB: The Unit Security Manager, together with the contractor, will submit an application for Department of Defense Common Access Card (CAC)-DEERS Enrollment, through the Contractor Verification System (CVS).

5.3.4. The contractor’s employees, at all times while on a military installation, shall wear visible contractor-provided identification either as a part of, or attached to, their outer clothing. The identification shall clearly identify the individual as being a contractor employee.

5.3.5. Contract personnel shall be cleared to access areas classified as Secret. Contract personnel must be able to obtain a SECRET clearance when working in these areas. Access areas requiring this clearance will be identified at the task order level. A Department of Defense (DoD) Form 254 is attached. For contractor employees who require government computer access only, a National Agency Check with Written Inquiries (NACI) must be submitted. For those contractor employees who require a higher level of investigation for a security clearance and government computer access, a National Agency Check with Local Agency Checks and Credit Check (NACLC), Secret or a Single Scope Background Investigation (SSBI), Top Secret, must be submitted. (AFI 31-501, Personnel Security Management Program, AFSSI 8522, Access to Information Systems.)

5.3.6. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel. At the completion of the contract or at the termination of any employee, the contractor shall return credentials to the Contracting Officer and Unit Security Manager.

5.4. Quality Control:

5.4.1 Quality Control Plan. The Contractor shall ensure quality service is maintained throughout the life of the contract, and shall prepare and implement (upon Government approval) a Quality Control Plan. The plan shall reflect the Contractor’s overall approach, internal controls and establishment of procedures for evaluating each of the major service areas (who, method, how often), for reporting to the Government, for resolving deficiencies, for identifying potential improvements, and for maintaining and making available to the Government, documentation (as listed in Deliverables Section) reflecting quality control inspections and any corrective actions taken. (CDRL A002).

5.5. Quality Assurance:

5.5.1 The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor's quality control program and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government may decrease the number of quality control inspections if merited by performance. The government will also investigate complaints received from various customers located on the installation. The contractor shall be responsible for initially validating customer complaints. However, the government representative shall make final determination of the validity of customer complaint(s) in cases of disagreement with customer(s).

5.5.2 Contract Officer Representative (COR):

5.5.3 The COR(s) is the authorized government representative(s) who will perform assessments of the contractor’s performance. Subsequent to contract award, the identity of the COR(s), with a letter defining their duties and authority will be promptly furnished to the successful bidder/offeror.

5.5.4 The COR(s) or alternate(s) will inform the contracting officer when discrepancies occur and will request corrective action. The COR(s) or alternate(s) will make a notation of the discrepancy on their assessment checklist with the date and time the discrepancy was noted and will request the contract manager (or authorized representative) to initial the entry on the checklist.

5.5.5 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer and not to the COR(s).

5.5.6 The services to be performed by the contractor during the period of this contract shall at all times and places are subject to review by the Contracting Officer or authorized representative(s).

5.6. Physical Security: The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, property, and materials shall be secured.

5.6.1. Key Control. The contractor shall establish and implement methods of ensuring that all keys/key cards issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons.

5.6.1.1. The contractor shall immediately report the occurrences of a lost or duplicate key to the contracting officer.

5.6.1.2. In the event keys, other than master keys, are lost or duplicated, the contractor shall, upon written direction of the contracting officer, rekey or replace the affected lock or locks; however, the government, at its option, may replace the affected lock or locks or perform rekeying. When the replacement of locks or rekeying is performed by the government, the total cost of rekeying or the replacement of the lock or locks shall be deducted from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

5.6.1.3. The contractor shall prohibit the use of keys issued by the government by any persons other than the contractor’s employees. The contractor shall prohibit the opening of locked areas by contractor employees to permit entrance of persons other than contractor’s employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer.

5.6.2. Lock Combinations. The contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures shall be included in the contractor’s Quality Control Plan.

5.7. Hours of Operation:

5.7.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: 0500-1530, Monday – Friday except Federal Holidays. The contractor may work, with prior approval of the contracting officer, overtime hours to ensure timely completion of work. During winter season, start times shall be adjusted to ensure walkways and entrances etc, are cleared of snow before start of government shift.

5.7.2. Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

5.8. Conservation of Utilities: The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities which include the following:

5.8.1. Lights shall be used only in areas where and when work is actually being performed.

5.8.2 Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees unless authorized.

5.8.3. Water faucets or valves shall be turned off after the required use has been accomplished.

5.8.4. Government telephones shall be used only for official government business.

5.9. Records: The contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in this PWS or required by the provisions of a mandatory directive listed in Applicable Publications and Forms. If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request.

5.10. Environmental Controls:

5.10.1. The contractor shall insure their employees are made aware that the performance of these services will occur in industrial areas. The industrial complex at Hill AFB has the potential to expose workers to hazardous materials which may include, but is not limited to: hexavalent chrome, cadmium, beryllium, lead and others. While the contractor will not perform duties in a marked and signed regulated area (where exposures are expected to exceed the permissible exposure limits set by 29 CFR1910. 10xx), there may be contact hazards with these materials during cleanup of dust, metal shavings, etc. The contractor shall insure proper protective measures and training are taken to ensure contracted employees are protected from these hazards. The 75th Air Base Wing (ABW) Bioenvironmental Engineering Flight has produced industrial hygiene assessments of these areas which are attached.

5.10.2. Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the Contracting Officer pursuant to notice requirements and negotiate a modification to the contract.

5.10.3. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately report the incident to Hill AFB Fire Dept at 777-1911. The liability for the spill or release of such substances rests solely with the contractor and its agent. A hazardous material control program shall be in effect to inform employees of potential hazards if and when chemicals are used. The MSDS will be maintained on file in accordance with OSHA 29 CFR 1910.1200 and will be provided to the employees for review of safety and health information.

5.10.4. Material Storage and Use. The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This may include hazardous materials. The contractor shall work with and dispose of various hazardous materials in accordance with AFI 32-7086, Hazardous Material Management and locally developed operating instructions.

5.11. Government Observations: Government personnel, other than Contracting Officers (COs) and Contract Officer Representative (CORs), may from time to time, with CO coordination, observe contractor operations. However, these personnel may not interfere with contractor performance or make any changes to the contract.

5.12. Safety Requirements: While performing work under this contract, the Contractor shall follow Appendix C and adhere to the following:

5.12.1. The Air Force is part of the OSHA Voluntary Protection Program (VPP). The Contractor is required to attain familiarization with the requirements of VPP. Information on the VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. VPP requirements apply to contractors whose employees work more than 1,000 hours per quarter. The contractor shall flow down these requirements to all subcontractors who qualify as applicable contractors under this contract. The Contractor shall provide from its OSHA 300 information the Total Case Incident Rate/Days Away Restricted or Transferred (TCIR/DART) rates by 15 January of each year to the CO for submission as part of the installation’s annual OSHA Voluntary Protection Program (VPP) self evaluation report.

5.12.2. Conform to the safety requirements in Appendix C.

5.13. Phase Out:

5.13.1. If there is a change in contractor or if the operation reverts to in-house, the incumbent contractor will provide familiarization, to the government or the follow-on contractor, whichever the case may be. During the phase-out familiarization period, the incumbent will be fully responsible for the operation of this PWS for 20 working days after contract award.

5.13.2. The government reserves the right to conduct site visits in all contractor operated facilities in conjunction with the solicitation of offers for the follow-on contract. In the event the follow-on contract is awarded to other than the incumbent, the incumbent contractor will cooperate to the extent required to permit an orderly change over to the successful contractor. With regard to the successor contractor’s access to incumbent employees, a recruitment notice may be placed in each facility.

5.14. Publications:

5.14.1. Compliance with all publications, regulations and operating instructions provided by the government is required when:

5.14.1.1. They pertain to the procedures for materials expediting herein and where the contractor is authorized by the statement of work to accomplish the work specified in the publication, regulation or operating instructions.

5.14.1.2. The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements.

5.14.1.3. The contractor is required to acquire and work on the latest version of the publication.

SAFETY, FIRE PROTECTION AND HEALTH SPECIFICATION

INDUSTRIAL SAFETY REQUIREMENTS

TYPE WORK: EQUIPMENT & FACILITIES MAINTENANCE

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

In context with AFMC Acquisition Reform initiatives, contract language will now focus on ensuring safety objectives are met through contractor risk management rather than the traditional method of providing specific government guidance for risk elimination. With this new “performance-based” language, the government will provide the contractor with areas of concern (Protection of government personnel and property) and the contractor will be free to meet those objectives in a manner that is consistent with corporate standards. The contractor will implement a safety program plan that ensures protection of Government personnel and property. The program will consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan and corresponding site specific checklist that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and will be used during the performance of the work described in the contract. The Safety Plan and checklist will be approved by 309 MXW/QPE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All contractor personnel shall be trained and qualified to perform their duties safely.

7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

1. The contractor shall notify 309 MXW Safety (Phone (801) 586-4176) who in turn will notify 75 ABW/SEG ((801) 777-3333) or 309 Wing Operations Center ( (801) 777-3238), or the Hill AFB Command Post (777-3007) after normal duty hours, and the designated Government Representative, i.e., the ACO, PCO, or COR within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to 309 MXW Safety. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

2. Ensure government contract lease agreements determine convening authority prior to executing the contract/lease agreements, and that all contracts ensure the following, IAW AFI 91-204, paragraph 2.4.6:

· The contractor will support and comply with the safety investigation and report requirements of AFI 91-204

· A record of all mishaps involving Air Force resources will be entered into the Air Force reporting system

· A record of mishaps involving other DoD resources will be forwarded to the involved agencies

· When a mishap involves a contract managed by DCMA, DCMA safety personnel will review the final safety report and send their response to HQ AFSC

3. Ensure contract/lease agreements for aerospace vehicles state: The Air Force is responsible for the investigation of mishaps involving aerospace vehicles. The Air Force is authorized to investigate mishaps involving non-accepted Air Force aerospace vehicles (Non-delivered vehicles for which the government has accepted responsibility; DD Form 250. Material Inspection and Receiving Report, HAS NOT been executed).

4. If requested by authorized government personnel (ACO, PCO, COR, safety representative, Fire Department, and Security Forces), the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

C. General Safety Requirements:

If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative.

Follow all applicable federal, USAF, state, and local safety directives outlined in Section II below.

Ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels.
Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the 309 MXW Safety Office.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall:

· Demonstrate a management commitment to employee safety and health

· Identify applicable rules and regulations

· Identify the roles and responsibilities of Management, Supervisors, Employees and Safety Coordinator

· Identify work to be performed and location of expected operations

· Provide a description of safety program, safety monitoring responsibilities, organizational structure, and contact information for on-site personnel

· Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures

· Identify employee safety and health training requirements and the documentation process

· Include emergency response plans and procedures that relate to protection of government personnel and property

· Include a workplace inspection frequency, to include the identity of the individual responsible for conducting the inspection

· Include hazard reporting procedures and identify individual(s) responsible for the correcting identified hazards

· Identify first aid and injury procedures

· Identify procedures for accident reporting and investigation

· Identify the process for tracking controlled hazards in contractors work area

· The contractor’s safety plan shall meet or exceed the referenced standards below.

The authorized Government Representative (309 MXW Safety Office) shall approve the contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification /Reporting Procedures, which are outlined in Section I, paragraph B of this Appendix C.
