Solicitation No. FA8224-12-R-0022
Solicitation Number: FA8224-12-R-0022 (Equipment & Facilities Support Services) Contractor Questions and Government Responses Date: 13 January 2012
1. The requirement states that the contractor must provide certification of a physical examination. Can the Government please describe the nature of this physical examination, is this to be work performance examination or is this a hazardous materials exposure base line examination?
Government response: The physical examination should include the required exams to qualify candidates medically for wear of the respirator, baseline and periodic evaluation for medical surveillance exam program (if indicated), audiogram if working within hazardous noise areas, and special examinations for expanded standards substances as prescribed by OSHA.
2. Is there a standard certification form desired by the Government for the doctor to certify a physical examination that will allow for proof of compliance without divulging protected patient information?
Government response: A signed statement from a licensed provider declaring that the worker meets criteria/medical standards as described in question one above. Actual medical data that qualifies as protected health information would not be required to be divulged.
3. The Contractor is also required to provide respirators and certified fit test, please confirm that the fit test must only comply with OSHA standards?
Government response: The fit-testing standards used must comply with OSHA 29 CFR 1910.134. A current certification of fit-test listing the brand/model and size of respirator fitted must be provided to the government.
4. After a review the various hazards chemical and environment exposures associated with the performance of this effort, will the Government provide appropriate replacement filters for the contractor provided respirators? If not will the cost be reimbursable under the T&M CLIN as required for task order performance?
Government response: Replacement filters shall be supplied by the contractor. The contractor needs to ensure they have the recommended filter type (manufacturer specific) as well as any other parts required to attach the filter to the respirator. Costs of the respirators and replacement filters will be reimbursable under the Over and Above CLIN (T&M).
