SECTION L - INSTRUCTIONS TO OFFERORS (ITO)

Reentry Field Support Equipment (RFSE) - Production

Reentry Field Support Equipment (RFSE) – Production

1.0 Program Structure and Objectives

The Government intends to award a single contract for the RFSE - Production effort using the lowest-price, technically-acceptable approach as defined by the DoD Source Selection Procedures effective 1 July 2011. However, the Government reserves the right to award no contract if the Source Selection Authority (SSA) determines it is in the Government's best interest. It is the Government’s intent to award without discussions.

Therefore, it is imperative that offerors submit an initial offer that contains the offeror’s best terms. However, if, during the evaluation period, it is determined to be in the best interest of the Government to hold discussions, offeror responses to evaluation notices and the final proposal revision will be considered in making the source selection decision.

2.0 General Instructions

2.1 Compliance

This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal. The offeror's proposal must include all data and requested information and must be submitted in accordance with (IAW) these instructions. The offer shall be compliant with the requirements as stated in this Request for Proposal (RFP), including but not limited to the Statement of Objectives (SOO) and Contract Data Requirements List (CDRL). Any offeror who submits an incomplete or non-conforming proposal may be considered non-responsive and eliminated from the competition.

2.2 Clear and Concise Proposals

The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal must not simply rephrase or restate the Government's requirements but rather shall provide convincing rationale to address how the offeror intends to meet these requirements in the form of descriptive plans unless otherwise indicated. An offeror shall assume that the Government has no prior knowledge of its facilities, engineering and manufacturing capabilities, and experience and will base its evaluation on the information presented in the offeror's proposal.

2.3 No Elaborate Documentation

Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.

2.4 Acceptance Period

The proposal acceptance period is specified in Section A of the solicitation.

Offerors providing less than the specified calendar days for Government acceptance may be considered non-responsive and may be eliminated from the competition.

2.5 Extra Copies of Unsuccessful Proposals

IAW Federal Acquisition Regulation (FAR) Subpart 4.8, Government Contract Files, the Government will retain one hard copy and one copy of the compact disc (CD) of all unsuccessful proposals. The Government will destroy extra copies of such unsuccessful proposals.

2.6 Point of Contact

The Contracting Officer (CO) is the sole point of contact for this acquisition.

2.7 Debriefings

The CO will promptly notify offerors of any decision to exclude them from the competitive range; whereupon, an offeror may request and receive a debriefing IAW FAR 15.505. Offerors excluded from the competitive range may request a pre-award debriefing or the offeror may choose to wait until after the source selection decision to request a post-award debriefing. However, offerors excluded from the competitive range are entitled to no more than one debriefing for each proposal. The CO will notify unsuccessful offerors in the competitive range of the source selection decision IAW FAR 15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing. An offeror desiring a debriefing must make a request IAW the requirements of FAR 15.505 or 15.506, as applicable.

2.8 Discrepancies

If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CO in writing with supporting rationale as well as the remedies the offeror is asking the CO to consider as related to the omission or error. The offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion.

2.9 Electronic Reference Documents

Some of the documents referenced in the solicitation are available on the Federal Business Opportunities (FBO) website, but some are not. For those documents that are not available on FBO, instructions for how to obtain the documents are available on FBO. Potential offerors are encouraged to

2.10 Oral Presentations

Oral presentations will not be allowed.

2.11 Communications

Exchanges of source selection information between Government and offerors will be controlled by the CO. Offerors are encouraged to protect their data to the best of their abilities and are to ensure that all data is free of any malicious software attachments and viruses.

2.12 Amendments to Solicitation

If this RFP is amended, all terms and conditions that are not amended remain unchanged. Offerors shall acknowledge receipt of any amendment to this request by the date and time specified in the amendment(s) by signing and returning the front page of each.

2.13 Submission, modification, revision, and withdrawal of

proposals

2.13.1 Submission

Proposals and modifications to proposals shall be submitted in both paper and CD (see paragraph 2.17) to the address located in paragraph

2.6 of this document, and show the time and date specified for receipt, the solicitation number, and the name and address of the offeror. Late proposals may be considered IAW FAR 15.208. In order to avoid being considered late or non-responsive, both the CD and paper copies of the proposal must be received by the time specified in the RFP.

2.13.2 Time of Receipt

Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the proposal wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.

2.13.3 Withdrawal

Proposals may be withdrawn by written notice received at any time before award. Proposals may also be withdrawn in person by an offeror or an authorized representative, if the identity of the person requesting the withdrawal is established and the person signs a receipt for the proposal before award.

2.14 Organization

The offeror shall prepare the proposal as set forth in the table, Proposal Organization (Table 2.1 below). The titles and contents of the volumes shall be as defined in this table, all of which shall be within the required page limits and with the number of copies as specified in Table 2.1. The contents of each proposal volume are described in the ITO paragraph as noted in the Table 2.1.

The title page of each volume must show:

2.15 Page Limitations

Page limitations are maximums. If exceeded, the Government will consider your proposal up to the page number limit specified. Page limitations may be placed on responses to Evaluation Notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the offerors. Title pages, table of contents, glossaries, pages dedicated solely to explaining abbreviations (abbreviation pages), index pages, or Solicitation Cross Reference Matrices will not count towards page limitations.

2.16 Page Size and Format

2.16.1 Pages

A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Page size shall be 8.5 x 11 inches, not including foldouts. Except for the reproduced sections of the solicitation document, the text size shall be no less than 12 point while using Arial font. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least 1 inch margins on the top, bottom, and side margins. Pages shall be numbered sequentially by volume. These page format restrictions shall also apply to EN responses and Final Proposal Revisions. These limitations shall apply to both CD and hard copy proposals.

2.16.2 Tables, Charts, Graphs and Figures

Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 11 x 17 inches in size. Any page exceeding 8.5 in width or 11 inches in height is considered a foldout. Each foldout will be considered 2 pages. Foldout pages shall fold entirely within the volume.

Foldout pages may only be used for large tables, charts, graphs, diagrams and schematics, not for pages of text. For tables, charts, graphs and figures, the text shall be no smaller than 10 point Arial font.

These limitations shall apply to both CD and hard copy proposals.

2.17 Copies of Offers on CDs

The content and page size of copies on CDs must be identical to the hard copies. CD copies will not be accepted without accompanying hardcopies. If there are any discrepancies between the hard copies and the CD copies of the proposal, the hard copies will be used for evaluation. For CD copies, indicate on each CD the volume number and title. Use separate files to permit rapid location of all portions, including subfactors, exhibits, annexes, and attachments, if any. Ensure all files submitted are editable by those reviewing and are not password protected. The offeror shall submit all volumes using virus-free recordable, (not re-writeable) CDs. Each volume shall be in a different directory on a CD. If files are compressed, the necessary decompression program must be included. The proposal’s files shall be submitted in a format readable by Microsoft (MS) Office 2010 or converted adobe portable document files (PDFs) will be accepted except for the following: Section J, Attachment 5: Solicitation Cross Reference Matrix (SCRM); Section J, Attachment 6: Pricing Matrix; Section J, Attachment 4:

Labor Rate Matrix; and Section J, Exhibit B: Initial Spares List. PDF files that are not searchable or that were scanned instead of converted will not be accepted.

2.18 Cost or Pricing Information

All cost or pricing information shall be addressed ONLY in the Price Proposal.

Cost benefit information, work hour estimates, and material kinds and quantities may be used in other volumes only as appropriate for presenting rationale for alternatives or design decisions.

2.19 Classified Information

Offerors shall ensure that proposals do not contain any classified information.

2.20 Cross-Referencing

Each volume shall be written on a stand-alone basis so that its contents may be evaluated without cross referencing to other volumes of the proposal.

Information required for proposal evaluation which is not found in its designated volume will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is permitted where (a) it is necessary, and

(b) its use would conserve space without impairing clarity.

2.21 Solicitation Cross Reference Matrix (SCRM)

The offeror shall provide a SCRM in the format listed in Section J, Attachment 5:

Solicitation Cross Reference Matrix (SCRM). The SCRM will explain where each requirement listed in the RFP and its attachments is located in the offeror’s proposal. It will also indicate the proposal reference information as it relates to Section L – Instructions to offerors, and Section M - Evaluation Factors for Award.

2.22 Indexing

Each volume shall contain a more detailed table of contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections.

2.23 Glossary of Abbreviations and Acronyms

Each volume shall contain a glossary of all abbreviations and acronyms used, along with an explanation for each.

2.24 Binding and Labeling

Each volume of the proposal must be separately bound in a three-ring, loose leaf binder large enough to permit the volume to lie flat when open. Staples shall not be used. The outside of each binder shall include a cover sheet, clearly marked as to volume number, title, copy number, solicitation identification, and the offeror's name. The same identifying data must be placed on the spine of each binder. Be sure to apply all appropriate markings including those prescribed IAW FAR 52.215-1(e), Restriction on Disclosure and Use of Data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection Information.

2.25 Distribution

The original proposal shall be identified. Proposals shall be addressed to the CO and mailed to the address in paragraph 2.6.

3.0 Summary of Evaluation Factors

The following is a list of factors, subfactors, and elements that will be used in evaluating proposals. For a more complete understanding of evaluation criteria, please review Section M - Evaluation Factors for Award.

4.0 Volume I - Price Factor

4.1 General Instructions

4.1.1 Price Evaluation Criteria

This section is to assist you in submitting data other than certified cost or pricing data that is required to assess the price evaluation criteria (as set forth in section M) of your proposed cost or price. Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal. The burden of proof for credibility of proposed prices rests with the offeror.

4.1.2 Price Data

Information beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your price position. All information relating to the proposed price including all required supporting documentation must be included in the price volume.

Under no circumstances shall this information and documentation be included elsewhere in the proposal.

4.1.3 Cost or Pricing Data Requirements

IAW FAR 15.402, 15.403-1 (b), and 15.403-3 (a), information other than certified cost or pricing data may be required to support price reasonableness. Information shall be provided IAW FAR 15.403-5. If, after receipt of proposals, the CO determines that there is insufficient information available to determine price reasonableness the offeror shall be required to submit other than certified cost or pricing data. If, during the source selection process, the CO determines that adequate competition no longer exists, then certified cost and pricing data may be required.

Recent guidance from the Director of Defense Procurement and Acquisition Policy provides instruction to Contracting Offices that if only one offer is received, “then the COs shall not depend on the standard at FAR 15-403-1(c)(ii) in determining the price to be fair and reasonable.

Rather, the CO shall use price or cost analysis IAW FAR 15.404-1 to make that determination.” The basis for the analysis will be either certified cost or pricing data or other than certified cost or pricing data as appropriate IAW FAR 15.403-1(c), DFARS 215-403-1(c) and FAR 15.403-3(b).

4.1.4 Rounding

Refer to Section J, Attachment 6: Pricing Matrix. Offerors shall populate the pricing and cost cells as appropriate. All CLIN unit prices shall be rounded to the nearest dollar.

4.2 Volume Organization

The Price volume shall consist of the following sections:

4.2.1 SECTION 1 - Table of Contents

The cost or price volume shall be prefaced by a Table of Contents and shall specify, by page number, the location of information requested in these instructions. Also provide abbreviations and acronyms used in the price volume.

4.2.2 SECTION 2 - Pricing Assumptions

Summarize all significant pricing assumptions, scope limitations and/or qualifications of the cost proposal. List each assumption provided in the solicitation and each qualification of the price volume, if any. Provide complete rationale for any assumption or qualification.

4.2.3 SECTION 3 – Estimating Techniques and Methods

The offeror shall include pricing information and supporting data, to include estimating methodology. With regards to your Accounting, Purchasing, and Estimating Systems, indicate whether or not you have Government approval.

If so, provide evidence of such approval. Copies of DCMA approval letters and/or certificates will suffice to show Government approval of your Accounting, Purchasing, and Estimating system. Also, identify any deviations from your standard procedures in preparing this proposal. If a Forward Pricing Rate Agreement has been negotiated with the Administrative Contracting Officer (ACO) covering the period of the proposal, provide appropriate comments and the date of agreement.

4.2.4 SECTION 4 –TEP

The offeror will complete both the Pricing Matrix, Section J, Attachment 6 as well as Section J, Exhibit B. The TEP is for award determination purposes only (although the proposed price elements that constitutes it will be contractually binding) and will be used in the award decision pending a determination that the proposed prices are considered fair and reasonable.

The TEP will be calculated IAW Section M.

4.2.5 SECTION 5 – Labor Rate Matrix

The offeror shall complete Section J, Attachment 4: Labor Rate Matrix IAW the instructions contained therein.

5.0 Volume II - Technical Acceptability Factor

5.1 General

The Technical Acceptability Volume must be specific and complete. Legibility, clarity, and coherence are very important. Your responses will be evaluated against the technical acceptability subfactors defined in Section M - Evaluation Factors for Award. Offerors should review criteria in Section M - Evaluation Factors for Award to ensure the minimum requirements are met. Using the instructions provided below, provide as specifically as possible the actual methodology you would use for satisfying these subfactors. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal.

Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.

5.2 Volume Organization

The proposal shall be organized according to the following outline specifically addressing the subfactors below IAW the SOO requirements.

5.2.1 Subfactor 1: Program Management

Description: The offeror shall provide a comprehensive management approach to achieve the requirements of RFSE SOO paragraph 5.1. The offeror’s proposal must (a) convey a sound, feasible, and effective approach and (b) demonstrate how that approach will be applied to the RFSE - Production effort.

The proposal must also include the following.

5.2.1.1 Management Plan

Provide a Management Plan that is accurate and complete relating to traceability from system level requirements. Management Plan must comply with SOO 5.1 and CDRL A007. The Management Plan provided with the proposal will be a draft version of the CDRL deliverable and will not count toward the initial delivery of this item.

5.2.1.2 IMS

The offeror shall provide an IMS IAW SOO 5.1 and CDRL A001.

The IMS used to be a separate CDRL, but it is now part of another CDRL called the Integrated Program Management Report. The IMS submitted for this proposal must conform to the IMS requirements listed in the Integrated Program Management Report CDRL, CDRL A001. The IMS must identify proposed schedule dates for all Management Plan events, reviews, accomplishments and criteria. Contractor’s IMS must show completion and delivery of the first deliverable no later than 20 months after contract award and completion and delivery of the last deliverable no later than 44 months after contract award to be considered technically acceptable. The IMS must provide enough detail to show that (a) all tasks are being completed, (b) that they are being done in a logical order, and (c) enough time is being allowed for completion of the task without compromising quality within the deadlines provided. The offeror shall include a delivery schedule of the CDRLs as a part of the IMS. For ease of evaluation, the offeror shall use a contract award date starting on January 1. The IMS is expected to be presented in the form of a Gantt chart and is required to be no more than ten (10) pages in length. To avoid issues with clarity, please also ensure the IMS is included electronically in Microsoft Project—version must be readable by Microsoft Project 2010. The IMS provided with the proposal will be a draft version of the CDRL deliverable and will not count toward the initial delivery of this item.

5.2.1.3 CSOW

The offeror shall include a CSOW as defined by SOO paragraph

5.1 and CDRL A005 for evaluation of this effort. The CSOW will be provided with the proposal and will be incorporated into the contract at contract award. The proposed CSOW shall define the tasks required for the RFSE - Production effort ensuring all minimum requirements of the Government provided SOO have been addressed. Enough detail must be provided to demonstrate that the offeror understands the tasks that must be completed in order to achieve the objectives listed in the SOO.

The proposed CSOW shall consist of tasking statements. Each tasking statement shall reference the CDRL items which will be delivered by that task. The proposed CSOW shall not contain informational notes (statements other than tasking statements), as the Technical Acceptability Volume provides ample opportunity for discussion and description of the offeror's approach, and the Management Plan and IMS provide the mechanisms for describing specific details of the offeror's approach. The tasking statements in the CSOW, the Management Plan, and IMS sections shall use a common numbering system.

5.2.1.4 Small Business Participation Plan

The offeror shall provide a small business participation plan that clearly demonstrates the course of action to be taken to meet the requirements of the small business subcontracting goals as identified in clause H-133. The small business participation plan is not to be confused with small business subcontracting plan. The small business participation plan shall also include the extent to which such firms are specifically identified in proposals and the extent of commitment to use such firms.

5.2.2 Subfactor 2: Production Management

Description: The offeror shall provide an approach for Quality and Manufacturing. Reference SOO paragraphs 5.4 and 5.5. The offeror’s proposal must (a) convey a technically-sound, feasible, and effective approach; (b) demonstrate how that approach will be applied to the RFSE program; and (c) provide evidence of how these capabilities, processes, and methodologies were successfully used on a previous program of similar technical complexity. The Government will determine what is of similar technical complexity. The proposal must also include the following.

5.2.2.1 Quality Assurance Program Plan (QAPP): Provide a

QAPP IAW SOO paragraph 5.4 and CDRL A010. The offeror’s Quality Assurance Program Plan must describe implementation of established quality assurance processes and what measures will be in place to provide early visibility of and confidence in the produced RFSE components. Acceptable offers will have ISO 9001-2008 certification or AS9100. Proof of an offeror’s ISO 9001- 2008 certification or AS 9100 certification must be included in offers. Additionally, acceptable offers will demonstrate an ability to identify process improvements and the ability to manage vendors and subcontractors. The offeror shall discuss what metrics are kept and how those metrics are used. Quality includes continual recertification of supply chain and subcontractors. A discussion of history of quality over the last three years must be included.

The QAPP is expected to be a draft version of CDRL A010 deliverable and will not count toward initial delivery of this item.

5.2.2.2 Manufacturing Plan: Provide a manufacturing plan IAW

SOO paragraph 5.5 (as they relate to manufacturing, assembly and checkout of the RFSE suite and its supporting components) and CDRL A006. The plan will include facilities that are rated for processing classified equipment at the SECRET level or higher.

Offerors shall ensure that proposals do not contain any classified information. The plan will discuss measures taken to reduce human induced defects in the build. Offerors must demonstrate an ability to handle heavy equipment and manage Nuclear Weapons Related Material. Offerors must describe the shelf life maintenance procedures and how they have been used. This is expected to be a draft version of CDRL A006 deliverable and will not count toward the initial delivery of this item.

6.0 Volume III - Past Performance Factor

6.1 Past Performance Information

Each offeror shall submit a past performance volume with its proposal, containing past performance information. This information is required on the offeror and any (a) significant subcontractors, (b) teaming partners, and/or (c) joint venture partners. Significant subcontractors are those who are (a) proposed to perform 15 percent or more of the proposed effort based on the total proposed price (including options) or (b) perform aspects of the effort the offeror considers critical to overall successful performance. Offerors are cautioned that the Government will use data provided by each offeror in this volume and data obtained from other sources in the evaluation of past performance. Offerors should review Section M – Evaluation Factors for Award, section 6.4 criteria to ensure the minimum PPI requirements are met.

6.2 General

Offerors shall submit specified PPI through the PPI Tool following the instructions in Attachment L-2: Instructions to Offerors, Past Performance Information (PPI) for Offerors. Note that line spacing and font size restrictions do not apply to the pages that are generated by the PPI tool. Contractors must then transmit a copy of Attachment L-5: Past Performance Questionnaire to the POC identified in PPI Tool for his or her evaluation. A Sample Letter for offerors to provide POCs is included as an attachment, Attachment L-6: Past Performance Questionnaire Cover Letter. The Government will need subcontractor consent to disclose any PPI from a company that is submitted as a teaming partner or major subcontractor on this effort, Attachment L-3. An offeror and its significant subcontractors shall complete the appropriate number of PPI. Up to four (4) contracts shall be for the prime offeror, and up to two (2) contracts shall be provided for each significant subcontractor, teaming partner, and/or joint venture partner. No more than ten (10) of the most relevant and recent contracts performed by the offeror, significant subcontractor, teaming partner, joint partner, and/or venture partner may be submitted. If the specific minimums will result in more than the maximum number of relevant and recent contracts, the offeror must state so and provide the required information for most significant contributors up to the maximum. If less than ten (10) total relevant and recent contracts are available, the offeror shall acknowledge and specifically state such. Include rationale supporting your assertion of relevance.

6.2.1 Recent and Relevant Contracts

Submit PPI on up to 4 recent prime offeror contracts that you consider most relevant in demonstrating your ability to perform the proposed effort.

For this solicitation, recent contracts shall be those that are ongoing or have completed within the last five years prior to RFP release. Include information on up to 2 recent contracts performed by each of your teaming partners and significant subcontractors that you consider most relevant in demonstrating their ability to perform the proposed effort.

Include rationale supporting your assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining recency and relevance, see Section M – Evaluation Factors for Award.

6.2.2 Specific Content

Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate. This may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to an unacceptable rating, since the problems encountered may have been on a more complex program, or an offeror may have subsequently demonstrated the ability to overcome the problems encountered. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. This may allow the offeror to be considered more favorably than without evidence of problem resolution. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems is required. Categorize the relevant information into the specific technical acceptability subfactors and price factor used to evaluate the proposal.

6.3 Consent and Authorization Letters

Along with the information required in this section, the offeror shall submit a consent letter, Attachment L-3: Sub-Contractor/Teaming Partner Consent Letter for the Release of Past and Present Performance Information to the Prime Contractor, executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse PPI to the offeror, so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a client authorization letter Attachment L-4: Client Authorization Letter, authorizing release to the Government of requested information on the offeror's performance.

6.4 Organizational Structure Change History

Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a roadmap describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation. Since the Government intends to consider PPI provided by other sources as well as that provided by the offeror(s), your roadmap must be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.

7.0 Volume IV - Contract Documentation

7.1 Solicitation

The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The offeror's proposal shall include a signed copy of (a) the model contract, (b) solicitation amendments, and (c) completed sections A through K.

7.1.1 Section A - Solicitation/Contract Form

Offeror must complete blocks 11, 13, 15, and 27. Blocks 21 and 22 which ask for unit price and amount are completed elsewhere in the offer. The offeror’s signature in block 27 constitutes an offer, which the Government may accept. The original copy must be clearly marked as ORIGINAL under separate cover and must be provided without any punched holes.

7.1.2 Section B - Supplies or Services & Prices or Costs

Supplies or Services and Costs or Prices: Do NOT input pricing information in Section B of the model contract. All required prices shall be proposed in the Pricing Matrix, Section J, Attachment 6.

7.1.3 Section F - Deliveries or Performance

Proposed Delivery Schedule: The offeror shall propose the delivery schedule by completing the blanks in Section F of the model contract.

7.1.4 Section I - Contract Clauses

The offeror shall comply with the clauses specified within Section I of the model contract. All laws regulations and statutory authorities will also be applied within the model contract.

7.1.5 Section K - Representations, Certifications, and Other Statements Complete representations, certifications, acknowledgments and statements. Offeror must have filled out an online provision 52.204-08 Annual Representations and Certifications (NOV 2011), Online Reps and Certs (ORCA) at https://www.sam.gov.

7.2 Exceptions to Solicitation Requirements

Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost, and specific requirements of the solicitation. This information shall be provided in the format and content of Table 7.2.

7.3 Other Information Required

7.3.2 Government Offices

Provide the mailing address, telephone and fax numbers and facility codes for the cognizant Contract Administration Office, DCAA, and Government Paying Office. Also, provide the name and telephone and e-mail address for the ACO.

7.3.3 Company and/or Division Address, Identifying Codes, and Applicable Designations Provide company and/or division's street address, county and facility code; CAGE code; DUNS code; TIN; size of business (large or small);

and labor surplus area designation. This same information must be provided if the work for this contract will be performed at any other location(s). List all locations where work is to be performed and indicate whether such facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location.

7.3.4 Attachments to the Solicitation: This information will be used as part of the CO’s determination of responsibility IAW FAR 9.104.

7.3.4.1 Ability to obtain resources

Submit evidence that your company possesses adequate financial resources to perform the contract.

7.3.4.2 Plans Required by Regulation or Policy

Submit the following plans IAW applicable FAR,DFARS, and/or SOO Parts.

7.3.4.3.1 Property Management Plan

IAW FAR 45.201(c)(4) and 52.245-1(f)(1) establish, implement and submit a property management plan.

7.3.4.3.2 Small Business and Small Disadvantaged

Business Subcontracting Plan.

All offerors other than Small Businesses shall submit a complete subcontracting plan IAW FAR 19.7, 52.219-09, DFAR supplement 252.219-7003 and 252.219-7004, and the following instructions. If the Offeror has an approved, unexpired master subcontracting plan (FAR19.704 (b) and 52.219-9(f)), or an approved, unexpired comprehensive subcontracting plan (DFARS 219.702), it shall submit an addendum with its proposal covering any additional information required by this solicitation along with a copy of the plan. The addendum will be incorporated by reference into any resulting contract along with the approved master or comprehensive plan. The enhanced small business requirement is not intended to obtain two plans from offerors but a single enhanced plan. Offerors are notified that while the Comprehensive Subcontracting Test Program has been extended through December of 2017, many of the individual plans that the Defense Contract Management Agency (DCMA) negotiated with different companies, may no longer be valid—expiration date of the program was

December 31, 2014. Offerors who would like to use a Comprehensive Plan, must contact DCMA to ensure their plans are still valid.

The purpose of this table is to show the roles and responsibilities of different members of an offeror’s team as those roles and responsibilities relate to the subfactors from paragraph 5.2. See paragraph 6.4 for more information.

In this example, the offeror team consists of 3 contractors, (A, B, and C). The roles and responsibilities for each of the contractors are broken out per the subfactors listed in paragraph 4.2.1.

INSTRUCTIONS TO OFFERORS

PAST PERFORMANCE INFORMATION (PPI) FOR OFFERORS

Offerors must use the PPI Tool (Use latest version from FBO.gov) in order to electronically submit the PPI portion of the Past Performance Volume IAW the RFP.

Downloading the PPI Tool The PPI Tool can be downloaded by performing the following steps (if you are unable to download the Tool, contact the CO for assistance):

1. Access the FedBizOpps (https://www.fbo.gov/) website.

2. Find the solicitation posting.

3. Locate the “ppi tool” link from the “All Files” column on the solicitation’s “Notice Details” tab .

4. Select the link and save the “ppi tool” to your computer. Name the file as the prime contractor + RFP number + file extension (e.g. XYZCompanyFA861710R6158.accdb).

Note: PPI Tools saved in Microsoft Office versions 2007 and greater will be saved with “.accdb” file extension.

Entering information in the PPI Tool After selecting and saving the tool, enter information by performing the following steps:

1. Open the saved PPI Tool.

2. Select the “Options” button from the “Security Warning” banner, if applicable

3. The “Security Alert” pop-up screen displays.

• Files saved using Microsoft Office 2010 will have the “Enable Content” button on the

“Security Warning” banner and therefore will not get a Security Alert pop-up.

4. Select the radio button “Enable this content” and then click “OK.” A setup pop-up screen displays.

Note: Once the offeror’s Name and RFP Number have been entered they can be edited by selecting the “Edit offeror And RFP Number” button from the “Contractor’s Menu”

6. Enter the offeror’s Name and RFP Number and then click the “Next” button. The application setup continues.

7. Choose the appropriate option by selecting the corresponding radio button and then click the

“Next” button. The “Contractor’s Menu” displays.

8. Click the “Step 1: Set up Business Relationships” button to create a business relationship, if applicable, for each business entity before proceeding throughout the PPI Tool (refer to Section L of the RFP for detailed instructions). Identify all prime and sub-prime organizations and categorize them according to the appropriate role in the proposed acquisition. The “Business Relationships” screen displays.

9. Click the “New” button to create a business relationship for the proposed acquisition. An additional “Business Relationships” screen displays.

10. Complete the fields as follows (fields marked on the screen with an asterisk ‘*’ are required):

• Contractor’s Name: Self-explanatory

• Role in Proposed Acquisition: Choose one of the four options – Prime, Sub, Joint

Venture, or Other (Explain). If the relationship is one of a teaming partner, mark “Other” and in the explanation state the relationship is a teaming partner relationship.

An explanation comment box will display when you select “Other.”

• Place of Work: Location where contractor will perform work.

• Percentage of Work: Identify percentage of work contributed by specified business entity

• Responsibilities: Detail proposed responsibilities of specified business entity.

11. Select one of the buttons at the bottom of the screen.

• Add – Saves the current business relationship and allows for the addition of a new one.

• Close – Cancels the current business relationship without saving.

Note: In order to edit or delete an existing business relationship in the list, double-click on it.

12. Select the “Close” button on the “Business Relationships” screen after all of the business relationships has been added.

13. Click the “Step 2: Enter Past Performance Information (PPI)” button to enter the Past Performance Information. The “Past

Performance” screen displays.

15. Complete the fields on each of the tabs as follows (fields on the screen marked with an asterisk ‘*’ are required):

• Contract Information Tab Contractor: Select from the dropdown the appropriate business entity.

Cage Code: Self-explanatory.

Contract Number: If you don’t have a contract number, enter “N/A.”

Program Title: Enter full name of program.

Contr Agency/Customer: Enter servicing contracting agency and customer (office symbols suffice).

DUNS Number: Self-explanatory

Delivery Task/Order: If the order is provided as a stand-alone reference, enter the task/call/delivery/purchase order number.

Contract Type: Enter Firm-Fixed-Price (FFP), Cost Plus Fixed-Fee (CPFF), Indefinite Delivery/Indefinite Quantity (ID/IQ), LH, Blanket Purchase Agreement (BPA), Cost Plus Incentive-Fee (CPIF), Cost Plus Award Fee (CPAF), etc. For additional clarification, click the question mark button.

Explain how your performance on this contract is relevant for each applicable factor or subfactor, as instructed under Past Performance in Section M of the RFP. Include any unique aspects that demonstrate relevancy in this effort. Provide evidence to support such relevance to the requirements as described in the RFP.

Include relevant information concerning your compliance with FAR 52.219-8, Utilization of Small Business Concerns, on the contract you are submitting, if applicable.

Identify whether a subcontracting plan was required for the contract you are submitting. If one was required, identify in percentage terms the planned versus achieved goals during contract performance. Explain why goals were not met, if applicable.

• POC & Key Individuals Tab Key Individuals: Click the “New” button to specify any key individual or individuals who participated in this program or who may support the proposed acquisition. Also indicate their roles for both acquisitions, previous and current. To edit or delete an existing key individual, double-click an item from the list.

Note: IAW FAR 15.305(a)(2)(iii), relevant contract efforts performed by from key personnel may be evaluated. If you wish to include the past performance of individual key personnel, select the “New” button for each individual and fill out the necessary information.
