SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements. The contractor will implement a safety program that ensures protection of Government personnel and property: The program will consist of, as a minimum:

1. A Safety Plan that addresses, as a minimum, the subjects listed in Safety Plan

Elements, Section II and to be used during performance of the work described in the contract. The Safety Plan will be approved by the procuring Safety Office prior to commencement of any work described in this contract.

a. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations, (i.e. fuel/defuel, engine runs, hazardous storage, etc.). Provide to the ACO for approval of these operations/locations and changes.

b. Routine and recurring surveillance of the procedures listed in the Safety Plan

c. Competent personnel to provide surveillance of the Safety Plan.

d. If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative. Approval shall be received prior to implementing the modification.

B. Mishap Notification/Reporting

1. The contractor shall notify the procuring Safety Office and the ACO, or a designated Government Representative (GR) within eight (8) hour of all mishaps or incidents involving damage to DOD property entrusted by this contract and employee injuries as a result of the contract operation. This notification requirement shall also include Foreign Object Damage (FOD) and physiological mishaps or incidents. A follow-up (written or e-mail) of this mishap/incident notification shall be sent within three calendar days to the Government Representative, the Procurement Contracting Officer (PCO) or the Administrative Contracting Officer (ACO) (with a copy to program manager), who will forward it to the Installation Safety Office. The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap.

2. Mishap notification/report shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective actions (taken or proposed)

h. Other pertinent information

3. If requested by Government Personnel or designated government representative, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance, training records and/or tools, materials and equipment involved, until released by the Procuring Safety Office.

a. Provide a process for ensuring subcontracts requiring subcontractors to comply with the safety provisions of this contract, as applicable.

b. Safety requirements listed herein that do not concern contractor’s operations or services shall be considered self-deleting as mutually agreed by the contractor, the Procuring Safety Office and the ACO.

SECTION II - SAFETY PLAN ELEMENTS

The contractor prepared Safety Plan shall clearly define procedures, personnel qualifications, facilities and required equipment, and other attributes necessary to fulfill the following elements. The GFR shall approve only those company procedures that meet the intent of the indicated benchmarks, with the exception of Mishap Notification/Reporting procedures. The contractor shall meet the Mishap Notification/Reporting requirements as written in Section I paragraph B in this Attachment 8.

The contractor is solely responsible for compliance with federal, state and local laws, the Occupational, Safety and Health Act (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, and the protection of their employees. Additionally, the contractor is responsible for the safety and health of all subcontractor employees.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. The Air Force is participating in the OSHA's Voluntary Protection Program (VPP). All Air Force contractors are required to familiarize themselves with the requirements of VPP.

Information on VPP can be accessed at http://www.osha.gov. The intent of the Air Force Safety Program is to prevent injuries and to communicate our safety expectations with the entire workforce at a government facility. Contractors shall be responsible for managing their safety and health program as part of OSHA compliance. The Contractor on an installation shall be aware of the hazards they may encounter while working at the site, recognize hazardous conditions and the sign and symptoms of workplace related illnesses and injuries, are familiar with the hazard controls (including safe working procedures) and the emergency evacuation procedures of the installation.
