This addendum to a solicitation provides evaluation criteria for a performance-based acquisition of laboratory support services. Pricing must use the attached schedule, and the lowest-priced technically acceptable offeror will be selected for award.

ADDENDUM TO 52.212-2
1. The Government will award this requirement to the Offeror who represents the best value in a Performance Price Tradeoff evaluation. Past performance is significantly more important than price. First, if applicable, the Government will evaluate all technical proposals to assign an “Acceptable” or “Unacceptable” rating. Then, the Government will assign a single past performance confidence rating based on the integrated assessment or recency, relevancy, and quality of past performance for each Offeror whose technical proposals was rated “Acceptable.”
The Government shall rank all Offerors by performance confidence rating and then by the Offeror’s total evaluated price. The Government wishes to award to the highest performance confidence rated Offeror at the best price for the requirement.
a. TECHNICAL
(1) The Government will evaluate the Offeror’s 5-page synopsis which clearly explains timely recruiting, hiring, training and retaining Medical Laboratory Technician-Team Leads (MLT-TL) (two (2) FTE); Medical Laboratory Technicians - Specimen Processing (MLT-SP) (eight (8) FTEs); Data Entry Clerks (four (4) FTE); Analytical Scientist (two
(2) FTEs), and Information Technology Support (three (3) FTEs) to ensure that the requirement is understood and determine if the company’s recruiting and hiring can produce satisfactory performance results.
b. PAST PERFORMANCE
(1) For past performance, the Government will assess the Offeror’s present and past performance records that meet the recency and relevancy criteria below. To be recent, the reference must have been on-going within a 3-year period from the date of issuance of the request for proposal. The Government will not evaluate any past performance information that fails to meet the recency criteria. To be relevant, the references should provide the same or similar labor category(s) or services as being requested in this solicitation.
(2) Using the Offeror’s references (Attachment 09A and 09B) as well as other information from PPIRS, FAPIIS or eSRS, as well as other databases, interviews, or non- Government resources, the Government will assess the results of the relevancy determination along with a thorough assessment of quality of services to assign an integrated performance confidence rating (as shown in the table below) that represents the overall evaluation of Offeror’s performance. The past performance evaluation will result in an overall assessment of the Offeror’s probability of meeting the solicitation requirements. The past performance evaluation considers each Offeror’s demonstrated recent and relevant record of performance in staffing, training and retaining Medical Laboratory Technician-Team Leads (MLT-TL) (two (2) FTE); Medical Laboratory Technicians - Specimen Processing (MLT-SP) (eight (8) FTEs); Data Entry Clerks (four
(4) FTE); Analytical Scientist (two (2) FTEs), and Information Technology Support (three (3) FTEs). Offerors without a record of relevant past performance or for whom information on past performance is not available will not be evaluated favorably or unfavorably. These Offerors will receive an “Unknown Confidence” rating for the past performance evaluation.
(3) As a result of the analysis, the Government will give each Offeror a Performance Confident Rating as follows:
Adjectival Rating Description Substantial Confidence Based on the Offeror’s recent/relevant performance record, the Government has a high expectation that the
Offeror will successfully perform the required effort.
Satisfactory Confidence Based on the Offeror’s recent/relevant performance record, the Government has a reasonable expectation that the
Offeror will successfully perform the required effort.
Neutral Confidence No recent/relevant performance record is available or the Offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The Offeror may not be evaluated favorably or unfavorably on the factor of past performance.
Limited Confidence Based on the Offeror’s recent/relevant performance record, the Government has a low expectation that the Offeror will successfully perform the required effort.
No Confidence Based on the Offeror’s recent/relevant performance record, the Government has no expectation that the Offeror will be able to successfully perform the required effort.
2. PRICE
a. The price section of the proposal must use the attached Pricing Schedule (Attachment 08).
The Offeror must clearly show the base year, options and total for each labor category and shall include a total evaluated price (grand total) in accordance with FAR 52.217-5, Evaluation of Options. The following will be used to evaluate proposed prices to ensure the price is fair and reasonable:
(1) Completeness: All prices shall be provided in the template, as required by the solicitation. The Government will evaluate completeness of the data fields as well as ensure the Offeror properly verified rounding, calculations, and certified/justified their prices.
(2) Reasonableness: The degree to which the proposed prices compare to the prices a reasonable, prudent person would expect to incur for the same labor categories and program requirements.
(3) The Government reserves the right to cancel the requirement (whole or in part). In the event the Government cancels the solicitation, the Government has no obligation to reimburse an Offeror for any proposal preparation costs (in accordance with FAR 52.232- 18, Availability of Funds). The Government’s obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made.
