This questionnaire is for the Atmosphere & Space Research, Development, and Program Analysis Support contract. Offerors submitting proposals in response to this solicitation are to provide this questionnaire directly to individuals, which have had experience working with them in the past 3 years on similar/relevant requirements. Individuals receiving this questionnaire are requested to complete it and return it directly to the government by fax to the address identified below no later than 7 January 2010 or sooner. Please complete this questionnaire based on the following guidance:
· Handwritten responses are sufficient.
· Indicate, based on the codes below, the contractor’s performance on the identified program. Assessments should reflect only contractor liable performance. The following is a definition of the scoring levels:
CODE PERFORMANCE LEVEL
E EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to your benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
VG VERY GOOD - The contractor’s performance meets contractual requirements and exceeds some (requirements) to your benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
M MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
U UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
N NOT APPLICABLE – Unable to provide a score.
· Please circle a letter corresponding to your rating and provide a narrative explanation for all answers rated other than Satisfactory. If more space is needed, use the back of this questionnaire. You are urged to supplement your own knowledge of the contractor’s performance with the judgment of others in your organization. In addition to completing the attached questionnaire for the identified program, we solicit your comments on other similar programs for which your activity has contracts with this offeror.
