1.0 PURPOSE

1.1 The purpose of this Statement of Work (SOW) is to define the requirements for the completion of the construction works to accomplish the repair of the roofing membrane and the incorporation of a new permanent fall protection system on Hangar #3, Building #7075 at Westover Air Reserve Base (WARB), MA.

2.0 SCOPE

2.1 This project incorporates two main activities for the barrel roof of Hangar #3, Building #7075.

2.1.1 The primary activity encompass the Seam Repair of the existing Thermoplastic Polyolefin (TPO) roofing membrane as required.

2.1.2 A subordinate activity involves the design of a New Permanent Fall Protection System (FPS), including the installation of its permanent anchoring points. The design- build undertakings of this activity will be coordinated through a submittal, which requires Government approval prior to commencement of any installation works.

2.2 Description of Existing Conditions

2.2.1 Roof Description; Hangar #3, Building 7075.

The main portion of the buildings is a barrel vaulted hangar with an existing TPO (thermoplastic polyolefin) roof membrane, laid over protective cover boards, over insulation, over wood decking, over steel structure. The north and south faces each have five rows of snow guards. Two story, masonry clad wings (lean-to’s), are located on the north and south sides of the building. Their roofing type is a TPO membrane system over insulation, over a concrete deck.

2.2.2 A roof plan with approximate TPO membrane seam locations, and diagrams with installation/repair procedure methods are included as Attachment D to this Statement of Work.

2.3 Roofing Membrane Parameters

2.3.1 Membrane: The membrane to be used as a patching material, shall be TPO (thermoplastic polyolefin) containing a reinforcing fabric scrim layer in compliance with ASTM D-6878-13 and shall have a thickness of 60 mils. Membrane shall be fungi resistant as demonstrated by "non sustained growth" or discoloration after an exposure of 21 days as specified in ASTM G 21. The new membrane should be tan, to match the color of the existing roof membrane. TPO product shall be compatible with existing JP Stevens / Johns Mansville 80 mil Thermoplastic Polyolefin EP membrane. Acceptable product manufacturers include; Johns Manville, Carlisle, Goodyear or approved equal.

2.4 TPO Membrane Repair/Installation

2.4.1 The repair will be performed on all seams at the entire barrel roof. The installation shall be a single ply with heat welded seams. An eight (8) inch wide TPO membrane patch will be used for the seam repair.

2.4.1 The Contractor shall perform the membrane repairs as indicated in this Statement of Work, in accordance with manufacturer’s requirements and recommendations, and following the guide specifications included in Attachment B.

2.5 New Permanent Fall Protection System Parameters

The integration of the New Permanent Fall Protection System (FPS) should encompass:

2.5.1 The Design of a FPS suitable for construction, inspection, and maintenance operations at Hangar #3, based on the provisions and requirements stated in the Unified Facilities Criteria (UFC), Unified Facilities Guide Specifications (UFGS) and Occupational Safety and Health Administration (OSHA). The design of the new FPS shall comply with USACE/ EM 385-1-1 and should encompass a full integrated personal fall protection equipment, systems, subsystems, and components. The FPS must comply with the provisions and requirements of American National Standards Institute, ANSI/ASSE Z359 Fall Protection Code, ANSI/ASSE A1264.1 Standard and DOL - 29 CFR Part 1910, Subpart D.

2.5.1.1 The material to be used for the fall protection equipment shall provide for metal compatibility in order to minimize corrosion, acceptable materials include Type 316 Stainless Steel.

2.5.2 The Installation at the barrel roofs on Hangar #3 of permanent anchorages (hard points) for safe use of fall arrest equipment and systems.

2.6 General Requirements

2.6.1 The Contractor shall perform site preparation, erect safety barricades, construction, restoration, repair, enhancement, maintenance, demolition, site clean-up, and demobilization of facilities off the sites as required for this work.

2.6.2 The Contractor shall furnish all temporary materials and equipment such as scaffolds, stair towers at each side of the hangars, staging, lifts, etc., to safely access and accomplish the construction works and inspection activities, as well as performing pre-proposal investigative tasks, as required for this work.

2.6.2.1 Contractor will provide non-rolling scaffold stair tower which will include at minimum walk-thru scaffold frames with appropriate cross braces, adjustable screw jacks with base plates, internal stair unit, outside and inside handrail for internal stair unit, steel, hook-on scaffold decks, guard rail posts and rails, and end panels.

2.7 Special Qualifications

2.7.1 Documentation and information including firm name, individual’s name and qualifications and experience shall be submitted to the CO within ten days of the Notice to Proceed to demonstrate compliance with the following requirements:

2.7.1.1 The Prime Contractor shall have personnel on staff with experience in similar projects and shall be assigned to administer this work.

2.7.1.2 The installation shall be performed by a Contractor with factory certification by the manufacturer of the systems. Contractor shall be qualified in the planning and/or installation and/or the construction management of systems similar to this project in the last five years. The Contractor shall provide a listing of similar past projects with a brief description of roles and responsibilities, location and contact information for reference.

2.7.1.3 The Contractor shall retain the services of a Registered Roof Consultant (RRC) to oversee and approve all aspects of the TPO membrane installation procedures submittal, prior to submission for Government review.

2.7.1.4 The General Contractor shall also retain the services of an Independent Registered Roof Observer (RRO) to provide full time on site monitoring and Field Quality Control of the work for compliance with approved TPO membrane submittal and roofing manufacturer's installation requirements.

2.7.1.5 The Contractor shall retain the services of a Licensed Structural Engineer to oversee and approve all aspects of the permanent FPS drawings and specifications prior to submission for Government review.

2.7.1.6 All planning and engineering work, including shop drawings, must be accomplished by or under the supervision of an engineer licensed or registered in the United States of America or its territories.

2.8 Applicable Documents

2.8.1 The Contractor shall identify and comply with all applicable federal, state, and local statutes; Department of Defense and/or Air Force instructions, manuals, handbooks, regulations, guidance, policy letters, and Executive Orders (EOs). It is the Contractor’s responsibility to identify and comply with all applicable requirements.

2.9 Project Activities

2.9.1 Project Activities shall begin after the issuance of a Notice to Proceed (NTP) by the Contracting Officer (CO). No physical work will be allowed to commence on the site until after the Work Plan and the Fall Protection Design are submitted and approved through the Contracting Office. During installation activities, the Contractor shall be responsible for:

2.9.1.1 Ensuring subcontractor performance and WP compliance including assurances on construction quality by providing construction management; complying with Base procedures regarding security, access and safety.

2.9.1.2 Conducting progress meetings on a weekly basis with Base personnel.

2.9.1.3 Providing the Base and the COR with weekly project status reports including photo documentation.

2.9.1.4 Ensuring safe work practices are employed; construction management.

2.9.1.5 Producing as-built drawings.

3.0 GENERAL PLANNING AND SUBMITTAL REQUIREMENTS

3.1 The Contractor shall provide efficient management of the work including accurate, on-time submittals of contract deliverables and timely identification and solution of impediments for a successful project execution. Technical requirements include early involvement in the process to allow for the development of the most cost-effective and technically sound solution. WARB will rely on the Contractor’s expertise in recognizing and addressing problematic issues and successful execution of this work. The Contractor shall perform all work in accordance with federal, state, and local statutes and regulations.

3.2 The Contractor shall perform all services required to complete investigations, construction plans and specifications related the new FPS. To meet the contract requirements, the Contractor shall prepare and furnish complete technical calculations, survey results, sketches, contract drawings, technical specifications, analyses, construction schedules, submittal schedules, and as-built drawings as required for the accomplishment of the project scope in accordance with the requirements. The Contractor shall provide all drawings, complete details, material samples, specifications, manufacturers/suppliers product data, standard color charts/catalogues for proposed materials/finishes as applicable, and the proposed work schedule, for the WARB review and approval prior to commencement of any work. Construction drawings and specifications shall be submitted for review and approval at 100% development stage. Construction will not begin until the Contracting Officer and WARB have approved the WP and planning documents.

3.3 Pre-proposal Investigative Requirements

3.3.1 The Contractor shall perform all site visits, surveys and investigations necessary to satisfy his/herself of all project conditions and requirements, existing conditions of the facility and material quantities necessary to accomplish the general intent of repairing the TPO roofing membrane and installing a new permanent FPS at the facilities. The Contractor’s services shall include, but not be limited to, any field investigations or surveying required for the accurate preparation and inclusiveness of the cost proposal, the construction plan and the ultimate completion of the TPO repair and FPS installation works.

3.3.2 The Contractor shall investigate the accuracy of all drawings and other as-built records applicable to the project scope and update them as necessary. The Contractor shall be responsible for physically investigating all accessible areas, including spaces above existing suspended ceilings as required. Non-accessible areas can be opened by Base personnel with prior coordination if the Contractor determines that the inspection is necessary to properly define the construction plan. The Contractor shall provide the Contracting Officer 14-day advance written notice prior to any scheduled inspection of concealed spaces which require government assistance.

3.4 Drawings

3.4.1 All contract drawings to be furnished shall be well prepared, complete, and accomplished in accordance with the best professional practice to show clearly and concisely the type and extent of work to be performed. The drawings shall be prepared in AutoCAD and dimensioned completely and accurately. Drawing border sheets shall be placed in a reference file using appropriate views to view the work shown. Each sheet or drawing shall include a graphic scale bar to indicate the appropriate scale for the work shown. Drawing dimensions shall be in English units. Standard fonts, material indications, and symbols for mechanical and electrical equipment shall be used. Project title sheets shall have an index that lists the sheet number, sheet name, and CAD file name.

3.4.2 Drawings shall be accomplished using AutoCAD. Drawings shall be accomplished on 24" X 36" D-size sheets. The drawings shall be legible when reduced to one half the original sizes. Design drawings will be on paper. As-built drawings will be on Mylar (1 set) and electronically in both AutoCADD and PDF format.

3.4.3 As-Built Drawings shall be on Mylar (1 set) and electronically in both AutoCADD and PDF format.

3.5 Technical Specifications

3.5.1. Specifications, including all technical and special conditions, shall be prepared by the Contractor utilizing the project specific specifications included as Attachment B. which contain the minimum requirements for the execution of this project. This Section includes the following:

Section 07 54 23.10 0220, Thermoplastic Polyolefin (TPO) Roofing This Section pertains to the roofing repair work to be performed. For the New Permanent Fall Protection System, the Contractor shall develop additional sections as necessary to define all work requirements identified in the complete construction plans. The contractor shall integrate FPS project specific specifications which comply with the requirements of UFGS-01 35 26 and USACE Safety and Health Requirements Manual EM 385-1-1. Additional sections for FPS shall comply with OSHA regulations and ANSI requirements.

3.6 Fall Protection System - Final Plan Submittal (100%)

3.6.1 For the 100% plan submittal, the Contractor shall provide those services necessary to prepare Construction Documents consisting of drawings, specifications, and other documents setting forth in detail the requirements for construction of the project for approval by the Contracting Officer. This work shall include, but not be limited to, the following:

3.6.1.1 Plan Analysis: An analysis describing the planning approach and recommendations including any assumptions and structural calculations used in the design of the new permanent FPS.

3.6.1.2 Drawings: Drawings shall be complete, indicate all work necessary for a complete project and include, but not be limited to roof plans, anchoring system layouts and standard manufacturer’s sections and details and those specially prepared by the Fall Protection manufacturer for any unique conditions that are encountered.

(a) Architectural – Comprehensive plans, sections and details showing the extent of existing material removal, repairs of existing TPO membrane and installation of new anchors as part of the new fall protection system installation. The plan shall establish the methods of attachments as recommended by the roofing and fall protection systems manufacturers.

(b) Structural – Identify all structural members required for the installation of the new fall protection system and address what measures must be taken during the installation the new fall protection system. Comprehensive plans, sections and details showing methods of attachments as recommended by the fall protection systems manufacturer.

(c) Mechanical and Electrical (if necessary) – Identify any existing mechanical and/or electrical equipment or devices that must be manipulated for the installation of the New Permanent Fall Protection System and address what measures must be taken to protect them during construction.

3.6.1.3 Construction Schedule: The Contractor shall provide a detailed construction schedule. The construction schedule shall show all work tasks, including material and equipment delivery timeframes, and indicate the anticipated critical path.

3.6.1.4 Specification: The Contractor shall provide technical specifications which identify all the work requirements based on those provided in this SOW. Additional specifications sections deemed necessary shall be based on; United Facilities Guide Specifications (UFGS) and be thoroughly edited and project specific.

3.6.1.5 Catalog Cuts: Complete package of catalog information, catalog cuts and manufacturer’s data for materials to be used as the basis for the plans and specifications, including the following: Roofing Membrane patching material, Fall Protection Anchoring System, Underlayment & Insulation replacement for any type of patching & repair, Installation Instructions, and Sample Warranties for TPO membrane and Fall Protection System. If any of the catalog cuts listed above are not used in the planning phase, the Contractor shall submit them during the construction phase as required in Paragraph 4.3, Product Data.

3.6.1.6 Contractor Review and Approval: The FPS submission shall be accompanied by a certification that the documents have been reviewed and approved by a Licensed Structural Engineer.

3.6.1.7 Project Disk: A single 650MB compact disk (CD) containing the following:

(a) All updated 100% AutoCAD drawing files, including complete As-Built drawings to reflect final constructed conditions.

(b) The entire corrected specification package for use on Microsoft Word or compatible software.

(c) Construction schedule

4.0 CONSTRUCTION EXECUTION

4.1 General Scope

4.1.1 The Contractor shall provide all labor, materials, and equipment to install, construct, and complete the work plan in accordance with Parts 2 and 3 of this Statement of Work. The Contractor shall also comply with WARB General Conditions.

4.1.2 The facility will remain occupied and operational during the performance of the work. The Contractor shall execute all measures and take every precaution to prevent moisture infiltration to damage uncompleted work or Government property. Necessary coverings or water cutoffs shall be in place at the end of each work shift. Any damage to a facility or its contents due to leakage during construction shall be repaired or replaced in a manner approved by the Contracting Officer at no additional cost to the Government.

4.2 Field Quality Control

4.2.1 The contractor is responsible for Quality Control, and technical review for all construction services provided under the contracted work.

4.2.2 The Contractor shall obtain the services of a Registered Roof Observer (RRO) who will perform a quality assurance review independent of all other quality control procedures. This may include requiring the contractor to furnish proof of their quality control measures and records of their Contractor Quality Control (CQC) program.

4.2.2.1 The RRO shall not be an employee of the contractor or sub-contractors.

4.2.2 The RRO will provide on-site monitoring and Field Quality Control of the work for compliance with drawings, specifications, and roofing manufacturer's installation requirements.

4.2.2.1 The RRO shall oversee and verify field quality control (QC) sampling by the construction Contractor in accordance with the construction specifications for the project. The construction Contractor shall submit all QC test results on a daily basis or as the tests are performed.

4.2.3 The RRO shall be on site any time roof construction is being performed, for the duration of the project. His primary duties are to provide Quality Assurance and insure the contractor and subcontractor’s compliance with the contract documents.

4.2.3.1 The Observer shall provide direct oversight of all testing and surveys performed by the construction Contractor or their subcontractor.

4.2.3.2 The Observer shall review inspections of roofing sections and joint/seam preparation and sealing/welding to determine that the materials and installation meet the requirements specified.

4.2.3.3 The Observer shall witness the field tests performed by the TPO and FPS manufacturers as part of the Warranty process. The dates of the inspection, problems identified, sections inspected/tested, and results of all tests shall be documented and provided in a written report within 7 calendar days of the project completion. Provide the written report in a format approved by the CO.

4.2.4 The RRO will supply recommendations and reports directly to the Government.

4.2.4.1 The RRO shall provide daily reports to the Government that shall contain :

(a) daily weather conditions,

(b) number of workers on site,

(c) list of visitors,

(d) reports of any accidents or incidents,

(e) type of work performed,

(f) amount of roof area repaired,

(g) amount of anchor installed, and

(h) the contractors compliance with specifications and details

4.2.4.2 The RRO shall take daily photographs (a minimum 12 photos) of every type of activity that shall include:

(a) tear off,

(b) cleaning,

(c) application of roofing membrane patches,

(d) installation of permanent fall protection anchor points,

(e) material storage/handling, and

(f) compliance with safety requirements.

Photos may be digital but one hard copy shall be made DAILY and kept on site.

4.2.5 The Contractor shall also arrange for a manufacturer's technical representative to visit the site during the installation for purposes of reviewing materials installation practices, adequacy of work in place and warranty on original membrane.

4.3 Product Data - New Fall Protection System

4.3.1 For the New Permanent Fall Protection System, furnish a typewritten information card for facility records and a photo engraved 0.032 inch thick aluminum card for exterior display. Card shall be 8 1/2 by 11 inches minimum. Information card shall identify facility name and number; location; contract number; approximate roof area; detailed fall protection system description, including maximum load capacity per anchorage; date of completion; installing Contractor identification and contact information; fall protection manufacturer warranty expiration, warranty reference number, and contact information. Install card at roof top or access location as directed by the Contracting Officer and provide a paper copy to the Contracting Officer. 2 signs will be required, one on each side of the bldg. Location TBD.

4.3.2 Submit catalog cuts and/or other manufacturer’s data for information and record purposes only. Submit information on the following products as required and in accordance with, General Conditions, if not submitted as a part of the plan submittals:

(a) Roofing Membranes

(b) Fall Protection Anchors

(c) Underlayment

(d) Insulation Systems

(c) Adhesives

(d) Installation Instructions

(e) Sample Warranties

5.0 WORKSITE ACTIVITIES AND COORDINATION

5.1 Coordination of Activities

The Contractor shall coordinate worksite activities with all applicable Base personnel to ensure the protection of human health and the environment; the prevention of damage to property, utilities, materials, supplies, and equipment and the avoidance of work interruptions. The Contractor shall provide physical security to work areas with security equipment and personnel. The Contractor must comply with Occupational Safety and Health Administration (OSHA) safety and health regulations and local safety office requirements. The Contractor is required to provide the CO copies of any OSHA report(s) submitted during the duration of the work.

6.0 PRE-FINAL INSPECTION

6.1 The Contractor shall conduct a pre-final walk through inspection to assess the conformance of the work on each roof to this SOW and Final WP. The Contractor shall publish the pre-final inspection findings in a pre-final inspection report.

6.2 Manufacturer’s Representative

6.2.1 In addition to presence of the Registered Roofing Observer, representatives or technicians from the roofing manufacturer shall also be present to inspect the installations.

6.3 TPO Membrane Pre-Final Inspection Report

6.3.1 Written inspection reports from both the roofing observer and the manufacturer’s representative shall be prepared and submitted. These reports shall include detailed findings of any and all deficiencies with recommended corrective measures.

7.0 FINAL INSPECTION

7.1 The Contractor shall conduct a final inspection and publish the findings in a final inspection report in accordance with the format specified at the pre-construction conference. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final report. The final inspection report shall (1) Certify that all items of the plan have been implemented and that the construction is complete including the Certificate of Inspection by the roofing and fall protection manufacturer; (2) As-built record drawings and specifications verifying that all developmental standards of the plan have been met. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer, and Acceptance of Real Property to the Base Civil Engineer (BCE) upon project completion. (see submittal register)

7.2 Certificate of Inspection by Permanent Fall Protection Manufacturer

7.2.1 Concurrent with the Final Inspection Report, provide certification by the permanent fall protection manufacturer that the fall protection systems are installed in accordance with the manufacturer’s requirements.

7.3 Close-Out Submittals

7.3.1 As-built drawings reflecting final constructed conditions and specifications set for the completed roofing and fall protection work.

8.0 PHASING

8.1 The work at this facility shall be commenced as soon as possible and be diligently prosecuted through its completion, inspection, and acceptance.

8.2 The Contractor shall not begin any construction or perform any other physical work at the construction site until all material submittals and methods of installation are received and approved by the Contracting Officer and all approved materials have been delivered to the Base or are immediately available for delivery as the work progresses. The Contractor must coordinate all materials, equipment, and Subcontractors so as to minimize the duration of the work.

9.0 SITE WORK

9.1 Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, monitoring and other types of wells, and the general site environment. This includes the protection of aircraft and equipment not in the affected zone from dust damage, and physical contact with machines and equipment.

10.0 SPECIAL REQUIREMENTS

10.1 The east elevation of the hangars is immediately adjacent to the flight line which is a restricted area of the Base. Contractor access to the work by both personnel and equipment will only be allowed from outside the restricted area.

10.2 Being adjacent to the flightline, the work site can be periodically considered as a very high noise area. Workers shall be equipped with necessary personal protective equipment.

10.3 The hangar can be subjected to high winds. The Contractor shall prevent the blowing of dust, debris and roofing materials from the roof at all times. Again, being adjacent to the flightline and airfield, blowing debris in the vicinity of parked or operating aircraft cannot be tolerated.

11.0 GOVERNMENT FURNISHED MATERIAL (GFM)

11.1 There are no materials, equipment, or supplies to be furnished by the Government for the performance of the work.

11.2 In conjunction with the project, Westover Air Reserve Base, Massachusetts, will provide the following support to the Contractors as detailed below:

11.2.1 Provide assistance to the Contractor in obtaining existing engineering plans, drawings, etc., to facilitate work required in the Statement of Work (SOW).

11.2.2 Arrange for a staging area for storing equipment and supplies. The security of equipment and supplies shall be the Contractor’s responsibility. The Base COR will provide a limited area for temporary storage of material and equipment which will be determined and mutually agreed at the Pre-construction Meeting. Contractor’s stored material and equipment shall at no time interfere with the ongoing operations of the Base.

11.2.3 Arrange for personnel identification badges, vehicle passes and/or entry permits with 4 days or more advance written notification. Request will be on 439 AW Form 1 to include: Company name, contract number, Contractor personnel names, citizenship, city and state (country, if not U.S.) of birth, Date of Birth, social security numbers, and duration of access required. Subcontractor requests must also include the name of the Prime Contractor and Prime Contract Number.

11.2.4 Arrange access to the work areas. The Base COR will provide access to the Base as required by the Contractor and the Subcontractors per the established Base security requirements.

11.2.5 Arrange for base CE and COMM to coordinate with the Contractor.

11.3 Drawings, Reports and Plans: Copies of drawings, which the Contractor shall update to reflect existing conditions, are available for use in Building 2450, Base Civil Engineering. The Contractor shall field verify all dimensions and conditions shown on any Government furnished drawing to ensure that all data and information presented, enumerated or noted within the drawings and/or specifications developed for this project are completely accurate in every regard.

12.0 WARRANTY

12.1 The Contractor shall complete all inspection and commissioning requirements prior to final inspection. The required warranties shall be issued in accordance with FAR

52.246.21 and as specified in individual technical specification sections.

12.2 Manufacturer's standard written no cost limit, 10 year warranty on materials and Contractor’s 5 year warranty on workmanship shall both be furnished. Warranty shall provide for repair or replacement of the membrane system, including other failures caused by defects in materials or workmanship. Refer to the technical specifications included as Attachment – B for specific requirements.

12.3 Furnish complete written warranty for the installation of the Fall Protection System.

12.4 Extended warranties offered by the Contractor and its Subcontractors or suppliers may be accepted at the Government’s discretion.

1.1 TEMPORARY PROTECTION

A. The Contractor shall protect all workmen, civilian and military personnel and the general public from injury. The Contractor shall coordinate and schedule all work with the Contracting Officer.

B. The Contractor shall conduct all work so as to cause the least interference possible with the normal activities of the operations of the airfield and surrounding areas.

1.2 SECURITY

A. All project work and conduct of construction activity must comply with all Base security regulations and policies.

B. Prior to any workers arriving at the site, the Contractor shall prepare and submit a 439 AW Form 1 requesting Identification Badges for all employees. Identifications Badges are to be obtained by submitting a letter to the Contracting Officer with the names of all the Contractor's employees including subcontractor personnel requiring badging. This listing shall show the project name and YTPM number, contract number, the full name, address, and social security number of all employees, and the Contractor's full address and telephone number. Badges shall be worn and be visible at all times while on the Base.

C. The Contractor shall be responsible for the return of all Identification Badges to the Contracting Officer after completion of the project. Final payment will not be made until all badges are returned.

D. The Contractor shall enter the project site only at a location approved by the Contracting Officer. Restrict all activities and movement of workers, supplies, and equipment to the designated project area and haul routes.

E. The Contractor shall not allow cameras on the project site nor photographs to be taken without written permission of the Contracting Officer.

1.3 UNDERGROUND UTILITIES, UTILITY INTERRUPTIONS AND SHUTDOWNS

A. The Contractor shall submit a notification prior to excavation or any ground disturbing activity. The notification shall be Form 103, BCE Work Request (Dig Permit) and shall be submitted to the Contracting Officer at least 30 days prior to the planned excavation. The Contractor shall obtain approval prior to excavation. The government will mark out known existing underground utilities in the work area. The marked location of the existing utilities is approximate. Excavation by power driven equipment is not permitted within three (3) feet of either side of the marked utility. Hand excavate on each side of the indicated obstruction and continue until uncovered or clearance for the new grade is assured.

B. Report damage to underground utilities or subsurface construction immediately to the Contracting Officer. Damage to marked utilities will be satisfactorily repaired or replaced by the Contractor at no additional cost to the government. Damage to unmarked utilities will be repaired or replaced at the government’s expense. Should the government engage the Contractor to perform the repairs or replacement; an equitable adjustment in the contract amount will be negotiated prior to the repair or replacement being made. Repair is defined as permanent, code compliant measures as approved by the Contracting Officer.

C. Interruptions shall be defined as any Contractor operation that interferes with access through, or use of facilities or areas including utilities, Energy Monitoring and Control Systems (EMCS), parking lots, generators, roads, or airfields.

D. Request for an interruption shall be submitted in writing at least fifteen (15) days in advance of the date desired and includes at least one alternate date. Include the following information: Location(s) of interruption(s), hours and dates of interruption(s), which services are affected and which facilities are affected. Contractor is cautioned that outages may not be granted on the date(s) requested. Contractor shall notify the Contracting Officer at least seven (7) days in advance of any cancellation of any scheduled interruption. Services shall not be interrupted until receipt of approval of proposed hours and dates are received from the Contracting Officer.

1.4 QUALITY ASSURANCE

A. All items of work not addressed in these General Conditions shall be completed in strict accordance with the manufacturers’ specifications.

B. The quality assurance procedures of this section are a minimum. Provide additional quality control as required by the Contracting Officer. Any additional quality control, which may be required, shall not constitute a change in the contract.

C. The government is not obligated to inspect the Contractor’s work, or to protect the Contractor from the consequences of his work. Government inspection is a general examination of the Contractor’s conduct and workmanship and is solely for the purpose of the government. Government inspectors do not have the authority to accept work, nor is government inspection to be construed as conclusive.

D. Government agents including inspectors, engineers and quality assurance evaluators are not authorized to change the contract without the written approval of the Contracting Officer; this lack of authority extends to all situations in which the action of these agents could be construed as constituting a change.

E. The quality of workmanship is subject to audit by government or government-designated inspectors at any time during the contract. The Contractor shall cooperate fully and provide all information necessary for this audit.

F. The Contractor shall institute immediate action to correct all variances from the contract or General Conditions and employ all procedures necessary to ensure future work shall conform to the requirements of the contract.

G. The Contractor shall submit all requests for changes in writing to the Contracting Officer. Do not proceed with changes without possession of written authorization of the Contracting Officer.

1.5 CONCEALED WORK

A. The Contracting Officer must approve all items of work including those which are to be concealed. The Contractor shall notify the Contracting Officer of his intention to conceal work 24 hours in advance of concealment.

1.6 SUBMITTALS

A. Approval must be obtained from the Contracting Officer for the material and equipment proposed to be incorporated into the work. Submittals shall be made no later than 30 working days after receipt of the Notice to Proceed or as specified by the Contracting Officer. The Contractor is to complete Air Force Form 3000, provided by the Contracting Officer, and submit the .pdf of the required materials for review. The Contracting Officer will return the submittals approved or disapproved within 10 working days after receipt, by AF Form 3000. If submittals cannot be made within 30 days after Notice to Proceed or as specified by the Contracting Officer, the Contracting Officer shall be advised of the difficulty and the Contractor shall request an extension of time for this task. Approval of submittals does not relieve the Contractor from performance in strict accordance with the contract or Statement of Work, nor does such approval constitute “acceptance” under other conditions of this contract.

B. Any and all “or equal” items or materials proposed by the Contractor to be used in the project shall be submitted for approval.

C. Items required to be submitted for approval are listed in the Statement of Work, and are a minimum.

1.7 SUPERINTENDENCE BY THE CONTRACTOR

A. At all times during the performance of the contract and until the work is completed and accepted, the Contractor shall directly superintend the work or assign and have on the worksite a competent Superintendent who is satisfactory to the Contracting Officer and has the authority to act for the Contractor.

B. The Superintendent is to oversee and coordinate all project related work and is required to be on site at any and all times physical work is being performed. The Superintendent shall be able to read, write, speak and understand the English language.

C. Failure to comply with this requirement may be deemed as cause for a non-compensatory stoppage and suspension of work until the deficiency is remedied.

1.8 NOTIFICATION PRIOR TO CONSTRUCTION OR DEMOLITION

A. In accordance with the requirements of the Massachusetts Department of Environmental Protection (MADEP), the Contractor shall prepare and submit form BWP AQ 06, Notification Prior to Construction or Demolition, to the Department on behalf of the government. The Contractor is also responsible for the remittance of the $100 notification fee, or all other required fees. Complete information is available from the Department or on the DEP Web site at mass.gov/dep.

1.9 STORAGE AND HANDLING

A. All new materials shall be transported, stored and handled according to the manufacturers’ written recommendations or as otherwise directed by the Contracting Officer.

1.10 SCHEDULING

A. GENERAL

1. All work shall be carefully scheduled to prevent any excessive shutdown time of utilities or services. The Contractor shall make written request to the Contracting Officer for approval of all work requiring shutdown 48 hours prior to commencement of such work.

B. PRE-CONSTRUCTION SCHEDULE REQUIREMENT

1. Prior to the start of work, prepare and submit to the Contracting Officer a Baseline Construction Schedule in the form of a Bar Chart Schedule in accordance with the terms in Contract Clause FAR 52.236-15 Schedules for Construction Contracts, except as modified in this contract. The approval of a Baseline Construction Schedule is a condition precedent to:

a. The Contractor starting demolition work or construction stage(s) of the contract.

b. Processing Contractor's invoice(s) for construction activities/items of work.

c. Review of any schedule updates.

2. Submittal of the Baseline Construction Schedule, and subsequent schedule updates, is understood to be the Contractor's certification that the submitted schedule meets the requirements of the Contract Documents, represents the Contractor's plan on how the work will be accomplished, and accurately reflects the work that has been accomplished and how it was sequenced (as-built logic).

C. SCHEDULE FORMAT

1. Bar Chart Schedule: The Bar Chart must, as a minimum, show work activities, submittals, Government review periods, material/equipment delivery, utility outages, on-site construction, inspection, testing, and closeout activities. The Bar Chart must be time scaled and generated using an electronic spreadsheet program.

2. Schedule Submittals and Procedures: Submit Schedules and updates in hard copy and on electronic media that is acceptable to the Contracting Officer.

D. CORRESPONDENCE AND TEST REPORTS:

1. Correspondence (e.g., letters, Requests for Information (RFIs), e-mails, meeting minute items, Production and QC Daily Reports, material delivery tickets, photographs) must reference Schedule Activities that are being addressed. Test reports (e.g., concrete, soil compaction, weld, pressure) must reference Schedule Activities that are being addressed.

E. ADDITIONAL SCHEDULING REQUIREMENTS

1. Any references to additional scheduling requirements, including systems to be inspected, tested and commissioned, that are located throughout the remainder of the Contract Documents, are subject to all requirements of this section.

1.11 DAMAGED PROPERTY

A. Work shall proceed in a manner which will minimize disturbance or risk of damage to surrounding structures, interior finishes, room furnishings, personal belongings, equipment or base infrastructure. The Contractor shall repair such items damaged by the Contractor in the course of carrying out the work at no additional cost to the government. All repairs shall match adjacent similar existing items in all aspects. All replacements will be in kind.

1.12 CLEANING UP

A. The Contractor shall at all times, keep the premises free from accumulation of waste materials or rubbish caused by work under this contract. All parts and equipment removed for replacement become the property of the Contractor and shall be removed from the premises. At the end of each work day and upon the completion of all work, the Contractor shall leave interior and exterior property in the condition in which it was found at the beginning of work.

1.13 SAFETY PRECAUTIONS

A. The Contractor shall take all necessary precautions in observing safety regulations in accordance with all Federal OSHA regulations, and shall assume the responsibility to guard against causing of fires and/or explosions and to protect government property. The Contractor shall perform the work in a manner consistent with security and with fire safety regulations especially with regard to exits and exit way access. Temporary closures shall not compromise security or fire safety.

1.14 ENVIRONMENTAL PROTECTION

A. There is no known or anticipated asbestos, lead nor any type of regulated hazardous material involved with the work of this project. However, in order to provide for control of all environmental pollution arising from construction activities, the Contractor and his subcontractors, in the performance of this contract, shall comply with all applicable federal state and local laws and regulations concerning environmental protection pollution control and abatement.

1.15 HAZARDOUS MATERIAL REPORTING

A. The Contractor shall maintain hazardous material inventories and material safety data sheets (MSDS) for all hazardous materials (as defined in CFR 1910.120, 40 CFR’s 355, 370, &

372) to be stored and used on this installation. Hazardous materials must be inventoried when received and at project completion. The amounts used shall be maintained for the project duration, and for the calendar year (ending 31 December).

B. Hazardous Materials Inventories, Material Safety Data Sheets and material quantities used shall be submitted to the Contracting Officer for approval utilizing 439 AW Form 20, CONTRACTOR INFORMATION SHEET, at the end of this Section. All hazardous materials shall be manifested, and prior to any hazardous materials leaving Westover Air Reserve Base, the manifest shall be signed by Base Environmental Engineering,

(413) 557-3036, (not the Contracting Officer’s Representative). The Contractor shall schedule (minimum of five days) all manifests with the Contracting Officer for proper coordination.

C. In the event of a spill, Contractor shall immediately notify the Base Fire Department at extension 911 as well as the Contracting Officer. The Contractor shall be solely responsible for the expense of any cleanup of such spill, and the cleanup shall be in accordance with the applicable provisions of 40 CFR Part 761.

1.16 HAUL ROUTES

A. The established haul access routes shall be the only routes used by the Contractor. The Contractor shall ensure that all vehicle operators associated with the project, or those of suppliers, are properly instructed before entering the Base.

1.17 TEMPORARY STAGING AND CONSTRUCTION AIDS

A. The Contractor shall provide, erect, maintain, and remove all necessary construction aids that are required for the performance of the work.

1.18 TEMPORARY UTILITIES

A. All reasonable required amounts of water and electricity essential to contract performance will be made available at no cost to the Contractor from existing systems, outlets and supplies. The water supply at the Munitions Complex is non-potable. Any temporary connections shall be in strict accordance with applicable OSHA standards and in compliance with all Health and Safety Codes and Regulations. The Contractor shall carefully conserve any utilities furnished without charge. Abuse of this service will result in denial of free use and will be subject to charges at market rates.

B. No telephone or data outlets are available for the Contractor's use. All services for desired communications will be arranged and paid for by the Contractor.

C. The Contractor shall provide sight-proof chemical sanitary toilet(s). The Contracting Officer will determine location. Chemical toilets will be subject to Government inspection by the Base Medical Officer. All sanitary deficiencies shall be corrected within 24 hours of the inspection.

1.19 CONSTRUCTION/DEMOLITION WASTE MANAGEMENT PLAN AND REPORTING

A. Prior to the start of the work, the Contractor shall prepare and submit for approval the Construction Waste Management Plan utilizing the form provided at the end of this section.

B. The Contractor shall record, document and report the total amount of construction/demolition debris and other solid waste that is generated and disposed of under this contract. The Contractor shall also record the total amount of debris and waste that has been diverted through sale, reuse, or recycling and the quantity of waste disposed by landfill or incineration. The waste shall be recorded by weight, but volume in cubic yards will also be acceptable.

C. Contractor will salvage and reuse the construction/demolition debris to the greatest extent possible. Waste materials that cannot be salvaged and/or reused and have value as being recyclable, shall be recycled. Only materials unable to be economically salvaged or recycled shall be transported to a landfill or incinerator. Revenues or other savings obtained for recycling or returns shall accrue to the Contractor, except revenues generated from Base recycling activities.

D. The records shall be made available to the Contracting Officer during construction and shall be submitted to the Contracting Officer prior to project closeout utilizing the Waste Management Summary Form also attached at the end of this section.
