About this file
This is a Past Performance Questionnaire template used to evaluate contractor performance for the 36th Wing Shuttle Transportation and Vehicle Rental Services solicitation (FA7000-10-R-0036) at Andersen Air Force Base, Guam. The questionnaire serves as a template for collecting historical performance data from contractors bidding on or referenced for the Blanket Purchase Agreement (BPA) for shuttle transportation services.
The questionnaire requires contractors to provide detailed past performance information across six evaluation criteria: Quality of Service, Schedule, Cost Control, Management of Key Personnel, Small Business Subcontracting, and Regulatory Compliance. Each criterion uses a five-point adjectival rating scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory), with mandatory written justification required for any rating of Marginal or Unsatisfactory. Respondents must complete the form with general information including contractor name, contact details, project descriptions, contract numbers, overall contract values, performance dates, and prime or subcontractor status. Completed questionnaires were to be submitted via email to the 36th Contracting Squadron points of contact (elena.gifford.1@us.af.mil and thomas.houle.4@us.af.mil) no later than 08 September 2026 at 10:00 AM Chamorro Standard Time. This document is a template and does not specify required products, services, pricing, or award information.
