About this file

This file is a Past Performance Questionnaire template for the Blanket Purchase Agreement (BPA) for Shuttle Transportation Services at Andersen Air Force Base, Guam. As an appendix to the solicitation, this template does not address specific required products or services but rather serves as an evaluation tool for assessing contractor qualifications. The questionnaire would be completed by offerors to demonstrate their prior experience and capability to perform shuttle transportation services. This document functions as part of the overall evaluation criteria for the BPA, which is being established by the 36th Contracting Squadron to provide nonpersonal transportation services for the 36th Wing and mission partners at Andersen Air Force Base in Yigo, Guam, supporting personnel travel to various locations including short-notice deployments, exercises, and contingency operations. Questions regarding the solicitation must be submitted by September 1, 2026, at 10:00 AM ChST.
