SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS

The following have been modified:

PROPOSAL PREP INSTRUCTIONS

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, in addition to those identified as evaluation factors.

Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two (2) separate parts; Part I - Price Proposal, Part II - Past Performance Proposal. Past Performance Proposals shall be sent through U.S. Army Aviation and Missile Research Development and Engineering Center (AMRDEC) to robert.peters.11@us.af.mil and jonathan.roddy@us.af.mil. AMRDEC instruction are detailed below in section C(4)(a).

B. Specific Instructions:

1. PART I – PRICE PROPOSAL

(a) Reserved.

(b) Complete blocks 14, 15, 16 and 20A, 20B, 20C of the RFP Section A, SF1442. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K.

(c) Complete block 19 of the RFP, SF 1442, if any solicitation amendment(s) to the original solicitation documents was/were issued. Amendments will not be counted against the page limits.

(d) Complete the Price Schedule (Attachment 5) for the Seed Project. The Price Schedule shall be the only place in the RFP that a price will be written. Section B shall not be filled in. This section will be filled in by the government if an award is made.

(e) Complete the necessary fill-ins and certifications in Sections K. Section K shall be returned in its entirety. For Sections C through I, the offeror shall submit only those pages that require a fill-in.

(f) Complete the Financial Data Form (Attachment 8).

(g) Scanned copy of the bid guarantee in this part and according to FAR 52.228-1 (Section I of solicitation).

(h) Provide a copy of the letter from the Small Business Administration (SBA) showing proof of 8(a) BD status with a bona fide place of business in Alaska.

(i) If none proposed, submit an Affirmation of no Exceptions, Deviations, or Waivers.

(j) Extreme care should be exercised to ensure that no price data of any kind are included in any other Part of the proposal.

2. PART II – PAST PERFORMANCE PROPOSAL

(a) Quality and Satisfaction Rating of Contracts: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) that demonstrates customer satisfaction with overall job performance and/or quality of completed and/or ongoing contracts. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems, such as cost overruns, extended performance periods, numerous warranty calls, etc. (See Section M, paragraph B)

(b) Performance Surveys/Questionnaires: Past Performance Questionnaires (PPQs) shall be used by the offeror in obtaining and providing past performance information. The government requires the offeror send out a PPQ to each of the Points-of-Contact (POC) identified in the Past Performance Proposal (paragraph (c) below).

The offeror shall send out the PPQs (Attachment 7), using the Past Performance Evaluation Cover Letter (Attachment 6), to each of its references.

THE RESPONSIBILITY TO SEND OUT THE PAST PERFORMANCE QUESTIONNAIRE RESTS

SOLELY WITH THE OFFEROR.

Once the questionnaires are completed by the POCs, the information contained therein will be considered source selection sensitive and will not be released outside the Government. The POCs shall forward their completed questionnaires directly to robert.peters.11@us.af.mil and jonathan.roddy@us.af.mil. -NOT

BACK TO THE OFFEROR.

(c) Past Performance Information (PPI): A list of up to five (5) of the most relevant contracts performed for any customer will be entered by the offeror in the PPI Tool. Instruction for the tool are located in attachment ten (10) of this solicitation. (See Section M, paragraph B)

Furnish the following information for each contract listed:

(1) Company/Division name

(2) Product/Service

(3) Contracting Agency/Customer

(4) Contract Number

(5) Contract Dollar Value

(6) Period of Performance

(7) Verified, up-to-date name, mailing and e-mail addresses, and telephone number of the contracting officer (Point-of-Contact)

(8) Comments regarding compliance with contract terms and conditions

(9) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.

(10) Include rationale supporting your assertion of relevance and identify aspects (scope, magnitude of effort, and complexity) of the contracts deemed relevant and how they relate to the proposed effort.

If an offeror has no past performance history of relevance, the offeror must affirmatively state it possesses no past performance history of relevance.

(d) The evaluation of past performance information:

(i) will consider the experience or past performance of an offeror’s parent, sister or affiliated companies where the firm’s proposal demonstrates that the resources of those companies will affect the performance of the offeror.

(ii) will consider the experience of predecessor companies if these companies can be linked directly to the offeror in the proposal.

(iii) will take into account approved joint venture (13 CFR 124.513) past performance,

(iv) will not take into account past performance regarding subcontractors,

(v) will pay particular emphasis on those trades required to complete the project work to include the following:

(1) Demolition

(2) Design

(3) Mechanical/HVAC/Plumbing

(4) Electrical

(6) Hazmat/Asbestos/Abatement

(7) Roofing/Insulation/Thermal & Moisture Control

(8) Civil work

(e) Relevant Contracts: Relevant past performance information must demonstrate design/build and build experience with multiple disciplines (as stated above). Submissions must only be for individual projects not entire programs. If you currently have a similar award to this program you may only submit an individual project not every task order for that contract. Projects should be similar in scope, magnitude of effort and complexity that are typically represented in task orders issued under the current MACC. Descriptions of these contracts can be found at https://www.fpds.gov/fpdsng_cms/index.php/en/?pageSource=loginPage using the ezSearch option. Use the following contract numbers for your search:

Provide data for up to 5 (five) relevant contracts held within the last three years (from RFP release date) the offeror wants the Government to consider. Include rationale supporting your assertion of relevance.

Note that the Government will not consider performance on an effort that concluded more than 3 (three) years prior to the issuance date of this solicitation. Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate.

C. Documents submitted in response to this RFP must be consistent with the following:

1. Requirements of the solicitation (CLIN and SOW) and government standards and regulations pertaining to the SOW.

2. Evaluation Factors for Award in Section M of this solicitation.

3. If a Part in a proposal exceeds the specified page limit cited in the Part Content, the excessive pages will not be evaluated.

4. Format for proposal Parts I, and II shall be as follows:

(a) Instruction on how to use AMRDEC can be found at https://safe.amrdec.army.mil/safe/guide.aspx. All proposals shall be submitted using AMRDEC Safe Access File Exchange (Safe) (https://safe.amrdec.army.mil/safe/).

(b) Each Part shall contain a Table of Contents. Cover pages and Table of Contents will not be counted against the page limit. However, pages depicting tables, charts, graphs and figures will count toward the page limit. The number of copies and page limits of each part is specified in Table 1 below.

(c) The proposals will be on 8 1/2” x 11” paper, except for fold-outs used for charts, tables, or diagrams, which may not exceed 11” x 17”.

(d) A page is defined as one face of a sheet of paper containing information.

(e) All documents shall be single-spaced. Spacing characteristics are applicable to charts and graphs.

(f) Written entries shall not be less than 11 font size. Size of print is applicable to charts and graphs.

(g) Page margins shall be a minimum of 1 inch top, bottom, and each side.

(h) The proposal shall be submitted in Adobe Acrobat portable document file (.pdf) format.

The same format used for the initial proposal shall be used for the final proposal resulting from any discussions and/or updates, if applicable.

(i) Each Part shall be complete in itself and shall not reference information contained in other Parts. Page 1 of a Part is defined as the first page after the Table of Contents. All pages shall be numbered.

(j) Price data of any kind shall only be present in the Price Proposal Part.

(k) Elaborate formats, bindings, and color presentations are not desired or required.

D. General Information:

1. Late proposals will be processed in accordance with FAR 52.215-1(c)(3),“Submission, modification, revision, and withdrawal of proposals.”

2. FEDERAL HOLIDAYS: The following Federal Legal Holidays are observed by this base:

3. JOINT VENTURE AGREEMENTS: Joint Ventures are allowable on competitive 8(a) BD set-asides; however, the joint venture agreement must be received by SBA prior to proposal due date and approved before award of any resulting contract. If you are contemplating a joint venture on this project, you must advise your assigned SBA Business Opportunity Specialist (BOS) as soon as possible. It is also recommended that the agreement be submitted as soon as practicable to ensure compliance with established regulations. Any corrections and/or changes needed can be made only when your BOS has adequate time for a thorough review before the proposal due date. NO CORRECTIONS AND/OR

CHANGES ARE ALLOWED AFTER TIME OF SUBMISSION OF PROPOSAL.

4. ORGANIZATIONAL STRUCTURE CHANGE HISTORY: Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap"

As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
