1.0 DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment (other than those listed in Section 3 of this Performance Work Statement (PWS)) and labor necessary to ensure that wastewater treatment plant (WWTP) services are performed at Holloman Air
Force Base (HAFB) in accordance with all local, state of New Mexico, and Federal laws.
1.1. GENERAL WASTEWATER TREATMENT PLANT OPERATIONS. WWTP, as described in
Appendix “C” of this PWS, operations shall be conducted in order to provide continuous, cost effective, and efficient treatment of all wastewater delivered to the plant. The plant will operate 24 hours a day, 7 days a week. Such operations shall include general operation of plant equipment, valves and piping, sampling and lab analyses, waste and effluent disposal, and other related services. All operations shall be accomplished in accordance with the treatment plant operations manuals where applicable, see Appendix
B. Plant operations and effluent discharge practices shall comply with all applicable federal, state, and local regulatory standards, including the NMED Discharge Permit requirements, see Appendix E. In the performance of the operation and maintenance of the WWTP the contractor shall adhere to all applicable
Occupational Safety and Health Administration (OSHA), National Institute for Occupational Safety and
Health (NIOSH), Air Force Occupational Safety and Health (AFOSH), Holloman Fire Prevention Guide for Contractors, and state mandated requirements.
1.2. PREVENTIVE MAINTENANCE INSPECTION (PMI) PROGRAM. The contractor shall implement a preventive maintenance inspection program to help prevent and correct deficiencies with equipment items, minimizing breakdowns and service interruptions, extending component service life, and maximizing operating efficiency. This program should incorporate methods to monitor trends for normal, abnormal or unusual conditions that would indicate problems that could arise. The contractor’s
PMI plan shall include inspections and services of lubrication, minor adjustment, and minor repair of components. A completed PMI plan shall be submitted to the contracting officer (CO) for acceptance within thirty (30) calendar days after contract award. This plan shall include a 52 consecutive week schedule indicating specific maintenance/inspection items to be accomplished. At the end of each year, each equipment item within every system included in this contract will have been addressed. The PMI program is to be established based upon manufacturers' recommendations, handbooks, operating/service manuals, contractor expertise, and general engineering judgments. The submitted plan shall include equipment components to be inspected and maintained, inspection and maintenance techniques, inspection and maintenance frequencies, and reporting methodology. The contractor shall update schedules as necessary to reflect any changes in equipment inventory.
1.2.1. PREVENTIVE MAINTENANCE WORK. Preventive maintenance work (PMW) shall include services of lubrication, minor adjustment, and minor repair of components. The contractor shall correct all equipment deficiencies identified during preventive maintenance (PM) inspections subject to the limits specified in the following Corrective Maintenance paragraph (1.3.1). When possible, equipment deficiencies detected during PM inspections shall be corrected prior to departing the job site.
1.2.2. PREVENTIVE MAINTENANCE RECORDS. The contractor shall maintain PM records for each item of equipment included within this contract, as described in Appendix D. These records shall include descriptions of periodic maintenance, scheduled versus actual completion dates, and the person who performed the work. The contractor shall ensure the records are accurate and current.
1.3. REIMBURSABLE COSTS. The Government shall provide an annual allowance for reimbursable costs for the basic year and each additional option year. Government reimbursable costs shall apply to corrective maintenance repair/replacement of material, including labor, and sludge disposal only. The
Government reserves the right to add or remove funding unilaterally from the reimbursable line item.
1.3.1. CORRECTIVE MAINTENANCE. Corrective maintenance is work that is required to return a system or components to proper operating condition. It is maintenance and repair work that is 80 hours or
Attachment 1 Page 4 of 14 less in labor and $2,500 or less in materials. Corrective maintenance is required on a routine basis as a result of preventive maintenance inspections. The contractor's liability under the firm fixed-price portion of the contract shall be limited to 80 total labor hours for completion and not more than $2,500 in total direct material and labor costs per job per approval of COR. Government approved repairs and improvements exceeding these per-job labor hours and material costs will be covered under the reimbursable line item in the bid schedule. For jobs exceeding the $2,500 material/labor costs, only the
Contracting Officer (CO) has authority to approve repairs and improvements. If repairs and improvements exceed the total cost of the reimbursable line item, the Government must verify if funding is available before proceeding In Accordance With (IAW) 1.3.2. of this PWS. Contractor shall provide two, preferably three independent price quotes and/or written justification stating otherwise, to help determine price fair and reasonable.
1.3.2. GOVERNMENT APPROVED REPAIRS, IMPROVEMENTS, AND REPLACEMENTS.
The government may approve repairs, replacements, and/or improvements to the WWTP over the threshold listed in 1.3.1 and not covered by any other part of this PWS. These repairs, replacements, and improvements may be approved if it can be shown that they are necessary due to government requirement or fault such as negligence, WWTP design fault, oversight, etc. The contractor shall submit a written request using the Request for Information (RI) Request Form (Appx K) to the Government for each repair, replacement and/or improvement. If the CO approves the request, funds, if available will be added to the reimbursable contract line item. Contractor shall provide two, preferably three independent price quotes and/or written justification stating otherwise, to help determine price fair and reasonable.
1.4. SLUDGE DISPOSAL. Sludge disposal services shall be provided by the Contractor at a frequency sufficient to maintain clean and orderly collection sites. Sludge shall be routinely collected and transported to a properly classified disposal site, with location subject to Contracting Officer
Representative (COR) approval. Initial disposal will be to the closest approved landfill, not to exceed a distance of 300 miles from the WWTP. Sludge collection, identification and disposal practices shall be accomplished in accordance with the most current 40 Code of Federal Regulations (CFR) 503 (all), 40
CFR 261, 40 CFR 257 paragraph 30, and Title 20 of the New Mexico Administrative Code (NMAC), Chapter 9, Part 1, Subpart VII, Sections 701, 702, 703, 704, 709, 712, and Subpart XI, Section 1109, see
Appendix B. The Contractor shall provide all necessary laboratory services for sludge analysis prior to disposal of sludge. Upon receipt of sludge analysis results and prior to the disposal process, the
Contractor shall submit all results to the COR and the Civil Engineer Installation Management-
Environmental Office (CEIE) in order for the Government to certify the sludge meets the requirements for landfill application. The Contractor shall maintain records of all laboratory analyses on file at the
WWTP site and available for CO and COR review. Original records, receipts, manifests, and logs shall be the property of the Government. The Contractor shall be reimbursed under the reimbursable line item in the bid schedule for sludge disposal, to include all costs for sludge analysis, transfer, removal, transport and tipping fees to the disposal site. Reimbursable at cost plus New Mexico Gross Receipts Tax.
1.4.1. During the life of the contract, the Holloman AFB Sludge Disposal Plan (Appendix H) requirements may change with respect to sludge handling and testing. In such an event, the contractor shall provide all sampling and analysis required by the permit modifications. Additionally the contractor shall still be required to provide disposal of the sludge. However, Holloman Airforce Base (HAFB) reserves the right to remove as much sludge as deemed necessary for other purposes. Again, all costs associated with sludge handling and testing requirements shall be reimbursable.
1.5. SAMPLING AND LABORATORY ANALYSIS. The contractor shall provide sampling and onsite laboratory analytical services for process control and to support base civil engineer and regulatory agency operating requirements with the exception of other regulatory tests determined by Civil Engineer
Installation Management-Environmental Office (CEIE). Such sampling and testing procedures shall be accomplished in accordance with applicable operating permit requirements and conditions. Unless
Attachment 1 Page 5 of 14 otherwise directed, all sampling and laboratory analyses shall be accomplished as described in 40 CFR, Part 136, see Appendix B. The Contractor shall utilize laboratory analysis results as a method to track trends in the system and to aid in determining changes to the operational process. Keep a complete set of laboratory records for all laboratory tests to include: date and time of sampling, type of sample, name of sample, location of sample, test performed, and test results. Assemble results of such laboratory analyses into reports, to include Air Force Form 1462, see Appendix B, to conform to the procedures and requirements of the forms and publications and Appendix E NMED Discharge Permit and deliver to the
COR and CEIE for submittal to the appropriate regulatory agency within five (5) business days of each new month.
1.6. DOCUMENTATION. The contractor shall prepare all related correspondence and operating reports. All documentation, records, schedules, etc., as described in this PWS that are the responsibility of the Contractor are the property of the Government and shall remain so even upon termination of the contract. The Contractor shall keep these items current at all times. Documentation and records shall be turned over to the Government upon completion of the contract or when requested by the COR. The
Contractor shall maintain on site records of all quality control inspections conducted by contractor personnel. Records shall include the name of the inspector, date of inspection, what was inspected, discrepancies found, corrective action taken, and date of corrective action. This documentation shall be made available to the Government during the term of the contract and at regular COR visits to the site.
The Contractor shall maintain copies of all records on site throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.
1.6.1. REQUIRED DOCUMENTATION. The contractor shall prepare, as a minimum, the following documentation and/or reports:
1.6.1.1. AIR FORCE FORM 1462 (WATER POLLUTION CONTROL UTILITY OPERATING
LOG). The Contractor shall submit to the COR and CEIE a completed AF Form 1462 by the 5th day of the following month being reported.
1.6.1.2. AIR FORCE FORM 487 (EMERGENCY GENERATOR OPERATIONS LOG). The
Contractor shall submit to the COR a completed AF Form 487, see Appendix B, by the 5th day of the following month being reported.
1.6.1.3. MONTHLY OPERATING REPORT. The Contractor shall submit copies to the COR of the completed monthly status report discussing project activities by the 5th day of the following month being reported. The report focuses on training, safety, effluent water quality and other areas. As a minimum, the monthly operating report includes:
1.6.1.3.1. Plant operations data, efficiencies.
1.6.1.3.2. General overview of notable activities.
1.6.1.3.3. Significant events.
1.6.1.3.4. Staff changes/recognition.
1.6.1.3.5. Outstanding work orders.
1.6.2. Contractor Full-Time Equivalent Reporting
The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for HAFB, NM via a secure data collection site. The
Attachment 1 Page 6 of 14 contractor is required to completely fill in all required data fields at http://www.ecmra.mil. This site is the electronic Contractor Manpower Reporting Application (CMRA).
Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the
FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.
*Reporting Period: Contractors are required to input data by 31 October of each year.
Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link.
However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
2.0 SERVICES SUMMARY.
Performance Objective PWS Para Performance Threshold
SS-1 Conduct General Wastewater Treatment Plant
Operations.
Provide continuous, cost effective, and efficient treatment of all wastewater. All operations accomplished in accordance with the treatment plant operations manuals and NMED Discharge Permit (Appendix G). Provide grounds maintenance and general housekeeping at plant property.
1.1., 3.3.1.
No Notice of Violations (NOV) permitted.
No more than 2 customer complaints per month for housekeeping and grounds maintenance.
SS-2 Maintain Preventive Maintenance Program.
System is operating continuously, safely, and effectively.
Corrective maintenance is performed timely and correctly.

90% of all scheduled PMIs are properly and timely completed.

Test results are accurate and timely. Tests are in accordance with applicable operating permit requirements, conditions and/or CEIE requirements.
1.5.

Discharge Permit parameters. Zero exceedances of the NMED Discharge Permit parameters.
SS-4 Dispose of Sludge.
Sludge is disposed of in accordance with (IAW) the
Holloman AFB Sludge Disposal Plan (Appendix H).

SS-5 Maintain Documentation and Records.
All documentation and records are maintained, kept current and available for review at all times.

3.0 GOVERNMENT FURNISHED PROPERTY/SERVICES/EQUIPMENT. The Government shall provide the following facilities, equipment, materials, and services identified in this PWS and Appendix
D.

3.1. PROPERTY.
3.1.1. PLANT FACILITIES. The Government shall make available the facilities described in Appendix
D. Facilities have been inspected for compliance with OSHA. No hazards have been identified for which workarounds have been established. The Government corrects OSHA hazards in accordance with base-wide Government-developed plans of abatement taking into account safety and health priorities. A higher priority for correction will not be assigned to the facilities provided hereunder merely because of this contracting initiative. The fact that no such conditions have been identified does not warrant or guarantee that no possible hazard exists, or that workaround procedures will not be necessary or that the plant, as furnished will be adequate to meet the responsibilities of the Contractor. Compliance with
OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the Contractor, and the Government will assume no liability or responsibility for the
Contractor's compliance or noncompliance with such responsibilities, with the exception of the aforementioned responsibility to make corrections in accordance with plans of abatement subject to base-wide priorities. The Contractor shall return the plant to the Government in the same condition as received, fair wear and tear and approved modifications excepted. Facilities shall be used for performance of this contract only. In the event that any Government Furnished Property, in whole or part, is removed from the contract, the contractor shall ensure all contractor-owned equipment and belongings are removed from the Government facilities within thirty (30) days of the modification.
3.2. EQUIPMENT.
3.2.1. REAL PROPERTY INSTALLED EQUIPMENT. All Real Property Installed Equipment
(RPIE) provided by the Government such as pumps, blowers, generator, etc., is listed in the Operation and Maintenance (O&M) Manual. The manual is located at the WWTP. The aforementioned equipment is identified in Appendix D of this PWS.
3.2.2. COMPUTER EQUIPMENT. The only Government-provided computer equipment is the
Operator Interface Unit (OIU) computer, with installed software, which is part of the controls system as described in the O&M manual. The manual is located at the WWTP. The aforementioned equipment is identified in Appendix D, paragraph 18.2.
3.3. SERVICES.
3.3.1. GROUNDS AND BUILDING MAINTENANCE. The Contractor shall be responsible for grounds maintenance 10 feet outside the fence perimeter as well as all grounds within the fence perimeter area to ensure the grounds remain free of debris and weeds. The Contractor shall be responsible for and ensure general housekeeping inside and outside is accomplished at all facilities inside the fenced area
(reference Appendix A). Maintenance of the building structures at the WWTP shall be provided by the
49th Civil Engineer Squadron.
3.3.2. REFUSE CONTAINERS AND COLLECTION. The Government shall provide one refuse container for general use and one refuse container for grit and screenings at the WWTP for the
Contractor's use. Refuse grit and screenings collection and disposal shall be provided by the Government.
The Contractor shall not place any grit or screenings that does not pass the paint filter test into the refuse container(s).
3.3.3. EMERGENCY MEDICAL SERVICES. HAFB will provide emergency medical transportation and treatment services for contractor personnel, if required, on a reimbursable basis.
3.3.4. INSECT AND RODENT CONTROL. 49th Civil Engineer Squadron shall provide insect and rodent control for all facilities (phone 572-3223/3224).

3.4. UTILITIES.
3.4.1. UTILITIES USAGE. Reasonable amounts of electricity, natural gas, and water as required for the performance of the contract shall be furnished by the Government.
3.4.2. TELEPHONE. The Government shall furnish two Class A telephone lines (commercial off-base), one Class C telephone line (on-Base only), and one telephone for the Administration Building. The
Government shall maintain the telephone lines up to the phone jack in the Administration Building.
3.4.3. CONSERVATION OF UTILITIES. The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions which preclude the waste of utilities.
3.5. REFERENCES AND TECHNICAL DOCUMENTATION.
3.5.1. GENERAL.
3.5.1.1. Publications and forms applicable to this PWS are listed in Appendix B. The Contractor is obligated to follow those publications and use those forms to the extent (that is, a specific procedure in a paragraph, section, chapter or volume) specified in the PWS. The Air Force (AF) Forms can be located at http://www.e-publishing.af.mil . Others can be downloaded from the internet or will be provided by the
Government.
3.5.1.2. The Government will provide follow-on requirements to the contractor when changes occur.
Supplements or amendments to listed publications from any organization level may be issued during the life of the contract. The contractor shall immediately implement those changes that result in a decrease or no change in the contract price and notify the CO in writing of such change. Should a decrease in contract price result, the contractor shall provide a proposal for a reduction in the contract price to the CO.
Before implementing any change that will result in an increase in contract price, the contractor shall submit to the CO a price proposal within thirty (30) days following receipt of the change by the contractor. Failure of the contractor to submit a price proposal within thirty (30) days following receipt of the change entitles the Government to performance according to such change at no increase in contract price.
3.5.2. MANUFACTURER’S EQUIPMENT LITERATURE, WHERE AVAILABLE. All equipment manufacturers’ literature and data is the property of the Government and is located at the treatment plant.
4.0 GENERAL INFORMATION.
4.1. Quality Control. The contractor shall develop and maintain a quality control program/plan (QCP) to ensure WWTP operation services are performed in accordance with commonly accepted commercial practices and services identified in this PWS (e.g. 4.6.10). The program/plan shall also include how the contractor will implement and maintain quality-accepted practices/procedures to identify, prevent, and ensure nonperformance and continual repeat of defective service does not occur. The written QCP shall be submitted to the CO for review no later than the pre-performance conference. The CO will review and either accept the QCP as written or provide feedback within ten (10) working days. If QCP is returned, contractor shall resubmit within 5 working days, until plan is accepted. The plan shall specifically address the items listed above as well as the contractor's strategy to provide quality workmanship, continual process improvement and for correcting deficiencies as required.

4.2. QUALITY ASSURANCE. The Government will conduct surveillance on the contractor’s performance under this contract. The Government has developed a contract specific Quality Assurance
Surveillance Plan (QASP). The Government will record all surveillance observations. When an observation indicates defective performance, the COR will require the contract manager or representative to initial the observation indicating acknowledgment of deficiency. The initialing of the observation does not necessarily constitute contractor concurrence with the observation, only acknowledgment that the contractor has been made aware of the defective performance. Government surveillance of tasks not listed in the Services Summary (SS) or by methods other than those listed in the SS may occur during the performance period of this contract. Such surveillance will be done according to standard inspection procedures of other contract provisions. Any action taken by the CO as a result of surveillance will be in accordance to the terms of this contract.
4.3. PERFORMANCE EVALUATION MEETINGS. The contract manager may be required to meet at least weekly with the COR and the Contracting Officer during the first month of the contract.
Subsequent meetings will be on a quarterly basis and/or as needed. The written minutes of all performance evaluation meetings shall be prepared by the Government. Should the contractor non-concur with the minutes; the contractor shall so state any areas of non-concurrence in writing to the CO within 3 working days of receipt of the signed minutes. The minutes will be included in the contract file.
4.4. GOVERNMENT REMEDIES. The CO shall follow FAR 52.212-4, Contract Terms and
Conditions-Commercial Items, for contractor’s failure to perform satisfactory services or failure to correct non-conforming services.
4.5. HOURS OF OPERATION. The contractor shall submit proposed hours of operation as to comply with this PWS in its entirety to the COR for approval.
4.5.1. EMERGENCY SERVICES CALLS. Upon Government notification by the COR, CEIE or CO, the contractor has 60 minutes to respond to emergency situations, after normal hours of operation set by the contractor, at no additional cost to the Government. For purposes of this PWS, any unforeseen occurrence or condition that poses an immediate threat to life, property, the environment, or the violation of the NMED Discharge Permit is considered an emergency situation and requires immediate action.
Examples of an emergency situation may include the possibility of a chlorine leak, jet fuel or Aqueous
Film Forming Foam (AFFF) spill that could enter into the treatment plant.
4.6. SECURITY REQUIREMENTS. HAFB is a closed military installation. The Air Force reserves the right to establish criteria for entry of all personnel, vehicles, and equipment onto HAFB. Access to the base may be further restricted as deemed necessary by the 49th Wing Commander (49 WG/CC) or his/her designee. Delays resulting from restricted access to HAFB will not be the basis for a claim by the contractor when the 49 WG/CC or his/her designee deems that such changes are necessary to protect the security of HAFB.
4.6.1. Illegal Aliens. The contractor shall not hire an illegal alien to work or perform any service on
Holloman AFB (HAFB). Employees who are not U.S. citizens can only be persons possessing a current resident alien immigrant status (Green Card) or if a non-resident immigrant must possess a current
Employment Authorization Document. While on HAFB, non-U.S. citizen employees must be able to produce for inspection the appropriate credential: Green Card or Employment Authorization Document.
Any illegal alien found on the installation shall be immediately detained by the base security forces and barred from the installation.
4.6.2. Listing of Employees. The contractor shall maintain a current listing of employees that require access to HAFB. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive
Attachment 1 Page 10 of 14 areas designated by the installation commander. The list shall be provided to the COR, sponsoring agency unit security manager, and contracting officer. An updated listing shall be provided when an employee's status or information changes (i.e. terminated for cause, reassignment, retirement, etc.) or when a new employee is added.
4.6.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles.
Contractor employees shall only be granted access to the installation to perform functions directly related to their respective contract.
4.6.3.1. The contractor shall ensure all employees are identified on a HAFB Visit Access Request (VAR).
The contractor shall submit VARs through the contracting officer to the 49th Security Forces Squadron
(49 SFS) Pass and Registration Office; VARs must be submitted no later than 7-duty/business days, and for foreign nationals, 14-duty/business days, prior to requiring access to HAFB. Recommend putting foreign nationals on a separate VAR.
4.6.4. Retrieving Identification Media. The contractor shall retrieve all identification media and/or visitor passes from employees who depart for any reason before the contract expires; i.e. terminated for cause, reassignment, retirement, etc. The contractor will notify the Government within 24 hours when unable to retrieve identification media from contract employees.
4.6.5. Traffic Laws. The contractor and its employees shall comply with base traffic regulations. Traffic regulations include, but are not limited to:
4.6.5.1. Contractor employees shall wear a seatbelt when operating or riding in any military, company, or privately-owned vehicle while on the installation.
4.6.5.2. Speed limits on HAFB are 30 MPH unless otherwise posted. Be aware of reduced speed limits when driving in the housing and school zones.
4.6.5.3. All vehicle operators shall not use cell phones unless the vehicle is safely parked or unless they are using a hands-free device. The wearing of any other portable headphones, earphones, or other listening devices (except for hands-free cellular phones) while operating a motor vehicle is prohibited.
Penalty: Privileges to operate a motor vehicle while on the installation may be suspended for 5 days, effective the first day after receipt of notice.
4.6.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their government, company, or private-owned vehicle while on HAFB.
4.6.7. For Official Use Only (FOUO). The contractor shall comply with DoDM 5200.01-V4/E3, DoD
Information Security Program: Controlled Unclassified Information (CUI), requirements. This manual sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.6.7.1. Records Management (RM) (2017). Records Management (RM) (June 2017). The contractor shall provide accountability and ensure all records and documents generated during the period of this contract are made available to the government at all times. All information may be maintained electronically. All publications and forms will be available for contractor's use through the Holloman
Publications Web Site. Any publications or forms that are required for the performance of this contract and are not available electronically are required to be maintained manually.

4.6.7.2. Freedom of Information Act (FOIA) and Privacy Act (PA) (2017). The contractor shall comply with DoDM 5400.7, DOD Freedom of Information Act (FOIA) Program, DoD5400-R_AFMAN
33-302 and AFI 33-332, Air Force Privacy and Civil Liberties Program. These regulations set policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding Personally Identifiable Information (PII) material.
4.6.7.3. Contractor Records (June 2017). The contractor shall maintain all government records IAW
AFMAN 33-363, Management of Records, paragraph 6.4., Contractor Support, in order to support the
COR who is responsible for all lifecycle record management and all duties assigned to the organization's
Record Professional (RP) (Tier-1 (T-1)). [AFI 33-360, Publications and Forms Management, Table 1.1 outlines description of the authorities for T-1.]
4.6.8. Reporting Requirements. Contractor personnel shall report the appropriate authority any information or circumstances of which they are aware which may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Employees shall be briefed by their immediate supervisor upon initial on-base assignment.
4.6.9. Physical Security. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
4.6.10. Key Control. The Government shall provide the Contractor with keys to the WWTP facilities on the contract start date. The Contractor shall establish and implement methods of ensuring that all keys for the WWTP facilities are continuously accounted for and are not used by unauthorized persons. The
Contractor shall develop procedures covering key control and replacement and shall include such procedures in the QCP. The contractor shall not duplicate any keys issued by the Government.
4.6.10.1. The contractor shall immediately report to the COR or Program Manager any occurrences of lost, duplicated keys or use by unauthorized persons.
4.6.10.2. If locks must be replaced due to damage, breach of security, or any other reason, such locks shall be replaced by the Contractor at no additional cost to the Government. One key for replacement locks shall be provided to the COR.
4.6.10.3. The contractor shall prohibit the use of keys, issued by the Government, by any persons other than the contractor’s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.
4.6.10.4. The HAFB Fire Department has installed KNOX-BOX® Rapid Entry System key boxes throughout the base for gaining access to secure areas at any time in the event of emergency. A KNOX-
BOX® is installed at the treatment plant gate and contains a gate key and the master key that opens all buildings on the plant property. The Fire Department shall control access to the KNOX-BOX® key box system.
4.6.11. Anti-Terrorism (AT) Awareness Level I Training. Antiterrorism (AT) Awareness Level I
Training. This is for contractor employees with an area of performance within a Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within ten (10) calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Certificates of completion for each affected

Antiterrorism Representative. AT level I Awareness Training is available at the following website:

4.7. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL
COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. The contractor shall continue to provide uninterrupted services IAW Department of Defense Instruction (DODI) 3020.37 and this PWS during crisis declared by the national command authority.
4.8. PERSONNEL. The contractor shall operate, maintain, and staff the WWTP with the appropriate number of personnel to satisfy all requirements of this PWS and the NMED Discharge Permit. The
Contractor shall provide a full-time onsite Contract Manager (CM) to oversee and coordinate the work described in this PWS. In addition to required certifications, the CM shall have a minimum of 8 years’ experience in work related to that which is to be performed under the contract.
4.8.1. SPECIAL QUALIFICATIONS. The contractor shall ensure all employees who are in responsible charge of plant operations, for any period of time, possess a current wastewater treatment certification issued by the state of New Mexico IAW NMAC 20.7.4.13, http://www.nmcpr.state.nm.us/nmac/parts/title20/20.007.0004.pdf . The contractor shall take into account that as of 2017 the base population is approximately 13,000 and the treatment plant is an activated sludge treatment plant.
4.9. CONTINUITY OF SERVICES. The Contractor shall be prepared to fully commence work on the start date of this contract. If needed, the incumbent onsite contract manager will assist with facility operations orientation for two weeks to ensure continuity of essential services.
4.10. STATE INSPECTIONS. Periodically state/federal authorities may conduct scheduled or unscheduled compliance inspections of the Holloman Air Force Base WWTP. If the contractor is contacted by inspection authorities, the contractor shall inform 49th Civil Engineer Squadron
Environmental Office (CEIE) (Phone: 572-3931) and COR immediately upon notification of the inspection team visit.
4.11. ENVIRONMENTAL CONTROLS. The contractor shall be knowledgeable of and comply with all applicable interstate, Federal, State, and local laws, regulations, and requirements regarding environmental protection.
4.11.1. CHEMICAL RELEASE EXERCISES/GENERAL DUTY CLAUSE. The contractor shall cooperate with the Government by participating in periodic chemical release emergency exercises. It is the contractor’s responsibility to know the Government provided General Duty Clause (Appendix G), comply with its contents, and educate all employees on its contents.
4.11.2. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws/regulations come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the CO pursuant to requirements of FAR 52.212-4(c) and negotiate a modification to the contract.
4.11.3. The contractor shall notify CEIE at 572-3931 and the COR within 24 hrs, or in the case of weekends or holidays the next business day, of any permit violations. Working hours for CEIE are 7AM to 4PM, Monday through Friday. 4.11.3.1. Your name and give HAFB point of contact (CEIE Water Quality rep.).
4.11.3.2. NMED Discharge Permit – DP-1127.
4.11.3.3. Telephone Number (Also provide Holloman point of contact number)
4.11.3.4. Required information from Part IV, Section C.35. of the NMED Discharger Permit – DP-1127.
4.11.3.4.1. A description of the noncompliance and its cause
4.11.3.4.2. A period of noncompliance including exact dates and times and if the noncompliance has not been corrected, the anticipated time it is expected to continue
4.11.3.4.3. Steps being taken to reduce, eliminate and prevent recurrence of the noncompliant discharge
4.11.4. NOTICES OF VIOLATION (NOV). In the event that any regulatory agency assesses a monetary fine against the Government for violations caused by the Contractor, and not the fault of the
Government, the Contractor shall reimburse the Government for the amount of that fine and receive a contractor discrepancy report.
4.12. LABORATORY EQUIPMENT. The Government will not provide any laboratory ware or equipment. Therefore, the contractor shall provide all ware and equipment necessary to satisfy the requirements of paragraph 1.5. of this PWS.
4.13. HAZARDOUS MATERIAL. In accordance with 29CFR 1910.1200, Air Force Instruction 32-
7086 Hazardous Materials (HAZMAT) Management, ACCMAN 32-7086 Hazardous Materials
Management Supplement 1, ACCMAN 32-7051 Chapter 11 Spill Management, Holloman AFB
Instruction 32-7011 Hazardous Materials Management and Holloman Hazardous Materials Emergency
Response Plan 32-1, ALL material determined to be hazardous as defined by Federal Standard 313, Paragraph 3.2 must be authorized for use via the AF Form 3952 approval process.
4.13.1. All contractors (construction, services, and suppliers) operating on HAFB shall report hazardous material usage to 49 CES/CEIE by providing an Air Force Environmental Information System (AF-
EMIS) Authorization Request Worksheet and Safety Data Sheets (SDS) for each hazardous material. The
49 CES/CEIE, Building 55, 572-3931 will determine whether tracking in the AF-EMIS database is applicable.
4.13.2. The AF-EMIS Authorization Request Worksheet shall be processed/completed through 49
CES/CEIE (or designated alternate location) for each material identified as hazardous PRIOR to being brought onto HAFB. Contact 49 CES/CEIE five (5) business/working days in advance for support with initiating/completing the AF-EMIS Authorization Worksheet and the AF Form 3952 approval process.
4.13.3. Any hazardous material found on HAFB that has not been approved shall be removed immediately until such time as it is properly approved.
4.14. MATERIAL STORAGE AND USE. The Contractor shall store all Hazardous Material
(HAZMAT) in a designated HAZMAT storage area. The Contractor shall anticipate receiving at least one assessment during the contract period to ensure their HAZMAT storage area is in compliance with
HAZMAT storage requirements outlined in this section.

4.14.1. The Contractor shall provide secondary containment for all HAZMAT being stored and used on base. Secondary containment shall be chemically inert to the HAZMAT being stored and impervious to absorption of the HAZMAT. The containment volume shall not be less than 10% of the total quantity being stored or the equal quantity of the largest container being stored whichever is greater in volume.
Separate secondary containment shall be provided for incompatible HAZMATs.
4.14.2. The Contractor shall ensure that the segregation of incompatible materials is accomplished at all times in his or her field office, storage, staging, and work areas. National Fire Protection Association
(NFPA) and OSHA required or specified flammable material and corrosive material storage lockers shall be used for the storage of all HAZMATS.
4.14.3. Ensure the use of protective measures such as drop cloths and tarpaulins when using HAZMATs to keep the work and storage areas free from drips and spills.
4.14.4. Keep all containers closed when not in use. At the end of the workday, or when finished using any material, return the container to a proper storage area.
4.14.5. Do NOT store HAZMAT outdoors where it can be exposed to precipitation.
4.14.6. Follow all manufacturers’ recommendations for storage and use of HAZMATs.
4.14.7. Ensure that all employees are given proper training in the use of the HAZMATS onsite and all personal protective equipment necessary for the use of HAZMAT on HAFB.
4.14.8. SDSs for each HAZMAT being used are required by OSHA to be available onsite to employees.
The Contractor shall have current copies available at all times where employees can access them in case of emergency.
4.15. SPILL CONTROL AND CLEAN UP. The Contractor shall be responsible for the cleanup and disposal of all spilled materials whether or not they are HAZMATs. This includes all materials used to contain and absorb the spill. Spills of HAZMATs or Hazardous Wastes shall be managed in accordance with the current Holloman Hazardous Waste Plan. Disposal of wastes generated from spill cleanup shall be the contractor’s responsibility.
4.16. WASTE MATERIALS. The Contractor shall turn in non-regulated waste such as fluorescent light tubes, and rechargeable batteries at base 90-Day Central Accumulation Point (Bldg. 149). Subject to limitations of what is acceptable, the Contractor shall turn in any potentially hazardous substance such as, grease, waste oil, as well as lead acid batteries, etc., at the base Defense Reutilization and Marketing
Office (DRMO) (Bldg. 112). Disposal of sludge from WWTP drying beds shall be accomplished in accordance with section 1.4. Sludge Disposal of the PWS.

PLANT DESCRIPTION AND WASTEWATER CHARACTERISTICS
1. PLANT DESCRIPTION
1.1. The purpose of this Appendix is to provide a general description of the processes and equipment for wastewater treatment at Holloman Air Force Base (HAFB). These descriptions are designed to present a general understanding of the systems involved, how they function, and how they are interrelated.
1.2. The WWTP was constructed in the mid 1990’s and was put into operation in July 1996.
