Questionnaire on Contractor’s Present and Past Performance
Request you provide the following information on the contractor’s present and past performance by placing an X in the space provided by the most appropriate response. Please provide comments on the lines provided after each question for all ratings other than Satisfactory Confidence. If more space is needed, continue your comments on a separate sheet of paper and attach it to this questionnaire prior to submittal to the 28th Contracting Squadron. You are asked to provide frank, concise comments regarding your overall assessment of the contractor’s performance on the contract identified. You are urged to supplement your own knowledge of the contractor’s performance with the judgment of others in your organization. The following definitions are applicable rating levels of the contractor’s performance on the identified program.

Substantial Confidence Based on the offerors recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort based on “very good” or “exceptional” performance where the contractor met the contractual requirements and exceeded expectations to meet the Government’s needs.
Satisfactory Confidence Based on the offerors recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort based on “satisfactory” performance where the contractor met the contractual requirements.
Neutral Confidence “No recent/relevant performance record” is available or the offerors performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.
Limited Confidence Based on the offerors recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort based on “marginal” performance where the contractor did not meet some of the contractual requirements.
No Confidence Based on. the offerors recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort based on “unsatisfactory” performance where the contractor did not meet most of the contractual requirements
