1. How to fill out a Request for Evaluation form
Go to the SharePoint and click on the “G-12 Request Form” in the left-hand menu.
Click on “Add New” at the top of the screen.
Blocks 1-4: Fill out the requester’s name, phone number, and email and the information of a backup POC for the requested item. A second POC is especially necessary for those who are likely to be deployed and uncontactable.
Block 5: Fill in the nomenclature of the item to be evaluated. Be specific.
Block 6-9: Include the manufacturer, part numbers, and URL for the Requested Item.
Block 10: Describe what power source you intend to use for the equipment (i.e.
battery, site power, and generator) and whether or not battery charging will be needed on site.
Block 11: Permanent installs are typically handled in an MCL or CEM depending on where the install will happen. The same type of evaluation will be needed and is sometimes tracked through the G-12 process.
Block 12: Zones are defined in the TO 21M-LGM30G-12. Some equipment is not allowed in zones below grade. Indicate if there are other TOs that are affected by this analysis.
Block 13: Fill in when and how long the requested item will need to be used and whether there are special circumstances. Will there be a mission impact without this item?
Block 14: Does the requested item have WIFI/Bluetooth capability or removable media? This is a cyber security concern. If the capability cannot be turned off, then the item maybe rejected for inclusion on the G-12 List.
Block 15: List the reason(s) why equipment is needed and how it is intended to be used. Sometimes this can affect the analysis. In addition, electrical configuration information will affect the outcome of the analysis.
Block 16: Sometimes the requested item has a similar item already on the list that can be used to conduct an analysis by similarity. Some items have multiple part numbers, manufacturers, etc. This information increases the likelihood of finding already existing test data.
Block 17: Is there any industry standard certifications on the part that might help with an analysis? If three sufficient documents may be found there is a chance that an analysis may be performed rather than requiring testing. This can significantly reduce the time it takes the item to get through the G-12 process.
Block 18: Configuration of the site is important as ordnance and sensitive equipment are stringently considered. Mark all the check boxes that apply.
Block 19: Contractors often request an item to be evaluated for contracted work. If questions arise, a POC or two from the listed office will be contacted to find the right path forward.
Block 20: Request the priority best suited to the situation. If the item is needed immediately or within a couple of months, priority 1 should be requested. If the item is needed in 4-6 months, request priority 2. All others fall under priority 3.
At the bottom of the form is a check tab that signals to the system to pass the task to the AFGSC offices for approval of priority. An option to upload documentation that may help with the evaluation or analysis of the requested item. Documentation of electromagnetic certification, test data, or on the electromagnetic performance can be uploaded here. Click Save at the top of the form to officially submit the form.
