SUMMARY OF CHANGES
The purpose of this Amendment is to answer questions from potential offerors and revise the quote due date and time. Please refer to the following:
1. The quote due date and time is hereby changed from:
29 July 2025 at 10:00 AM PDT to 30 July 2025 at 10:00 AM PDT
2. Could you please confirm whether you have a physical or digital copy of OSHA 29 CFR 1926 and NFPA 1402 standards referenced or advise on the appropriate method to obtain them?
RESPONSE: Both can be found online https://www.osha.gov/laws-regs/regulations/standardnumber/1926 https://www.nfpa.org/codes-and-standards/nfpa-1402-standard-development/1402
3. On section 2.1.A of the SOW, it shows 4ea. Does “4 EA” refer to four individual anchors in total, or does it represent multiple groups of four anchors (e.g., per location or per tower)?
RESPONSE:4 anchors total.
4. Are there as-built drawings or diagrams available showing the location and type of each fire rappelling anchor at
Facility 1550?
RESPONSE: No
5. Is there any information available regarding the configuration, type, size and physical orientation of the installed anchors (e.g., eye type, recessed, surface-mounted), including their expected direction of loading? This will help confirm compatibility with our testing equipment (Hydrajaws M2000 or equivalent).
RESPONSE: Please see Attachment 3 - Amendment 0002 - Test Point Photos
6. Are any photographs of the anchors or their installed locations available to review?
7. Are the anchors located at ground level, or will elevated access be required?

10. The Statement of Work specifies that testing is to be performed “in the direction of intended use.” However, standard hydraulic testers apply a load perpendicular (in tension) to the surface. Are anchors mounted in a way that accommodates this test orientation?
RESPONSE: Please see Attachment 3 - Amendment 0002 - Test Point Photos to assist contractors with making that determination.

RESPONSE: Yes
If so, what is the process for arranging escort access?

13. Has this anchor system been previously tested or certified?
If so, is a copy of the prior inspection report available for review?
14. The Statement of Work requires photographic documentation in the final report. Are photos permitted during the initial inspection or site walk? Are there restrictions or a process for requesting approval?
RESPONSE: Any photos that need to be taken must be taken by escorting personnel.
15. Are there any specific personal protective equipment (PPE) requirements or base safety policies we should be aware of for inspection and proof load testing activities at Facility 1550?
