About this file

This document is a Past Performance Information Form (PPIF) related to a federal contract opportunity for Missile Alert Facility (MAF) Refuse services at Minot Air Force Base in North Dakota.

The PPIF requests information from a customer/contract agency about a contractor's past and present performance on contracts, including the contractor's ability to hire and maintain qualified personnel, the contractor's overall performance rating, and whether the customer would award another contract to the contractor. The form also asks if the contractor was ever issued a cure or show cause notice, or if the contract was terminated. The completed PPIF should be returned to the 5th Contracting Squadron at Minot AFB by the offer due date for the related solicitation, which is for a 1-year contract with 4 option years for refuse removal and disposal services at the 15 MAFs, as outlined in the Performance Work Statement and Quality Assurance Surveillance Plan.

PAST PERFORMANCE INFORMATION FORM

The 5th Contracting Squadron, Minot AFB, North Dakota, is in the process of selecting a contractor for Missile Alert Facility (MAF) Refuse services. One of the considerations in quote evaluation is the verification of the Quoter’s past and present performance on contracts, which reflect the Quoter's ability to perform on the proposed effort. We depend on information received from agencies, such as yours, which have had first-hand experience with an Quoter for the evaluation of the Quoter's performance on those contracts.

Please complete this form and provide comments wherever applicable. Please sign the document in the space provided. Please note, we may contact the Quoter on problem areas or concerns identified to see what corrective actions have been taken or will be taken. However, the individual's name completing the questionnaire will not be disclosed to the Quoter.

Please return the completed form as soon as possible but no later than the offer due date noted in the Solicitation. Please obtain this date from your MAF Refuse contractor.
