STATEMENT OF WORK
PROJECT GENERAL REQUIREMENT: The Contractor shall provide the government with the following:
100% As Built package drawn AutoCAD 2012 format, drawn to Minot AFB standards.
Corrected Cost Estimate on CostWorks software and Microsoft Adobe.
Construction schedule from Notice to Proceed to Completion, in Microsoft Word or Excel format.
Corrected List of submittals on “AF Form 66.”
Corrected Statement of Work in Microsoft Word format
PROJECT DESCRIPTION: The design shall outline the work needed for a complete, finished, operating, and functional facility ready for use by the government. The design package shall include all work stated below pertaining to building 527, located on Minot Air Force Base, ND:
AS-BUILT DRAWINGS: The Government will furnish record drawings to the Contractor from which the Contractor shall develop and produce the proposed 35% designed drawing, cost estimate and specifications. Specific data will be included for general guidance and concept, but will not constitute a basis for design. Contractor shall verify all dimensions in the AS-BUILT drawings.
Period of performance will be 150 days for this project.
Install drywall in rooms as indicated in drawings.
Demo existing hallway and drop ceiling as indicated in drawings.
Demo and replace existing flooring as indicated in drawings.
Demo and replace existing openings as indicated in drawings.
Demo and replace existing hard ceiling as indicated in drawings.
Demo existing drop ceiling as indicated in drawings.
Install new drop ceiling as indicated in drawings.
Demo and replace existing light fixtures as indicated in drawings.
Demo and replace existing HVAC as indicated in drawings.
Demo existing sink and associated plumbing and wood paneling.
DESIGN POLICY AND CRITERIA: The project design will follow the codes and regulations listed below:
UFC 3-501-01 Electrical Engineering UFC 3-520-01 Interior Electrical Systems UFC 3-530-01, Design: Interior, Exterior Lighting and Controls International Plumbing Code (IPC).
International Fire Code (IFC).
National Fire Protection Association (NFPA) Codes and Standards International Mechanical Code (IMC).
UFC 3-401-01, Mechanical Engineering UFC 3-410-01, Heating, Ventilating, and Air Conditioning systems.
International Building Code (IBC).
National Electrical Code (NEC).
Americans with Disabilities Act (ADA).

PROJECT REQUIREMENTS
1. DIVISION 01 – GENERAL REQUIREMENTS
1.1. Contractor shall submit a schedule of contract on the form “AF 3064.”
1.1.1. Liquidated damages shall be assessed for work that is not finished within the scheduled performance period.
1.2. Contractor shall schedule a pre-design conference with the base SABER Office.
1.3. Contractor shall perform a pre-work inspection of the work area.
1.3.1. Contractor is responsible for knowing the presence of any Asbestos, Polychlorinated Biphenyls (PCB), and Lead that will interfere with the work on site.
1.3.1.1. Any effort to abate asbestos, PCB’s, or lead on site must comply with all federal, state and local laws.
1.4. Contractor shall not install any products containing PCB’s.
1.5. Contractor shall provide government with a warranty certificate for any products with a warranty of two or more years.
1.6. Contractor shall provide written notification to the Contracting Officer (CO) two weeks prior to commencement of work.
1.7. Work shall be performed to the satisfaction of the CO.
1.8. Working hours for the Contractor will be Monday through Friday, between the hours of 7:30 a.m.
and 4:30 p.m.
1.9. Contractor shall maintain a safe work area and shall remove all construction rubble from the work site to a Contract Authorized onsite construction dumpster or remove from Minot AFB on a daily basis.
1.10. MSDSs, proposed container sizes and rates of use must be submitted to the 5 CES Environmental
Management Office (via the CO) for review and approval at least 15 duty days prior to any materials being brought onto any portion of the installation. The Contractor is required to have and maintain Material Safety Data Sheets (MSDSs) on all materials that are used. The Contractor is responsible for training his employees to properly use any authorized hazardous materials and properly handle hazardous wastes. A monthly chemical usage report will be submitted to the CES Environmental Management Office (via the COR) that lists the amount of materials (with associated MSDSs) used and locations of usage. After completion of the project, a final total chemical usage report will be submitted to the CES Environmental Management Office (via the COR) that lists the amount of materials (with associated MSDSs) used and locations of usage. **Submittal required
1.11. All contractor-generated hazardous wastes will be removed from the installation the same day it is generated and disposed of off base by the contractor. All hazardous waste characterization and proper disposal is the responsibility of the contractor. This shall be done in a manner to conform to the State of North Dakota Health and Sanitation Codes. In no manner will the Government be responsible for disposals. No hazardous waste or excess materials will be left behind on Air Force property after conclusion of the project.
1.12. Contractor shall be knowledgeable of and comply with all applicable Federal, State and local regulations and requirements regarding environmental protection.
1.13. The Contractor is solely responsible for compliance with EM 385-1-1 Safety and Health Requirements Manual and OSHA standards for the protection of their employees. The Contract Manager shall ensure specific safety requirements in Air Force Occupational Safety and Health (AFOSH) standards are complied with by Contractor Personnel when non-compliance would clearly present the potential to harm or damage government resources. The Air Force is not responsible for ensuring that Contractors comply with "personal" safety requirements that do not present the potential to damage government resources.
1.14. Contractor shall complete and follow all instructions on AF Form 103 before start of work on base. A copy of AF Form 103 instruction must be on the construction premises at all times while work is being performed.
1.15. When welding, cutting, brazing or use of any open flame device is to be used, an AF Form 592 permit must be obtained from the Base Fire Department (Building 896) prior to beginning such work.
During such work, a fully charged and operable fire extinguisher, appropriate for the task, shall be provided by the Contractor. Contractor shall process form for approval.

2. DIVISION 02 – EXISTING CONDITIONS
2.1. Conduct Asbestos/Lead based paint survey in suite.
2.2. Remove and abate all asbestos necessary to complete work.
2.3. Conduct mold survey in room 150.
2.4. Remove and abate all materials affected by mold as necessary.
3. DIVISION 03 – CONCRETE :
3.1. Demo and remove existing concrete slab in new reception room in front of room 150 as indicated in drawings.
3.2. Apply clear form epoxy to concrete floor as indicated in drawings.
3.3. Patch and repair indicated concrete as necessary.
4. DIVISION 04 – MASONRY:
4.1. Install CMU blocks and mortar in wall in front of room 150 to cover hole left from existing hard ductwork.
5. DIVISION 05 – METALS :
5.1. Replace existing wooden studs that are exposed in room 149A and 149B and replace with 10’ tall steel studs as indicated in drawings.
5.2. Install 10’ steel studs for furring in front of walls in rooms 149A, 149B, and 150.
5.3. Install 10’ steel studs for previous hallway entrance as indicated in drawings.
6. DIVISION 06 – CARPENTRY
6.1. Install drywall as indicated in drawings in rooms 149, 149A, 149B, 150, and previous hallway entrance.
6.2. Remove existing wood paneling from walls as indicated in drawings.
6.3. Repair or replace damaged drywall where necessary.
7. DIVISION 07 – THERMAL & MOISTURE PROTECTION:
7.1. Install wall insulation in room 149A as indicated in drawings. *Submittal Required*
8. DIVISION 08 – OPENINGS:
8.1. Demo existing doors, frames, and hardware in rooms 149, 149A, 149B, and 150.
8.2. Install office wooden doors, wooden frames, and hardware to consist of hinges, doorknob, closers, kickplates, keyed lockset and wall bumpers in rooms 149, 149A, 149B, 150 and previous hallway entrance.*Submittal Required*
9. DIVISION 09 – FINISHES
9.1. Demo and replace existing carpet and cove base with tile carpeting in rooms 149, 149A, 149B, and
150. *Submittal Required*
9.2. Install new flooring tiles and cove base in reception room in front of room 150. *Submittal
Required*
9.3. Demo and replace all existing cove base in rooms 149, 149A, 149B, and 150. *Submittal Required*
9.4. Demo and remove existing tile flooring in hallway as indicated in drawings.
9.5. Install floor transitioning strips between existing doors and concrete where tile flooring was removed in hallway as indicated in drawings. *Submittal Required*
9.6. Demo and remove existing drop ceiling grid and tiles in rooms 149, 149A, 149B, 150, and hallway as indicated in drawings.
9.7. Demo and replace existing hard ceilings in rooms 149, 149A, and 149B as indicated in drawings.
9.8. Install new drop ceiling in room 150 and the new reception room to include ceiling grid and ceiling tiles. *Submittal Required*
9.9. Prepare and paint all new and existing walls in rooms 149, 149A, 149B, 150, and the new reception room in front of room 150. Additionally, prepare and paint all new hard ceilings in rooms 149, 149A, and 149B as indicated in drawings. *Submittal Required*
9.10. Remove existing wallpaper from hallway as indicated in drawings.

10. DIVISION 21 – FIRE SUPPRESSION:
10.1. Repair and replace existing fire suppression sprinkler heads in rooms 149, 149A, 149B, and 150 as necessary. *Submittal Required*
11. DIVISION 22 – PLUMBING:
11.1. Demo and remove existing sink, faucet, associated piping and cabinetry in room 150. Cap remaining plumbing lines.
12. DIVISION 23 –HEATING, VENTILATION, AND AIR CONDITIONING:
12.1. Demo and remove all existing HVAC (thermostats, diffusers, returns, radiators, etc) in rooms 149, 149A, 149B, and 150 as indicated in drawings
12.2. Install new HVAC (thermostats, diffusers, returns, etc) for rooms 149, 149A, 149B, and 150 that provides proper coverage of layout as indicated in drawings. *Submittal Required*
12.3. Remove existing wall air conditioners as indicated in drawings.
