About this file

This is a Past Performance Questionnaire (Section L Attachment) for a Giant Voice Replacement system procurement by the 87th Contracting Squadron of the Air Force Air Mobility Command. The questionnaire is issued by Contracting Officer Lizbette Zalybniuk and is designed to collect performance feedback from organizations with prior experience working with potential contractors.

The procurement involves a turnkey Giant Voice System at Joint Base McGuire-Dix-Lakehurst that requires design, engineering, installation, and testing of a complete outdoor mass notification system. The system must integrate new siren and command equipment with forty-two existing government-furnished towers and central control stations, including AtHoc network interfaces. Responses to the questionnaire are due by September 20, 2026, at 1000 hours and should be submitted electronically to Mr. Noah Whiting (noah.whiting.1@us.af.mil) or Lizbette Zalybniuk (Lizbette.Zalybniuk.1@us.af.mil). The questionnaire requests detailed contract identification information, customer/agency details, respondent identification, and comprehensive performance ratings across seven technical performance areas using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Performance areas assessed include technical requirement compliance, installation schedules, proficiency with Mass Notification platforms and AtHoc software, Air Force network security compliance, integration with existing AF infrastructure, systems engineering practices, environmental safety procedures, documentation quality, risk management, and overall contractor suitability for future similar work. The questionnaire specifically restricts responses to actual respondents with prior experience and explicitly excludes responses from potential contractors.
