Dix Food Service
Attachment 1 Past and Present Performance Questionnaire
1. The 87 Contracting Squadron is in the process of selecting a contractor for a Full Food Service program. The services are required for the 7 dining facilities on the Dix area of JB-MDL. The service encompasses dining facility management functions, food preparation, facility sanitation and janitorial duties.
2. One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies, such as yours, which have had firsthand experience with an offeror.
3. Our areas of interest in the offeror are summarized in the enclosed questionnaire. Request that you provide your completed questionnaire no later than 6 May 19. You may email your questionnaire to michael.petty.5@us.af.mil. Please email Michael Petty if you have any questions. Do not send the completed questionnaire back to the offeror. Your completed questionnaire will become a part of the official Source Selection records.
4. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
