SUBJECT: Request for Proposal (RFP) Admendment 01, Communications Operations & Maintenance Functions (COM-F) III, FA4452-18-R-0037, HQ AMC AMC/A6I.
1. This is a Combined Synopsis/Solicitation RFP Amendment 02, please see changes in red. This RFP is a competitive best value subjective tradeoff source selection conducted in accordance with Federal Acquisition Regulation (FAR) 15, Source Selection, as supplemented by the Defense Federal Acquisition Regulation Supplement (DFARS), the Air Force Federal Acquisition Regulation Supplement (AFFARS), and the specific requirements outlined in this request with attachments. The Performance Work Statement (PWS) (Attachment 1) details the effort to be accomplished. The North American Industry Classification System (NAICS) for this acquisition is 541513, Computer Facilities Management Services, small business size standard of $27.5M. The resulting Single Award IDIQ contract will have a five-year ordering period of 13 Feb 20 – 12 Feb 25 and a six month Extension of Services 13 Feb 25 – 12 Aug 25. The initial GDSS Task Order (TO), which the Government anticipates awarding with the COM-F III IDIQ to meet the IDIQ minimum guarantee, will have a one-year base period, and four option periods with an anticipated 60 day phase-in period beginning 01 Mar 20 and full performance to begin 01 May 20. The Government anticipates awarding Subsequent TOs immediately after awarding the COM-F III IQIQ and initial GDSS TO. The Subsequent TOs will also have a one-year base period, and four option periods with an anticipated 60 day phase-in period beginning 01 Mar 20 and full performance to begin 01 May 20, The Government reserves the right to delay award or not award any or all of those task orders.
2. This RFP is not authorization to begin performance, and in no way obligates the Government for any costs incurred by the offeror associated with developing a proposal. The Government reserves the right not to award an IDIQ and TO in response to this RFP. Prior to commencement of any activities associated with performance of this requirement, the Government will issue a written directive or contractual document signed by the Contracting Officer with appropriate consideration established.
3. A Bidder’s Library will be provided for Contractors who intend to propose on the COM-F III Requirement. The Bidder’s Library will provide 101 Briefings and information on the “Systems” Architecture via ARL Secure Access File Exchange (https://safe.arl.army.mil/) to assist contractor in preparing proposals. All persons that will have access to or will be given the opportunity to view the information provided must sign a Non-Disclosure Agreement (NDA) prior to accessing the information. Non-Dislosure Agreements, Attachment 19, has been posted to FBO for contractors to submit and return to gloria.hoybach@us.af.mil and sarah.myers.3@us.af.mil, once contractor provides NDA for ALL person granted access to the Bidder’s Library the government will provide a link to ARL Secure Access File Exchange.
4. A Pre-proposal conference will be conducted on 26 Mar 19 at 2:00 p.m. Central Time for the purpose of answering questions regarding this solicitation. Offerors may attend via teleconference or in person at the 763 SCONS at 510 POW MIA Drive, Building P-40, Suite E1004, Scott Air Force Base, 62225-5420, please see Provisions and Clauses, Attachment 6 for more information.
5. The due date for RFP proposals is 15 Apr 19 at 4:30 PM (Central), all electronic submissions shall reference the complete RFP number in the subject line. LATE PROPOSALS WILL NOT BE ACCEPTED. ADDENDUM TO FAR 52.212-1
INSTRUCTIONS TO OFFERORS -- COMMERCIAL ITEMS
1.0 General Instructions
(a) The offeror's proposal must include all data and information requested by this Instruction to Offerors (ITO) and must be submitted in accordance with these instructions. The proposal shall be compliant with the requirements as stated in the Performance Work Statement (PWS), Contract Data Requirements List (CDRL) and Request for Proposal (RFP). Non-conformance with the instructions provided in this ITO may result in an unfavorable proposal evaluation.
(b) The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities and experience and will base its evaluation on the information presented in the offeror's proposal.
(c) Elaborate brochures or documentation, binding, detailed artwork, or other embellishments are unnecessary and are not desired.
(d) FAR 52.212-1(c) is amended as follows: The offeror agrees to hold the IDIQ and initial GDSS Task Order prices in its proposal firm until 13 Feb 20. The offeror agrees to hold the Subsequent Task Order prices in its proposal firm until 01 May 20.
The solicitation issue date of 15 Mar 19 and proposal due date of 15 Apr 19 at 4:30 P.M. (Central Time) as specified in FAR 52.212-1 (f) (1) and the RFP. The offeror shall make a clear statement in Table 1.2a Volume I that the proposals for the COM-F III IDIQ and the initial GDSS Task Order are valid through 13 Feb 20, The offeror shall also make a clear statement in Table 1.2a Volume I that the proposals for the Subsequent TOs for ACAS, CAMPS, DMR, GATES, GCCS, MEIS 3, MEIS 4, MAF C2, and MAFPS are valid through 01 May 20.
(e) In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of all unsuccessful proposals. Unless the offeror requests otherwise, the Government will destroy extra copies of such unsuccessful proposals.
1.1 General Information
1.1.1. Point of Contact

1.1.2. Debriefings
The CO will promptly notify offerors of any decision to exclude them from the competitive range (if discussions are held); whereupon, they may request and receive a debriefing in accordance with FAR 15.505. Offerors excluded from the competitive range may request a pre-award debriefing or they may choose to wait until after the source selection decision to request a post-award debriefing. However, offerors excluded from the competitive range are entitled to no more than one debriefing for each proposal. The PCO will notify unsuccessful offerors in the competitive range of the source selection decision in accordance with FAR 15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing. Offerors desiring a debriefing must make their request in accordance with the requirements of FAR 15.505 or 15.506, as applicable.
1.1.3. Discrepancies
If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CS and PCO in writing with supporting rationale as well as the remedies the offeror is asking the CS and PCO to consider as related to the omission or error. The offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion. This reservation includes matters of additional or substitute pages of the initial proposal.
1.1.4. Electronic Reference Documents
All referenced documents for this solicitation are available on the Federal Business Opportunities (FedBizOpps) website at http://www.fedbizopps.gov. Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation.
1.1.5. Oral Presentations
Oral presentations will not be permitted.
1.1.6 Communications
The CS will control exchanges of source selection information between Government and offerors. Email may be used to transmit such information to offerors only if the email can be sent and received encrypted and includes “Source Selection Information – See FAR 2.101 & 3.104” in the Subject line.
1.1.7 Amendments to Solicitation
If this Request for Proposal (RFP) is amended, all terms and conditions that are not amended remain unchanged. Offerors shall acknowledge receipt of any amendment to this request by the day and time specified in the amendment (s).
1.1.8 Organizational Conflict of Interest (OCI)
At the time of submitting a proposal, offeror must state affirmatively if it has OCI issues and must submit an Organizational Conflict of Interest Identification and Mitigation Plan (OIMP). The Government-accepted OIMP shall be incorporated by reference into a resultant Contract.
1.2 Organization/Number of Copies/Page Limits
The offeror shall prepare a proposal for award of the COM-F III IDIQ, the initial GDSS Task Order, and Subsequent TOs (ACAS, CAMPS, DMR, GATES, GCCS, MEIS 3, MEIS 4, MAF C2, and MAFPS) as set forth in the Proposal Organization Tables below (Table 1.2a and 1.2b). The titles and contents of the volumes shall be as defined in these tables, all of which shall be within the required page limits and with the number of copies as specified in Tables 1.2a, and 1.2b. The attachments identified in these tables shall be submitted electronically to the CS and PCO. The contents of each proposal volume are described in the ITO paragraph as noted in the tables below:

1.2.1 Page Limitations
Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and excess copies will be returned to the offeror as soon as practicable. Page limitations may be placed on responses to Evaluation Notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the offerors. Each page shall be counted except the following:
Cover pages, tables of content, tabs, glossaries, indexed lists of tables and figures, and TO Attachments 4, 5, 6, 9, 13, 15, 16, and 17.
1.2.1.1. Page Formatting
(a) A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Page size shall be 8.5 x 11 inches. Pages shall be typed with no smaller than single line spacing. Except for the reproduced sections of the solicitation document, the text size shall be no less than 10 point. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least 1-inch margins on the top and bottom and 1 inch side margins. Pages shall be numbered sequentially by volume. These page size and format restrictions shall apply to responses to ENs.
(b) Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 8.5 x 11 inches in size. For tables, charts, graphs and figures, the text shall be no smaller than 8 point. Pages shall be numbered consecutively within each volume. The page limits prescribed are maximum page limits for each volume. All proposal information shall be submitted in either MS Word, PDF, MS Excel format unless otherwise indicated in the solicitation.
These page size and format restrictions shall apply to responses to ENs. Each volume shall be submitted as a separately labeled electronic file.
1.2.2 Pricing Related Data
All pricing data shall be addressed ONLY in the Price Proposal and Contract Documentation Volumes. Cost tradeoff information, work hour estimates, and material kinds and quantities may be used in other volumes only as appropriate for presenting rationale for alternatives or design and tradeoff decisions.
Pricing information shall be addressed in Table 1.2a Volume III and Table 1.2b Volume III, Price. Include supporting data to substantiate the reasonableness of the proposed prices. At a minimum for the IDIQ price proposal, supporting data shall include labor category position descriptions fully burdened hourly rates in Attachment 9, IDIQ Labor Category and Pricing Workbook. The fully burden rate purposed for each IDIQ labor category shall consider and include any mid or swing shift differential rates. The COM-F III IDIQ Table 1.2a Volume III price proposal shall include the entire 5-year ordering period. At a minimum for the initial GDSS and Subsequent TOs price proposals, shall include allocation of personnel by labor category, labor hours for Task and Subtasks, and fully burdened hourly rates in Attachment 16, TO Pricing Template. All significant assumptions, scope limitations, and/or qualifications of the proposal shall be addressed and explained. The initial GDSS and all Subsequent TOs Table 1.2b Volume III shall include a two month phase-in period (if applicable), ten month base period, and four twelve-month option periods. Information on all TO pricing proposals shall show the consolidated total price for performance of each TO.
1.2.3 Cross-Referencing
To the greatest extent possible, each volume shall be written on a standalone basis so that its contents may be evaluated with a minimum of cross-referencing to other volumes of the proposal. Information required for proposal evaluation, which is not found in its designated volume, will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is permitted where its use would conserve space without impairing clarity.
1.2.4 Indexing
Each volume shall contain a more detailed table of contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections and will not count against page limitations.
1.2.5 Glossary of Abbreviations and Acronyms
Each volume shall contain a glossary of all abbreviations and acronyms used, and with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.
1.2.6 Tables and Figures
Include an indexed list of Tables and Figures. This index will not count against page limitations.
1.2.7 Title Page and Labeling
Each volume must include a title page, which includes:
a) Solicitation number
b) Name, address, telephone number(s) and e-mail address of the offeror.
Be sure to apply all appropriate markings including those prescribed in accordance with FAR 52.215-1(e), Restriction on disclosure and use of data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection Information
1.3 Electronic Proposal
Complete proposals include an electronic submission (via email), no later than the required proposal due date. Use separate files to permit rapid location of all portions, including subfactors, exhibits, annexes, and attachments, if any. The electronic submission shall be submitted in a format readable by Microsoft (MS) Office 2016 suite of applications.
1.4 Proposal Submission Requirements
Proposals are to be submitted by the due date specified in the RFP. Offerors are to submit one electronic copy of their proposal via email to Ms. Sarah C. Myers at sarah.myers.3@us.af.mil and Ms. Gloria S. Hoybach at gloria.hoybach@us.af.mil
Email proposals must be received by the proposal due date.
2.0 IDIQ COM-F III (Phase 1)
2.1 Table 1.2a Volume I – IDIQ Executive Summary
In the executive summary volume, the offeror shall provide the following information: see Table 1.2(a), indicates a three page limit
2.1.1 Narrative Summary
The narrative summary of the entire proposal should be concise, to include addressing the significant risks, and highlighting any key or unique features, excluding price. The salient features should tie in with the technical subfactors defined in Addendum to FAR 52.212-2, Evaluation – Commercial Items. Any summary material presented here shall not be considered as meeting the requirements for any portions of other volumes of the proposal.
2.1.2 Table of Contents
Include a master table of contents of the entire proposal.
2.2 Table 1.2a Volume II – IDIQ Technical Volume
2.2.1 General
The Technical Volume should be specific and complete. Legibility, clarity, and coherence are very important. Your responses will be evaluated against the Technical subfactors defined in Addendum to FAR 52.212-2, Evaluation – Commercial Items. Using the instructions provided below, provide as specifically as possible the actual methodology you would use for accomplishing/satisfying these subfactors. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
2.2.2 Format and Specific Content
2.2.2.1 Technical
In the technical volume, address your technical solution for meeting or exceeding the Government’s threshold performance or capability requirements of each technical subfactor.
2.2.2.3 COM-F III IDIQ Volume Organization
The IDIQ Technical volume shall be organized according to the following general outline:

(2) Teaming Matrix (if applicable)
(3) Table of Contents
(4) List of Table and Drawings
(5) Glossary of Abbreviations and Acronyms
(6) Mission Essential (Subfactor One)
(7) Knowledge of AMC C2, ITV, and Business Systems (Subfactor Two)
(8) Labor Categories, Descriptions, Education/Certifications, and Experience (Subfactor 3), Attachment 9, Columns A through F, of the IDIQ Labor Category and Pricing Template and does not require a narrative.
2.2.2.4 Subfactor 1 – Mission Essential, IAW DFARS PGI 207.105(b)(20)(c) Each offeror shall provide a detailed and complete Mission Essential Contractor Services Plan. Requirements for Subfactor 1 – Mission Essential are described in Addendum to FAR 52.212-2, paragraph 2.1.1 Subfactor 1: Mission Essential.
2.2.2.5 Subfactor 2 – Knowledge of AMC C2, ITV, and Business Systems Each offeror shall provide a detailed and complete narrative demonstrating their knowledge. Requirements for Subfactor 2 – Knowledge of AMC C2, ITV, and Business Systems are described in Addendum to FAR 52.212-2, paragraph 2.1.1.2 Subfactor 2: Knowledge of AMC C2, ITV, and Business Systems.
2.2.2.6 Subfactor 3 – Labor Categories and Descriptions
Each offeror shall provide labor categories, descriptions, education/certifications, and experience required to achieve the requirements of the PWS in Attachment 9, IDIQ Labor Category and Pricing Template. Requirements for Subfactor 3 – – Labor Categories and Descriptions are described in Addendum to FAR 52.212-2 paragraph 2.1.1.3 Subfactor 3: – Labor Categories and Descriptions.
2.3 Table 1.2a Volume III Price - IDIQ Labor Rates
2.3.1 General Instructions
2.3.1.1 Price Reasonableness and Completeness
This section is to assist you in submitting pricing data that is required to evaluate the reasonableness of your proposed price. Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal. The contractor shall submit pricing information to support the completeness and reasonableness of their proposed prices. The pricing information submitted should fully support PWS requirements for each major functional element specified in the PWS. Proposals should be sufficiently detailed to demonstrate their completeness, reasonableness. The burden of proof for credibility of proposed prices rests with the offeror.
2.3.1.2 Price Data
Data beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your price position. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price volume and in Attachment 9, Columns G through P, IDIQ Labor Category and Pricing Template. Under no circumstances shall this data and documentation be included elsewhere in the proposal.
2.3.1.3 Cost or Pricing Data Requirements
The contracting officer has determined there is a high probability of adequate price competition in this commercial acquisition and certified cost and pricing data under FAR 15.406-2 will not be required. Upon examination of the initial offers, the contracting officer will make a determination of price reasonableness, and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit data other than certified cost and pricing data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price IAW FAR 15.403-3.
2.3.1.4 Rounding
All unit and Attachment 9 prices provided shall be rounded to the nearest dollar. Extended prices shall be divisible by the number of units proposed (i.e. 12 months) with no fractional cents. All loaded labor rates shall be rounded to the nearest penny.
2.4 Table 1.2a Volume IV – IDIQ Contract Documentation
2.4.1 Model Contract/Representations and Certifications
The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The offeror's proposal shall include a signed copy of the RFP.

2.4.4 Exceptions to Solicitation Requirements
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost, and specific requirements of the solicitation. This information shall be provided in the format and content of Table 2.4.4.

2.5 Other Information Required
2.5.1 Authorized Offeror Personnel
Provide the name, title, and telephone number of the company/division point of contact regarding decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the Government.
2.5.2 Company/Division Address, Identifying Codes, and Applicable Designations Provide company/division's street address, county and facility code; CAGE code; DUNS code; TIN; size of business (large or small); and labor surplus area designation. This same information must be provided if the work for this contract will be performed at any other location(s). List all locations where work is to be performed and indicate whether such facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location.
3.0 Initial GDSS and Subsequent Task Orders (Phase 2)
3.1 Table 1.2b Volume I – Initial GDSS Task Order Technical Volume
3.1.1 General
The Technical Volume should be specific and complete. Legibility, clarity, and coherence are very important. Your responses will be evaluated against the Technical subfactors defined in Addendum to FAR 52.212-2, Evaluation – Commercial Items. Using the instructions provided below, provide as specifically as possible the actual methodology you would use for accomplishing/satisfying these subfactors. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
3.1.2 Format and Specific Content
3.1.2.1 Technical
In the technical volume, address your technical solution for meeting or exceeding the Government’s threshold performance or capability requirements of each technical subfactor.
3.1.2.2 Technical Risk
Address Technical Risk associated with those aspects of your proposed technical approach you consider to have the potential for disruption of schedule, increased costs, degradation of performance, the need for increased Government oversight, and/or the likelihood of unsuccessful contract performance. Classify each risk in accordance with DoD Source Selection Procedures Table 2B, Technical Risk Rating Method. Provide the rationale for each risk and its rating, including quantitative estimates of the impact on cost, schedule, and performance. Describe the impact of each identified risk in terms of its potential to interfere with or prevent the successful accomplishment of other contract requirements (for example: PWS), whether or not those requirements are identified as subfactors. Propose a realistic "workaround" or risk mitigation for identified risks that will eliminate or reduce risk to an acceptable level. Identify and classify any new risks introduced by such risk mitigation.
3.1.2.3 Initial GDSS and Subsequent Task Order Volume Organization The initial GDSS Task Order Technical volume shall be organized according to the following general outline:

3.1.2.4 Subfactor 1 – Initial GDSS TO Technical Approach Each offeror shall provide a detailed and complete technical approach demonstrating their capabilities, knowledge, and expertise. The technical approach shall represent the offeror’s approach to meeting overall PWS requirements. Identify any best practices/innovations proposed. Requirements for Subfactor 1 – Initial GDSS Technical Approach are described in Addendum to FAR 52.212-2, paragraph 2.2.1.1 Subfactor 1: Initial GDSS Technical Approach.
3.1.2.5. Subfactor 2 –Initial GDSS TO Staffing Approach
Each offeror shall provide a plan with their approach to overall staffing. The staffing plan should represent the offeror’s approach to performing PWS tasks by identifying necessary personnel resources. Requirements for Subfactor 2 – Initial GDSS Staffing Approach are described in Addendum to FAR 52.212-2 paragraph 2.2.1.2 Subfactor 2 Initial GDSS Staffing Approach.
3.1.2.6. Subfactor 3 – Subsequent Task Order Staffing Approach Each offeror shall provide a plan with their approach to overall staffing. The staffing plan should represent the offeror’s approach to performing PWS tasks by identifying necessary personnel resources. Requirements for Subfactor 3 – Subsequent TOs Staffing Approach are described in Addendum to FAR 52.212-2 paragraph 2.2.1.3 Subfactor 3: Subsequent TOs Staffing Approach.
3.2 Table 1.2b Volume II –Past Performance
3.2.1 General
Each offeror shall submit a past performance volume with its proposal, containing past performance information in accordance with the format contained in the Past Performance Information Sheet (PPIS) (Attachment 10). Your submissions will be evaluated against the Technical subfactors defined in the Addendum to FAR 52.212-2, Evaluation – Commercial Items. This information is required on the offeror and significant subcontractors, teaming partners, and/or joint venture partners who are proposed to perform aspects of the effort the offeror considers critical to overall successful performance. Offerors are cautioned that the Government will use data provided by each offeror in this volume and data obtained from other sources in the evaluation of past performance.
Along with the information required in this paragraph, the offeror shall submit a Teaming Consent Letter (Attachment 13) executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a Client Past Performance Authorization Letter (Attachment 11), authorizing release to the Government of requested information on the offeror's performance.
The Government shall consider the entire COM-F III IDIQ size and magnitude when considering demonstrated recent and relevant record of performance, as well as the quality of that performance. Therefore, the offeror shall consider the entire COM-F III IDIQ size and magnitude when submitting past performance products and services that meet the contract requirements.
3.2.2 Past Performance Information
Each offeror is highly encouraged to submit the information shown in Past Performance Questionnaire (PPQ) (Attachment 12), for each relevant contract ten calendar days prior to the date for receipt of proposals. Failure to submit early proposal information will not result in offeror disqualification.
As soon as practicable, offerors shall complete Section 1 of the attached PPQ (Attachment 12) and email it to points of contact (POCs) the offeror has listed in the PPIS (Attachment 10). The POCs listed in the PPIS’s should represent the individuals most knowledgeable about the offeror’s performance on the identified contract or program, such as the following: (1) Contracting Officer’s Representative; (2) Program/Project Manager; (3) Contracting Officer. The POCs will complete the questionnaires and forward them by email directly to both CO and CS, Ms. Sarah C. Myers sarah.myers.3@us.af.mil and Gloria S. Hoybach at gloria.hoybach@us.af.mil. RESPONDENTS TO THE QUESTIONNAIRES SHALL NOT SEND THE COMPLETED INFORMATION SHEETS BACK TO THE OFFEROR. The Source Selection Evaluation Board (SSEB) will conduct follow-up with any POC as necessary. PPQs should be received by the CO and CS via email ten calendar days prior to the date set for receipt of proposals. The CO will use discretion in accepting PPQs received after the requested date.
3.2.3 Relevant/Recent Contracts
Submit information in accordance with the PPIS (Attachment 10) on no more than three recent contracts that you consider most relevant in demonstrating your ability to perform the proposed effort. Also, include information on no more than two recent contracts performed by each of your significant subcontractors that you consider most relevant in demonstrating their ability to perform the proposed effort. Task Orders submitted will count toward the three contracts limit for the prime offeror and two contracts limit for each significant subcontractor. Include rationale supporting your assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining recency and relevance, see Addendum to FAR 52.212-2, Evaluation – Commercial Items, paragraph 2.5 – Factor 2 Past Performance Factor. Each PPIS shall not exceed five pages. The Government may consider past performance in the aggregate in addition to on an individual contract basis. In the case of offerors for which there is no information on past contract performance or where past contract performance information is not available, the offeror may not be evaluated favorably or unfavorably on the factor of past contract performance (see FAR 15.305(a)(2)(iv).) In this case, the offeror’s past performance is unknown and assigned a performance confidence rating of “neutral.” Although the SSEB will not rate an offeror that lacks recent, relevant past performance favorably or unfavorably with regard to past performance, the SSA may determine that a “Substantial Confidence” or “Satisfactory Confidence” past performance rating is worth more than a “Neutral Confidence” past performance rating in a best value tradeoff. Any additional information or contracts over the maximums (3 contracts limit for the prime offeror and 2 contracts limit for each significant subcontractor) will not be considered for evaluation. Page limitations will also be followed in accordance with paragraph 1.2 above.
3.2.4 Specific Content
Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate. This may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to a limited or no confidence rating, since the problems encountered may have been on a more complex program, or an offeror may have subsequently demonstrated the ability to overcome the problems encountered. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. This may allow the offeror to be considered a higher confidence candidate. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems is required. Categorize the relevant information into the specific Technical subfactors and Price factor used to evaluate the proposal.
3.2.5 Organizational Structure Change History
Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
3.3 Table 1.2b Volume III– Initial GDSS Task Order Price
3.3.1 Price Reasonableness, Completeness, and Balance
This section is to assist you in submitting data other than certified cost or pricing data that is required to evaluate the reasonableness of your proposed price. Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal. The contractor shall submit pricing information to support the completeness and reasonableness of their proposed prices. The pricing information submitted should fully support PWS and TO Appendices requirements for each major functional element specified in the PWS. In addition, a breakdown should be provided for price traceability, which shows the skill mix, labor categories, number of hours, and labor rates. Information shall also be segregated by the performance periods and total price for each performance period. List the grand total for all performance periods. Additionally, unbalanced pricing poses an unacceptable risk to the Government and may be a reason to reject an offeror’s proposal. Proposal should be sufficiently detailed to demonstrate their completeness, reasonableness, and balance. The burden of proof for credibility of proposed prices rests with the offeror.
3.3.2 Price Data
Data beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your cost/price position. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price volume and in Attachment 16, TO Pricing Template. Under no circumstances shall this data and documentation be included elsewhere in the proposal.
3.3.3 Cost or Pricing Data Requirements
The contracting officer has determined there is a high probability of adequate price competition in this commercial acquisition and certified cost and pricing data under FAR 15.406-2 will not be required. Upon examination of the initial offers, the contracting officer will make a determination of price reasonableness, and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit data other than certified cost and pricing data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price IAW FAR 15.403-3.
3.3.4 Rounding
All unit and extended prices provided shall be rounded to the nearest dollar. Extended prices shall be divisible by the number of units proposed (i.e. 12 months) with no fractional cents. All loaded labor rates shall be rounded to the nearest penny.
3.3.5 Volume Organization
The Price volume shall consist of the following:
Ensure the price proposal includes detailed price amounts of all resources required to accomplish the task, (i.e. labor categories, labor hours, number of employees for each labor category, rates, etc.). These hourly requirements are to include (but separately identify) subcontractor(s). Offerors are cautioned to propose the most efficient organization consisting of the optimum utilization of management, labor categories, labor hours, and costs per task that realistically portray the support required to provide the services described in the PWS to the level described in the services summary. Your price proposal shall include the two month phase-in period (if applicable), ten month base period, and four twelve-month option periods.

Addendum to FAR 52.212-2, Evaluation-Commercial Items
1.0 SOURCE SELECTION
1.1 Basis for Contract Award
This is a competitive best value subjective tradeoff source selection conducted in accordance with Federal Acquisition Regulation (FAR) 15.3, Source Selection, as supplemented by the Defense Federal Acquisition Regulation Supplement (DFARS), and the Air Force Federal Acquisition Regulation Supplement (AFFARS). These regulations are available electronically at the Air Force (AF) FARSite, http://farsite.hill.af.mil. The Government will select the best overall proposal, based upon an integrated assessment of Technical/Technical Risk, Past Performance, and Price. A contract(s) may be awarded to the offeror who is deemed responsible in accordance with the FAR, as supplemented, whose proposal conforms to the solicitation’s requirements (to include all stated terms, conditions, representations, certifications, and all other information required by Addendum to FAR 52.212-1, Instruction to Offerors-Commercial Items, of this solicitation) and is judged, based on the evaluation factors and subfactors to represent the best value to the Government. The Government seeks to award to the offeror who gives the Air Force the greatest confidence that it will best meet, or exceed, the requirements. This may result in an award to a higher rated, higher priced offeror, where the decision is consistent with the evaluation factors, and the Source Selection Authority (SSA) reasonably determines that the technical and/or overall business approach and/or past performance of the higher price offeror outweighs the cost difference. The SSA will base the source selection decision on an integrated assessment of proposals against all source selection criteria in the solicitation (described below). While the Government source selection evaluation team and the SSA will strive for maximum objectivity, the source selection process, by its nature, is subjective; and therefore, professional judgment is implicit throughout the entire process.
1.2 Number of Contracts to be Awarded
The Government intends to award one COM-F III IDIQ contract from FA445218R0037 and one initial GDSS Firm Fixed Price (FFP) Task Order concurrently. The Government also anticipates awarding subsequent TOs in support of COM-F III following award of COM-F III IDIQ and initial GDSS Task Order, however, the Government reserves the right to make no award at all.
1.3 Discussions.
The Government intends to award without discussions, but reserves the right to conduct discussions if necessary. If, during the evaluation period, it is determined to be in the best interest of the Government to hold discussions, offeror responses to Evaluation Notices (ENs) and the Final Proposal Revision (FPR) will be considered in making the source selection decision. If the offeror’s proposal has been evaluated as acceptable at the time discussions are closed, any changes or exceptions in the FPR are subject to evaluation and may introduce risk that the offeror’s proposal be determined unacceptable and ineligible for award.
1.3.1 If the Government determines that, after the initial evaluation of proposals, discussions are necessary, the Government will make a competitive range determination (limited to a restricted number of the highest rated proposals) in order to establish a list of offerors with which it will conduct discussions.
1.3.2 Discussions will be initiated by the Procuring Contracting Officer (PCO) upon issuance of ENs to the offerors within the competitive range.
