SSP Attachment 3 Section L - Instructions, Conditions, and Notices to Offerors

THIS SECTION IS FOR SOLICITATION PURPOSES ONLY. THIS SECTION WILL BE

PHYSICALLY REMOVED FROM ANY RESULTANT AWARD, BUT WILL BE DEEMED

INCORPORATED BY REFERENCE.

52.215-1 INSTRUCTIONS TO OFFERORS—COMPETITIVE ACQUISITION

The offeror agrees to hold the prices in its offer firm for 220 calendar days from the date specified for receipt of offers.

The following instructions cover the preparation and submittal of the offeror’s proposal for this solicitation.

Offerors must follow the instructions contained herein. Offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined not eligible for award.

ADDENDA TO 52.215-1, PROPOSAL PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of three (3) separate parts; Part I – Completed RFP Documents, Part II – Price Proposal, and Part III – Past Performance Information.

B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.

C. Specific Instructions:

1. PART I – Completed RFP

(a) Part I, Complete blocks 12, 17a, and 30a, b, and c of the SF1449. In doing so, the offeror agrees to the contract terms and conditions as written in the SOLICITATION, with attachments. Additionally, the completed RFP shall consist of the completed and signed RFP, to include any amendments issued, with a cover letter delineating any exceptions taken to the RFP terms and conditions with accompanying rationale. However, offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined not eligible for award. Offerors shall ensure that all clauses and provisions that require “fill in” information are appropriately completed, including the proposed prices associated with the contract line items in the Schedule of the RFP.

2. PART II – PRICE PROPOSAL - Submit one (1) electronic copy of the completed Pricing Schedule (electronic copies must be compatible with Microsoft excel)

(a) Insert proposed unit and extended prices in the attached Pricing Worksheet to include all option periods. The extended amount must equal the whole dollar unit price multiplied by the number of units.

Additionally, the whole dollar unit price must be divisible by twelve (12). Future modifications and price changes will be rounded down to numbers that are within 2 decimal points and dividable by 12. The Pricing Worksheet must be completely filled in. The proposal must be submitted for a base period plus four (4) Option Years.

3. PART III – PAST PERFORMANCE INFORMATION - Only references for recent and relevant contracts will be considered. Submit one (1) electronic copy.

(a) Quality and Satisfaction Rating for Contracts Completed in the Past Five Years: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, etc.

(b) Performance Surveys: The government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The government will use information submitted by the offeror (Present/Past Performance Questionnaire) and any other sources of information available to the Government to assess past performance. Provide completed Present/Past Performance Questionnaires of no more than three (3), of the most relevant contracts performed for Federal agencies or commercial customers within the last five (5) years from the issuance date of the RFP. Relevant contracts include those with work similar in size and scope to the services described in the Performance Work Statement, to include experience in Supervision of personnel, providing equipment, materials and supplies, necessary to perform all Levels of Custodial Service in accordance with the Performance Work Statement.

The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed:

(i) Company/Division name

(ii) Product/Service

(iii) Contracting Agency/Customer

(iv) Contract Number

(v) Contract Dollar Value

(vi) Period of Performance

(vii) Verified, up-to-date name, address, FAX & telephone number of the contracting officer or contracting officer’s representative.

(viii) Comments regarding compliance with contract terms and conditions

If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past performance documentation that is both recent and relevant to this requirement.

(c) Past Performance Questionnaire: The Present/Past Performance Questionnaire (see

Attachment fill in attachment number) will be one means used by the Government to obtain present/past performance information. The offeror shall send out and track the completion of the Present/Past

Performance Questionnaires. The responsibility to send out and track the completion of the Present/Past Performance Questionnaires rests solely with the offeror - i.e., it shall not be delegated to any other entity.

Exert your best effort to ensure that at least two Present/Past Performance Questionnaire are completed and submitted directly to the Government no later than the date established in the RFP for receipt of proposals. Questionnaires sent directly from the offeror to the Government will not be accepted. Completed Present/Past Performance Questionnaire must be emailed to: jorge.alvarez.8@us.af.mil. & tricia.esannason@us.af.mil

(d) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. A Letter from all subcontractors must be provided with the proposal who will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.

General Information

A virtual site visit and virtual pre-proposal conference will be conducted by the 628th Contracting Squadron (628 CONS). Additional details will be forthcoming.

INFORMATION REGARDING SUBMISSION OF PROPOSAL: Proposals must be emailed electronically. The Solicitation Number, and the name of the offeror must be in the subject line. Emails postmarked after the solicitation has closed will be late proposals and will be processed in accordance with FAR 52.215-1(c)(3).

FEDERAL HOLIDAYS: The following Federal Legal Holidays are observed by this base:

JOINT VENTURE AGREEMENTS - Joint Ventures are allowable on competitive 8(a) set-asides, however, the joint venture agreement must be received by SBA prior to proposal due date and approved before award of any resulting contract. If you are contemplating a joint venture on this project, you must advise your assigned SBA Business Opportunity Specialist (BOS) as soon as possible. It is also recommended the agreement be submitted as soon as practicable to ensure compliance with established regulations.
