About this file

This document is a comprehensive quarterly inspection report for the Aircraft Fire Training Facility (AFTF) at Hurlburt Field Air Force Base in Florida, covering multiple inspection periods from 2021 to 2024. The report details extensive technical assessments across multiple systems including structural components, electrical systems, pneumatic systems, fuel systems, cooling systems, and training infrastructure. Key findings include multiple non-compliant and serviceable conditions, with recommended work orders addressing issues such as propane system valve replacements, structural member oxidation, safety modifications, and infrastructure improvements.

The inspection report highlights significant safety concerns, particularly with the AFTF, which is noted as "NOT CERTIFIED FOR HIGH RISK LIVE FIRE TRAINING" due to multiple infrastructure deficiencies. Recommended work orders total multiple line items focused on replacing oxidized components, improving safety features, upgrading electrical and plumbing systems, and ensuring compliance with NFPA, OSHA, and Air Force technical orders. The reports consistently recommend comprehensive overhauls of various systems, with estimated repair and replacement costs spanning multiple work orders. Despite the identified issues, the facility remains partially operational, with technicians performing ongoing maintenance and monitoring.

Serviceable Non-Compliant - Troop ramp does not meet TO 35E-1-2-13-1 (2015) standards. Recommend removal, disposal and replacement of existing troop ramp (and debris) with a TO 35E compliant hot dipped galvanized troop ramp 6' x 16' (SOW #009) 21-Q4. No change 24-Q1

Low wing height creates a fall hazard for firefighters during a training evolution and non-compliant.

Recommend installing a step to offer a step up and step down or a simulated rescue ladder in the center of the leading edge of the wing to prevent firefighter injury from falling (SOW #010) 21-Q4. No change 24- Q1

Non-Compliant doesnot meet both means of egress missing step on low wing and troop ramp not up to TO no change 24-Q1

AIRCRAFT EXTERIOR

High Wing Assembly front and rear connection points are deformed from what appears to be pulling. At the lowest point there is a 2" gap and the gap decreases the higher you go on the wing. Recommend 5" stitch welds every 12" the length of the seam to increase safety to prevent the front half from separating durign live fire training evolution where the steel members can soften and lose integrity (SOW #005) 21- Q4. No change 24-Q1

Good condition - There is a slight bow that will increase if columns are not isntalled on appropriately sized footing concrete. Recommend removing low wing assembly, installing concrete footing engineered for the location weather conditions and wind factor and reinstalling low wing assembly (SOW #007) 21- Q4. No change 24-Q1

Serviceable - Lower cross member in fuselage is showing signs of wear and oxidation. Recommend closely monitoring condition after each training evolution. 21-Q4 no change 24-Q1

Inboard and Outboard Engine stands are good condition, but the top of each cowling is not positively connected to the High Wing assembly except by 3x small dimension bolts - Recommend drilling 12x new bolt holes in both cowlings and install A325 galvanized structural bolts to positively connect High Wing to Inboard and Outboard Cowlings and prevent unexpected collapse (SOW #006) 21-Q4. SPECIAL NOTES:

I have reviewed the report and pictures in support of the Inspection report furnished, e-mail correspondence and conversations regarding the fire training facility construction, condition, and needed compliance repairs. Based on the information furnished, we have performed in a manner consistent with that degree of care and skill ordinarily exercised by members of the same profession currently practicing under similar circumstances at the same. To the best of my knowledge, information and belief, the inspection revealed general conformance and safety issues which are subject to applicable standards. In my professional opinion the minor conformance and safety issues noted are NOT cause for a facility lockout. However I do believe the issues addressed in this report should be prioritized and repairs or upgrade be made as soon as possible for complete compliance.
