EXECUTIVE SUMMARY

Tetra Tech EM Inc. (Tetra Tech) was tasked by the U.S. Air Force to conduct a Hazardous Materials

Survey (survey), specifically an asbestos survey and a lead-based paint (LBP) survey in Dormitory 90334 at Hurlburt Field in Okaloosa County Florida (subject property). The survey was conducted as part of the project Title I Design Services for Repair Dormitory 90334 (Tier 2) (Project No. FA8903-08-D-8781-

0147, Task 100-AMC-T98781.0147), to provide architectural and engineering design for renovations of the dormitory building.

The installation hosts Headquarters Air Force Special Operations Command (AFSOC), the 1st Special

Operations Wing (1 SOW), the United States Air Force Special Operations School (USAFSOS), and the

Air Combat Command's (ACC) 505th Command and Control Wing; the installation has been in operation as an U.S. military base since September 1942. Dormitory 90334 is currently used to house U.S. military personnel stationed at the base, and is surrounded by a mix of other types of buildings on base. The footprint of the dormitory encompasses approximately 9,700 square feet or 0.22 acre. The dormitory is bounded by a parking lot and office building to the north, green space and a food court to the east, an office building to the south, and green space and an office building to the west. Tetra Tech worked with

Hurlburt Field representatives to obtain access to interior areas of the building in order to conduct the survey.

During the survey, Tetra Tech collected 96 samples of suspect asbestos containing materials (ACM) and

11 samples of suspect LBP for laboratory analysis.

The following findings were based on survey observations and analytical results from samples collected during the survey of the dormitory:

• Regulated ACM was identified in the subject property building at the following locations:

Red sheet flooring with backing on the first floor in the custodial closet underneath 12x12 brown floor tile

Tetra Tech recommends no specific abatement actions for this confirmed ACM prior to renovation of the subject property building. According to the National Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations, if the ACM identified is less than 160 square feet, no notification is necessary and the material does not have to be removed by a licensed asbestos abetment contractor.

Per information provided by base representatives, previous abatement of asbestos containing 9x9 floor tile and associated mastic occurred at the dormitory. The base http://en.wikipedia.org/wiki/1st_Special_Operations_Wing http://en.wikipedia.org/wiki/1st_Special_Operations_Wing http://en.wikipedia.org/wiki/USAF_Special_Operations_School http://en.wikipedia.org/wiki/Air_Combat_Command http://en.wikipedia.org/wiki/505th_Command_and_Control_Wing

103IG1987 ES-2

representatives could not confirm that all the asbestos containing 9x9 floor tile and associated mastic was removed. During the survey, Tetra Tech did not observe any remaining 9x9 floor tile, but because of limitations on destructive sampling some asbestos containing 9x9 floor tile and associated mastic could be present under existing flooring materials.

Tetra Tech recommends that if during renovations any 9x9 floor tile and associated mastic is encountered, it be assumed to contain asbestos and be removed by a licensed asbestos abatement contractor before any renovation work disturbs the floor tile and associated mastic.

• According to the laboratory results, all painted surfaces sampled contained less than 1 milligram per square centimeter (mg/cm2) lead; therefore, no LBP was identified during the survey. Since some levels of lead were detected in the paint, the contractor performing the renovation work must comply with the Occupational Safety and Health Administration (OSHA) Lead in Construction Standard, Title 29 of CFR, Part 1926.62.

Survey specifics, analytical protocols, complete findings, and recommendations from the survey are detailed throughout the remainder of this report.

1.0 INTRODUCTION

Tetra Tech EM Inc. (Tetra Tech) was tasked by the U.S. Air Force to conduct a Hazardous Materials

Survey (survey), specifically an asbestos survey and a lead-based paint (LBP) survey in Dormitory 90334 at Hurlburt Field near Fort Walton Beach, Florida (subject property). The survey was conducted as part of the project Title I Design Services for Repair Dormitory 90334 (Tier 2) (Project No. FA8903-08-D-

8781-0147, Task 100-AMC-T98781.0147), to provide architectural and engineering design for renovations of the dormitory building. The primary purpose of the survey was to assess potential impacts to the subject property from hazardous substances. The scope of the survey included an inspection of the subject property building for the presence of asbestos containing building materials (ACM) and LBP.

The survey was completed in accordance with industry standard practice for hazardous materials surveys.

Tetra Tech’s Project Manager for the survey was Mr. Jeffrey Mitchell, Certified Asbestos and LBP

Inspector. The survey team included Mr. Jeffrey Mitchell and Ms. Lona Pearson, Certified Asbestos and

Lead-Based Paint Inspector. The survey strategy and sample methodology were developed based on the possibility that all parts of the subject property building, both inside and out, could be impacted by future renovation or demolition plans. Due to limitations on destructive sampling methods, additional suspect materials not detected may be located in walls, voids, or other concealed areas. Assumptions and deviations regarding the subject property building surveyed are identified in Section 8.0. Further survey work may be needed to comply with all local, state, and federal requirements regulating ACM and LBP prior to any renovation/demolition of the building not cited in this survey report.

Tetra Tech conducted the survey from August 30 through August 31, 2011. One purpose of the survey was to evaluate the subject property for the presence, quantity, locations, and characterization of ACM that may require abatement prior to any renovation/demolition activities, in accordance with National

Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations as adopted by the

U.S. Environmental Protection Agency (EPA). The intent of the asbestos NESHAP regulations is to protect the public (and workers) by minimizing release of asbestos fibers during activities involving processing, handling, and disposal of ACM.

Tetra Tech also conducted a screening for the presence, quantity, and locations of LBP exceeding lead hazard levels, which would require Occupational Safety and Health Administration (OSHA) worker safety precautions during remodeling activities. The subject property building was constructed prior to

1978, and LBP likely was used in the build-out of these structures. The LBP survey was conducted

103IG1987 2

according to protocols similar to the single-family housing inspection procedures in Department of

Housing and Urban Development (HUD) guidelines (HUD 1997).

Tetra Tech provided these services consistent with the level and skill ordinarily exercised by members of the profession currently practicing under similar conditions. This statement is in lieu of other statements either expressed or implied. The scope of services performed in execution of this evaluation may not be appropriate to satisfy the needs of other users, and use or re-use of this document, the findings, conclusions, or recommendations is at the risk of said user. Although a reasonable attempt has been made to identify suspect ACM and LBP in all parts of the building, the inspection techniques used are inherently limited in the sense that only full demolition procedures can reveal all building materials of a structure and therefore all areas of potential ACM or LBP. This survey report does not warrant against future operations or conditions that may not be consistent with its recommendations. Moreover, because of some limitations on destructive sampling during the survey, completion of the survey does not guarantee identification of all hazardous materials, ACM, or LBP—hazardous materials may be present in voids of walls or ceilings.

Section 2.0 of this report discusses the site structure. Section 3.0 specifies field survey and analytical protocols for the asbestos survey. Section 4.0 presents the field survey and analytical protocols for the

LBP screening. Section 5.0 presents the asbestos findings. Section 6.0 describes the LBP findings.

Section 7.0 offers recommendations based on the survey findings. Section 8.0 specifies assumptions and deviations. Section 9.0 lists sources referenced in developing this report.

103IG1987 3

2.0 SITE STRUCTURE

Dormitory 90334 is located at Hurlburt Field in Okaloosa County, Florida. Dormitory 90334 is a three-story structure that encompasses approximately 29,000 square feet. The building was constructed in the

1950s of concrete and some cinderblock walls with a rolled asphalt roof. Some renovations have occurred to the building, including covering the roof with a pitched steel roof. Interior finishes include concrete, drywall, and cinder block walls. Flooring materials include carpet, floor tile with associated mastic, and concrete. The bathrooms in each dorm room have ceramic tile on the walls and floors. The building is currently occupied and is used as military housing (see Appendix A, Figure 1).

103IG1987 4

3.0 ACM FIELD SURVEY AND ANALYTICAL PROTOCOLS

Tetra Tech made every effort to inspect all areas of the subject property building. Minor demolition of materials (destructive sampling) was required during the survey effort. The inspector took care to ensure that the subject property remained unoccupied during sample collection. Asbestos samples were collected in accordance with NESHAP as adopted by EPA and the Asbestos Hazard and Emergency

Response Act of 1986 (AHERA) protocols. AHERA defines “asbestos containing building material”

(ACBM) as any building material or product that contains more than 1 percent (%) asbestos. Suspected

ACMs were grouped as homogeneous areas if the material was similar in appearance and texture;

however, if the inspector decided that a material (for example, wall texturing) was not similar in appearance and texture to other materials in the building, the inspector distinguished the material as unique and collected samples of each unique material accordingly. Due to limitations on destructive sampling methods, additional suspect materials not detected may be present in walls, voids, or other concealed areas. Assumptions and deviations regarding the building surveyed are identified in

Section 8.0.

Bulk samples of suspected ACM were collected to ensure that each distinct layer of material was represented in the sample. A wetting agent was applied to friable surfaces prior to sample collection to reduce the potential for fiber release. All samples collected were placed in plastic bags, labeled, and sealed immediately upon collection. To prevent cross-contamination between samples, the sampling instruments were wiped clean using a wet, lint-free cloth after collection of each sample. A unique sample identification number was assigned to each sample.

The samples remained in the inspector’s custody until sent to the laboratory. Upon completion of sampling activities, the bulk samples were sent, along with Tetra Tech’s chain-of-custody documentation, to EMLab P&K in San Bruno, California. Suspect ACM samples were analyzed per EPA Method

600/R-93/116 by EMLab P&K using Polarized Light Microscopy (PLM) analysis. EMLab P&K is a member of the National Voluntary Laboratory Accreditation Program (NVLAP)-certified laboratory, certification number 200728-0. For Quality Assurance/Quality Control (QA/QC) purposes, Tetra Tech analyzed one duplicate sample for every 20 samples. Section 5.0 of this report summarizes the ACM analytical results. Sample locations are shown on Figures 2 through 5 in Appendix A. Appendix B includes the ACM analytical results and chain-of-custody forms for the bulk samples. Appendix D includes a photolog of suspect ACM sampled, and Appendix E includes laboratory and asbestos inspector certifications.

103IG1987 5

4.0 LBP SCREENING AND ANALYTICAL PROTOCOLS

Tetra Tech made every effort to inspect all areas of the building. HUD Guidelines for the Evaluation and Control of LBP in Housing (1997) suggests that paint applied before 1978 could contain lead.

A screening of suspected LBP was performed according to protocols similar to the single-family housing inspection procedures in the HUD Guidelines. Tetra Tech collected LBP chip samples of suspect painted surfaces that possibly would be impacted during renovation/demolition activities. Paint containing greater than or equal to 1.0 milligram per square centimeter (mg/cm2

A 2-square-inch-square template was used to accurately measure the sample size. The template was placed on the painted surface where the paint was observed. While held in place manually, the template was outlined with a permanent marker. A clean piece of paper was taped to the bottom of the template to create a funnel effect. The paint inside the template area was scraped off using a small chisel. The paint chip samples were collected in hard plastic containers, labeled, and sealed immediately upon collection.

To prevent cross-contamination between samples, the sampling instruments were wiped clean using a wet, lint-free cloth after collection of each sample. A unique sample identification number was assigned to each sample. The inspector took care to ensure that the subject property remained unoccupied during sample collection.

) or 0.5% by weight lead by laboratory analysis is considered LBP.

The samples remained in the inspector’s custody until sent to the laboratory. Upon completion of sampling activities, the samples were sent, along with Tetra Tech’s chain-of-custody documentation, to

Quantem in Oklahoma City, Oklahoma. Quantem is a member of the Environmental Lead Lab

Accreditation Program (ELLAP). The paint chips collected were analyzed for total lead. The results are summarized in Section 6.0. Screening and sampling locations are shown on Figure 2 in Appendix A.

Copies of the analytical results and chain-of-custody forms are included in Appendix C, and Appendix E includes laboratory and LBP inspector certifications.

7.0 RECOMMENDATIONS

Based on survey observations and the sample analytical results, Tetra Tech recommends the actions summarized below before renovation of the subject property building.

7.1 ACM

• Approximately 30 square feet of red sheet flooring with backing is located in the first floor custodial closet under 12x12 brown floor tile. The flooring material is represented by samples H-1-C-FT2-1, -2, and -3. Laboratory results indicated that the flooring material contained 30 percent chrysotile asbestos. Tetra Tech recommends no specific abatement actions for this confirmed ACM prior to renovation of the subject property building. According to the National Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations, if the ACM identified is less than 160 square feet, no notification is necessary and the material does not have to be removed by a licensed asbestos abetment contractor prior to renovation activites.

• Per information provided by base representatives, previous abatement of asbestos containing 9x9 floor tile and associated mastic occurred at the dormitory. The base representatives could not confirm that all the asbestos containing 9x9 floor tile and associated mastic was removed.

7.2 LBP

According to the laboratory results, all painted surfaces sampled contained less than 1 mg/cm2 lead;

therefore, no LBP was identified during the survey. Since some levels of lead were detected in the paint, the contractor performing the renovation work must comply with the Occupational Safety and Health

Administration (OSHA) Lead in Construction Standard, Title 29 of CFR, Part 1926.62.

103IG1987 15

8.0 ASSUMPTIONS AND DEVIATIONS

The entire dormitory building was visually surveyed for suspect ACM and LBP. The building is currently in use for housing military personnel in training at the base. Not all dorm rooms were accessible because the building is currently occupied. A representative number of dorm rooms and all common areas were accessible to conduct the survey. Access to these representative dorm rooms, along with information provided by the base representative, provided the survey team with sufficient information to complete the survey. According to information provided by base representatives, ACMs have been removed from the dormitory, specifically 9x9 floor tile and associated mastic present throughout the dormitory in the past. No specific dates when the material was removed could be provided. Signs of this floor tile can be seen in the hallways. Base representatives could not confirm if all this flooring material had been removed. Because not all dorm rooms were accessible, some of this asbestos containing 9x9 floor tile and associated mastic may still be present under carpet or other floor coverings in the dormitory. If during renovations any 9x9 floor tile and associated mastic is encountered, Tetra Tech recommends it be assumed to contain asbestos and be removed by a licensed asbestos abatement contractor before any renovation work disturbs the floor tile and associated mastic. The removed waste must be transported to a disposal site able to accept non-friable ACM.
