SECTION 01 11 00

IDIQ Repair and Replace Roofs Miscellaneous Base Buildings Master Spec.

PART 1. GENERAL

1.1. SUMMARY

SUMMARY OF WORK

The Scope of Work to be performed under this project is located at Hurlburt Air Force Base, Florida.

The work to be performed under this project consists of providing all the labor, equipment, tools, and materials necessary to refurbish, repair and/or replace roofing systems on various facilities maintained and operated by the 1 Special Operation Civil Engineering Squadron.

All work contained in this Section shall be completed in a professional and workmanlike manner, per Manufacturers' specifications and requirements; in accordance with the roofing industry standard and practices; and in accordance with latest edition of Local, State and Federal Codes.

All work shall comply with the requirements and specifications in this Section and each task order’s Scope of Work documents, as defined below. If there is conflict between requirements of these items and the particular Roof Specification, the Contractor shall comply with the most stringent requirement, unless the Contractor through direct negotiation with the Contracting Officer receives written approval from the Contracting Officer to comply with the less stringent of the conflicting requirement.

1.2. SCOPE OF WORK DOCUMENTS

A. All work shall be in conformance as specified in individual task orders including the furnishing of all materials, labor, plant, tools, equipment, and services necessary and incidental thereto. All work must be coordinated through the Base Civil Engineering staff liaisons to minimize the interruption of service. The work required to be performed by the Contractor consists of, but is not limited to, the following principal items contained in the below PROJECT

1.3. PROJECT DESCRIPTION

1.3.1. Architectural

Roof refurbishment, repairs and/or replacement may include, but is not limited to, the following types of roofing systems:

Shingles:

Asphalt fiberglass and organic.

Built-Up Roof Membrane (BUR):

Asphalt, Coal Tar, and Modified Bitumen.

Metal Roof Panel:

Retrofit metal roofing and framing system, Architectural standing seam roof panels, corrugated, structural and non-structural. All new Roofs shall have a high solar reflective rating.

Coatings:

Elastomeric and Energy-Star rated

Single-Ply Membrane:

PVC, EPDM, TPO

1.3.2. Structural

Selected facilities may require structural roof deck and framing repairs including, but not limited to:

Masonry wall cleaning and re-pointing.

Wood decking (planks and plywood) and framing (dimensional lumber, pre-fabricated truss and timber).

Addition of steel purlin supports for standing seam metal decking as required by current building codes.

Misc. metals for roof access complying with OSHA safety codes.

1.3.3. Mechanical

Selected facilities may require, but not be limited to, refurbishment, repair, removal and re-installation of existing HVAC, fresh-air intake and exhaust systems.

1.3.4. Electrical

Selected facilities may require, but not be limited to, refurbishment, repair, removal and re-installation of existing electrical equipment and lightning protection systems.

1.3.5. Painting

Painting of surfaces for new or existing materials by the contractor as repair or replacement, and all work incidental thereto.

1.3.6. Asbestos

Remove asbestos containing materials from roof and adjacent components of facilities, i.e., Asbestos shingles, roof penetration mastic, and soffits, etc.

1.3.7. Special Construction

All projects will require installation of roof-top fall protection systems, and may require structural roof deck and framing upgrades for fall protection systems. Also, mechanical equipment repairs may be required.

1.4. EXISTING WORK

Remove or alter existing work in such a manner as to prevent injury to the public and operations personnel or damage to any portions of the building or existing work which remain.

Repair or replace portions of existing work which have been altered during construction operations are to match existing or adjoining work, as approved by the Contracting Officer. At the completion of operations, existing work must be in a condition equal to or better than that which existed before the new work started.

1.5. "AS-BUILT" DRAWINGS (RECORD DRAWINGS)

A. When available, maintain at the job-site two sets of full size contract drawings, marking in red to show all variations between the construction actually performed and the requirements indicated or specified in the contract documents, including buried or concealed construction.

B. Where a choice of materials or methods is permitted or where variations in scope from that of the original contract are authorized, mark the drawings to define the construction actually performed.

C. Use standard drafting practices to represent such changes and include supplementary notes, legend, and detail as necessary to clearly portray the as-built construction.

D. Upon completion of the work, deliver to the Contracting Officer both sets of marked red-line drawings for approval and acceptance.

E. When contractor is required to provide design, provide drawings on computer disk in AutoCAD format and PDF to contracting officer at project completion along with both sets of marked drawings.

1.7. SUBMITTALS

SD-06 Test Reports

Submit the following items to the Contracting Officer:

Investigation reports Daily checklist

Daily logs reporting on the progress of daily activities or attesting that the work has been installed in accordance with the contract plans and specifications.

Frequency of these submittals will be as determined by the Contracting Officer.

1.8. SAFETY/ACCIDENT PREVENTION PLAN SUBMITTALS

Submit an Accident Prevention Plan in accordance with Section 01 35 26

1.9. OSHA INPECTIONS:

The contractor shall be subject to no-notice inspections under Occupational Safety and Health Act (OSHA) Program by inspectors of the Department of Labor.

At a minimum, the contractor must be familiar and comply with general safety requirements IAW DOL regulations, OSHA 1926/1910, as well as applicable AFOSH standards and AFR's. These standards and AFR's are available in the 1 SOW/SEG Safety Office, Building 90210 Suite 224, phone 884-2614.

1.10. PRE-construction CONFERENCE

The contractor will be required to attend a pre-construction conference at AFSOC 1 SOCONS/PKB, Building 90339, Hurlburt Air Force Base, or at an alternate location chosen by the contracting officer.

1.11. NOTICE TO PROCEED:

The Contracting Officer will establish a required start and completion date for each task order under this contract. The Contracting Officer will issue a Notice to Proceed (NTP) prior to the scheduled start date for each new construction project.

Any Government expenses incurred setting up a false start by the contractor shall be annotated in their performance rating. Contractor delays in starting a project on time that is not the contractors fault will not be annotated as a deficiency.

1.12. WORK SCHEDULING

Work Schedules for all on-site activities must be reviewed and approved by the Contracting Officer prior to start of the project and all activities must be coordinated with the Contracting Officer once work has begun. Due to the critical nature of the work location all work pertaining to this contract must be completed without disrupting and/or minimal disruption to the operation of the occupied facilities.

It may become necessary from time to time for the Government to stop the contractor from working due to high priority or sensitive missions. The contractor will cease work as directed by the Contracting officer. The IDIQ Repair and Replace Roofs Miscellaneous Base contractor may be required to repair or replace items within the scope of the contract to support the continued operation of the facility during this time as directed by the Contracting officer. For each calendar day and/or portion of a calendar day the contractor is removed from the job site, the contract schedule will be adjusted by one calendar day. When possible, the Government will provide 12 hour advance notification each time the work restrictions are invoked.

The roofing installation must be made watertight at the end of each day’s activity to prevent water infiltration into the completed roofing system installation. Complete all flashings and terminations as the roofing installation progresses. At the edge of the competed roofing system installation, extend the roofing membrane a minimum of 6” beyond the edge.

Seal the roofing membrane to the surrounding deck or substrate surface with hot asphalt or foam sealant. Remove all temporary night seal materials prior to continuing with the roof installation.

1.12.1 Regular Work Hours

Working hours normally range between the hours of 7:00 AM and 4:30 PM (0700- 1630) excluding Saturdays, Sundays and Federal Holidays. If work must be done during periods other than above, additional Government inspection and Support personnel may be required. See additional requirements in Section 1.13.2.

1.12.2 Work Outside Normal Duty Hours:

Notification must be given to the Contracting Officer ten (10) working days in advance of the Contractor’s intention to work during other periods to allow assignment of additional inspection forces when the Contracting Officer determines that they are reasonably available. If such support is reasonably available, the Contracting Officer may authorize work to be performed during periods other than normal duty hours/days. If inspections are required to be performed in excess of the normal duty hours/days solely for the Contractors need, the cost of the governments Civil Engineer’s inspection may be negotiated using overtime rates that will become incorporated into a task order modification making the applicable deduction from the total task order.

Work beyond normal hours/days will be required for major utility outages or due to natural disasters and any required government inspections will not be an additional charge to the contractor.

No construction work is to take place over the Christmas/New Year holiday period. This period will start one full work day before the Federal Christmas holiday and end the first work day after the Federal New Year Holiday. The Contractor may request to work during this time at the discretion of the Contracting Officer.

1.12.3 Government Holidays:

The following is a list of Federal Holidays observed by all Federal Installations:

New Year's Day, January 1 Martin Luther King, Jr., 3rd Monday in January President Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4 Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October

Veterans Day, November 11 Thanksgiving Day, 4th Thursday in November Christmas Day, December 25

NOTE: Any of the above holidays falling on a Saturday will be observed on the preceding Friday; holidays falling on a Sunday will be observed on the following Monday.

1.13 TASK ORDERS:

Performance of service shall be accomplished by the contractor by individual task orders and they will be issued by the Contracting Officer prior to the beginning of specified performance periods during the period of the contract.

The contractor is not required to accept any project during any period which would cause that amount stated in the IDIQ Contract to be exceeded. Contractor invoices are required to be input per the IDIQ Contract Section G Wide Area Workflow Instructions.

1.14 AUTHORIZED GOVERNMENT REPRESENTATIVE:

Personnel authorized to initiate projects for the government against Task Order will be specified on each order.

1.15 SPECIAL PROVISIONS FOR MEASUREMENTS:

A. Prior to the issuance of a "project", the Contracting Officer's representative and the contractor shall together measure each building/facility to determine the quantities of work involved in the Bid Schedule. After the quantities have been computed, they shall be placed on forms (which when executed by the Contracting Officer will constitute a "project" in accordance with the "Task Orders, and/or these Special Instructions) supplied for the various groups in the Bidding Schedule and total costs determined. This form will be signed by the Contracting Officer's representative and the contractor for measurement of work and signed by the Chief, Engineering Division, or his duly assigned representative for verification of the requirement for the work for each request. Three copies are required; one copy for the Contracting Officer, one copy for the contractor's use and one copy for contracts inspection.

If the contractor refuses to take part in the measuring, the contractor will be required to accept the measurements made by the government.

B. No work can be performed until a "Task Order" is issued by the Contracting

Officer, or his duly assigned representative.

C. In the event more than one building/facility is covered by a "project", separate measurements and total costs shall be made for each building/facility involved in the "project."

D. Materials found to be deteriorated after the original survey, including defective materials uncovered during construction, shall be promptly brought to the attention of the Contracting Officer. The Contracting Officer or the Contracting Officers Representative will be the determining authority as to how much, or what materials, will be replaced. Materials replaced by the contractor without prior approval will be at the Contractor’s expense.

1.16 INSPECTION AND ACCEPTANCE:

Inspections and final acceptance will be made on each separate building/facility as completed by the contractor with the following additional provisions:

A. The contractor shall give the Government a minimum of fourteen (14) days notice for all requested inspections. Inspections shall be staggered such that two inspections will not occur at the same time.

B. The contractor shall make sure that the work is ready for all inspection.

The presence of an unreasonable number of deficiencies, as determined by the Contracting Officer's representative, may cause the inspection to be rescheduled.

1.17 CONTRACT PROGRESS REPORT

A. Contractor progress reports shall be made weekly in a timely manner and in accordance with the contract documents.

B. Contractor shall use the Contract Progress Report form, AF Form 3065.

C. Contractor shall submit a completed form to include only those items of work applicable to each project.

D. “Demobilize/Close-Out Documents” has been assigned a value of 3%. This amount will be withheld from final payment until such time as all project record documents (including "As-Built" drawings), warranties, maintenance service, etc., have been turned over to the government. The contractor and subcontractors are advised to prepare these documents as the work progresses and not wait until the end of the project. These documents must be turned over to the government prior to the final inspection.

1.18 CONTRACT PROGRESS SCHEDULE

The contractor must provide a copy of the Contract Progress Schedule for review by the Contracting Officer and the Construction Project Manager as required by the Contracting Officer. If disapproved, the contractor shall resubmit the revised Contract Progress Schedule within 2 days of the date of the disapproval. The Contract Progress Schedule must be approved within 10 days of the date of the disapproval. No construction work shall start without an approved Contract Progress Schedule. Contract Progress Schedules are required for projects lasting 60 days or more, reference contract clause

FAR 52.236-15.

1.19 ALLOTTED WORK TIME:

When a project is issued it will specify a required start date and a required completion date computed in accordance with the Period of Performance Schedule. The contractor may start work at an earlier date and complete at an earlier date than specified provided he gives the Contracting Officer two

(2) days advance notice in writing. This earlier start date must be approved by the Contracting Officer. If earlier start date is approved, the Contracting Officer will re-compute required completion date to comply with the allotted work time.

Note: All orders having a Period of Performance that is in the time frame of the day before Christmas through the day before the first work day after the New Year Holiday requires compensation of each work day affected by the Christmas and New Year holidays.

1.20 MEASUREMENT AND PAYMENT:

Measurement shall be in accordance with the unit as indicated in the Approved progress report/Bid Schedule. Payment for work covered under these specifications applicable in the Bid Schedule shall be calculated on the basis of the unit price offered by the contractor and the proposed quantity.

IN – PLACE QUANTITIES

A. The Contractor shall be paid for all work provided to the Government as part of the individual Task order. The Government will verify and compensate the Contractor for the in place quantity of materials provided by the Contractor as part of the Task order.

QUANTITY OF MATERIALS

A. Several line items are determined by the quantity of material used in

Task order. The quantity of materials used to determine the appropriate line item is the total quantity per Task order.

LABOR, EQUIPMENT AND MATERIAL COSTS

A. Unless otherwise specified in the following paragraphs, all required labor, equipment, material, and incidentals required to perform line item work per specifications should be included in the line item, i.e.

include temporary perimeter fencing, warning tape, safety gear, ladders, forklifts, lifts, scaffolding, dumpsters, Material delivery and removal.

OVERHEAD,PROFIT AND G & A

A. All line items include the Contractors expense for overhead, profit and general & administrative. No additional fees will be paid to the Contractor for overhead, profit and G & A expenses such as insurance, bonding and managerial expenses.

INCIDENTAL WORK

A. Incidental work including but not limited to: erection of barricades and

FOD prevention, administrative expenses, re-work, etc, shall be included in the base items listed in the bid schedule and no separate payment will be made for these items or any other item not listed in the bid schedule unless approved by the Contracting Officer on an individual case by case basis.

ESTIMATE OF NON-PRE-PRICED ITEMS

A. Items required by the Task order statement of work that are not included in the pre-priced line items shall be organized at the bottom of the proposal, listing a short description of work, quantity, unit price and total price. Non-pre-priced item costs shall be supported by price quotes from 3 verifiable sources.

NON-STANDARD WORK HOURS

A. Any task orders required to be done outside the bases standard work hours shall be priced as detailed above and then multiplied by the percentage for non-standard work hours on the bid schedule. Work performed during non-standard work hours at the option of the contractor shall be paid based on standard work hour prices.

1.21 FIRE REGULATIONS:

The contractor will be required to comply with the fire protection requirements in the United Facilities Criteria (UFC) 3-600-01 governing Fire Protection Systems.

1.22 ESCORT IN RESTRICTED AREAS:

Each Contractor crew will be escorted for work in restricted areas. Escorts shall be scheduled by the Contractor through the Contracting Officer 15 days prior to starting work in restricted areas. Escort will be provided by the

Government only during normal working hours, unless authorized by the Contracting Officer, such as for weekend work.

1.23 SEVERE WEATHER:

It is the responsibility of the contractor to be informed and to anticipate the onset of severe weather which could adversely affect his work.

Upon receipt of a severe weather warning, the contractor will take immediate action to tie down, remove, protect, or secure his materials and equipment in order to reasonably assure that Government property will not be damaged.

If the Contractor fails or refuses to secure all work materials and equipment to the satisfaction of the Contracting Officer’s Representative, the work will be accomplished by Government personnel and the cost thereof charged to the Contractor.

1.24 EXISTING UTILITIES:

Record drawings showing existing utilities are located in Building 90053, Hurlburt AFB. Contractor shall avail himself of the drawings. Any utility line shown on the record drawings (or made known to the contractor) and damaged during construction work will be repaired immediately by the contractor at no cost to the government.

1.25 PROJECT VERIFICATION:

The contractor shall visit the job site to verify location, conditions, and dimensions prior to starting work.

1.26 TOILET:

Toilet facilities are not available for use by contractor personnel. The contractor shall provide portable toilet facilities.

1.27 STORM Water Pollution Prevention

The Contractor shall take positive action to prevent work-generated refuse from leaving the job site. The Contractor shall employ Best Management Practices (BMP) in the form of sand bags, silt fences and straw rolls when necessary and comply with Environmental Protection Agency (EPA) regulations for the National Pollution Discharge Elimination System (NPDES).

1.28 OCCUPANCY OF PREMISES/ACCESS REQUIREMENTS/WORK PLAN

Building(s) will or will not be occupied during performance of work under this Contract. If occupied, Contractor shall post notifications detailing type of work being performed (i.e. Roofing Work Construction Area, Material Storage Area) in a prominent location in the work area.

The contractor shall notify the Contracting Officer of intent to start a minimum of 24 hours prior to time work is to commence in any specific assigned area. Before work is started or the notice to proceed is issued, the Contractor is to submit a Work Plan to the Contracting Officer or Contracting Officers Representative for approval. The plan must include a sequence of procedures, means of access, space plan for storage of materials and equipment, and use of personnel and vehicle approaches to the facility, corridors, hallways, and stairways. The Work Plan will entail a sketch of work area and/or a written description of the Work Plan. A plan is required whether the building is occupied or not.

1.28.1 Facility Contents

1.28.1.1 Responsibilities

Closely coordinate the schedule of work with the Contracting Officer. Unless otherwise stated, remove all piping, equipment, cable, conduit, lighting, miscellaneous supports, etc., as required to perform the work. Any movable or fixed lighting, miscellaneous supports, equipment, piping, etc., remaining in the work area must be covered and protected from damage. Store and protect all relocated items from damage throughout the construction period. After completion of work and prior to final acceptance, replace removed items in their original location. If materials are damaged in improper removal, the contractor is required to replace the material at their own expense.

Typical demolition of systems must not start until replacement equipment/materials are on site. Not all roofing systems will require “all” replacement equipment/materials on site prior to starting work, i.e. lightning protection.

1.28.1.2 Damage To Existing Equipment

At the sole discretion of the Contracting Officer and/or Project architect/engineer, the Contractor is to repair and/or replace any damage to adjacent and/or existing materials, equipment or systems which has occurred from activities or neglect by the Contractor at no cost to the Federal Government.

1.28.1.3 New Material and Construction Equipment

Only materials and construction equipment designated for performance of contract work are to be stored at the construction site or located in Government-controlled warehouses or shop facilities.

1.29.2. Construction Area Access And Control

1.29.2.1. Access and Control Requirements

Exclusive use or control of any area will not be allowed as the Government must have access to all areas at all times. All work must be scheduled around operational constraints.

1.29.3. Vehicle Parking

1.29.3.1. Requirements

Job and support vehicles for workers must be parked in designated parking areas within reasonable access to the worksite. All vehicle operators must comply with all traffic laws including mandatory seat belt usage, speed limits and hauling restrictions. Contractor is responsible for repairing or replacing any facility/grounds damaged areas, unless notified otherwise by the

1.30. SEPARATE CONTRACTS:

The Government reserves the right to award separate contract or contracts for work other than work covered by this contract, or to perform repairs or construction with indigenous forces. Such work will be programmed by the Contracting Officer in such a manner and sequence that work under this contract can proceed with a minimum of interference and inconvenience to the Government and the contractors. The Government reserves the right to perform repairs or construction with indigenous forces whether covered by this project or not.

1.31. TRAFFIC LAWS:

Employees are cautioned to obey all speed laws, parking regulations and traffic control devices. Failure to do so can result in loss of driving privileges and debarment. Privately owned contractor vehicles are prohibited from driving on the flight line, taxiways and aircraft parking areas, except when specific permission has been granted by Base Operations. Flight line work requires an A Class driver’s license. In addition, seat belt usage is mandatory on Base and hand held cell phone usage while driving is strictly prohibited.

1.32. HEALTH AND SAFETY:

1.32.1. Health and Safety Plan:

The Contractor shall prepare, maintain, implement, and comply with Air Force, OSHA, EPA, state and local health and safety regulations regarding the proposed work effort. To the extent practical, the Contractor shall tailor existing corporate Health and Safety Plans (HSP’s) to the current effort and shall use EPA guidelines for designating the appropriate levels of protection at the sites.

1.32.2. Accidents and Medical Information:

In the case of an emergency, call 911 and tell the operator the location of the emergency is on Hurlburt AFB.

Accidents involving injuries to Air Force personnel or damage to government property involving the contractor may be reportable USAF mishaps and must be reported to the Contracting Officer and the Ground Safety Office (AFSOC/SEG), phone 884-2829.

The Contractor is responsible for reporting all accidents incidental to the Contractor's operation to the Contracting Officer. Notification is to occur on the same day of the mishap or accident takes place with any employee or subcontractor working under the contract.

The name of the Contractor’s physician(s) and preferred hospital (for use in the event of industrial injury of employees) shall be posted by the Contractor at the site of the work in a prominent place where it can easily be seen by the workers. This information shall also be furnished to the Director of Base Medical Services, 1 SOMDG/SG.

1.33. STORAGE, TRANSPORTATION AND PROTECTION

1.33.1. Storage

Notwithstanding Contract Clause, FAR 52.236-10, Operations and Storage Areas, subject to approval by the Contracting Officer and in turn, availability and need, a storage/staging area shall be provided at the subject project site.

Contractor may be assigned a storage area upon written request to the Contracting Officer. Contractor's request must indicate dimensions of container, trailer, size of storage area, and utilities required. All trailers and containers are to be in good physical and safe condition with locations compliant to NFPA 241.

Contractor is responsible for contacting the local phone company to request a telephone number and line for their office trailer.

Contractor is responsible for the storage and protection of all his materials and equipment, whether incorporated into the job or not.

1.33.2. Transportation

Transport and handle products in accordance with manufacturer's instructions.

Promptly inspect shipments to ensure the products comply with project requirements, proper quantities, and are undamaged. Protect products subject to deterioration with impervious sheet covering. Provide ventilation to prevent condensation and degradation of products.

1.33.3. Hazardous Materials

Hazardous Materials and petroleum product containers must be stored on an impervious surface with secondary containment. Incompatible materials must be segregated and have separate containment systems. Additional HAZMAT requirements are provided in Section 01 57 20 "ENVIRONMENTAL PROTECTION". Submit a complete list of Hazardous Materials (including all paint) which are used to perform the work. The Contractor shall submit their list of hazardous materials either before commencing the work on any task order, or after the contract is awarded, prior to any work beginning.

1.33.4. Site Protection Plan

Structures, utilities, sidewalks, pavements, and other facilities immediately adjacent to excavations must be protected against damage. The Contractor must comply with COE EM-385-1-1 and is required to protect areas adjacent to the construction site.

1.34. PROTECTION OF WORK

Prior to performing any construction or demolition work notify the Contracting Officer, fourteen (14) days prior to the start of work, to enable the Contracting Officer or Ground Safety Office (AFSOC/SEG) to review the protective measures being taken to prevent hazards to employees and possible damage to adjacent structures and/or finishes or materials.

Store and protect products in accordance with manufacturer's instructions.

Store products with seals and labels intact and legible. Store sensitive products in weather tight, climate controlled enclosures in an environment favorable to the product. Products acceptable for exterior storage must be on sloped supports above ground.

Provide and install temporary dust suppression measures as required for the work being performed at the building site. If interior leaking occurs, the government property must be protected from water damage.

1.35. GOVERNMENT-FURNISHED MATERIALS - None

1.36. ON-SITE PERMITS

1.36.1. Utility Outage Requests and Connection Requests

Schedule work to hold outages to a minimum.

Utility Outages and Connection Requests required during the execution of work that affect existing systems must be arranged at the convenience of the Government which may require scheduling outside of the regular working hours or on weekends, at no additional cost to the Government.

Each Utility Outage and Connection Requests must state the system involved, area involved, approximate duration of outage, and the nature of work involved.

1.36.1.1. Requirements

A written request for a utilities outage (electricity, communication, water, compressed air, gas, steam, air conditioning, etc.) must be submitted to the Contracting Officer at least fourteen (14) calendar days in advance of the desired outage. Approval is required prior to scheduling any outage. All limitations noted on the approved outage authorization must be carefully observed. Final verbal coordination must be made four (4) hours prior to the outage by calling the Contracting Officer. A utilities outage without prior approval and coordination is prohibited.

Since many systems are critical to facility operations, the number and duration of utility outages must be limited to a four (4) hours maximum at any one time, unless otherwise approved by the Contracting Officer due to unusual circumstances. Coordinate all work efforts with sufficient manpower, materials, and equipment to complete the work within the authorized outage window.

Existing utility services may be interrupted only when approved by the Contracting Officer. Schedule the interruption at a time of minimum demand on the utility, which is convenient to the Government. Perform all outages on weekends, holidays, or evenings at no additional cost to the Government.

The Contractor is to make all necessary arrangements to schedule and provide connections to existing utilities and to cause minimum interruption to system operation. All temporary utility hook-ups and disconnects are to be made by the Contractor.

If the Contractor fails to comply with a scheduled outage, the cost to reschedule the outage will be deducted from the final contract payment.

Government ‘may’ deduct liquidated damages (LD’s) for a contractor’s non-performance in a scheduled power outage. However, any additional charges not listed in the LD’s have to be based on the government’s actual expenses.

1.36.2. Work Clearance Request

All construction work requires a Work Clearance Request, AF Form 103. This permit is processed by the Contractor. It is required prior to the start of any construction at the project site and must be posted in a conspicuous location on site during all construction activities. The normal processing time for a work clearance request is 10 working days.

Obtain (from the 1 SOCES CEG Construction Representative), prepare, and properly complete AF Form 103. Provide coordination for location of buried structures and utility lines before beginning any work involving digging/excavation. The 1 SOCES CEG Construction Representative must be the first and the last to coordinate on the AF Form 103.

1.36.3. Excavation, Brush/Tree Clearing And Welding Permits

Submit Excavation Requests, Brush/Tree Clearing Requests, and Welding Requests to the Contracting Officer. Permits must be obtained by the Contractor, within fourteen (14) calendar days, for Submittal to the Contracting Officer.

Post permits at a conspicuous location in the construction area.

Burning of trash or rubbish is not permitted on any USAF facility.

1.37. SALVAGE AND DISPOSAL OF MATERIAL AND EQUIPMENT

1.37.1. Removal

Remove and/or relocate only those materials or items of equipment specifically indicated in the drawings and specifications. Perform removal operations in such a manner that adjacent areas, installed equipment, or existing utilities are not damaged. Repair all openings that occur due to removal or demolition operations to match adjacent, existing surfaces. Repair any damages incurred during removal operations at no additional cost to the Government.

1.37.2. Housekeeping

Provide sufficient personnel, labor, and equipment to ensure compliance with all housekeeping requirements. Job sites are to be kept clean on an ongoing, daily basis. All areas are to be thoroughly cleaned prior to leaving the jobsite at completion of project work.

1.37.3. Salvage of Material And Equipment

All salvageable materials or items to be removed must remain the property of the Federal Government. Salvageable items will be indicated for each facility in the construction documents and/or drawings. Salvageable items must be transported to and deposited as required per SOW.

Salvageable material must be segregated and separated into categories before transport. All salvageable items delivered to DRMO must be accompanied by a properly prepared DD Form 1348. This form will be furnished by the Contracting Officer. Furnish a signed copy by DRMO reflecting the turn in materials to the Contracting Officer.

Maintain property control records for material or equipment designated as salvage. Contractor's system of property control may be used if approved by the Contracting Officer. Contractor is responsible for storage and protection of salvaged materials and equipment until disposition by the Contracting Officer.

Material to be salvaged and re-installed by the Contractor must be protected during removal and stored to prevent damage.

For reference purposes salvageable is defined as: items, material, equipment which can be refitted, reworked, and restored and put to use or sold. The Contracting Officer has sole discretion as to determining whether any particular item is salvageable.

1.37.4. Waste Disposal

Remove non-salvageable material and debris from work area and dispose of daily unless a dumpster/container is on the project site.

The Contractor shall satisfy the following requirements for the transportation of waste materials off site:

a. The Contractor shall inspect all vehicles prior to their leaving the project site to ensure environmental compliance.

b. The Contractor shall deliver site material only to the approved disposal facilities on the shipping manifest.

c. The Contractor shall be responsible for any and all actions necessary to remedy situations involving material spilled.

d. Site material will be stored and transported in bulk containers.

e. Prior to leaving the site, a load inspection will be conducted by the

Contracting Officer or representative. The Contractor shall submit the following:

1. A complete and accurate manifest, signed by the Hurlburt Hazardous Waste Program Manager.

2. Utilization of the proper DOT-approved shipping container.

3. Proper labeling in accordance with DOT regulations.

4. Bill of Lading traceable to the manifest when required.

5. Validation that all waste shipment containers are in sound condition and are not leaking.

6. Validation that a certificate of insurance is in force.

Refer to Section 01 57 20 "ENVIRONMENTAL PROTECTION" for further waste disposal and record keeping requirements.

1.37.5. Cleanup

Remove all waste materials, except indicated salvaged items, generated by the construction from U.S. Government property at the end of each work day and deposited. Disposal must be in accordance with federal, state, county and city ordinances and at the expense of the Contractor.

Upon completion of the construction each day, leave the work premises in a clean, neat and workmanlike condition, satisfactory to the Contracting Officer.

Remove bonding adhesive, bituminous markings and other contaminants from finished surfaces. In areas where finished surfaces are soiled with asphalt or any other source of soiling caused by work of this or other sections, consult manufacturer of surfaces for cleaning advice and conform to those instructions.

1.38. COMMUNICATION SECURITY

Government telecommunications networks are continually subject to interception by hostile/unfriendly intelligent organizations. Therefore, the DOD has authorized the military departments to conduct COMSEC monitoring and recording of telephone calls from, or terminating at, DOD organizations. The Contractor shall assume the responsibility for ensuring frequent dissemination of this information to all employees dealing with official DOD information.

1.39. DETERIORATED CONSTRUCTION AND FINISH

The Contracting Officer's representative will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement and whether the surfaces are suitable for painting.

1.40. WORK CREWS

The contractor will be required to have a minimum of one (1) crew (2 people or more) for work covered by this contract, and provide additional crew(s) as the work justifies. Each crew shall be capable of operating simultaneously and independently of the other crew(s).

1.41. OZONE DEPLETING AGENTS:

No ozone depleting agent will be utilized in this contract.

PART 3. EXECUTION

3.1. ACCESS to Hurlburt

AFB

Master Spec.

A. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract.

Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

B. The contractor shall submit a written request on company letterhead to the Contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter is required regardless of the length of visit and will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The Contracting officer will endorse the request and forward it to the contract specialist for processing into the Security Forces data base. Contractors requiring base access for meetings or site visits must complete the Hurlburt CAB (Contractor Access Badge) Affidavit.

Contractors requiring access to work on base will need to complete the Hurlburt CAB Affidavit and a Form I-9 (Employment Eligibility Verification). These documents must be delivered to the Hurlburt AFB East contractors Gate Pass and ID Office in a sealed envelope with a point of contact (POC) and phone number on the front of the envelope. You may also mail or FEDX the documents to:

Anticipate a seven-day lead time for Security Forces to process the documents and conduct the required background check. Once the background check is complete, a copy will be returned to the POC listed on the envelope. The prime contractor can provide paperwork for their subcontractor(s) and submit as many documents per envelope as necessary.

C. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate and social security card to obtain a vehicle pass. All deliveries to contractor work sites on Hurlburt AFB main base must go through the East Contractors Gate, located off Mr. Luther King Blvd. Trucks trying to enter the other gates will be turned around and told to use the East Contractors Gate.

Contractors’ work trucks carrying equipment and tools are also required to use the East Contractors Gate.

D. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

E. When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management citing the appropriate paragraphs as applicable.

F. Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

G. Failure to comply with these requirements may result in withholding of final payment.

3.2. Hurlburt Deliveries

3.2.1. Non-Hazardous Deliveries

All deliveries of construction material and equipment to job sites (including cement trucks) must use the East Contractors Gate for access. To the maximum extent possible, deliveries must be scheduled between 0800 and 1500 hours to avoid the peak traffic volume times. The driver of each delivery vehicle and their passengers must have the following:

a. A valid photo ID; and

b. A valid bill of lading; and

c. A known delivery point on Hurlburt/Facility# or specific location laydown area

d. An on-base or local phone number and POC to vouch for delivery

Only deliveries that can be verified will be allowed to proceed. Special deliveries, deliveries after normal business hours, or deliveries that cannot negotiate through the barricades must be coordinated in advance.

All delivery vehicles are inspected by Security Forces personnel prior to being allowed to proceed onto the installation.

3.3. RESPONSIBILITIES

The Government reserves the right to revoke badging authority, escort authority and/or base access privileges for any person, at any time and for any reason. The Contractor understands that the Government has made every attempt to identify potential delays associated with background checks and badging requirements and that such delays have been considered in the construction schedule. Delays resulting from the Contractor's failure to follow the badging guidelines above shall not be considered grounds for contract extension or compensation.

3.4. EXCAVATION

3.4.1. Dig/Utility Locate Permit Requirements

All excavation work requires a Dig/Utility Locate Permit. The Contractor shall complete the dig/utility locate permit prior to excavation.

Perform excavation only after the permit is received and then with extreme caution to prevent damage to the existing buried utilities in the area. All open trenches must be adequately barricaded.

3.4.1.1. Location of Buried Structures and Utility Lines

Accurately locate and stake structures and utility lines indicated by the Government provided Utility Locators on Hurlburt. Provide a drawing indicating the full extent of digging/excavation (width, depth, and length of trench or hole) and attach to the AF Form 103. If unidentified underground utilities are encountered during excavation, notify the Contracting Officer and cease operations until they are properly identified.

3.4.1.2. Excavation, Trenching, and Backfilling

Open only those trenches for which material is ready to be placed. As soon as inspected and approved by the 1SOCES CEG Construction Representative, trenches shall be backfilled and tamped as required by the drawings and specifications. As a minimum, the topsoil must be replaced and the disturbed area must be grassed by sod, watered and maintained for a minimum of two feet from the edge of the excavation. Mark or barricade construction work which may present a hazard.

3.4.2. Restrictions

All excavation at the work site within ten (10) feet of located underground utilities must be accomplished by hand only.

3.4.3. Barricades

Guardrails, fences or other suitable barricades and warning lights must be placed at all open excavations/trenches which are adjacent to paths, walkways, sidewalks, vehicle parking areas, and other pedestrian or vehicle thoroughfares. Operational yellow flashing lights shall be placed a minimum of every twenty (20) feet along open trenches as warnings during night time hours.
