GENERAL REQUIREMENTS

July 2009

THE GENERAL REQUIREMENTS SPECIFIED WITHIN THIS DOCUMENT PROVIDE A BASELINE

STANDARD OF THE REQUIREMENTS FOR ALL CONTRACTS ISSUED AT SCOTT AFB. IF THESE

REQUIREMENTS AND THE STATEMENT OF WORK CONFLICT, THE MOST STRINGENT

STANDARD SHALL APPLY.

I. GENERAL

A. DESCRIPTION OF WORK: The contractor shall furnish all personnel, equipment, tools, materials, supervision, and other items and services necessary to design, manage, and accomplish a broad range of maintenance, repair, alteration, and/or new construction work on various real property facilities. All work required by the project documents shall be accomplished by the contractor even though minor items required may not be specifically mentioned in the above listing.

B. LOCATION: The work is located at Scott Air Force Base, Illinois. The Base is located approximately 20 miles east of St. Louis, Missouri, south of Interstate Highway 64.

C. STANDARD WORKDAY/WEEK: Work shall normally be accomplished during the standard 8-hour day, 5-day week, Monday through Friday, between the hours of 7:30 A.M. and 4:30 P.M. local time, excluding Federal Legal holidays and non-work days resulting from holidays occurring on weekends. If desiring to work other than the standard 8-hr day, submit a written request at least three work days prior to the proposed performance of work to the Contracting Officer (CO). State the proposed performance of work, day/days, time/amount of work, and reason why permission should be granted to perform work during non-standard hours.

LEGAL HOLIDAYS

Scott Air Force Base recognizes the following holidays:

D. BASE AND SAFETY REGULATIONS: Comply with all applicable Air Force regulations and directives pertaining to security, safety, occupational health, fire, traffic, and personnel clearances while on Scott Air Force Base.

E. SAFETY BRIEFING: Primary contractors will brief subcontractors on safety, occupational health, and fire prevention regulations, emphasizing the requirement to obtain a welding permit, when welding, cutting and brazing operations are required, and obtain authorization from the Base Fire Chief when open burning is intended. Maintain documentation that all subcontractors have been briefed.

F. FORCE PROTECTION/ANTI-TERRORISM STANDARDS: All construction shall use UFC 4-010-01, DoD

Minimum AT Standards for Buildings issued under the authority of DoD Instruction Number 2000.16, DoD Antiterrorism Standards which requires DoD Components to adopt and adhere to common criteria and minimum construction standards to mitigate antiterrorism vulnerabilities and terrorist threats.

II. SUBMITTAL REQUIREMENTS

A. SCHEDULE OF MATERIAL SUBMITTALS: For the majority of projects, a material submittal schedule will not be provided by the Government. Solicitation instructions will specify if offerors will be provided a material submittal schedule or if they are to include in their proposal a material submittal schedule. If the requirement for a material submittal schedule is absent from the individual solicitation documents, the following guidelines apply. After award, the contractor awarded the contract shall complete and submit a material submittal schedule for approval by the Government. The contractor shall identify all planned submittals dates on the schedule. Submit the schedule to the Contracting Officer within 30 calendar days after contract award or within another agreed to timeframe as directed by the CO.

B. SUBMITTALS: Submit to the CO for approval all items as required by the solicitation documents, specifications and submittal register. Material submittals shall comply with Unifed Facilities Criteria (UFC), the Scott Air Force Base Architectural Compatability Plan, the Air Mobility Command (AMC) Landscape Design Guide, all other design guides as applicable to a specific project, and comply with all other contract and soliciation documents. The aforementioned design guides can be viewed at:

The Contracting Officer may request submittals in addition to those required when deemed necessary to adequately describe the work. Each submittal shall include an original AF Form 3000, Material Approval Submittal, and three (3) copies of supporting documentation. When available, the Government may request electronic copies of submittals via email, in lieu of hard copies. The availability and practicality of submitting electronically versus hardcopy will be discussed and agreed to during the initial pre-construction conference after contract award. In general, submittals shall be provided a minimum of 14 calendar days in advance of scheduled purchase to allow satisfactory time for government review. Additionally, unless stated otherwise, submissions on test results and other submissions of that nature shall be made within 14 days, after the work or test is performed. Each submittal shall be complete and in sufficient detail for ready determination of compliance with the contract requirements. Submittals shall include such items as Contractor’s or manufacturer’s shop drawings, descriptive literature, catalog cuts, diagrams, operation charts or curves, samples, certifications, test reports, test cylinders, operating manuals, installation procedures and recommendations, templates, and other descriptive literature. Catalog cuts and other descriptive data which may have more than one model or type shall be clearly marked to show the model, type, size, and all optional accessory equipment the Contractor proposes to install. Submittals on component items forming a system or interrelated items shall be submitted at one time as a single submittal in order to demonstrate that the items have been properly coordinated and will function as a unit. Submittal items that are disapproved will be promptly corrected and resubmitted within 10 calendar days by the Contractor. No materials shall be installed until submittals are approved by the Contracting Officer. The approved submittals shall be available at the job site for inspection by the Contracting Officer at all times. All submittals involving color selections, patterns and other decorative architectural features shall be submitted at the same time for both interior and exterior materials.

C. AS-BUILT DRAWINGS:

1. During the course of construction, the contractor shall keep an accurate record of as-built conditions by computer upgraded disks. When available, the Government may provide record drawings of applicable facilities for use by the Contractor to aid in developing as-built drawings. Required computer and software material is the Contractor’s responsibility. Drawings software is AutoCAD 2008. Specifications software is minimum Microsoft Word 2007.

2. All variations from the contract drawings, for whatever reason including those made because of the selection of optional materials, shall be clearly indicated. These variations shall be shown in the same general detail utilized in the initial contract drawings. The as-built drawings shall be maintained in a current condition during the course of construction, readily available for review by the Contracting Officer pg 2 of 10 http://www.wbdg.org/ccb/browse_cat.php?o=29&c=4 http://www.wbdg.org/ccb/browse_cat.php?o=33&c=129 or the Construction Management Inspector at all times. The as-built drawings shall be furnished to the Contracting Officer not later than ten work days after the final inspection date.

3. As part of the as-built drawings a separate set of plans shall be marked showing areas containing existing/remaining asbestos containing materials and lead based paint and methods of encapsulation, if appropriate.

D. GEOBASE REQUIREMENTS: All geospatial deliverables (CADD or GIS format), whether obtained via survey or any other data collection process, shall be measured in meters. The coordinate system for all geospatial data will be the Illinois State Plane Coordinate System 1202 West. The vertical datum will be North American Vertical Datum 1988 (NAVD 88). The horizontal datum will be NAD83. For more information see “GEOBASE” attachments.

E. REAL PROPERTY RECORDS: At the completion of construction and before final payment, the following information, by building number, will be provided by the contractor to the Contracting Officer on a completed DD form 1354 for record purposes. The contractor shall be furnished a copy of the 1354 Worksheet which will aid in thoroughly completing the DD form 1354. The contractor shall:

1. Provide a draft 1354 to the Contracting Officer at least 45 days prior to the contract completion date. At a reasonably scheduled time upon receipt of the draft, a representative from the real property office with the contractor will do a thorough walk through and annotate any findings or corrections required.

2. Prepare an accounting of all installed/constructed property, by building number (if multiple buildings are included in the project).

3. List Government-furnished/contractor-installed items and contractor-furnished/contractor-installed items.

4. List complete nomenclature of each item:

a. Manufacturer.

b. Model Number.

c. Serial Number.

d. Electrical rating, full nameplate data.

e. Capacity (MBTU,KVA, Tons, Sprinkler Heads, Etc.).

f. Other pertinent information (Warranty/guarantee information-start date, stop date)

g. Cost of each item (Equipment cost only)

5. List items including, but not necessarily limited to the following:

a. Electrical equipment (special lighting fixtures, metering equipment, overhead door operators, exterior floodlights and poles, fire alarm and burglar alarm systems, etc.)

b. Mechanical equipment (commercial refrigeration equipment, heating equipment, air conditioning systems equipment, pumps, boilers, sprinklers, meters, and other items of mechanical equipment)

c. Other items that are considered to be real property installed equipment

6. The real property office is the point of contact to assist with any questions, comments or concerns. The Contracting Officer can provide an example of a completed 1354. Both the worksheet and draft copy will be available at the onset of the project.

III. WORK COORDINATION:

A. GENERAL: The contractor shall coordinate all work schedules with the CO and the Construction Management

Inspector prior to the start of work.. Notify the Contracting Officer or the Construction Management Inspector five days prior to moving onto the work-site to commence work. You must provide notification the day you move back to the work-site after an interruption of work of two or more work days. Before initiating any work under on this contract, the contractor shall meet with the CO and inspector and agree on:

• a sequence of procedures;

• a means of access to premises and buildings;

• placement of dumpsters and haul routes

• delivery of materials and use of approaches;

• use of corridors, stairways, elevators, and similar means of egress;

• communications;

• the location of partitions and barriers;

• eating spaces, and restroom for contractor’s employees;

• any other incidentals to the project.

1. Interruption of Work. If it becomes necessary to interrupt work activities in buildings and/or areas for construction purposes (except as otherwise described in the contract), the Contractor shall request permission, in writing, from the CO, or designated representative(s), at least ten (10) working days in advance. The Contractor shall submit written requests for approved street closings ten (10) working days prior to closing of the street.

2. Temporary construction. Any contractor installed temporary construction intended to prevent interruption of normal work activity or loss of utility services is subject to approval of the Base Civil Engineer through the CO.

B. INSPECTION: Obtain the Construction Management Inspector's approval prior to accomplishing tasks such as closing walls or performing tests. The request for such approval shall be given with sufficient advance notice to permit an inspection of the area before the work or test is accomplished. You are permitted to continue the work or test, in strict conformance with the project drawings and specifications, in the event the Construction Management Inspector fails to arrive at the project site at the designated time. The Construction Management Inspector's approval or inspection does not relieve the contractor of compliance with the project drawings and specifications.

C. WATER UTILITY WORK: The water utility lines on Scott AFB are owned and operated by American Water

(AW). Contractors shall coordinate with AW when conducting any work pertaining to the water utility lines.

Compliance with AW’s requirements does not relieve the contractor from complying with Scott AFB requirements (i.e. – submittals, as-builts, coordination, etc.).

D. PROJECT SUPERINTENDENT: You must employ a full-time superintendent for the project. The superintendent’s duties shall include the overall management of the project, coordination of the sub-contractors work, and liaison with the Contracting Officer and the Inspector. No work shall be performed without the presence of the superintendent or a competent representative on the job-site. A competent representative is as defined by the Occupational Saftey and Health Administration (OSHA) Standards for Construction Industry part 1926.32, Definitions (29 C.F.R. 1926 OSHA). This paragraph supplements FAR clause 52.236-6, Superintendence by the Contractor.

E. 24-HOUR POINT OF CONTACT: Provide the Contracting Officer with a 24-hour number or numbers where his authorized representative can be contacted at anytime day or night. This number should be provided at the pre-construction conference, but not later than the first day of contract work at the site or ten (10) days after the Notice to Proceed date.

F. CONTRACTOR’S OFFICE SPACE: Office space required for the Contractor’s operation shall be the responsibility of the Contractor. Any office space to be located on Scott AFB, must be approved by the Contracting Officer.

G. CONTRACTOR’S STORAGE AREA: You will be allowed to have one storage trailer at the construction site.

The location must be approved by the Contracting Officer. All other trailers, storage, and equipment will be stored at a site designated by the Inspector. The government is not responsible for damages or losses. This paragraph supplements FAR clause 52.236-10, Operations and Storage Areas.

H. SITE AND STORAGE AREA MAINTENANCE: You are responsible for general maintenance of your construction site and storage area(s) on a daily basis. This includes, but not limited to, lawn mowing, ice and snow removal, and disposal of trash and debris.

I. DUST CONTROL: Maintain all excavations, embankments, stockpiles, access roads, and all other work areas free from excess dust to such reasonable degree as to avoid causing a hazard or nuisance to base personnel or surrounding facilities. Approved temporary methods consisting of sprinkling, chemical treatment, light bituminous treatment or similar methods will be permitted to control dust. Dust control shall be performed as the work proceeds and whenever a dust nuisance or hazard occurs. Additionally, the Contractor shall ensure that any equipment hauling rock, soil, or other debris shall be adequately affixed with a tarp or other means of containment so as not to liter parking lots or roadways on Scott AFB. Finally, due to the nature of the mission of Scott AFB, contractors shall be responsible to pickup, clear and/or sweep away any mud or debris from roadways left by heavy equipment or trucks. Contractors shall use whatever means necessary to ensure equipment and trucks do not leave mud and debris on the roadways, to the satisfaction of the Contracting Officer.

J. PATCHING AND REPAIRING: After the work has been completed, any contractor caused damages to

Government-owned structures, facilities, streets, curbs, walks, lawns, etc., shall be properly patched, repaired, or replaced to the complete satisfaction of the Contracting Officer. Any holes, cuts, breakages, and damages made in finished surfaces of the area in which work has been accomplished, shall be properly repaired and the disfigured areas then painted over or otherwise repaired and resurfaced to match the existing finishes to the complete satisfaction of the Contracting Officer.

K. DISPOSITION OF REMOVED GOVERNMENT PROPERTY: All materials and equipment to be removed by the contractor and not reinstalled or recycled under this contract, including debris generated by this project, shall be removed at regular intervals from the site and disposed of off-base by the contractor. Off-base disposal of materials shall be in conformance with applicable Federal, state, and local pollution regulations at the contractor’s expense.

L. EXCAVATIONS & LANDSCAPING: The contractor shall backfill all excavations, within one week unless ongoing work in an excavation precludes such action. Under no circumstances shall the contractor stockpile removed earth material on the construction site unless it is to be used in another portion of the same project.

For all disturbances to grounds, the contractor shall complete final grading within one week of the disturbance/backfilling. The contractor shall complete final landscaping or seeding within three (3) weeks of the beginning of a planting season to all areas available for such work.

M. FINAL CLEANUP: Upon completion of the project, all construction equipment and debris will be removed from the site(s) and left in an acceptable condition approved by the CO. This paragraph supplements FAR 52.236-12, Cleaning Up.

IV. PERMITS:

A. GENERAL: Contractor is required to investigate, submit for any permits necessary for the design and construction of this project. All permit applications shall be submitted to the Contracting Officer Representative. The Contractor shall pay all permit fees. The following sections contain information on some of the various permits the Contractor may be required to obtain for contracts. This is not meant to be a complete list of all required permits. It is still the responsibility of the contractor to obtain all required permits for any construction, whether included in this document or not.

B. SPECIAL PERMITS INCLUDE:

1. EMERGENCY POWER GENERATOR PERMIT: The contractor shall contact the 375th CES/CEAN, Air Quality Program Manager, to find out the requirements for an emergency power generator prior to having the generator delivered to the base. The generator shall be certified by the Family Emissions Limits (FELs). The POC is Mr. Mark McCoy at (618) 256-2167.

2. BURN PERMITS: The Base requires the contractor to obtain a USAF Welding, Cutting, and Brazing Permit from the Base Fire Department prior to conducting any operation that will create a spark or flame.

Contact: Scott AFB Fire Department, Point-of-Contact is Assistant Chief Carl Hodges (618) 256-5130. If an open burn permit is required, the permit request shall be routed through the 375 CES/CEAN, Air Quality Program Manager, no-later-than (3) months prior to actual open burn effort.

3. NOTICE OF PROPOSED CONSTRUCTION OR ALTERATION TO FEDERAL AVIATION

ADMINISTRATION(FAA): Some construction activities may require advance notification to the FAA prior to commencing activities when exceeding certain height restrictions as described in 14 Code of Federal Regulations, part 77 pursuant to 49 U.S.C., Section 44718. When required by regulation FAA Form 7460-1 must be completed and provided to the Regional FAA Office 45 days prior to commencing construction activities. 4. FAA NOTICE TO CONSTRUCT: IAW with Federal Air Regulation Part 77, when building within 20,000' of an active runway a FAA Notice-To-Construct is required to be submitted 45 days prior to the start of any construction or equipment used in the construction that will penetrate a 100 to 1 slope from the nearest point of the runway. There are exceptions. The contractor is responsible for ensuring the notice is sent and approval obtained prior to the start of the project for which they have responsibility.

5. NPDES PERMIT: As part of the NPDES permitting process for the state of Illinois, the contractor will be required to submit a Storm Water Construction Permit to the Illinois Environmental Protection Agency (IEPA) prior to any construction activities. General information can be found at http://www.epa.state.il.us/water/forms.html.

6. NOTICE OF INTENT: Notice of Intent (NOI) is a general permit to discharge storm water from construction site activities. NOI shall be signed, certified and stamped by an Illinois Registered Engineer before submitting to the 375th CES/CEAN Water Permits Manager for local coordination, and signatures.

Storm Water NOI’s, Construction Plans, and SWPPP will be submitted electronically, along with 3 hardcopies, to 375th CES/CEAN for local coordination, and signatures. Final submittal of packages will then be submitted by CEAN to the Illinois Environmental Protection Agency Division of Water Pollution Control for the sewer installation and connection. For specific questions and guidance please contact the Water Permit Manager at 618-256-2092.

7. WATER PERMIT: An application for construction/operation must be submitted to American Water Co. for water main installation and connection. Permit applications, with instructions, from the State of Illinois are available on the IEPA web site at http://www.epa.state.il.us/water/forms.html#permits-drinking-water. The contractor may contact the American Water Customer Service at 1-800-422-2782 or infoaw@amwater.com with questions.

8. AMERICAN WATER WORK PERMIT: All work affecting the water utility system shall be reviewed and approved by American Water. As such, the contractor must adhere to the design and construction provisions of the AW Water Work Permit. Prior to any water-related construction work, the contractor shall submit a completed AW Water Work Permit to American Water. American Water is located on Scott Air Force Base at 34004 New Mexico Street and can be contacted at (618) 744-9631.

9. SEWER PERMIT: The extension of the existing sanitary sewer will require a permit. Permit application shall be signed, certified and stamped by an Illinois Registered Engineer before submitting to the 375th CES/CEAN Water Permits Manager for local coordination, and signatures. Final submittal of packages will then be submitted by CEAN to the Illinois Environmental Protection Agency Division of Water Pollution Control for the sewer installation and connection. 10. ABOVEGROUND/UNDERGROUND STORAGE TANK PERMITS: The Contractor will complete a permit application to install any aboveground storage tank above 110 gallons to include generator sub-base tanks. A permit is required for the installation or work on an underground storage tank or associated piping. Forms can be found on at the following website http://www.state.il.us/osfm/PetroChemSaf/home.htm. Coordinate permit applications with the Environmental Office at (618) 256-2092.

11. EXCAVATION: Contractor shall apply for and obtain a Government Excavation Permit, also known as a “digging permit” (AF Form 103, Base Civil Engineer Work Clearance Request), prior to any excavation.

The permit can be obtained from the CEOEP (Civil Engineering, Operations - Planning), Building 530.

There is no charge for the permit. The digging permit (AF Form 103) is required for the Government to locate and mark existing underground utilities as well as for the Government’s internal notification and coordination. The Contractor will not begin any below-grade work until this permit has been completed and approved. The Contractor shall make every attempt to identify and locate all valve or utility shut-off locations for use in the event of accidental damage. The Contractor shall note any utilities discovered during excavations that are missing or incorrect on the contract drawings (including any erroneous or missing dimensions on Government record drawings), and clearly identify those discoveries on the as-built drawings.

Initiate the request for a digging permit a minimum of five (5) working days prior to performing any excavation or ground penetration. When requesting the permit, identify the area on a site plan. The site plan shall clearly delineate the proposed work area and if possible, the nearest building, cross street to the location of digging, etc. Mark the location of the excavation or ground penetration with white, water soluble, spray paint, or white utility marking flags. Ensure the markings are maintained until the area is cleared by all utilities. In the event that a representative for the designated utility does not respond, the Contractor shall immediately notify the Contracting Officer. After identified utilities have been cleared through each affected agency or office, you will receive a copy of the digging permit. You must have the digging permit on the job site during the excavation process and exercise every precaution during excavation to prevent damage to the existing utilities. Immediately notify CEOEP and the Construction Inspector if any utilities are damaged during the excavation process.

V. UTILITIES AND SERVICES:

A. COMMERCIAL UTILITIES: You are permitted the use of the utility services such as electric, gas, sewer, and water. These temporary connections such as portable sanitary facilities are at your expense and are subject to approval by the CO. You are responsible for any relocation of existing and/or new construction of commercial utilities to include but not limited to cable, gas, telephone, electrical, etc. Contact the applicable utilities and pay for all relocation and/or new utility construction and project costs. All exterior equipment provided under the contract shall be painted Sherwin Williams product #SW4009 (“Espresso Brown”).

B. TEMPORARY WATER SERVICE CONNECTIONS: Notify American Water 10 days in advance of temporary water service requirements. All temporary water connections shall be performed by the contractor at the expense of the contractor. Where appropriate, backflow devices will be used. Disconnection of temporary service lines installed and used by the contactor and any subcontractors will require complete removal of the service line at contractor’s expense

C. UTILITY SERVICE INTERRUPTIONS: The contractor shall submit written notification not less than 14 work days in advance for each planned interruption of a utility service. No single interruption shall exceed eight hours. Utility service interruptions shall not be permitted unless approved by the Contracting Officer. Service interruptions shall be accomplished at other than normal work hours if feasible. As a general rule, water distribution service will not be interrupted. All water connections will be made via “hot” tap on the main and shall be observed by American Water personnel. American Water shall be the responsible party to determine what shall constitute sufficient reason to interrupt water service. Interruption of water service due to scheduled construction work involving water system components shall be coordinated with AW at least 10 work days in advance to allow for proper water outage planning and water outage notification to affected parties. AW Staff pg 7 of 10 http://www.state.il.us/osfm/PetroChemSaf/home.htm shall operate all valves and hydrants during any outage plan test and implementation of the scheduled water system outages and service restoration. The Contractor performing work requiring the interruption of water service shall provide all material, labor, and equipment required to minimize the extent and duration of a water outage and shall ensure restoration of service in the most efficient and complete manner in accordance with accepted industry standards and practices. This may require one or more line stops to preclude an extensive outage to multiple facilities. The contractor shall coordinate water outage plans and requirements with AW staff accordingly. The contractor shall comply with all base and state environmental requirements.

D. ABANDONMENT OF WATER LINES: Service lines to be abandoned shall be excavated at the curb stop or corporation stop and the valve closed off. The service shall be physically separated so as to have a minimum of three feet horizontal clearance between the main and the cut service line. The location of the disconnection shall be provided to AW on project as-built drawings.

Water mains to be abandoned shall be cut and depressurized, then capped with a mechanical cap or concrete cover. Valves shall be closed, and the valve boxes removed. All fire hydrants shall be fully removed. In the event a shut off valve is not available, the Contractor will install a temporary line stop to enable separation of the water mains. Certain circumstances where settlement may be a structural problem may require that the main be filled with flowable fill.

E. ENABLING/DISABLING FIRE ALARMS PANELS: The contractor shall bear all costs associated with enabling/disabling fire alarm panels. All fire alarm panels shall be properly operating prior to the contractor securing at the end of each workday. To have a fire alarm panel enabled/disabled the contractor may contact Scott AFB fire alarm maintenance contractor (contact Operations Support, (618) 256-2698) or the Contractor may have a fire alarm contractor of their choice, provided the following Scott AFB requirements are met: 1) fire alarm contractor must maintain current NICET2 (National Institute of Certified Engineering Technology) license or current NBFAA (National Burglar/Fire Alarm Association (Practical Fire) license; 2) any contractor/sub-contractor working on Scott AFB fire alarm transmitters must have current General Radio Operator License Element 1 & 3 issued by Federal Communications Commission; and 3) the Scott AFB Fire Alarm Service Contractor must be present at all final acceptances for fire alarm systems. An appointment shall be coordinated three (3) to five (5) workdays in advance of proposed inspection date/time. Please contact the Fire Alarm QAE (Operations Support), (618) 256-2698, to schedule an acceptance inspection of the fire alarm system.

F. FIRE HYDRANT CONNECTION: Temporary connections to fire hydrants require prior coordination and approval from the water distribution system owner, American Water Operations & Maintenance, to ensure the proper operation of the hydrant and installation of an appropriate backflow prevention device. The Contractor shall be responsible for obtaining all devices and bearing all costs associated with providing, installing, and testing the backflow device as required by the Illinois Plumbing Code.

VI. MAINTENANCE INFORMATION:

A. OPERATIONS AND MAINTENANCE MANUALS: Where required by the other parts of the specifications, the Contractor shall provide the Contracting Officer the specified number of copies of operation and maintenance instruction manuals. Manuals shall contain complete instructions on operating and maintaining the equipment, including piping and wiring diagrams. The manual shall be assembled in book form having a cover indicating the contents by equipment or system name and project title. Manuals shall be submitted a minimum of ten work days before the Contractor conducts the on-site operation and maintenance instruction. If such instruction is not required by the other parts of the specifications, manuals shall be provided at the final inspection.

B. SPARE PARTS LIST: Where required by the other parts of the specifications, the Contractor shall provide the

Contracting Officer the specified number of copies of spare parts lists. The lists shall provide the supply source, item description and price of all parts and shall be cross referenced to equipment diagrams or pictures. Spare parts lists shall be assembled in book form and may be separate from or integral with the operation and maintenance instruction manuals. Spare parts lists shall be submitted with the related instruction manuals.

C. ON-SITE INSTRUCTION: The contractor shall provide the services of a qualified person, where required by other parts of the specification, to instruct government personnel in the proper operation and maintenance of newly installed systems and equipment. The instruction shall be in accordance with the procedures and methods recommended by the equipment manufacturer. The Contractor shall schedule the on-site instruction with the Contracting Officer, but such instruction shall not occur until ten days after the receipt of operation and maintenance instruction manuals.

D. WARRANTY ITEMS: There shall be a maximum 24-hour response time by the Contractor on all warranty items installed under this contract and identified as either malfunctioning or inoperative after acceptance of the item. The Contractor shall notify both the Contracting Officer or his designated representative and the Inspector of the date, place, and time that the contractor will be on location to determine the cause of the problem. Response time shall be defined as the time from when the Contracting Officer contacts the Contractor until the time the Contractor is present at the location of the equipment with the appropriate personnel to determine the problem and what is necessary to correct the problem. Warranty items shall be defined as all equipment, components, appurtenances, and wiring installed and the labor and materials necessary to install said items.

Corrective action shall be completed within 5 work days from the time the Contractor visits the location of the malfunction or inoperative equipment. Where parts are required to correct a malfunction, corrective action shall be completed within 5 work days from the time parts are received.

If the Contractor fails to respond within the time frame indicated above, the government shall declare the contractor non-responsive and shall take all actions necessary to correct the problem with government resources. All costs incurred as a result of the government taking this action shall be properly accounted for and back charged to the contractor.

E. SHORING AND SAFETY: The contractor shall provide all shoring as required in accordance with local, state, federal and OSHA requirements for excavations utilized for this project's scope of work. All workers shall be protected from danger. Additionally all excavations shall be secured as necessary to prevent unauthorized personnel from entering the work sites and/or excavations.

F. SUBSTITUTION OF MATERIALS AND EQUIPMENT: The Contractor shall submit plans and elevations drawings drawn to 1/4" equals 1' scale if the Contractor proposes a piece of equipment and/or accessories on other than that specified. The drawings shall show all piping, electrical connections, sizes of equipment, location of piping, and electrical penetrations, and all other associated equipment and materials. The Contracting Officer reserves the right to reject or accept any or all of the proposed changes based on completeness, clarity, and other requirements of the plans and specifications.

When the Contractor requests approval of substitute materials and/or equipment, it shall be understood and agreed that such substitution, if approved, will be made without cost to the government, regardless of changes in connections, spacing, electrical service, and all other appurtenances.

G. BRAND NAMES: References in specifications to any brand name equipment or materials shall be regarded as establishing a standard of quality and shall not be construed as limiting competition. The Contractor may submit proposed equal equipment and/or materials for government consideration during the submittal process of the contract after award.

H. ENVIRONMENTAL WORK: If contract requires specific environmental work be performed, no non-environmental work shall be accomplished which will alter the environmental work specifications or cause contamination of non-contaminated materials and areas.

I. PRODUCTS NOT ALLOWED: Materials containing asbestos, transformers containing PCB fluid, and the use of lead based paints or paints containing chromates are prohibited.

J. OZONE DEPLETING CHEMICALS: Ozone Depleting Chemicals, Class I and II listed in Attachment 2 of AFI 32-7080 are to be recovered by the Contractor and returned to the Government. The Ozone Depleting Chemicals may not be sold or given away to private industry unless otherwise specified by the government.

K. RECYCLE: Recycle all ferrous and non-ferrous metals from the project. The Contractor shall also recycle general administrative refuse associated with this project. This refuse includes cardboard, one gallon mark 1 and 2 plastic bottles, type 1 and 2 plastic soda bottles, aluminum and steel soda cans, and mixed paper. All recyclable material shall be turned into the recycling center. The Base Recycling Center, 34020 New Mexico Street will accept these items Monday through Friday between the hours of 7:30 A.M. and 4:00 P.M. The telephone number for the Recycling Center is (618) 256-6431.

L. PHOTOGRAPHS: When permitted, the Contractor shall provide a minimum of twelve 4" x 6" color photographs prior to beginning actual construction and at two week intervals thereafter for the duration of the project construction. The Contractor shall coordinate with Public Affairs (618-256-4206) and the facility user prior to taking any photographs of the construction area. Photographs shall be taken at different angles/locations so that the existing conditions, along with the construction progress, are fully represented.

Photographs shall be taken inside and outside for all projects involving interior and exterior work. Photos shall be submitted electronically. The photograph file name/title shall include a general description of the photo along with the date the photo was taken. Photos shall be submitted in accordance with the submittal schedule.

M. NATURAL GAS CONNECTION: It shall be the Contractor's responsibility to make arrangements with

AMEREN/IP for work involving the natural gas service. The Contractor shall bear all costs associated with moving or extending gas lines/service as well as all costs for meter installation. Existing natural gas lines are owned by Illinois Power Company. The Contractor shall complete all hookups to natural gas at completion of job.

N. PESTICIDE APPLICATIONS: All pesticide applications must be approved by the Contracting Officer through the Environmental Management Flight (EMF). A label and Material Safety Data Sheet (MSDS) of the insecticide must be available for review. After treatment (if approved), the amount (meaning insecticide + water), percentage used (.05%, 1.0%, etc.) of the insecticide and total square feet of treatment must be submitted to the EMF.

O. RADIOACTIVE MATERIAL (RAM): In accordance with AFI 40-201, the use of any device (i.e. soil density meters, XRF meters, etc) that contains Radioactive Material (RAM) must be approved by the Base Radiation Safety Officer (RSO), Bioenvironmental Flight, @ (618) 256-7307, at least 30 days prior to bringing RAM onto Scott AFB.

P. BACK FLOW PREVENTION DEVICES: In accordance with AFI 32-1066, all newly installed back flow prevention devices must be inspected by a certified inspector within one week of installation and a follow-up inspection performed 3-months later. The Contractor shall submit appropriate documentation of these inspections in accordance with the submittal register.

Q. ENVIRONMENTAL MANAGEMENT SYSTEM - Scott AFB is committed to conducting its mission in a responsible manner that will protect human health, natural resources and the environment. Contractors will reduce environmental risks, promote pollution prevention, and meet all Environmental regulatory requirements during the execution and completion of the contract.

II. SUBMITTAL REQUIREMENTS
1. Interruption of Work. If it becomes necessary to interrupt work activities in buildings and/or areas for construction purposes (except as otherwise described in the contract), the Contractor shall request permission, in writing, from the CO, or designated representative(s), at least ten (10) working days in advance. The Contractor shall submit written requests for approved street closings ten (10) working days prior to closing of the street.
2. Temporary construction. Any contractor installed temporary construction intended to prevent interruption of normal work activity or loss of utility services is subject to approval of the Base Civil Engineer through the CO.
