1 DESCRIPTION OF WORK

1.1 SCOPE OF WORK

The contractor shall provide all personnel, tools, supplies, vehicles, equipment, labor, and applicable licenses, permits, and/or certificates necessary to service Grease Traps located at Goodfellow Air Force Base (GAFB), Texas. Qualified personnel skilled in the trade must perform in compliance with the contract and industry standards. The government encourages the contractor to use best industry practices, current technologies and innovative processes and products to achieve optimum performance of this contract.

The contractor shall be responsible for pumping, inspecting, cleaning, scraping, transporting, and disposing of waste from grease traps as outlined in this Performance Work Statement (PWS).

Ensure all grates, covers, grills, and hookups are returned to their original position before departing the area. In addition to the trap, all drainage lines shall be clear of grease, oil, dirt, lint or other material to ensure they are kept free flowing. Any spills, debris, or damage to the surrounding area by the contractor shall be reported immediately to the Contracting Officer’s Representative (COR) and shall be returned to the original condition as determined by the Contracting Officer (CO). The contractor shall provide adequate equipment to perform services within varying distances from water sources. Contractor shall record amount of collection on a change of custody receipt after each service location.

1.1.1 GREASE TRAPS

The contractor shall pump and fully evacuate each grease trap unless trap volume is greater than the tank capacity on the vacuum truck in which case the transporter shall arrange for additional transportation capacity so that the trap is fully evacuated within a 24-hour period. Residual or recharge water must be higher than lower inlets. Contractor shall clear obstructions in inlet/outlet pipes within the pits/traps to ensure unrestricted flow. Additonally, the contractor will inspect and perform cleaning on all grease traps annually.

1.1.2 SERVICE SCHEDULING

All work will be conducted throughout the year IAW Appendix A. Prior to conducting service, the contractor shall notify the COR seven (7) working days prior to commencement of the work.

1.1.3 SERVICE DETAILS

The contractor shall service all grease traps listed in Appendix A of this contract. Buildings 334, 800, 906 and 3220 shall be serviced on a monthly basis. Buildings 213, 222, 303, 927 and 3201 shall be serviced on a quarterly basis. All grease traps must be cleaned and inspected annually.

1.1.4 CLEAN UP

Contractor shall ensure each collection site is cleaned and surrounding areas are restored to previous condition after servicing each trap, pit or tank. If any spillage and/or leakage occurs the COR should be notified immediately. The contractor shall be held responsible for cleaning up all spillage and/or leakage that occurs during service at no additional cost to the government within 4 hours of the COR being notified. If a lid is dropped into a trap by the contractor, the contractor is responsible for removing the lid within 4 hours. During the timeframe needed to retrieve the dropped lid, the contractor shall ensure any unsafe area is secured until the lid has been secured. Contractor shall perform in depth cleaning and scraping of tanks annually as well as periodic treatment as needed.

1.1.5 DISPOSAL LOCATION

Contractor shall transport and dispose of all grease, sludge, mud and wastewater at a Texas Commission on Environmental Quality (TCEQ) certified Type V disposal site located off- base in the state of Texas.

The contractor is responsible for all permits or fees associated with the use of off-base disposal locations. Prior to contract start, the contractor shall submit in writing to the CO proposed disposal locations. These sites must be approved by the COR, CO, and Environmental Manager before the proposed sites can be used.

1.1.6 CHANGE OF CUSTODY AFFIDAVIT

The contractor shall provide a change of custody receipt to the COR after each tank is emptied. A copy of the chain of custody affidavit shall be submitted to the City of San Angelo no later than 7 days after completed service prior to invoicing the total waste collected and disposed of during the month. Collection point, amount collected, and disposal site shall be noted on the Affidavit.

2 SERVICE SUMMARY

The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each element. The Services Summary (SS) and the Contractor’s Quality Control Plan provide information on contract requirements, the expected level of Contractor performance, and the conventional method of government validation and confirmation of services provided. These thresholds are critical to mission success. Procedures, as outlined in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1 QUALITY CONTROL

Develop and implement procedures to identify, prevent, and ensure non-performance, and a continual repeat of poor service does not occur. Submit a written Quality Control Plan to the contracting officer for review and feedback. Provide a written description of the quality control system to include identification of the commercial practices and procedures to be used, such as those of the Professional Grounds

Management Society (PGMS), and the American National Standards Institute (ANSI). Include the inspection system requirements of FAR 52.246-4 in this document. Submit the plan no later than the pre-performance conference. The plan shall specifically address the contractor’s strategy to provide quality workmanship, continual process improvement, and correcting deficiencies as required. Update the plan as changes occur and changes shall be submitted for review and acceptance by the CO. The contractor’s quality control plan will contain, as a minimum, the following items:

• A description of the inspection system to cover all services. Description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspector(s).

• A description of the methods to be used for identifying and preventing defects in the quality of service performed.

• A description of how the records will be kept. Records must document all inspections and corrective or preventive actions taken.

2.1.1 CONTRACTOR INSPECTION RECORDS

Records of inspections shall be kept and made available to the Government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.

2.2 QUALITY ASSURANCE

The Government may use any method to inspect and evaluate the contractor’s performance to ensure that services have fulfilled the contract obligations pertaining to quality and quantity. Inspections include validating actual work performance, physically checking an attribute of the completed task, validating a management information report, investigating customer complaints, and conferring with facility managers. The CO or COR will use the contractor’s work schedule or a modified version thereof, to record validation results. Results of the validation then become the official Air Force record of the contractor’s performance. When a performance threshold is unsatisfactory or contractor performance not accomplished, the COR will initiate and provide the contracting Officer a Contract Discrepancy Report (CDR) DD Form 2772 for issuance to the contractor. The contractor shall respond to the CDR IAW instructions provided and return it to the Contracting Officer within one (1) calendar days of receipt.

2.2.1 UNSATISFACTORY PERFORMANCE

Unsatisfactory Performance is defined as the service which does not meet most contractual requirements, and recovery is not likely in a timely manner. Failing to meet the performance threshold, for any performance objectives, in any one month, may result in a warning or letter of concern. An unsatisfactory inspection is recorded and may require re-performance of the service. Failing to meet the performance threshold objectives for any two or more consecutive may constitute an immediate Progress Meeting with the Multi-functional Team.

3 GENERAL INFORMATION

3.1 MISSION

The overall Civil Engineer mission at Goodfellow AFB is to plan, maintain, operate, and protect the infrastructure, facilities, and environment for assigned Airmen, families, and civilian employees.

3.2 CONTRACTING OFFICER AND CONTRACTING OFFICERS REPRESENTATIVE. (QUALITY CONTROL) The CO will appoint a primary and alternate representative for the management of the day-to-day activities of the contract. The identity, title, and authority of this representative will be provided in writing to the Contractor after the contract award.

3.3 HOURS OF OPERATION

Workdays shall be from 0730 to 1600, Monday thru Friday; except on the Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Buildings 3220, 303, 334 & 800 will not be scheduled between the hours of 1100 &1300. No weekend work allowed unless approved by the Contracting Officer. If weekend work is required Contractor shall submit request in writing to Contracting Office a minimum of 72 hours in advance. The Contractor shall provide written or phone call notification to the Contracting Officer’ Representative seven (7) calendar days prior to arriving on base for scheduled services.

3.4 RECOGNIZED FEDERAL HOLIDAYS

Ten (10) Federal holidays are set by law. The Contractor is not required to provide service on the following holidays. Under current definitions, the following four are set by date:

If any of the above falls on a Saturday, then Friday shall be observed as a holiday. Similarly, if one falls on a Sunday, then Monday shall be observed as a holiday.

The other six are set by a day of the week and month:

3.4.1 BASE CLOSURES

Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert will be resume upon access to installation.

3.5 PERFORMANCE OF SERVICE DURING CRISIS OR HEIGHTENED SECURITY

Contractor providing services designated as essential by a Department of Defense Component are expected to use all means at their disposal to continue to provide services, IAW with this PWS. The services provided under this contract are deemed mission essential during crisis and emergencies and shall continue normal operation. The CO will determine the circumstances which constitute a crisis based on direction from Security Forces and base leadership.

3.6 CONTRACT PERSONNEL

3.6.1 CONTRACTOR PERSONNEL

Present a neat appearance and be easily recognizable while on the installation in conjunction with this contract. This can be accomplished through the wearing of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that it identifies personnel easily and quickly for reasons of safety and personal protection.

3.6.2 EMPLOYMENT RESTRICTIONS

The Government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

3.7 SAFETY REQUIREMENTS AND REPORTS

Conduct and perform services per the Department of Labor, OSHA requirements found in 29 CFR 1910 and 29 CFR 1926, and Air Force Safety & Health Standards, including AFMAN 91-203, Air Force Occupational Safety, Fire and Health Standards. Provide a verbal mishap report to the CO within two hours of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five (5) calendar days of each occurrence.

3.8 ENVIRONMENTAL REQUIREMENTS

Contractors are not exempt from air emission, storm water, hazardous waste, and other environmental compliance rules and regulations. The Contractor shall comply and ensure that all subcontractors comply with all applicable federal, state, and local laws, regulations, ordinances, and standards related to environmental matters. The Contractor shall also comply and ensure that all subcontractors comply with all specific instructions or directions given to the Contractor by Goodfellow AFB Environmental Office.

3.9 TRAFFIC LAWS

Contractor and employees shall comply with Texas transportation code, base traffic regulations and posted signs while on the installation.

3.10 WEAPONS, FIREARMS, AND AMMUNITION

Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned vehicle or privately-owned vehicles while on Goodfellow AFB.

3.11 CONTROLLED UNCLASSIFIED INFORMATION (CUI)

The Contractor shall comply with DOD 5400-7- R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and with DoDI 5200.48 for marking, handling, transmitting, and safeguarding and destroying CUI material.

3.11.1 OPERATIONS SECURITY (OPSEC) REQUIREMENTS

Organizations and personnel supporting the 17th Training Wing have OPSEC requirements associated with their activities and support. The Contractor will comply with the 17th Training Wing OPSEC Program. The basis for the OPSEC program is AFI 10-701.

Contractor personnel will receive OPSEC Awareness Education and Duty-Related Training within 90 working days of start date and annually thereafter. OPSEC Awareness Education and Training will be provided or coordinated through the government.

3.12 REPORTING REQUIREMENTS

Contractor personnel shall report to appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, Contractor personnel, resources, and classified or unclassified defense information.

3.13 PHYSICAL SECURITY

The Contractor shall be responsible for safeguarding all Government property and controlled forms provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

CONTRACTOR/EMPLOYEE BASE PASS AND IDENTIFICATION, SPECIAL CLEARANCES AND

VEHICLE PASSES

The Contractor shall comply with all requirements and procedures IAW DAFFAR 5353.242-9000, Contractor access to Air Force installations. All documentation shall be submitted at the pre-performance conference and as changes occur.

3.13.1 BASE ACCESS

a. The contractor and DoD ID card holding sponsor shall ensure a Defense Bio-Metric Identification System (DBIDS) identification card or pass is obtained for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Installation access is restricted to only authorized persons and their vehicles. Authorization to enter Goodfellow will be granted following a process of identity proofing and vetting, at minimum for each unescorted visitor, to determine fitness of the individual requesting and/or requiring access to the base and issuance of access credentials.

Prior to allowing access to Goodfellow, a minimum of a National Crime Information Center (NCIC) will be completed on all visitors (16 years of age and older), requesting unescorted access to validate the visitor’s fitness. Authorization to enter Goodfellow will be immediately denied in instances revealing the following items: felony conviction within 10 years, any violence within 7 years, indecent acts with minors, violence with a weapon, terrorist threats, repetitive history of criminal activity or any other misconduct the commander determines may have a negative effect on good order and discipline or installation security.

All information is subject to review and recommendation by Defense Force Commander (DFC) as delegated by TRW/CC. All personnel denied entry will be issued a letter containing instructions on how to appeal the decision.

b. The contractor shall submit a written Base Access List (BAL) request on company letterhead to the 17th Civil Engineering Squadron (CES) POC listing the following: contract number, location of work site, start and stop dates, days of the week entry/access is required, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The 17th CES POC will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) will provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate, and be listed on the submitted BAL to obtain a vehicle and personal pass. Contract employees will report to Pass and Registration with 30 days from the date of the BAL to receive the access card/pass. After 30 days, a new BAL must be provided.

Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

c. During performance of the contract, the contractor and base sponsoring agent shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. Workers are ONLY authorized on the installation when required to complete work in accordance with the active contract.

Entering the installation for private/personal business or for any reason not associated with the obligations of the current contract is “unauthorized” and subject to criminal prosecution. Entering/attempting to enter a federal installation with a fraudulent credential or using a pass that is no longer valid can result in a federal charge of “trespassing” and, at the discretion of the Installation Commander, can result in subsequent barment from the installation. Requesting 24/7 access to the installation is generally not authorized. The sponsoring agent will determine if the contractor requires access to the installation outside the contract work hours and will preannounce access to Security Forces through an established Civil Engineering or Comm Focal Point work-order location.

d. When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management citing the appropriate paragraphs as applicable.

e. Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. Personnel failing to return DBIDS credentials may be denied a return entry into the installation until accountability of the pass is resolved.

f. Failure to comply with these requirements may result in withholding of final payment.

3.13.2 BAL RENEWAL

All BALs are (re)accomplished each time employees or personnel change, not to exceed 180 days.

Oversight for BAL updates and establishment of procedures to ensure Physical Access Control System (PACS) credentials and locally created access credentials from individuals who no longer require installation access is the responsibility of the 17th CES POC.

3.13.3 BAL TERMINATION

When an employee (regardless of position) is terminated by the Contractor, all DBIDS passes are to be returned to the Contract Officer/Administrator or to the Visitor Control Center. If a local issued access credential/pass is not returned, the CO may take administrative action, or the Installation Commander may consider permanent debarment to the installation. Immediate access denial may be initiated by Pass & Registration updating the DBIDS database until disposition of the DBIDS pass is resolved.

3.13.4 SECURITY REQUIREMENTS

The contractor shall ensure contractor employees comply with base operations plans/instructions for Force Protection Condition (FPCON) procedures, Random Antiterrorism Measures (RAMS) and local search/identification requirements. The government will provide/make available Anti- Terrorism (AT) Level I training/information at the Contractor’s request. This information can be provided through the 17 TRW Contractors AT Level I awareness pamphlets/binders, through computer-based training programs, or through the unit/installation Antiterrorism Officer.

3.13.5 COMMERCIAL VEHICLE INSPECTION

All contractor employees will need to access the base through the Commercial Vehicle Inspection Area (CVIA) between the hours of 0600 – 1400, Monday – Friday. Once the CVIA closes at 1400 hours and on weekends, vehicle inspections will be conducted in the parking lot of the Visitor Control Center. All vehicle compartments must be opened by the driver prior to the vehicle being inspected by a Security Forces Member. Once cleared, all vehicles must proceed through the Jacobson (South) Gate. The Contractor should plan and allow for delays because of increased and ever-changing security procedures.

3.13.6 CONTRACTOR VEHICLES

Contractor vehicles must be maintained in a safe and operational condition at all times while on Goodfellow AFB. All contractor vehicles will maintain proof of insurance and valid registration inside the vehicle and drivers must carry a valid driver’s license anytime a company vehicle is operated on the installation or associated property.

3.13.7 SECURITY AUTHORITY

The Contractor shall brief all employees that they shall immediately leave any area upon notification by the CO, COR, constituted authority, Security Forces, or when directed as occasional operational requirements, real world activities, or emergencies dictate. Restricted/Controlled Areas. Contractor performance within restricted/controlled areas shall require Contractor compliance with various levels of security and established personnel security entry control procedures. The Contractor shall ensure all contractor employees performing services in secured areas are processed for a controlled area, limited access, or visitor badge by the Entry Controller of the restricted/controlled area.
