About this file
This is a Past Performance Questionnaire used by the 82nd Contracting Squadron of the Air Force to evaluate offeror performance on a continuation contract for exhaust hood cleaning and certification services at Sheppard Air Force Base and Sheppard Annex at Lake Texoma. The service provider must supply all personnel, equipment, tools, supplies, expertise, and materials necessary to perform kitchen exhaust hood and duct cleaning.

Section 1 requests contract identification details including contractor name, CAGE code, contract number, type, program title, period of performance, dollar value, and work description. Section 2 collects customer or agency identification information and geographic scope of services. Section 3 gathers respondent identification including name, title, contact information, position, length of service on the subject contract, and suggested alternate points of contact. Section 4 contains 15 performance rating questions using a standardized six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable) covering areas such as contract requirements compliance, data quality, quality processes, environmental safety, contract management, customer relations, problem resolution, Performance Work Statement compliance, emergency response, material management, cost-saving innovations, workforce hiring and retention, and advance notification of unforeseen costs. The questionnaire also requests information on any contract terminations for default or convenience. Past Performance Questionnaire
The 82nd Contracting Squadron of the Air Force is in the process of selecting a source for Exhaust Hoods. This requirement is a continuation contract to provide Exhaust Hoods cleaning and certification services at Sheppard AFB and Sheppard Annex at Lake Texoma08. The service provider shall provide all personnel, equipment, tools, supplies, expertise, materials and other items necessary to perform cleaning kitchen exhaust hoods and ducts.
One of the considerations in proposal evaluation is the verification of the offerors past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. To meet the acquisition milestones, we request your written response no later than 20 July 2026 @ 10:00am CT. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to cole.jones.6@us.af.mil and jessica.muller@us.af.mil. Your completed questionnaire will become a part of the official Source Selection records.
Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
