About this file
This document is a Past Performance Questionnaire issued by the 82nd Contracting Squadron of the Air Force to evaluate contractor performance for a continuation contract to provide Exhaust Hoods cleaning and certification services at Sheppard Air Force Base and Sheppard Annex at Lake Texoma, Texas. The service provider must supply all personnel, equipment, tools, supplies, expertise, and materials necessary to perform kitchen exhaust hood and duct cleaning operations. Responses to this questionnaire are due by 20 April 2026 and should be submitted electronically to Cole Jones (cole.jones.6@us.af.mil) or Jessica Muller (jessica.muller@us.af.mil), the Contracting Officer. Completed questionnaires become part of the official source selection records and will inform the government's evaluation of offeror past performance capabilities.
The questionnaire is structured in five sections requiring respondents to provide contract identification details, customer or agency information, respondent identification, and performance ratings across 15 evaluation criteria using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable). Key performance areas assessed include contractor compliance with contractual requirements, data quality and integrity, implementation of quality processes, environmental safety procedures, contract management effectiveness, cooperation with program staff, problem resolution, Performance Work Statement compliance, emergency response capability, material management, cost reduction initiatives, workforce hiring and retention, and advance notification of unforeseen costs. The questionnaire also requires disclosure of any contract terminations for default, convenience, or pending terminations. A final section requests the respondent's overall assessment of whether they would award the contract to this contractor based on demonstrated performance.

Past Performance Questionnaire
The 82nd Contracting Squadron of the Air Force is in the process of selecting a source for Exhaust Hoods. This requirement is a continuation contract to provide Exhaust Hoods cleaning and certification services at Sheppard AFB and Sheppard Annex at Lake Texoma (approximately 2 hour drive/120 miles from Sheppard AFB). The service provider shall provide all personnel, equipment, tools, supplies, expertise, materials and other items necessary to perform cleaning kitchen exhaust hoods and ducts.
One of the considerations in proposal evaluation is the verification of the offerors past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. To meet the acquisition milestones, we request your written response no later than 20 April 2026. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to cole.jones.6@us.af.mil and jessica.muller@us.af.mil. Please call Cole Jones at 940-676-2803 prior if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.
Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
