PART 1 - GENERAL
1.1 WORK COVERED BY CONTRACT DOCUMENTS:
A. Project Description: The goal of this project is to update the chiller in the Mechanical Room of the building (B1080), as well as associated pumps and control system. The scope of work shall be in accordance with contract documents as a whole to achieve the goal. The scope of work consists of the following:
1. Demolition and removal of specified equipment and settings in the Mechanical Room and on top of the roof, (including removal of asbestos and lead paint identified, and abandoned pad, anchor bolts, brackets, pipes, wires and cables, etc. that are on the floor, walls, and roof).
2. Fixing and patching the walls and the floor of the Mechanical Room after demolition;
renovating the supporting structure for an air-cooled chiller on the roof, and repairing the roof after that.
3. Procurement and installation of an air-cooled chiller system with two pumps, chemical feeder, piping system with related valves and branches, air separator, and accessories, as well as electrical power distribution, instruments and controls according to engineering design.
4. The Mechanical Room, (both interior and exterior), shall also be repaired and/or renovated for fuel gas piping, sanitary pipe, floor drains, exhaust fan, louvers and wall painting.
5. Integration of the updated controller to the campus-wide EMCS system via VLAN. (VLAN drop will be provided by Government). This work includes updating the GUI at the front-end of the EMCS System in Building 1402.
6. Test, adjust, and balance the water supply system at the supply side (Mechanical Room).
Contractor shall also perform and pass the field performance test for commissioning.
7. Upgrade electrical system by replacing two panelboards with one motor control center and one panelboard.
8. Upgrade interior lighting system in the mechanical room by replacing fluorescent luminaires with LED.
B. Location: The work shall be located at Sheppard Air Force Base (SAFB) which is located in Wichita County, in the northern part of Wichita Falls, TX. The project is VNVP 181605, Replace Chiller, Cooling Tower, and Controls, B1080. Project performance period will be a total of sixty (60) calendar-days construction time from the notice to proceed (NTP) date. The lead time for equipment and materials is not included in the 60-days construction time period, which shall be reasonably allocated based on the item of the longest lead-time for procurement.
The Customer or POC has not requested the project to start on a specific date.
C. Phasing: Not used.
1.2 REFERENCES: NOT USED.
1.3 SUBMITTALS: Government approval is required for submittals with a “G” designation and may be required on any of the following items. Items requiring submittals will be listed on the Schedule of Material Submittals. The contractor shall provide any submittals required by the contract documents which have been omitted from the Schedule of Material Submittals. Provide copies of all submittals per the requirements in Section 01 33 00 SUBMITTAL PROCEDURES.

A. SD-01 Preconstruction Submittals:
AF Form 103 Existing Damage Work Plan
1.4 CONTRACT DRAWINGS: Contractor shall immediately check furnished drawings and notify the government of any discrepancies.
1.5 OCCUPANCY OF PREMISES: The contractor will perform all work with the building area occupied.
A. The contractor shall coordinate all work with the facility managers, the contracting officer (CO), and the construction inspector for a safe and smooth execution of the project. Before work is started, the contractor shall arrange with the CO a sequence of procedure, means of access, and space for storage of materials and equipment.
B. The contractor shall rent a temporary chiller (or heater) during the project execution to avoid interruption to the building AC.
1.6 EXISTING WORK: In addition to "FAR 52.236-9, Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements":
A. Remove or alter existing work in such a manner as to prevent injury or damage to any portions of the existing work which remain.
B. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the CO. At the completion of operations, existing work shall be in a condition equal to that which existed before new work started as approved by the CO.
C. The contractor shall provide documentation in the form of drawing markups and/or photographs of existing damage within the work area prior to beginning construction. Any damage that is found that was not documented as existing shall become the responsibility of the contractor to repair.
1.7 ON-SITE PERMITS:
A. Authorization for Work: The contractor shall complete AF Form 103, Base Civil Engineering Work Clearance Report (commonly referred to as a “Digging Permit”) prior to starting any work regardless if excavation will be necessary. Submit the completed form to the construction inspector.
B. Approval for Excavation: Before performing any excavation, grading, trenching or other operations whereby existing underground utilities may be damaged, the contractor shall complete AF Form 103. The form, as received by the contractor, will list points of contact and telephone numbers for each of the various underground utilities (telephone, gas, water, electrical, etc.) existing in the work area. It is the contractor’s responsibility to contact the point of contact for each utility listed, request that any utilities existing in the work area be marked, and obtain each point of contact's certification on the form. After each point of contact has been contacted and has certified such by signing or initialing the form, the form must then be delivered to the construction inspector.
1.8 LOCATION OF UNDERGROUND UTILITIES:
A. The government (or owner, in the case of privately owned utilities) is responsible for marking the location of utilities in the work area. Typically, flags and/or paint will be used to mark the earth and/or pavement to provide semi-permanent identification of the utility location. The

government will not make any indications or marks of the depth of the utility below the existing grade. The contractor shall allow sufficient time for the government to accomplish this task at no additional cost to the government. Once the utilities are marked, the contractor is responsible for maintaining the flags/paint at their proper location(s).
B. No excavation shall be permitted until all existing area utilities have been marked. The contractor is responsible for repairs to any underground utilities damaged as a result of this work if the damaged utility is within a lateral zone of three (3) feet on either side of the government/commercially-provided mark indicating the location of that utility. This three (3) foot zone on either side of the mark provided is a hand-dig only area. Because depth of the utility is not provided, the contractor assumes responsibility for any depth determination necessary.
1.9 WORK PLAN: After the pre-work conference is completed and prior to starting work, the contractor shall submit for government approval, a detailed work plan indicating how the work will be accomplished, start to finish. The required work plan is a separate document from the AF Form 3064. Items required in the work plan include, but are not limited to, the following:
work area involved, extent of work to be accomplished, equipment to be used, manpower available, timing and sequence of work elements, and subcontractors involved. The contractor may not proceed with any work until the work plan has been approved by the government. As a minimum, the contractor shall re-submit the work plan each month to include updates as necessary.
1.10 GOVERNMENT-FURNISHED MATERIAL AND EQUIPMENT: NOT USED.
1.11 SALVAGE MATERIAL AND EQUIPMENT: Items designated to be salvaged remain the property of the government. Segregate, itemize, deliver, and off-load the salvaged property at the CO designated on SAFB storage area. Salvageable equipment: (Gas fired) Water Heater, Water Heated Heating Radiators.

PART 1 - GENERAL

1.3 SPECIAL SCHEDULING REQUIREMENTS:
A. The contractor shall provide written notice to the CO at least fourteen (14) calendar days before starting the scheduled work.
B. Permission to interrupt any roads and/or utility service must be requested in writing a minimum of fifteen (15) calendar days prior to the desired date of interruption.
C. Should the contractor’s work require that any contracted services supplier’s equipment be removed, installed, reinstalled, or be otherwise affected or that the supplier’s personnel must be rescheduled to accommodate the contractor’s work, the contractor shall coordinate with the affected supplier.
1.4 CONTRACTOR ACCESS AND USE OF PREMISES:
A. Base Access: Except as stated below, the contractor must obtain personnel passes for all personnel before they may enter SAFB. Delivery drivers possessing a bona-fide Bill of Lading with SAFB delivery destination normally do not require personnel passes. The CO is the point of contact for personnel passes.
B. Search Pit: Contractor shall access SAFB via the Missile Road gate as indicated on the drawings. All vehicles entering the gate are subject to search. Vehicles over three-quarter (3/4) ton gross weight such as tractor trailers, delivery vans, haul trucks, and concrete trucks shall be searched before base access is permitted. Unless the contractor makes special arrangements at least seventy-two (72) hours in advance, such vehicles will be denied base entry outside normal search pit hours at no additional cost to the government.
1. Deliveries to SAFB must be made to the Missile Road Gate between 7:00 a.m. and 4:00 p.m.
(0700 – 1600 hours), Monday through Friday (federal holidays excluded). Upon arrival at the gate, drivers must provide proper photo identification, Bill of Lading identifying the contract number and items being delivered, and the delivery destination on SAFB. Trucks should be packed to allow search prior to base entry. For security purposes, expect delays when entering SAFB.
C. Personnel Contacts: Provide a list of contact personnel of the contractor including telephone numbers for use in the event of an emergency. At a minimum, the contractor shall provide the name and cell phone number of the designated on-site project superintendent. As changes occur and additional information becomes available, correct and change the information contained in previous lists.
D. Base Regulations: Ensure that contractor personnel employed on SAFB become familiar with and obey SAFB regulations including safety, fire, traffic, and security regulations. Keep within the limits of the work and avenues of ingress and egress. Wear hard hats in designated areas.

Do not enter any restricted areas unless cleared for such entry and required to do so by the contract. Mark contractor equipment for identification.
1. Tobacco Use on SAFB: Tobacco use is prohibited on SAFB, except in “Designated Tobacco Areas.” Tobacco includes, but is not limited to, cigars, cigarettes, electronic-cigarettes (e-cigarettes), stem pipes, water pipes, hookahs, and smokeless products that are chewed, dipped, or sniffed.
2. Use of Cellular Telephones on SAFB: Vehicle operators on SAFB will not use cell phones unless the vehicle is safely parked or unless they are using a hands–free device. The wearing of any other portable headphones, earphones, or other listening devices (except for hands– free cellular phones) while operating a motor vehicle is prohibited. Other tasks with the potential for driver distractions such as eating and drinking, operating radios, CD players, global positioning equipment, and so on should only be done when the vehicle is safely parked, when possible.
3. Radioactive equipment: The contractor shall obtain authorization to bring any nonexempt radioactive material (such as density gauges or moisture meters) onto SAFB by contracting the Nuclear Regulatory Commission, Region IV Office, using the NRC Form 241. Before bringing any radioactive material onto SAFB, the contractor shall forward a copy of the NRC Form 241 to:

E. Working Hours:
1. Typical Work Hours: 7:30 a.m. to 4:30 p.m. (0730 – 1630 hours) Monday through Friday, exclusive of federal holidays.
2. Missile Road Search Pit Hours: 6:00 a.m. to 6:00 p.m. (0600 – 1800 Hours) Monday through Saturday. The search pit is closed on Sundays and federal holidays.
3. Work Outside Regular Hours: At the government’s option and with approval of the CO, work may be permitted on non-typical hours at no additional cost to the government. Make application fifteen (15) calendar days prior to such work to allow arrangements to be made by the government for inspecting the work in progress, giving the specific dates, hours, location, type of work to be performed, contract number and project title. Based on the justification provided, the CO may approve work outside regular hours. During periods of darkness, the different parts of the work must be lighted in a manner approved by the CO.
F. Occupied and Existing Building(s): Do not enter the building(s) without prior approval of the CO. Provide dust covers or protective enclosures to protect existing work that remains and government material during the construction period as indicated on the drawings. The government will remove and relocate other government property in the areas scheduled to receive work unless indicated on the drawings to be removed by the contractor.
G. Utility Cutovers and Interruptions: When a utility outage is necessary to perform the contract work in an occupied facility, regardless of whether or not the work area itself is occupied, the contractor shall perform the outage during non-duty hours at no additional cost to the government, unless otherwise approved by the CO.
H. Use of Facilities: Contractor employees are not authorized to use base service establishments such as the shoppettes, service station, exchange, bowling alleys, pools, theaters, or commissary. Contractor employees are authorized to eat at any exchange food activity with the exception of government dining facilities (mess facilities). Contractor employees who are

retired military, a military dependent or guard/reserves and in possession of a valid military identification card may use SAFB services as authorized. If an unauthorized civilian is found to be wrongfully using base service establishments, their name will be forwarded to the SAFB Wing Commander for appropriate action. Actions can include, but are not limited to, being barred from SAFB.

ADMINISTRATIVE REQUIREMENTS

1.3 COORDINATION:
A. Coordinate construction operations included in different sections of the specifications to ensure efficient and orderly completion of each part of the work. Coordinate construction operations that depend on each other for proper installation, connection, and operation.
B. Administrative Procedures: Coordinate scheduling and timing of required administrative procedures with other construction activities and activities of other contractors to avoid conflicts and to ensure orderly progress of the work. Such administrative activities include, but are not limited to, the following:
1. Preparation of the schedule of values if required.
2. Installation and removal of temporary facilities and controls.
3. Delivery and processing of submittals.
4. Progress meetings if required.
5. Closeout activities.
C. Conservation: Coordinate construction activities to ensure that operations are carried out with consideration given to conservation of energy, water, and materials. Coordinate use of temporary utilities to minimize waste.
1.4 SITE PROJECT SUPERINTENDENT: The contractor must have at least one qualified project designated superintendent capable of reading, writing, and conversing fluently in the English language on the job site at all times while work is in progress. In addition, if a quality control (QC) representative is required on the contract, then that individual shall also have fluent English communication skills. The site project superintendent is responsible for the quality of the work on the job and ensuring that all work performed by the contractor’s forces and by subcontractor(s) is in full compliance with the construction documents. The site project superintendent is responsible for the overall construction activities at the site, including scheduling, quality, and production. The site project superintendent must maintain a physical presence at the site during all construction activities. Prior to the preconstruction conference, the contractor shall provide the name and cell phone number of the designated on-site project superintendent in writing to the CO. This information shall remain on file with the CO throughout the duration of the contract and shall be updated in writing by the contractor in the event of and at least two (2) calendar days prior to a personnel change. The designated on-site project superintendent shall be required to attend the pre-construction conference.
1.5 CHARACTER OF WORK AND MECHANICS: The contractor shall execute the work in the most professional manner by qualified, careful and efficient mechanics skilled in the trade. The Contractor’s work must be executed in strict accordance with the contract documents and prevailing industry standards. Only certified journeymen in respective trades, or apprentices under the direct supervision of certified journeymen, may install, supervise installation of, or alter or repair electrical and mechanical systems. Electrical and mechanical systems include, but are not limited to: pipe, plumbing, HVAC systems, electrical wiring, fire protection systems, welding, equipment and devices. The CO will recognize as certification a current state or municipal tested and issued plumbing/gas/air conditioning and electrical license upon approval. The license must be available for government inspection at the pre-work conference and from mechanics and

electricians at the jobsite throughout contract operations. For those trades not requiring a license, the contractor shall have a minimum of three (3) years’ experience in the trade. The government reserves the right to request work references.
1.6 AVAILABILITY OF CADD DRAWING FILES: After award and upon request, the electronic "Computer-Aided Drafting and Design (CADD)" drawing files will only be made available to the contractor for use in preparation of construction data related to the referenced contract subject to the following terms and conditions.
Data contained on these electronic files shall not be used for any purpose other than as a convenience in the preparation of construction data for the referenced project. Any other use or reuse shall be at the sole risk of the contractor and without liability or legal exposure to the government. The contractor shall make no claim and waives to the fullest extent permitted by law, any claim or cause of action of any nature against the government, its agents or sub consultants that may arise out of or in connection with the use of these electronic files. The contractor shall, to the fullest extent permitted by law, indemnify and hold the government harmless against all damages, liabilities or costs, including reasonable attorney's fees and defense costs, arising out of or resulting from the use of these electronic files.
These electronic CADD drawing files are not construction documents. Differences may exist between the CADD files and the corresponding construction documents. The government makes no representation regarding the accuracy or completeness of the electronic CADD files, nor does it make representation to the compatibility of these files with the contractor’s hardware or software.
In the event that a conflict arises between the signed and sealed construction documents prepared by the government and the furnished CADD files, the signed and sealed construction documents shall govern. The contractor is responsible for determining if any conflict exists. Use of these CADD files does not relieve the contractor of duty to fully comply with the contract documents, including and without limitation, the need to check, confirm and coordinate the work of all contractors for the project.
If the contractor uses, duplicates and/or modifies these electronic CADD files for use in producing construction data related to this contract, all previous indicia of ownership (seals, logos, signatures, initials and dates) shall be removed.
1.7 PROJECT CORRESPONDENCE: All project correspondence with SAFB personnel and all contractual issues/questions shall be coordinated with the CO and contract administrator.
1.8 ELECTRONIC MAIL (E-MAIL) ADDRESS: The contractor shall establish and maintain electronic mail (e-mail) capability along with the capability to open various electronic attachments in Microsoft® Office®, Adobe® Acrobat®, and other similar formats. Within ten (10) calendar days after contract award, the contractor shall provide the CO a single (only one) e-mail address for electronic communications from the CO related to this contract including, but not limited to contract documents, invoice information, request for proposals, and other correspondence. The CO may also use email to notify the contractor of base access conditions when emergency conditions warrant, such as hurricanes, terrorist threats, etc. Multiple email addresses will not be allowed.
It is the contractor's responsibility to make timely distribution of all CO initiated e-mail with its own organization including field office(s). The contractor shall promptly notify the CO, in writing, of any changes to this email address.
1.9 REQUEST FOR INFORMATION (RFI):
A. General: Immediately on discovery of the need for additional information or interpretation, the

contractor shall prepare and submit an RFI in the form specified. Coordinate and submit RFIs in a prompt manner so as to avoid delays in the work.
B. Content of the RFI: Include a detailed, legible description of item needing information or interpretation.
1. Project name.
2. Project number.
3. Date.
4. Name of contractor.
5. RFI number, numbered sequentially.
6. RFI subject.
7. Specification section number and title and related paragraphs, as appropriate.
8. Drawing number and detail references, as appropriate.
9. Field dimensions and conditions, as appropriate.
10. Contractor's suggested resolution. If contractor's solution(s) impacts the date of completion or has a cost impact, the contractor shall state impact in the RFI.
11. Attachments: Include sketches, descriptions, measurements, photos, product data, coordination drawings, and other information necessary to fully describe items needing interpretation.
a. Include dimensions and details of affected materials and assemblies on attached sketches.
C. RFI Forms: Use form at end of section. Electronic word file of RFI form is available upon request.
D. Government’s Response: The government technical representative will review each RFI, determine action required, and respond appropriately. Allow a reasonable amount of working days for the response for each RFI.
1. The government technical representative’s action may include a request for additional information, in which case the time for response will date from time of receipt of additional information.
2. If the contractor believes the RFI response warrants change in the date of completion or the contract amount, notify the CO in writing within ten (10) calendar days of receipt of the RFI response.
3. An RFI received from the contractor that does not contain at minimum items 1, 2, 3, 4, 5, 6, 10, and 7, 8, or 11 above will be returned to the contractor without response.
1.10 MEETINGS:
A. Preconstruction Conference: The government will schedule and conduct a preconstruction conference at a time convenient to the government. The meeting will be conducted by the CO and will include briefings to the contractor by SAFB agencies.
1. Attendees: The government’s representatives, the contractor, the project superintendent, the major subcontractors, and other concerned parties shall attend the meeting.
2. The meeting intent is to review responsibilities and personnel assignments.
3. Agenda: The meeting agenda may include the following:

B. Progress Meetings: The government may request progress meetings at regular intervals depending on scope and length of a particular project.
1. Attendees: The governments representatives, each contractor, subcontractor, supplier, and other entities concerned with current progress or involved in planning, coordination, or performance of future activities shall be represented at these meetings. All participants at the meeting shall be familiar with the project.

C. Beneficial Occupancy Inspection (BOI):
1. A BOI may be conducted when it is in the best interest of the government to occupy, use, or accept a portion of the work prior to closing out a project contract.
2. Submit request in writing for a BOI to the CO. On receipt of the request, the government will coordinate the BOI with the contractor, using agency, civil engineering, and inspections and will note unfulfilled requirements. The government will prepare the BOI report. CO will notify the contractor of items identified by the government that must be completed or corrected.
a. Re-inspection: Request re-inspection when the work identified in previous inspections as incomplete is completed or corrected.
D. Final Inspection: Submit a request in writing for final inspection to the CO. On receipt of request, the government will coordinate the final inspection with the contractor, using agency, civil engineering and inspections and will note unfulfilled requirements. The government will prepare the final inspection report. The CO will notify the contractor of items identified by the government that must be completed or corrected before final inspection will be approved.

PART 1 - GENERAL
1.1 REFERENCES: NOT USED.
1.2 SUBMITTALS: Government approval is required for submittals with a “G” designation and may be required on any of the following items. Items requiring submittals will be listed on the Schedule of Material Submittals. The contractor shall provide any submittals required by the contract documents which have been omitted from the Schedule of Material Submittals. Provide copies of all submittals per the requirements in Section 01 33 00 SUBMITTAL PROCEDURES.
A. SD-01 Preconstruction Submittals:

1.3 ACCEPTANCE: Prior to the start of work, prepare and submit a baseline construction schedule to the CO for acceptance if the project duration is greater than sixty (60) days. The baseline schedule shall show sequence of construction by activity broken down on a weekly basis to the hundredth percentile. Schedule shall list installed value of component parts in sufficient detail to serve as a basis for computing values for progress payments. Submission and acceptance of an error free baseline schedule and updates are a condition precedent to processing the contractor's pay request.
Schedules for projects including multiple phases, buildings, and/or locations must be broken down by phase, building, and/or location as well as activity as discussed above.
1.4 SCHEDULE FORMAT: Construction schedule is required to be on AF Form 3064. A sample form is provided at the end of this section.
1.5 SCHEDULE PREPARER: The contractor shall designate an authorized representative to be responsible for the preparation and required updating (activity status) of schedules and reports.
The designated authorized representative shall be experienced in scheduling projects similar in nature and complexity to this project.
1.6 UPDATED SCHEDULES: The contractor shall update the construction schedule when directed by the CO. The updated schedule shall reflect actual activity progress and plan for completing the remaining work. Submit copies of purchase orders and confirmation of delivery dates as directed.
1.7 WEEKLY PROGRESS MEETING: The contractor’s superintendent is required to meet on a weekly basis with the construction inspector to agree on a percentage of work accomplished for each part of the work. Construction inspector shall fill out an AF Form 3065 Contract Progress Report and provide a final report to the contractor for submission to the CO. A sample progress report is provided at the end of this section.
1.8 DAILY LOG:
A. Contractor shall ensure that a daily log is prepared and maintained at the job site for review by the construction inspector. Use log provided at end of section.
B. Log shall include, but is not limited to:
1. List of subcontractors at site
2. Approximate count of personnel at site
3. Accidents and emergency procedures
4. Field orders and significant decisions
5. Work in progress
C. Submit a duplicate copy of log to the construction inspector by 1000 hours each day.

1.9 THREE WEEK LOOK AHEAD SCHEDULE: The contractor shall prepare and issue a three week look ahead schedule to provide a more detailed day-to-day plan of areas of work to be completed.
Five (5) hard copies of the three week look ahead schedule shall be delivered to the CO no later than 0800 hours (8:00 a.m.) each Wednesday. The three week look ahead will be reviewed during the weekly coordination meeting between the contractor, CO, construction inspector, and airfield manager. The time of weekly coordination meeting will be scheduled at the NTP meeting.
1.10 CORRESPONDENCE AND TEST REPORTS: All correspondence (e.g., letters, RFIs, e-mails, meeting minute items, production and QC daily reports, material delivery tickets, photographs, etc.) shall reference schedule activities that are being addressed. All test reports (e.g., concrete, soil compaction, weld, pressure, etc.) shall reference schedule activities that are being addressed.
1.11 ADDITIONAL SCHEDULING REQUIREMENTS: Any references to additional scheduling requirements, including systems to be inspected, tested, and commissioned, that are specified within the remainder of the contract documents, are subject to all requirements of this section.

CONTRACT PROGRESS SCHEDULE

1. Prepare three copies of AF Form 3064, in accordance with these instructions and submit to the contracting officer at the time stated in the contract or as established by the contracting officer.
2. Blocks 1, 2, and 3. Enter the contract number, starting date, and completion date as shown on the contract.
3. Block 8. Enter the title of the project as shown on the contract.
4. Block 9. Enter name and address of your firm. The three copies submitted to the contracting officer must be dated and signed by the contractor or an auathorized representative of the firm.
5. Lines 1 through 16. Major elements of work (whether unit price or lump sum contract) such as excaavation, concrete work, carpentry, engineering, installation of communication electronic facilities, etc., will be shown in Column B in logical seequence and in sufficient detail to identify the element.
6. The percentage of each listed work element to the complete job will be shown opposite each element in Column C.As an example, if 3 work elements are shown: excavation, concrete work, and backfill, and it is estimated that excavation is 50% of the total job, concrete 30%, and backfill 20%, these percentages will be shown opposite each element. The total must equal 100%.
7. Columns D through Q will be used to show planned periodic progress, as required by the contracting officer, during the period of the contract. Only the top half of the space under these columns will be used by contractors. Using the example cited above and assuming that the contract period for performance is 8 weeks, and it is planned to accomplish 10% of the excavation work the 1st week and 20% each week until excavation is complete, 10 would be shown in the upper half of Line 1 under Column D, 20 under Column E, and 20 under Column F for a total of 50%. If it is planned to accomplish the concrete work during the 4th, 5th, and 6th weeks in equal amounts, 10 would be entered under Columns G, H, and I on Line 2. Assuming the backfill will be accomplished during the 7th and 8th weeks, 10 would be entered under Columns J and K on Line 3. If the contract extends beyond 14 weeks, additional copies of AF Form 3064, with appropriate alterrations, may be spliced on as a continuation of performance record.
8. Total amount of work planned at intervals as shown in Columns D through Q will be totaled and entered in the top half of the space under the appropriate column on Line 16. Again using the example cited above, the totals under Columns D through K would read 10, 20, 20, 10, 10, 10, 10, 10, for a total of 100%. These totals will be used a lot an overlay graph using Lines 67 through 15, Beginning with Column D and extending to the right that number of columns (weeks) authorized by the contract for performance time.
9. Beginning at the loweer left corner, line 15, column D, the graph will be plotted using a broken or dotted line showing the cumulative total progress for planned intervals. Again using the example cited above, the dotted line would, for the 1st week, extend from line 15 to line 14 under column d for 10%. The second week the dotted line would extend from Line 14 to Line 12 under Column E for 20%, making a cumulative total of 30%. The dotted line should end (100%) at the intersection of Line 5 and Column K.
10. In the event the contract is modified by the contracting officer under terms of the contract, changing the progress of work as originally scheduled, adding or deleting work or changing the original completion date, a revised progress schedule will be prepared and submitted to the contracting officer for approval. In preparing the revised schedule, the amount of work completed will be considered, together with the changed new completiondate set forth in the contract modification. Considering these factors, the work under the contract will be rescheduled over the new total performance time in the same manner that the original schedule was prepared. The new completion date will be entered in Block 3. "First Revised Schedule," "Second Revised Schedule," etc., will be entered immediately underneath the form title. Time of submission of the revised schedule will be determined by the contracting officer.
11. At such, intervals as may be established by the contracting officer, the contractor will submit to the contracting officer AF Form 3065. "Contract Progress Report," indicating the percentage of work accomplished for each work element during the established reporting period. Accurate reports are of the utmost importance to the contractor and to the Government, since the percentage of completion, or progress, thus reported is used in administration of the contract and may be used in connection with approval of partial payments under the contract. Care should be taken to plan the work in such manner that it can be accomplished as stated in this schedule. If for any reason it becomes known that any part of the progress under this contract will be delayed, this fact should be reported to the contracting officer immediately.

PART 1 - GENERAL
1.1 SUMMARY:
A. The CO may request submittals in addition to those specified when deemed necessary to adequately describe the work covered in the respective sections.
B. Units of weights and measures used on all submittals are to be the same as those used in the contract drawings.
C. Each submittal is to be complete and in sufficient detail to allow ready determination of compliance with contract requirements.
D. Proposed deviations from the contract requirements are to be clearly identified. Include within submittals items such as: Contractor's, manufacturer's, or fabricator's drawings; descriptive literature including (but not limited to) catalog cuts, diagrams, operating charts or curves; test reports; test cylinders; samples; O&M manuals (including parts list); certifications; warranties;
and other such required submittals.
E. Submittals requiring government approval are to be scheduled and made prior to the acquisition of the material or equipment covered thereby. Pick up and dispose of samples not incorporated into the work in accordance with manufacturer's Safety Data Sheets (SDS) and in compliance with existing laws and regulations.
1.2 DEFINITIONS:
A. Submittal Descriptions: Submittals requirements are specified in the technical sections.
Submittals are identified by Submittal Description (SD) numbers and titles as follows:
1. SD-01 Preconstruction Submittals: Submittals which are required prior to start of construction (work), issuance of contract NTP, commencing work on site, or the start of the next major phase of the construction on a multi-phase contract, includes schedules, tabular list of data, or tabular list including location, features, or other pertinent information regarding products, materials, equipment, or components to be used in the work.
2. SD-02 Shop Drawings: Drawings, diagrams and schedules specifically prepared to illustrate some portion of the work and to show how multiple systems and interdisciplinary work will be coordinated.
3. SD-03 Product Data: Catalog cuts, illustrations, schedules, diagrams, performance charts, instructions and brochures illustrating size, physical appearance and other characteristics of materials, systems or equipment for some portion of the work. Samples of warranty language when the contract requires extended product warranties.
4. SD-04 Samples: Fabricated or unfabricated physical examples of materials, equipment or workmanship that illustrate functional and aesthetic characteristics of a material or product and establish standards by which the work can be judged. Color samples from the manufacturer's standard line (or custom color samples if specified) to be used in selecting or approving colors for the project. Field samples and mock-ups constructed on the project site establish standards by which the ensuing work can be judged. Includes assemblies or portions of assemblies which are to be incorporated into the project and those which will be removed at conclusion of the work.
5. SD-05 Design Data: Calculations, mix designs, analyses or other data pertaining to a part of work.
6. SD-06 Test Reports: Report signed by authorized official of testing laboratory that a material, product or system identical to the material, product or system to be provided has

been tested in accord with specified requirements. Testing must have been within three (3) years of date of contract award for the project. Report which includes findings of a test required to be performed by the contractor on an actual portion of the work or prototype prepared for the project before shipment to job site. Report which includes finding of a test made at the job site or on sample taken from the job site, on portion of work during or after installation. Also includes investigation reports, daily logs and checklists, final acceptance test and operational test procedure.
7. SD-07 Certificates: Statements printed on the manufacturer's letterhead and signed by responsible officials of manufacturer of product, system or material attesting that product, system or material meets specification requirements. Must be dated after award of project contract and clearly name the project. Document required of the contractor, or of a manufacturer, supplier, installer or subcontractor through the contractor, the purpose of which is to further quality of orderly progression of a portion of the work by documenting procedures, acceptability of methods or personnel qualifications. Also includes confined space entry permits and text of posted operating instructions.
8. SD-08 Manufacturer's Instructions: Preprinted material describing installation of a product, system or material, including special notices and SDS concerning impedances, hazards and safety precautions.
9. SD-09 Manufacturer's Field Reports: Documentation of the testing and verification actions taken by manufacturer's representative at the job site, in the vicinity of the job site, or on a sample taken from the job site, on a portion of the work, during or after installation, to confirm compliance with manufacturer's standards or instructions. The documentation must be signed by an authorized official of a testing laboratory or agency and must state the test results; and indicate whether the material, product, or system has passed or failed the test.

10. SD-10 Operation and Maintenance Data: Data that is furnished by the manufacturer, or the system provider, including manufacturer's help and product line documentation necessary to maintain and install equipment. This data is needed by operating and maintenance personnel for the safe and efficient operation, maintenance and repair of the item. This data is intended to be incorporated in an operations and maintenance manual or control system.
