SECTION L – INSTRUCTIONS TO OFFERORS

PROPOSAL PREPARATION INSTRUCTIONS AND INFORMATION TO OFFERORS

1.0 General Instructions: This section provides general guidance, as well as specific instructions on format and content, for preparing proposals. The Offeror's proposal must include all information requested and must be submitted in accordance with these instructions. Compliance with these instructions is mandatory and failure to furnish a complete proposal, at the time of proposal submission, may result in the proposal being unacceptable to the Government and may eliminate the Offeror from further consideration for award. It is the Offeror's responsibility to ensure the completeness of the proposal submitted. With respect to Price proposals, the burden of proof for credibility rests with each Offeror. Evaluation of a proposal will be conducted solely on the basis of the information contained within it and the Government will not assume that an Offeror possesses any capabilities not specified. Inconsistencies among the parts of each proposal shall be explained. Any unexplained inconsistency may raise a fundamental question of the Offeror's understanding of the requirement and/or ability to perform. Offerors are advised that their proposals are presumed to represent their best efforts and most complete responses to this solicitation. Proposals that simply reiterate the contents of this solicitation, or contain only superficial responses, may be considered unacceptable. Evidence of experience, capability, and qualifications that clearly demonstrate and support the Offeror’s claims are essential. The absence of such evidence will adversely influence the evaluation of the proposal. Offerors must clearly identify any exceptions to the solicitation terms and conditions and provide complete supporting rationale. Non-conformance with the instructions provided in these Instructions to Offerors may result in an unfavorable proposal evaluation.

The Government intends to execute a Multiple Award Indefinite Delivery, Indefinite Quantity Firm Fixed Price

(FFP) contract for the Joint Base San Antonio (JBSA) Noncomplex Construction (NC2) requirement.

The Government intends to select up to five (5) active contractors and two (2) inactive contractors. The

Government intends to evaluate proposals and award contracts without discussions with Offerors (except clarifications as described in FAR 15.306(a). Therefore, the Offeror's initial proposal should contain the Offeror's best terms from a technical, past performance, and a price standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary or in the best interest of the

Government. The base ordering period is for five years plus five (5) one year option periods. The total contract performance will not exceed ten years. Nonperforming contractors may be off ramped IAW Section G of the resulting contract. If an active contractor is off ramped, an inactive contractor may be on ramped. On ramping is defined as switching a qualified inactive contract holder to active status to compete for subsequent requirements under the fair opportunity process identified at FAR 16.505. Awardees are required to recertify their small business status prior to the completion of the fifth year of the contract IAW FAR 52.219-28.

In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain a copy of all unsuccessful proposals.

The Contracting Officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists, no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the Contracting Officer to determine the reasonableness and affordability of the price.

1.2 Type of Acquisition: This acquisition is a Total Small Business set aside. Proposals may only be submitted by Small Business firms, registered in the North American Industrial Classification System Code (NAICS) of

236220. The Government intends to reserve one active contract award for a certified WOSB registered in the

WOSB Program Repository IAW FAR 19.1503(b)(3) and one active contract award for a SDVOSB. NOTE:

OFFERORS MUST ANNOTATE ALL SOCIOECONOMIC/SMALL BUSINESS RESERVES FOR WHICH

THEY QUALIFY AND DESIRE TO COMPETE IN AS A PART OF THE VOLUME I – EXECUTED

CONTRACT DOCUMENTATION/SUPPORTING DOCUMENTATION.

1.3 Discrepancies and Ambiguities: If an offeror believes the requirements in these instructions contain an error, omission, ambiguity, or are otherwise unsound, the offeror shall immediately notify the Contracting

Officer in writing with supporting rationale no later than ten (10) calendar days after the RFP release.

1.4 Discovery of Mistakes: Mistakes in a contractor’s proposal that are disclosed after award shall be processed substantially in accordance with the procedures for mistakes in bids at FAR 14.407-4

1.5 Team/Team Member(s)/Teaming Partner(s): The Government will consider the Offeror and all subcontractors a “team” when evidence of a signed Teaming Arrangement (TA) or Letter of Intent (LOI) is provided. This RFP will refer to the Offeror and all identified teaming subcontractors as a “team.” This RFP will refer to all identified teaming subcontractors as “team members” or “teaming partners.” To be valid under this solicitation, a Teaming Arrangement (TA) or Letter of Intent (LOI) must commit the parties to performance under this contract.

1.6 Preproposal Conference/Seed Project Site Visit: A preproposal conference and site visit will be held at

JBSA Ft. Sam Houston on 23 March 2020 at 09:30 – 12:00. The preproposal conference and site visit is an opportunity for industry to inspect the existing conditions of the Seed Project facility and communicate questions, concerns, or suggestions to the Government prior to proposal submission. All questions and answers will be in written form and provided to offerors through beta.sam.gov. Attendance is not mandatory but is highly encouraged.

1.7 Points of Contact (POC): The POCs for this acquisition are the Contracting Officer (CO), Mr. A. Ryan

Rodriguez, and the Contract Specialist (CS), Ms. Laura Herring. Written requests for clarification shall be sent to the CO/CS. All communication must be coordinated through the CO/CS using the following contact information:

1.8 Questions and Answers Period: All questions regarding this solicitation must be submitted in writing. All questions and subsequent answers will be posted to the beta.sam.gov website. It is the responsibility of the offeror to continuously monitor the site for updates. All questions must be submitted electronically to the individual(s) listed in paragraph 1.7 above no later than 1400 hrs CST 7 business days after the site visit. Any questions submitted after this period may not be answered.

1.9 Notifications/Debriefings: Contractors who are eliminated from further consideration for award will receive a notification with a basis of the determination only. Debriefings are available IAW FAR 15.505 and 15.506.

(a) Preaward Debriefing:

(i) The offeror may request a preaward debriefing by submitting a written request for debriefing to the Contracting Officer within 3 calendar days after receipt of the notice of exclusion from the competition or exclusion from the competitive range.

(ii) At the offeror’s request, this debriefing may be delayed until after award. If the debriefing is delayed until after award, it shall include all information normally provided in a post award debriefing.

(iii) If the offeror does not submit a timely request, the offeror need not be given either a preaward or a postaward debriefing. Offerors are entitled to no more than one debriefing for each proposal.

(b) Postaward Debriefing:

(i) An offeror, upon its written request received by the agency within 3 calendar days after the date on which that offeror has received notification of contract award in accordance with 15.503(b), shall be debriefed and furnished the basis for the selection decision and contract award.

(ii) To the maximum extent practicable, the debriefing should occur within 5 calendar days after receipt of the written request.

(iii) An offeror that was notified of exclusion from the competition, but failed to submit a timely request, is not entitled to a debriefing.

(iv) Government accommodation of a request for delayed debriefing pursuant to 15.505(a)(2), or any untimely debriefing request, does not automatically extend the deadlines for filing protests.

PROPOSAL PREPARATION INSTRUCTIONS

2.0 The following instructions are to aid in the evaluation process: To ensure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Proposals must be complete, self-sufficient, and respond directly to the requirements of this solicitation; alternate proposals will not be accepted. The response shall consist of four (4) separate volumes; Volume I – Executed Contract Documentation/Supporting

Documentation, Volume II - Technical, Volume III Past Performance, Volume IV Price.

2.1 Proposal Submission: Offerors are responsible for submitting proposals so as to reach the Government office designated in this solicitation by the time specified in this solicitation. Both an electronic and 6 original hard copies are due prior to the proposal submission date. The electronic copy shall be submitted using the DoD

SAFE website https://www.arl.army.mil/www/safe.cfm. Offerors will be required to contact the Contracting

Officer and Contract Specialist to obtain a code to upload source selection documents. It is the responsibility of the offeror to request the code and submit the proposal prior to the time and date set for proposal submission.

The original hard copy proposal package must be delivered to the address in Block 7 of the SF 1442 prior to the time set for closing of proposals; see Block 13A of the SF 1442. Exchanges of source selection information after submission of the initial proposal packages between the Government and offerors will be controlled by the

Contracting Officer.

All electronic documents must be in .pdf format and shall follow the instructions in paragraphs 2.2 and 2.3 below. Title of the documents should state the corresponding proposal volume/title and include the offerors

CAGE code (i.e. “Vol 1 Executed Contract Documentation/Seed Project Basis of Estimate and Offeror

CAGE#”).

Contractors should request base access for hand carried proposal delivery NLT 48 hours prior to the proposal due date/time by sending an email request identifying the company name and list of individuals requesting access to the Contracting Officer and Contract Specialist identified in paragraph 1.7.

Proposals may also be submitted by mail to the 502d Contracting Squadron at JBSA-Lackland, TX. Mailed proposals must contain six original hard copies. Title of the documents should state the corresponding proposal volume/title and include the offerors CAGE code (i.e. “Vol 1 Executed Contract Documentation/Seed Project

Basis of Estimate/ CAGE#”). The required original hard copy proposal may be mailed to the following address:

Any proposal, bond, amendment, or revision that is received at the designated Government office after the exact time specified for receipt of proposals will be “late” and will not be considered unless the Contracting Officer determines the criteria set forth in FAR 15.208 exists.

Proposals shall be complete, clearly presented, and include sufficient detail for effective evaluation as detailed in section M of this solicitation and for substantiating validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements but rather provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of the offeror’s facilities and/or experience and will rely heavily on the information presented in the offeror's proposal. Proposals shall be neat, indexed (cross-indexed as appropriate) and assembled in an orderly manner.

Elaborate artwork and expensive visual and other presentation aids are neither necessary nor desired. Include only information that is relevant to this source selection.

2.2 Organization/Number of Copies/Page Limits: A complete proposal shall consist of four (4) volumes:

Volume I – Executed Contract Documentation/Supporting Documentation, Volume II - Technical, Volume III

Past Performance, and Volume IV Price. Specific guidance regarding the content of each volume will be discussed further below. Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal. When Evaluation Notices (ENs) are necessary, the

Government will issue them by electronic means to prospective offerors. Page limitations may be placed on responses to these ENs and such limitations will be provided at the time the EN is issued.

2.3 Page Size, Format and Limit: The proposals will be on 8 1/2” x 11” paper, white or ivory stock. Charts, tables or diagrams may not exceed 11” x 17”. A page is defined as one face of a sheet of paper containing information. This includes all appendices, charts, graphs, diagrams, tables, photographs, etc, unless placed on

11” x 17” sheets, in which case they will be counted as two (2) pages. Covers for volumes, tables of contents, indices, title pages, cross reference indices and section dividers/tables will not be included in the page count if they are inserted solely to provide ease to the reader in locating volumes/sections of the proposal. They will be counted if they contain any other information, (e.g. diagrams, extraneous data, etc). Pages marked “This page intentionally left blank” will not be counted. Pages in excess of the page count stated in the table below will not be read, and will be removed and returned to the offeror. Type face shall not be less than 12-font, with 1” margins on all sides. Smaller, legible font size, no smaller than 6-point, may be used for charts and tables. An electronic page is defined as each face of an 8 ½” X 11” electronic sheet of paper containing information. Page limits apply to all electronic files and hard files.

All pages of each part shall be appropriately numbered and identified with the Request for Proposal (RFP) number. Pages in excess of the maximum will not be evaluated.

Cost or Pricing Related Data. All cost and/or pricing data shall ONLY be addressed in Volume II Subfactor 2 –

Seed Project and Volume IV Price.

Proposals should be titled appropriately so as to easily discern each volume. Each Volume shall contain a more detailed table of contents to delineate the subparagraphs within that file. Tab indexing shall be used to identify sections. Indexing is not included in the page limitations.

3.0 SPECIFIC PROPSAL SUBMISSION INSTRUCTIONS: The proposal shall be prepared in accordance with these instructions and shall be evaluated in accordance with the evaluation criteria and evaluation standards in Section M, Evaluation Factors for Award. Proposals shall include necessary information to enable the evaluators to form a definitive conclusion concerning the offeror's ability to perform the required construction.

3.1 VOLUME I – EXECUTED CONTRACT DOCUMENTATION/SUPPORTING DOCUMENTATION

The Government will review Volume I for completeness. The Contracting Officer may conduct clarifications to address the completeness of Volume I deliverables. It is the offeror’s responsibility to verify the executed contract documentation and supporting documentation meet the requirements of Sections L & M of this solicitation. Additionally, offerors must annotate all Socioeconomic/Small Business reserves for which they qualify and desire to compete in. Offerors are not limited to competition under one socioeconomic group.

Offerors shall include a single page cover sheet identifying all Socioeconomic/Small Business reserves for which they qualify and desire to compete in.

3.1.1 – Executed Standard Form 1442/Seed Project Basis of Estimate/Letter from Surety/Signed

Teaming Agreements, if applicable

(a) Submit the Standard Form 1442 and all pages originally included in the RFP. Submit one (1) copy.

Complete blocks 14, 15, 16, 17, 19, and 20a-c of the SF 1442, Solicitation, Offer and Award. An authorized official of the firm submitting the offer must sign and date the SF 1442 in block 20a-c. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP, Sections A through M. Complete the representations and certifications in Section K or the System for Award

Management (SAM).

(b) The prime contractor must provide proof of bonding capacity to support the NC2 program for JBSA.

To be considered, the contractor must have a letter from a surety on surety letterhead that says the contractor has a relationship with the company and required bonding capacity can be obtained.

Provide proof of bonding capacity with bonding capability letters from an approved surety listed with the Department of Treasury. The list of approved sureties can be found at http://www.fiscal.treasury.gov/fsreports/ref/suretyBnd/c570_a-z.htm. Letters must be notarized and signed by someone who can represent the surety. Letters signed by agents are unacceptable unless accompanied by a Power of Attorney granting proper authority to the agent. To be considered, the contractor must be capable of providing an aggregate bonding capacity of at least $8,000,000.00. The contractor shall furnish the required seed project bid guarantee IAW FAR 52.228-1.

(c) Signed Teaming Agreements (if applicable), submit signed Teaming Agreements or signed Letters of

Intent and a complete, coherent presentation of team abilities, as applicable. All team members must have a signed Teaming Agreement (TA) or Letter Of Intent (LOI) that commits the parties to performance under the basic IDIQ contract. If Teaming Partners will be used, a complete, coherent presentation of team abilities must be included.

(i) If any first-tier subcontractor is projected to perform more than 20% of all construction work on the basic IDIQ contract, then a signed TA or LOI shall be provided for that subcontractor.

(ii) First-tier subcontractors projected to perform less than 20% of all construction work on the contract may be included as team members at the Prime’s discretion to ensure coverage of SOW scope. If included as a team member, a signed TA or LOI shall be provided for that subcontractor.

(iii) A subcontractor without a TA or LOI with the Offeror is NOT considered as part of the “team” and will not be evaluated as such.

(iv) Past Performance Information submitted for subcontractors without a TA or

LOI will not be evaluated.

(v) The Government will consider the Offeror and all subcontractors a “team” when a signed TA or LOI is provided in Volume I.

(vi) A Joint Venture (JV) will be considered a sole business entity.

(vii) Mentor-Protégé (M-P) is a relationship and not a business entity. M-Ps require formal TAs or LOIs to qualify for consideration as a team member.

(viii) Primes can team with no more than three (3) partners to meet requirements of the RFP. Proposals will be evaluated based on the team, not just the Prime.

(ix) Offerors who propose as a Joint Venture (JV) are not permitted to submit a separate proposal independent of the JV.

(x) Offerors are limited to one offer as a Prime contractor and one additional

Teaming Arrangement.

(xi) An offeror who does not submit as a Prime can serve as a teaming partner for two primes.

(xii) The proposal shall present a complete, coherent presentation of team abilities as applicable.

3.2 VOLUME II – TECHNICAL (FACTOR 1)

3.2.1 SUBFACTOR 1 – Executive Summary

(a) The executive summary should, at a minimum, address the offeror’s key technical, performance and management features in regards to performing the requirements of the solicitation. The purpose of this section is to provide the Government evaluators with a clear and concise introduction to the content and structure of the Technical proposal. This should be a "big picture" explanation and provide the

Offeror’s overall approach and supporting rationale for the technical performance/management of the

NC2 requirements.

3.2.2 SUBFACTOR 2 – Seed Project

(a) Offeror shall provide a solution to the Seed Project Repair Building Exterior, Bldg. 2792, JBSA-Ft.

Sam Houston. The offeror must provide a detailed project schedule, a detailed work breakdown structure (WBS) and a redacted Basis of Estimate. The Seed Project deliverables must demonstrate a clear understanding of all tasks and elements of work required to complete the project.

(b) Project Schedule: Submit a detailed project schedule. The schedule must show major measurable line items of the construction project, the percentage/value each line item represents in the total proposal cost and a timeline of when each line item is scheduled to be completed.

(c) Work Breakdown Structure (WBS): Provide a detailed work breakdown structure for the entire seed project. This analysis must identify the elements of work, labor mixes, materials, equipment and labor hours to complete the project. The WBS must demonstrate a clear and concise approach, which the Prime Contractor and Teaming Partners will use to complete the seed project.

(d) Basis of Estimate: Provide a complete basis of estimate for the Seed Project that identifies the elements of work, labor mixes, materials, equipment and labor hours to complete the project. The estimate should show detailed listing of RS Means line item numbers, description of items, units of measure, quantity of units required, and REDACTED unit costs/total cost.

3.2.3 SUBFACTOR 3 –Management Plan and Qualifications

(a) Management Plan/ Qualifications: The offeror shall summarize their overall management plan/approach. This plan shall include the offerors overall management approach with regard to organization, coordination, development, monitoring and control of the construction process. The plan shall also describe the offeror’s local site office organization with regard to levels of management, supervisory experience, personnel authority, and specific positions required to manage requirements through project completion. The plan shall also describe the offeror’s material purchasing system and local capabilities. Describe how warranty/maintenance support will be handled by your organization. Describe interface with home/site office, subcontractor operations and construction teams. Describe interface and partnering with government project managers, contracting officials, inspectors, and users.

(b) The offeror shall submit an organizational chart delineating the key functional areas within the Prime

Contractor’s corporate structure and their relationships and interactions between the headquarters staff and key field staff personnel as well as all teaming partners and subcontractors that depicts a systematic approach to manage all of the requirements in the project specifications as required.

(c) Quality Control Plan: The offeror shall provide a general description of their quality control plan that outlines the offeror’s intended approach to provide quality control for the NC2 program.

(d) The offeror shall provide a personnel overview that shows offeror’s personnel have achieved the minimum qualifications. The contractor’s key personnel who will be involved in the day-to-day operations of this contract and subsequent task orders shall include, as a minimum, personnel qualified in the areas identified below. The following sub-paragraphs identify the minimum requirements, certifications, licenses, etc. required for personnel performing activities or acting in a specific capacity:

(i) Program/Project Manager (PM): The Project Manager must have a minimum of 10 years of experience as a Project Manager or Superintendent on projects of similar size and complexity.

The Project Manager is primarily responsible for the overall management of the project. The PM is the primary POC for the Contracting Officer for all contractual issues with regards to project/program execution and is responsible for the overall management of this contract. The

PM is to ensure that quality work is accomplished on schedule and qualified personnel and subcontractors are assigned to complete performance. It is the PM’s responsibility to ensure that contractor personnel and subcontractors possess the appropriate qualifications and experience to complete specified project tasks and that they comply with all statutes, regulations and the contract requirements. The PM shall have available and provide to the CO, upon request, all required employee or subcontractor certifications and qualifications.

(ii) Project Superintendent(s): A Project Superintendent shall not manage multiple project across

JBSA installations. The Project Superintendent can manage up to eight (8) projects at a single installation (Lackland includes Kelly Field Annex, Medina Annex (Chapman Annex), and Port

Annex/Fort Sam Houston includes Medical Center Annex and Grayson Street Annex/Randolph includes Sequin Auxiliary Field and Canyon Lake). The project superintendent must have a minimum of 10 years’ experience in construction with at least 5 of those years as a superintendent on projects similar in size and complexity. The individual must be familiar with the requirements of Emergency Management (EM) Publication 385-1-1 and have experience in the areas of hazard identification and safety compliance. The individual must be capable of interpreting a critical path schedule and construction drawings. The qualification requirements for the alternate superintendent are the same as for the project superintendent.

(iii) Quality Control (QC) Manager: The contractor shall have a QC Manager with a minimum of three (3) years’ experience in construction and at least two (2) years of hands-on construction.

The QC Manager must report to an officer of the firm and not be subordinate to the Project Superintendent or Project Manager.

(iv) Site Safety and Health Officer (SSHO): Provide an SSHO that meets the requirements of

USACE EM 385-1-1 Section 1. The SSHO must ensure that the requirements of 29 CFR

1926.16 are met for all projects. Provide a Safety oversight team that includes a minimum of one

(1) person at each project site to function as the Site Safety and Health Officer (SSHO). The

SSHO or an equally-qualified Alternate SSHO must be at the work site at all times to implement and administer the Contractor's safety program and government-accepted Accident Prevention

Plan. The SSHO and Alternate SSHO must have the required training, experience, and qualifications in accordance with USACE EM 385-1-1 Section 01.A.17, and all associated sub-paragraphs. The SSHO may also serve as the Quality Control Manager. The SSHO may also serve as the Superintendent. However, the SSHO may not serve as SSHO, Quality Control

Manager and Superintendent (all three positions) concurrently

3.3 VOLUME III PAST PERFORMANCE (FACTOR 2)

Offerors shall provide adequate past performance information on completed or current contracts considered most relevant in demonstrating the ability to perform the proposed work as identified within the specifications. Offerors must provide recent and relevant past performance information. Offerors are required to explain what aspects of each contract are deemed relevant to complexity and magnitude as related to the requirements of this solicitation. Failure to provide required relevancy description may impact the integrated performance confidence assessment rating, (i.e. aspects of previous projects should match work elements identified in the Solicitation/specifications).

Offerors are cautioned that the Government will use data provided by the teaming partners as well as the prime contractor. The offeror shall provide no more than six (6) recent and relevant past performance references. The offeror shall provide at least two (2) recent and relevant past performance references from the prime contractor. The Government reserves the right to obtain data from other sources such as the

Contractor Performance Assessment Reporting System (CPARS) and the Past Performance Information

Retrieval System (PPIRS), but it’s the responsibility of the offeror to provide no more than six (6) recent and relevant past performance references.

Lack of any past performance will not automatically disqualify an offeror, but it is a factor that is considered in the Best Value Subjective Tradeoff Source Selection described in this section. If the offeror claims there is no past performance, then that status must be identified to the Contracting Officer in the Volume III Past

Performance Summary Page.

In conducting the Past Performance evaluation, the Government reserves the right to use both the information provided in the Offeror's Past Performance Proposal Volume and information obtained from any and all sources, including sources outside the Government. Other sources may include, but are not limited to, the Contractor Performance Assessment Reporting System (CPARS) and/or similar systems, Past

Performance Information Retrieval System (PPIRS) using all CAGE/DUNS numbers of contractors who are part of the partnership or joint venture identified in the Offerors proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity (FAPIIS), Electronic Subcontract Reporting

System and any other known sources not provided by the Offeror. The Government will consider the recency and relevancy of the information, the source of the information, context of the data and general trends in the Offeror's performance. This evaluation is separate and distinct from the Contracting Officer's responsibility determination. The assessment of the Offeror's past performance will be used as a means of evaluating the Offeror's probability to successfully meet the requirements of the RFP.

3.3.1 – Recency

Provide a summary of previous/current contracts not to exceed one (1) page of past performance information per reference. The summary should explain what aspects of each contract are deemed recent as related to the requirements of this solicitation. The Government will only consider recent performance on a project effort completed within the last three (3) years prior to the date of issuance of this solicitation. Ongoing projects may be used as long as they are at least 80% complete as of the date of issuance of this solicitation (A progress report signed by a Government Contracting Officer or equivalent must be provided to prove actual completion percentage).

(i) The summary should not exceed 1 page per reference and include:

(A) Name of project (Contract number, if applicable)

(B) Name and address of customer or Government agency

(C) Name, telephone, fax number and/or email of customer contact or contracting officer

(D) Dollar value

(E) Period of performance/Completion date

(F) Brief description of work completed

3.3.2 – Relevance

Provide a summary of previous/current contracts not to exceed two (2) pages of past performance information per reference. The Summary should explain what aspects of each contract are deemed relevant as related to the requirements of this solicitation. Relevant past performance information must demonstrate construction experience managing concurrent projects with similar scope and complexity of NC2 requirements. In determining relevance, consideration will be given to program similarity, project complexity, technical diversity, contract/subcontract management, schedule, and magnitude. Project work elements include renovation, alteration and repair, and construction. This information may include data on efforts performed by teaming partners, if such resources will significantly influence the performance of the proposed effort.

(i) The summary should not exceed 2 pages per reference and include:

(A) Name of project/program (Contract number, if applicable)

(B) Name and address of customer or Government agency

(C) Name, telephone, fax number and/or email of customer contact or contracting officer

(D) Dollar value

(E) Period of performance/Completion date

(F) Brief description of work completed, and why the effort is relevant

(G) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions, any mitigating circumstances and any corrective actions taken

3.3.3 – Quality

a. The quality assessment consists of an in-depth evaluation of the Past Performance

Information Sheets, Past Performance Questionnaire responses, CPARS or similar systems, interviews with Government customers and, if applicable, commercial clients.

b. The offeror shall provide a complete Present/Past Performance Information (PPI) Sheet for each reference (see Section J, Attachment 17). All information provided in the Present/Past

Performance Information Sheet should be recent and relevant.

c. The Government will use the PPIs to coordinate the completion of a Past Performance

Questionnaire (PPQ) with each reference (see Section J, Attachment 18). It is the responsibility of the offeror to provide past performance references that are current with the correct contact information.

3.4 VOLUME IV PRICE (FACTOR 3)

Provide a complete basis of estimate for the Seed Project that identifies the elements of work, labor mixes, materials, equipment and labor hours to complete the project. The estimate should show detailed listing of RS Means line item numbers, description of items, units of measure, quantity of units required, and unit costs. Additionally, the basis of estimate shall provide actual rates (i.e. profit, overhead, labor burden, bonding, City Cost Index) and the proper application of these rates to derive at the proposal grand total. This basis of estimate will be used to evaluate the Total Evaluated Price

(TEP).
