5.1.1 Instructions. The CO has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the CO will review this determination and if, in the CO’s opinion, adequate price competition exists no additional cost information will be requested. However, if at any time during this competition the CO determines that adequate price competition no longer exists offerors may be required to submit other-than-certified cost or pricing data to the extent necessary for the CO to determine the reasonableness of the price of the proposal.

Offerors shall complete only paragraph (b) of the provision FAR 52.212-3 if the Offeror has completed the annual representations and certification electronically via the System for Award Management
(SAM) website at https://www.sam.gov. Offerors who have not completed the annual representations and certifications electronically shall complete only paragraphs (c) through (p) of the provision FAR 52.212-3 and return it along with their proposal.
Tab 2: Pricing Information. Offerors shall review the entire Schedule of Supplies/Services in this Request for
Proposal (RFP) and submit under Tab 2 of Volume I of their proposal both a unit price and extended amount price for each Firm Fixed Price (FFP) Contract Line Item Number (CLIN), rounded up to two decimal places.
The extended amount price must equal the unit price multiplied by the number/quantity of units. The proposed prices shall be based on the Offeror’s own approach and the Government’s Performance Work Statement (PWS) requirements as set forth herein. Offerors who change the quantities stated in the solicitation shall render the proposal ineligible for award. The Offeror shall submit only pricing information that will be evaluated for award or become part of the resulting contract. Supply costs, associated General and Administrative (G&A) costs, profit and/or fees should be included in the unit price for each Firm Fixed Price (FFP) Contract Line Item
Number (CLIN) listed in the RFP Schedule of Supplies/Services. Wages will be subject to SCA/CBA increases over the contract life.
RFP Attachment 1 is the Contract Price Lists. Attachment 1 is an Excel document with three worksheets titled: Base Year CLIN 0001, 1st Option Year CLIN 1001 and 2nd Option Year CLIN 2001. Each worksheet list the 23 alteration services that will be performed under the LOTS for CLINs 0001, 1001, 2001. There are 12 LOTS in each Period of Performance. The Quantities for each service are estimated for a full year of performance; each period of performance may vary in estimated quantities and must be calculated separately, on the corresponding worksheet. The offeror will individually price each service to establish the “Service Total Amount” for that service. The “Service Total Amount” is established by taking the “Service Unit Price” and multiplying it by the “Service Quantity”. The “Estimate Amount (12
LOTS)” will be established by adding the sum of all 23 “Service Total Amount” together. “UNIT PRICE
(Each LOT)” will be established by taking “Estimate Amount (12 LOTS)” divided by 12.
The offeror shall use the information from Attachment 1 to complete the BID SCHEDULE. UNIT PRICE on Attachment 1 will equal to the UNIT PRICE on the BID SCHEDULE. The “Estimate Amount (12
LOTS)” on Attachment 1 will equal to ESTIMATED AMOUNT on the BID SCHEDULE. All units must be rounded to two (2) decimal points. Conflicting information may result in rejection of proposal.
Tab 3: Financial Disclosure Release Statement. Provide a release statement authorizing the Government to request information from current financial institutions or lenders must be submitted on official letterhead. In addition to this authorization, the offeror should include the financial institution or lender’s address, and a point of contact including email and phone number to be contacted with the authorization on the attached Offeror’s
